Psychiatric aspects of temporal lobe epilepsy.

This study is an attempt to investigate cases of temporal lobe epilepsy, the starting point being a large number of psychiatric patients. The data presented in this study suggest that a certain number of schizophrenic-like psychoses are produced by temporal seizural activity. There is some evidence that if there is a centrencephalic seizure at the beginning of such a psychotic state, the patient's behavior will have confusional characteristics (perhaps a postictal centrencephalic state). Some findings suggest that the side localization of the temporal lobe focus affects the feature of the psychotic state. None of the nine patients with periodic psychoses had schizoid or paranoid premorbid personality. As to the interictal behavioral disorders of epileptics, we have noticed that two patients with hysteroepilepsy and two with compensation neurosis had dominant temporal lobe focus. Of 10 cases with behavior disorders, all had grand mal or petit mal epilepsy, and none had a temporal lobe focus. Although this study is not based on sufficient data, it may provide working hypotheses for further research.

Munchausen by proxy syndrome: the triad of abuse, self-abuse, and deception.

The Munchausen By Proxy Syndrome (MbPS) is a complicated phenomenon whereby a peculiar threefold relationship between perpetrator, victim, and medical staff develops. This triad is based on abuse of the child, deception of the doctor, and, in many cases, self-abuse or passivity of the victim towards the perpetrator's deeds. We have analyzed the interrelationship between the perpetrator, the victim, and the medical authority, in light of the mother's pathology.

Ganser syndrome: a review of 15 cases.

Using the diagnostic criteria found in the literature, 15 new cases of Ganser syndrome were examined. A relationship was found between specific premorbid personality traits and severity of the syndrome; a high percentage of the sample belonged to an ethnic minority and was suspected of having symptoms of premorbid neurological pathology.

[Epithelioma cuniculatum. Apropos of a case with review of the literature].

A case of Epithelioma cuniculatum arising on a leg ulcer in a 85 years old lady is described. 102 cases have been reported before, mainly located on the sole of the foot: this rare tumor may be recognized by the gross appearance of a slow-growing exophytic cauliflower growth, exuding foul smelling material from numerous sinuses, which eventually invades underlying tissue sometimes including bone, and which rarely metastasizes to the regional lymph nodes. Histological examination shows a well-differentiated squamous epithelial proliferation without cellular atypia, with numerous crypts and sinuses. The diagnosis is difficult as the cytological regularity does not allow malignancy to be evoked. It is the conspicuity of the invasion of the deep tissues with sufficiently large samples that makes it certain. The differential diagnosis includes keratoacanthoma, pseudo-epitheliomatous hyperplasia mainly represented by pyoderma vegetans and "classical" squamous cell carcinoma. From a nosological point of view the authors totally assimilate Epithelioma cuniculatum and Gottron's papillomatosis cutis carcinoïdes regrouped under the name of cutaneous verrucous carcinoma, emphasizing the analogy with verrucous carcinomas of mucous membrane, giant condyloma of Buschke-Lowenstein, oral florid papillomatosis.
