Ventilation with nitrous oxide during open cholecystectomy increases the incidence of postoperative hypoxemia.

The effect of intraoperative use of air versus nitrous oxide (N2O) on postoperative oxygen (O2) saturation in blood was evaluated in 40 ASA Class I and II patients undergoing elective, open cholecystectomy. Patients were allocated randomly to two groups on the basis of whether they received air (Group A, n = 20) or N2O (Group B, n = 20) intraoperatively. Oxygen saturation was recorded on arrival of the patients in the ward, 24 h, and 48 h postoperatively. Although mean O2 saturation did not differ significantly (P > 0.05) between the groups over the first 24 h postoperatively, it was significantly higher (P < 0.05) in Group A as compared to Group B 48 h postoperatively. Incidence of hypoxemia (O2 saturation < 90%) was 40% in Group B as compared to 0% in Group A at the end of 48 h postoperatively. We conclude that the use of N2O during cholecystectomy is associated with a higher incidence of hypoxemia postoperatively.

Measles associated hepatobiliary disease: an overview.

In this report the literature on all cases of documented measles with hepatobiliary disease was reviewed to describe the characteristics of measles associated hepatitis. We found 27 patients described. Their age ranged from 9 to 59 years. Male to female ratio excluding military facilities was 1:1. The clinical and laboratory presentation appeared to follow one of two patterns. The first was suggestive of hepatocellular dysfunction characterized by aminotransferase elevation. It was encountered in 24/27 cases, seven of them with atypical measles. This form of liver disease tended to be asymptomatic. It appeared early and resolved in a few days. The second pattern was characterized by cholestasis and jaundice. It was described in three cases. This type of hepatitis became apparent when measles began to recede and persisted for two weeks or longer. The variation in time of appearance of the two types of liver disease may imply distinct pathogenetic mechanisms.

Causes of conjunctivitis and keratoconjunctivitis in Karachi, Pakistan.

The causes of conjunctivitis and keratoconjunctivitis in 388 patients who attended eye casualty departments in Karachi, Pakistan, during a 5 month period were investigated. Most of these infections were diagnosed as adenovirus (291, 75%) or bacterial (71, 18.3%). Of the remainder, 9 cases (2.3%) were caused by herpes simplex virus and 7 (1.8%) by Chalmydia trachomatis. There was no evidence of typical active trachoma in this urban population. Bacteria or Candida albicans were also grown from 44 of the adenovirus cases (15%). Many of the bacteria grown from eyes in this study were resistant to antibiotics, probably because of inadequate and/or inappropriate self-medication with antibiotics in this community.

Pathology of mesenteric lymph nodes of buffaloes and cattle with special reference to salmonellosis.

Enlarged mesenteric lymph nodes were collected from ninety buffaloes and sixty cattle slaughtered at Faisalabad abattoir. Among these, salmonellae were isolated from lymph nodes of 32 (21.33%) animals. Maximum preponderance of salmonellosis was recorded in animals over two years of age. Enlargement, pale to dark red in color, increased consistency and even calcification were the main gross pathological lesions. Histopathological lesions included thickened capsule, typical lymphofollicular reaction, accumulation of oedematous fluid and haemorrhages.
