Unpredictable response to vasodilator therapy in primary pulmonary hypertension.

A 49-year-old man with severe primary pulmonary hypertension unresponsive to multiple vasodilators showed beneficial response to intravenous (i.v.) isoproterenol with significant decrease in the pulmonary vascular resistance and increase in the cardiac output. However, the patient became dependent on i.v. isoproterenol and eventually died suddenly following an episode of bradyarrhythmia. Autopsy confirmed severe primary pulmonary hypertension. As the response to vasodilator therapy can be unpredictable in PPH, sequential hemodynamic evaluation of drugs is necessary for finding an optimal therapeutic agent.

The incidence of temporomandibular disorder signs in patients wearing complete dentures compared to patients with a natural dentition.

The aim of this study was to determine the incidence of signs of a temporomandibular disorder in patients wearing complete upper and lower dentures and to compare this to the incidence of signs in dental out-patients with a natural dentition. One hundred and sixty patients were assessed using a questionnaire and clinical examination. The results showed statistically significant differences between the two groups regarding the presence of temporomandibular disorder signs, the dentate group having a higher incidence (P < .04).

Reversibility of furazolidone-induced changes in testes and secondary sex characters of White Leghorn cockerels.

Furazolidone (Fz) was fed to 4 groups of cockerels 15 w of age at 0, 400, 800 and 1200 mg/kg feed for 3 w and then was withdrawn for another 3 w. In a second experiment conducted simultaneously cockerels were fed these same Fz levels for 6 w and then were withdrawn from Fz feed for 4 w. In both experiments, 800 and 1200 mg Fz/kg feed resulted in decreases in comb and wattle size and in the weight of testes. The testes had smaller seminiferous tubule diameters and increased relative and decreased absolute volumes of interstitium. Spermatogenesis was not observed in cockerels fed 800 and 1200 mg Fz/kg feed. Withdrawal of Fz resulted in increased comb and wattle size, weight of testes and diameter of the seminiferous tubules. There was also an increase in absolute volume of interstitium along with decreased relative interstitium volume. Spermatogenesis was restored.

Modified Laryngeal Mask Airway--a useful aid to fiberoptic intubation.

Modified Laryngeal Mask (MLMA) aided fiberoptic intubation (FOI) using larger than 6 mm ID endotracheal tube was studied in 25 adult patients and compared to unaided FOI in another 25 patients. MLMA aided FOI was successful in 96% cases compared to 56% in the unaided group. Furthermore, the time to successful intubation was increased by 67.1% when FOI was attempted without MLMA aid which was statistically significant (P < 0.001). However, maximum fluctuations in the heart rate and mean arterial pressure in the immediate post-intubation period were statistically insignificant between the groups. We conclude that MLMA not only adds to success rate of FOI but also significantly reduce the FOI time in adult patients.

Hypobaric spinal anaesthesia with bupivacaine (0.1%) gives selective sensory block for ano-rectal surgery.

Twenty adult male patients undergoing anorectal surgery in the jackknife position under spinal anaesthesia were studied for the anaesthetic properties of 5 ml hypobaric 0.1% bupivacaine. The patients were positioned in the prone, jack-knife position with a pillow under the hips and with an operating table break angulation of 30 degrees with head down tilt of 20 degrees. In this position a 25-gauge Quincke spinal needle was inserted intrathecally through L3-4 and 5 ml solution, prepared by mixing 1 ml bupivacaine 0.5% with 4 ml of distilled water with a specific gravity of 1.001 at 20 degrees C, was given over 15-20 sec. Onset time, progression and upper level of sensory blockade evaluated by pin prick, and the extent of motor block (1 = full motor movement at ankle and knee joint, 2 = restricted motor movements, 3 = full motor block, no movements) were measured at one minute intervals for the first five minutes, then every five minutes for 30 min. The number of dermatomes blocked was also noted. The median level of cephalad sensory blockage was of L1, with a range from T10-L3. On average, nine dermatomes were blocked (range 7-12). Motor blockade was not observed in any patient. Changes in heart rate and blood pressure were minimal. The average duration of postoperative analgesia was 339.5 +/- 182.9 min. Post-spinal headache was not observed in any patients. In conclusion, 5 ml intrathecal hypobaric bupivacaine, 0.1%, provided excellent perioperative analgesia without motor blockade and haemodynamic stability in patients undergoing anorectal surgery in jackknife position.

Egg shell penetration tendency of different Salmonella serotypes by attached ring color method.

To investigate the extent of Salmonella penetration through the egg shell, 200 eggs were dipped in red (for 3 minutes) and then in green (for 6 minutes) aqueous bland food color solution for the detection of positive penetration test areas. Each egg with positive penetration area, 5 spots of 1 cm in diameter, was marked for the attachment of steel cylinders (1 cm in diameter and height). These cylinders were filled with the test strain of Salmonella. Among 19 serotypes S. pullorum and S. gallinarum were nonmotile while the other 17 were motile. Among a total of 180 eggs (900 points) maximum (30%) penetration was in area III, where salmonellae invaded through cuticle, shell, inner and outer shell membranes, followed by area II (14.77%) and area I (4.6%). It was very well evident that penetration of salmonellae to the contents of eggs was maximum, while in area II the penetration was to outer shell membrane and in the least cases through the cuticle and shell. Penetration in area I is not significant and to some extent in area II as well, while invasion in area III is highly significant.

Ventilation with nitrous oxide during open cholecystectomy increases the incidence of postoperative hypoxemia.

The effect of intraoperative use of air versus nitrous oxide (N2O) on postoperative oxygen (O2) saturation in blood was evaluated in 40 ASA Class I and II patients undergoing elective, open cholecystectomy. Patients were allocated randomly to two groups on the basis of whether they received air (Group A, n = 20) or N2O (Group B, n = 20) intraoperatively. Oxygen saturation was recorded on arrival of the patients in the ward, 24 h, and 48 h postoperatively. Although mean O2 saturation did not differ significantly (P > 0.05) between the groups over the first 24 h postoperatively, it was significantly higher (P < 0.05) in Group A as compared to Group B 48 h postoperatively. Incidence of hypoxemia (O2 saturation < 90%) was 40% in Group B as compared to 0% in Group A at the end of 48 h postoperatively. We conclude that the use of N2O during cholecystectomy is associated with a higher incidence of hypoxemia postoperatively.

Measles associated hepatobiliary disease: an overview.

In this report the literature on all cases of documented measles with hepatobiliary disease was reviewed to describe the characteristics of measles associated hepatitis. We found 27 patients described. Their age ranged from 9 to 59 years. Male to female ratio excluding military facilities was 1:1. The clinical and laboratory presentation appeared to follow one of two patterns. The first was suggestive of hepatocellular dysfunction characterized by aminotransferase elevation. It was encountered in 24/27 cases, seven of them with atypical measles. This form of liver disease tended to be asymptomatic. It appeared early and resolved in a few days. The second pattern was characterized by cholestasis and jaundice. It was described in three cases. This type of hepatitis became apparent when measles began to recede and persisted for two weeks or longer. The variation in time of appearance of the two types of liver disease may imply distinct pathogenetic mechanisms.
