[Multiple primary cancers associated with gynecologic malignancies].

Seventeen multiple primary cancers including 16 double cancers and one triple cancer were found in 316 patients with gynecologic malignancies who were treated in our department from 1984 to 1988. All pathologic slides but one were reviewed, and cases with possible metastasis or recurrence were not included in this study. The incidence of multiple primary cancers in gynecologic malignancies was 5.4%. Multiple primary cancers were encountered in 4.4% of 205 cervical cancers (including carcinoma in situ), 15.2% of 33 endometrial cancers, and 8.6% of 58 ovarian cancers (including low potential malignancy), respectively. The most frequent sites of other cancers were seen in the large intestine and rectum (5/17), breast (4/17), and gynecologic organs (3/17). Higher incidences were seen in our study that in those in domestic literature. This is probably because detailed anamnesis and gastrointestinal series were obtained in most gynecologic malignancies (especially in endometrial or ovarian cancer).

[Spontaneous complete remission in a patient with acute monocytic leukemia].

A spontaneous complete remission was observed in a 47-year-old female with acute monocytic leukemia. Resolution of all abnormalities, including systemic papule, thrombocytopenia, increased numbers of immature monocytoid cells in the peripheral blood and bone marrow, elevation of serum lysozyme and LDH, and trisomy 8 on chromosome analysis, occurred without any treatment. Moreover, the remission was not associated with any infection or blood transfusion, and is persistent for 12-month duration.

[Study on the steroid sulfatase (STS) activity in normal individuals and patients with abnormal sexual differentiation].

The STS (steroid sulfatase) gene which has been assigned to the short arm of human X chromosome (band p22) is thought to have escaped from Lyon's inactivation. For that reason, the STS enzyme activities differ between male and female according to the number of X chromosomes in cells. In this report, the STS enzyme activities were studied in different human tissues such as placentas, lymphocytes, and cultured fibroblasts of normal individuals and sex anomaly patients. Tritium labelled dehydroepiandrosterone sulfate (DHA-S) was used as the reaction substrate. The placental STS activities between normal male and female subjects showed a significant difference in spite of wide variances that were ascertained not to be the effects of methodological alterations involving enzyme purification, substrate concentration, and activity calculation (units per mg of protein or DNA). Further, lymphocytes and fibroblasts which had low levels of enzyme concentration compared with placentas, the STS enzyme activities were also significantly different between both sexes. These results confirmed that the STS gene in cells of tissue tested here seemed to be inactive at the gene level and followed the gene dosage effect to some extent. The enzyme activity was also studied in 17 patients with sex anomalies using lymphocytes and cultured fibroblasts. The cells of 45,X Turner Syndrome and of Klinefelter syndrome with 47,XXY or other karyotypes, showed slightly lower levels of enzyme activity when compared with control values of normal males or females. The enzyme activity in intersexual disturbances, especially XX male and XX true hermaphrodites, showed intermediate levels between normal male and female values. This result may give support to the concept that at least one X chromosome in these diseases is genetically abnormal due to X-Y interchanges, something that has been partly proved recently by analysis of H-Y antigen and Y-specific DNA probes. The present study on the STS enzyme activity revealed the presence of a gene dosage effect of STS gene between males and females not precise but rather rough in quantity, and it pointed out problem, some of which were related to genetic and environmental factors modifying the STS gene expression in normal individuals as well as sex anomaly patients.

A comparative study of the estrogen receptor ratio in myometrium and uterine leiomyomas.

Estrogen receptor (ER) content in myoma and myometrium was determined by radio-receptor assay (RRA), and ER staining was performed by immunocytochemical staining, i.e. the peroxidase-antiperoxidase (PAP) method, in uterine samples from eight patients with uterine leiomyoma. ER content in myoma tended to be hither than that in myometrium, but the difference was not significant. The distribution pattern of ER staining was thought to be an important criterion. When the data were compared on the basis of the number of cells in the site where the largest number of positive cells aggregated, the number for myoma was significantly higher than that for myometrium (P less than 0.05). These results suggest that myoma contains more ER positive cells to obtain estrogen effects than myometrium.

Clinicopathologic study of squamous cell carcinoma of the ovary.

Clinicopathologic study was performed on 10 squamous cell carcinomas of the ovary. All four patients with stage II or III lesions had deteriorated within 1 year after the operation, and four of six patients with stage I lesions had survived over 5 years. Clinical findings of the patients with ovarian SCC, including age, chief complaint, clinical stage, and outcome of the patients, were similar to those of common epithelial cancer. The effectiveness of chemotherapy was not shown in this study. Histopathologic study revealed that squamous cell carcinoma may arise not only from epidermis, but also from squamous metaplastic epithelium of respiratory gland.

[Three cases of Klinefelter syndrome in childhood--endocrinological and gonadal changes in puberty].

Three children at the ages of 4, 10 and 12 years, with external genital malformation, were diagnosed to be with the Klinefelter syndrome by chromosome analysis. To clarify the pubertal changes in this syndrome, all of them were studied for physical and endocrinological examinations and two underwent testicular biopsy. Before puberty any remarkable abnormality were not observed in the hypothalamus-pituitary-gonadal axis and in the physical status. After the onset of puberty they started showing increases of the basal levels of plasma FSH and LH with over-response to LH-RH stimulation test. During the period of this study the levels of plasma testosterone were in the normal range and increased gradually with age. One boy showed a transient high level of plasma testosterone at early puberty. The reactions of plasma testosterone to HCG stimulation of all cases showed the normal pattern. The histological examination of the testis revealed that the number of spermatogonia was reduced in both cases compared with that of normal boys reported by Mancini et al. These findings indicate that most endocrinological and histological abnormalities in adult Klinefelter syndrome occur after the onset of puberty. These changes may be induced at puberty by hypergonadotrophic condition which result from slightly impaired testicular function which is present before puberty.

[Effect of breathing fluctuations on cerebral blood flow in demented patients and its correction method using end-tidal CO2 concentration].

During mouthpiece respiration of Xe-133 for a measurement of regional cerebral blood flow (rCBF), the breathing pattern of patients fluctuated and it caused a change of end-tidal CO2 concentration that had an excellent correlation with PaCO2 in patient without respiratory disease. The end-tidal CO2 concentration of demented patients varied within lower ranges than senile control group. The range of fluctuation on the end-tidal CO2 concentration was dependent on the type and the degree of dementia, and it fluctuated most widely at the middle stage of Alzheimer disease. Mean cerebral blood flow increased by 13.9% for each 1% increase in end-tidal CO2 concentration (3.6%/mmHg PaCO2) in the case of demented patients without cerebrovascular disease. To improve the reliability of rCBF in demented patients, especially in Alzheimer disease, the correction of rCBF data for end-tidal CO2 concentration should be performed.

[Mammary gland carcinoma treated by large-dose medroxyprogesterone acetate of which pulmonary metastases disappeared and a selection of hormone, chemoimmunological treatment].

A patient who underwent radical mastectomy was treated by tegafur and tamoxifen. Pulmonary metastases were found four months after operation. Lymphokine activated killer (LAK) and auxiliary interleukin-2 (IL-2) administration was performed, with no change (NC). However, administration of large-dose medroxyprogesterone acetate induced complete disappearance of pulmonary metastases after more than nine months.

[Intravesical BCG therapy in patients with recurrent superficial bladder tumors].

Intravesical instillation of BCG has recently become a treatment for superficial bladder tumor, and favorable results are reported. We have also obtained favorable results by performing intravesical BCG therapy to totally 32 cases of superficial recurrent bladder tumor and carcinoma in situ (CIS) one indicated for total cystectomy. The subjects were 29 cases of Ta and T1 and 4 cases of Tis, which are classified into 27 cases for treatment and 5 cases for prevention of recurrence after TUR. As the method for administration, 120 or 80 mg of BCG was dissolved in physiological saline and infused intravesically every week a total of 8 times. As a result of designating the patients who received the treatment more than 4 times as the subject, CR and PR were 19 cases (73%) and 5 cases (19%), or respectively, out of 26 assessable cases, and effective rate including PR was 93%. No recurrence has been observed so far in any of 5 cases treated for prevention of recurrence. Side effects which required treatment were observed in 13 cases (41%), but treatment could be continued by symptomatic therapy in most of the cases. Treatment was discontinued in 4 cases, 3 of which were hepatic disorders. It is considered that intravesical instillation of BCG is a therapeutic procedure which is good for trial in future in the light of the great efficacy obtained in the present study.

[Cystic thyroid masses: ultrasonographic and pathologic correlation].

Forty-nine patients with 63 cystic thyroid masses who had undergone preoperative sonography were retrospectively reviewed. 52 lesions (83%) were benign and others (17%) were malignant. Among various sonographic findings of cystic thyroid masses, oval cystic lesions with polyp or dome like solid component projecting into the lumen were all diagnosed adenomatous goiter. Irregular cystic structures with more than 2 cm finger like pedunculated mass extended into and/or out of the lumen were all diagnosed papillary carcinoma. Small oval cysts (less than or equal to 1 cm) with strong echo were all diagnosed colloid goiter. The other sonographic type of cystic thyroid masses had somewhat malignancy (12-30%), not having characteristics which differentiate benign from malignant lesions. Pathologic findings of malignant lesions showed that cancer cells existed rather in pericystic portion than in cyst wall except for finger like solid component projecting into the lumen which was papillary carcinoma itself. Ultrasonically guided needle biopsy should be performed to get samples of above portions.

[Ultrasonography of ganglions in the extremities].

Ultrasonography of 8 cases of cystic mass in extremities (7 ganglions, 1 popliteal cyst) were reviewed and evaluated. The lesions were demonstrated as round, oval or flattened shaped masses with no echo signal. However, in most cases, there was no sign of posterior echo enhancement, which may be the effect of posterior bony structure. It seemed important to handle probe with adequate palpation, which enables examiner to get further anatomical information.

Captopril renal scintigraphy in a patient with bilateral renal artery stenosis.

A 41-yr-old woman with bilateral renal artery stenosis (RAS) and renovascular hypertension is presented. In this patient, the routine [99mTc]diethylenetriaminepentaacetic acid renal scintigraphy without Captopril was normal and the subsequent study with Captopril showed a marked decrease in glomerular filtration rate of the right kidney alone. Percutaneous transluminal angioplasty of the functionally affected right kidney immediately evoked new renin-dependent hypertension caused by the untreated left RAS. This patient illustrates two major points: (a) as a diagnostic tool, the sensitivity of Captopril renal scintigraphy may be compromised by an inability to detect bilateral RAS, but (b) after angioplasty, it can be useful for evaluating the treated kidney and in revealing contralateral disease that may benefit from angioplasty.

[Alveolar hydatid disease of the liver in children--radiographic findings].

The alveolar hydatid disease of the liver is uncommon in childhood. Six were less than 15 years old in our hospital. Among the first three patients, two had hepatomegaly, and the third patients had liver dysfunction. The latest three patients were detected by serological test and ultrasonographic examination in the mass screening when the lesions were too small to produce symptoms. Then CT and angiography were done for more precise examination. For the good prognosis, an early diagnosis and complete removal of the lesion are indispensable.

[Combination chemotherapy using cyclophosphamide, adriamycin, and cisplatin in recurrent or advanced endometrial cancer--a preliminary report].

Four patients with recurrent or advanced endometrial cancer have undergone combination chemotherapy with Cyclophosphamide, Adriamycin and Cisplatin (CAP). All drugs were administered by I.V. on day 1 in the following doses: Cyclophosphamide 500 mg/m2, Adriamycin 50 mg/m2 and Cisplatin 50 mg/m2. The treatment was repeated every 4 weeks and continued as long as there was disease progression. Two complete clinical responses and two partial responses were achieved. Based on these good results, we have initiated post-operative prophylactic chemotherapy using CAP in high risk patients. Adverse effects including myelo-suppression, nausea, and vomiting, and alopecia were seen in almost all patients. In no case, however, did any patient experience life-threatening toxicity. Based on our experience, CAP therapy appears tolerable when used per our schedule.

Effect of alpha 2-adrenoceptor antagonist on platelet activation during insulin-induced hypoglycaemia in type 2 (non-insulin-dependent) diabetes mellitus.

The role of epinephrine in platelet activation and the effect of an alpha 2-adrenoceptor antagonist, midaglizole, during insulin-induced hypoglycaemia in Type 2 (non-insulin-dependent) diabetes mellitus were examined. The action of midaglizole as a platelet alpha 2-antagonist was confirmed by in vitro studies using platelet-rich plasma and washed platelet suspension. Hypoglycaemia was induced by a bolus injection of short-acting insulin in 24 diabetic patients. They were divided into two groups, a control group (n = 12) and an alpha 2-group (n = 12), and midaglizole was administered orally 60 min before insulin injection in the latter. Blood glucose and plasma C-peptide levels were significantly decreased (p less than 0.005) by insulin injection in both groups. Counter-regulatory hormones, including epinephrine, and arginine vasopressin were similarly increased at the hypoglycaemic nadir compared with the levels at 0 min in both groups. Plasma beta-thromboglobulin was increased at the hypoglycaemic nadir (165.5 +/- 12.6 ng/ml) compared with the level at 0 min (121.0 +/- 11.5, p less than 0.005) in the control group, whereas no significant increase was demonstrated in the alpha 2-group. These results suggest that plasma epinephrine plays an important role in platelet activation during hypoglycaemia in Type 2 diabetes mellitus, and that the platelet activation is prevented by alpha 2-adrenoceptor antagonist.

Automated synthesis system with computer control for the production of [1-11C]fatty acids.

To accommodate the increasing need for [1-11C]fatty acids for use in routine medical studies and the large amounts of radioactivity required, a more advanced automated synthesis system for the preparation of [1-11C]fatty acids has been constructed. The synthesis of [1-11C]fatty acids has been completely automated using a new separation unit (isobaric Sepaltor) for the extraction procedure, which had caused some difficulties with automation. The yield of [1-11C]palmitic acid was 20-30 mCi, and that of 3-methyl [1-11C]heptadecanoic acid was 2-7 mCi. The time required for the synthesis was less than 30 min from the start of the 11CO2 trapping. This system has been used for more than 20 preparations of [1-11C]palmitic acid and more than 20 preparations of 3-methyl [1-11C]heptadecanoic acid.
