[Postnatal development of evoked potentials in the rat somatosensory cortex].

The developmental changes of evoked potentials in the somatosensory cortex were studied using Long-Evans rats of postnatal age from day 2 to day 55. A hole of appropriate size was made in the skull under urethane anesthesia (1.4 g/kg) and somatosensory evoked potentials (SEPs) were recorded with a silver-ball electrode. Electrical stimulation was applied to whisker C 3 follicle. The electrode was moved on the dura over the hemisphere and SEPs were displayed on an oscilloscope. When a maximum SEP was obtained 100 successive responses were digitally processed with a time resolutions of 100 microseconds and averaged. From the averaged SEP, various peak latencies and amplitudes were calculated. In response to electrical stimulation of the whisker follicle, a short-latency positive wave appeared on postnatal day 2 (PND 2) and the second positive wave was recorded on PND 8 and a negative wave appeared on PND 10. The latency of these waves shortened, while the amplitude increased with age. SEPs in developing rats attained adult pattern on PND 17, although both the values of latency and amplitude reach the limit on PND 55. The placing reaction was also tested from PND 9 to PND 15 to examine the functional development of whisker. All subjects displayed positive placing reaction of chin hair on PND 10. At this age all components appeared in the SEP, 1st positive, 2nd positive and negative waves.

[Predictable factors in healing of gastric ulcer, comparative analysis of local morphologic factor of gastric ulcer and suppression of intragastric pH].

Predictable factors in healing of 48 patients with gastric ulcer by PPI or H2-RA on 4 and 8 weeks of treatment were investigated. We evaluated the following factors such as patient's profile, local morphologic factors of gastric ulcer and suppression rate of 24 hr intragastric pH between healed and unhealed patients using single and multiple variable analysis. Results obtained from multiple variable analysis, the significant and independent factors of intractable gastric ulcers on the 4th week of treatment were ulcer size in over 20 mm and solitary ulcer. On the contrary, those of 8th week of treatment were shortage of holding time of intragastric pH over pH4 (less than 16.7 hrs) during treatment, the site of ulcer at lesser curvature of gastric angle and the shape of ulceration which showed mucosal overhanging. Considering with single and multiple variable analysis, we concluded that, in early phase, local shapes of the ulceration were important predictors in the healing of gastric ulcer, and the after that, the inhibition of intragastric acidity were significant factors in healing process of gastric ulcer.

Two rare cases of Diphyllobothrium latum parvum type infection in Korea.

Two rare cases of human infection with parvum (dwarf) type of Diphyllobothrium latum [syn. D. parvum (Stephens, 1908)], were discovered in Korea. The first case was a 46-year old housewife, from whom a kind of pseudophyllidean tapeworm eggs was detected in the feces. She was treated with praziquantel and purged, and a complete strobila with scolex, 120 cm in total length, was recovered. She recalled that she had eaten raw trouts at a raw-fish restaurant near the Chungju Lake. Another patient was a 22-year old medical student (male), who used to eat raw sea-foods. He discovered a chain of tapeworm proglottids, 15 cm in length, discharged spontaneously in his stool and brought it for identification. The worms from the two cases were compatible with D. parvum (Stephens, 1908) of which the taxonomic significance has long been questioned. After a detailed morphological study and review of literature, we designated the worms as D. latum parvum type. This is the first report on the occurrence of this rare type of D. latum infection in Korea.

Correlation of sonographic findings with histopathological changes of the bile ducts in rabbits infected with Clonorchis sinensis.

Clonorchiasis is an important parasitic disease of humans in Korea. The present study intended to compare sonographic findings with histopathological changes in experimental clonorchiasis. Eighteen New Zealand white rabbits were infected with metacercariae of Clonorchis sinensis, and examined 4, 10, and 22 weeks post-infection (PI). Four infected rabbits were treated with praziquantel 10 weeks PI and were examined 12 weeks after treatment. Sonography revealed mild to severe dilatation of the intrahepatic ducts (IHDD) and slightly increased periductal echoes in 12 out of 14 rabbits at 4 weeks PI, and all of the animals after 10 and 22 weeks PI and 12 weeks after treatment. The histopathological lesions were duct dilatation, mucosal hyperplasia, and periductal fibrosis, which progressed from 4 weeks to 22 weeks PI and even in treated rabbits. The dilated intrahepatic ducts over 1 mm diameter were detected by sonography. The present results indicate that sonographic findings are well correlated with histopathological lesions in rabbit clonorchiasis except for early phase of light burden of infection. The sonography has a limitation in discriminating residual sequelae of the ducts after praziquantel treatment.

[A new index of disease activity in Crohn's disease].

A comprehensive assessment including subjective and objective parameters seems to be essential for evaluation of disease activity in Crohn's disease. The aim of the present study was to develop an activity index (AI) of Crohn's disease, composed of both subjective and objective variables. Data composed of a total 132 clinical examinations in 100 patients with Crohn's disease were used to determine the AI. Three physicians independently evaluated disease severity in each patient. Stepwise multiple regression analysis was carried out with the overall evaluation as a dependent variable, and with 18 parameters as independent variables. Analysis showed that the following seven variables had a significant correlation with physician's overall evaluation: abdominal pain, abdominal tenderness, complications, C-reactive protein (CRP), body temperature, diarrhea, and body mass index (BMI). AI was expressed as follows: AI = 3.5 x abdominal pain + 3 x abdominal tenderness + 3 x complications + CRP + 3 x body temperature + 4 x diarrhea - 0.4 x BMI. AI values below 15 corresponded to inactive disease, values between 15 and 25 to mild disease, values between 25 and 35 to moderate disease, and values above 35 to severe disease. This study suggests that the activity index is useful for evaluation of the effect of medical treatment in Crohn's disease.

Differential expression of two members of FGF receptor gene family, FGFR-1 and FGFR-2 mRNA, in the adult rat central nervous system.

The fibroblast growth factor (FGF) receptor gene family now contains four members that encode homologous receptor-tyrosine kinases (RTKs) to each other. We have demonstrated that one of the members, FGFR-1 (also termed as flg), is expressed in the widespread but specific neuronal populations in the adult rat central nervous system (CNS). In the present study, we examined the expression pattern of another member, FGFR-2 (also termed as bek) and compared it with that of FGFR-1. In contrast with FGFR-1, we detected FGFR-2 expression primarily in the fiber tracts of the adult rat CNS, suggesting that the oligodendrocytes are the main sites of the FGFR-2 expression. These findings indicate that FGF may regulate neurons and glial cells through different subtypes of its cognate receptor.

Influence of heat shock, drugs, and radiation on karyotype of Leishmania major.

Leishmaniasis is one of the important tropical diseases in the world. Although it is not prevalent in Korea, imported cases have been recorded. The karyotype of Leishmania sp. has been observed to be variable by localities or by strains, but the karyotype of a strain is known to be stable. This study was performed to observe if the karyotype of a Leishmania sp. would be changed under some stressful conditions. The karyotype, analyzed by pulsed field gradient gel electrophoresis, was not grossly changed by heat shock, chemotherapeutics, UV illumination, and gamma irradiation. Radiation destroyed the chromosomes mechanically, but subcultured organisms after irradiation showed unaffected karyotype. The present findings suggest that the karyotype of a Leishmania strain is so stable that it is not altered by temporary stimulation with heat, drugs, and radiation.

[The long-term follow-up study of Japanese patients with ulcerative colitis].

To elucidate the long-term outcome of the ulcerative colitis we analyzed 124 patients suffering for more than 10 years concerning the long-term prognosis, cumulative rate of colectomy, cumulative risk of cancer development and cumulative survival rate. Long-term prognosis were divided to three categories as good, fair and poor, according to present disease activities, social availability and subjective judgement. One hundred and twenty four patients were divided to 66 patients with good prognosis, 27 fair and 31 poor (2:1:1). The rate of patients having active disease at each year were followed-up annually for 10 years according to final three categories of the prognosis. The serial curve of annual rate of having active disease of the patients with poor prognosis was the highest, fair prognosis the second, good prognosis the lowest. The factors affecting the final prognosis were the period before initiation of proper treatment, the severity of the first attack or the elder age (> 40 year old) at the first attack. Twenty six patients were undergone colectomy in relation to ulcerative colitis. The cumulative colectomy rate was 16.5% at 10 years and 38.5% at 15 years after the onset. Malignant disease or severe dysplasia of the colon developed in 4 patients 10 to 14 years after the onset. Cumulative survival rate was equal to the expected survival curve during 16 years. The main causes of death were intracranial hemorrhage and colorectal malignant disease.

An index of disease activity in patients with ulcerative colitis.

Quantification of disease severity was studied in 72 patients with ulcerative colitis, who had undergone total 85 clinical courses. We performed a multiple stepwise regression analysis, according to Truelove and Witts' classification, with disease severity as a dependent variable, and with 18 clinical, laboratory, and endoscopic parameters as independent variables. Results showed that disease severity in patients with ulcerative colitis is significantly influenced by five factors, namely, bloody stool, bowel movements, erythrocyte sedimentation rate (ESR), hemoglobin (Hb), and serum albumin. The activity index (AI) developed for ulcerative colitis is expressed as follows: AI = 60 x blood stool + 13 x bowel movements + 0.5 x ESR - 4 x HB - 15 x albumin + 200. Index values below 150, values between 150 and 220, and values above 220 nearly corresponded to mild, moderate, and severe disease, respectively, in Truelove and Witts' classification. We believe that the activity index is useful in evaluation of the effect of medical treatment in patients with ulcerative colitis. Its most important value will be in therapeutic trials.

Intestinal absorption of carnitine in experimental azotemia.

We studied carnitine absorption in rats rendered azotemic by subtotal nephrectomy (RF) and compared the results with those obtained in a sham-operated group on liberal food intake (NL) and those pair-fed (PF) with the RF group. Animals with short-term (2 weeks post-nephrectomy) and long-term (5-6 weeks post-nephrectomy) RF were studied. In vivo recirculating perfusion was used. A significant reduction in carnitine absorption (expressed per unit length) was noted in the short-term RF and PF groups when compared to the NL controls. When the data were corrected for intestine weight, the short-term RF and PF groups showed comparable absorption rates with the NL group. These findings suggest that in animals with short-term RF, weight loss is responsible for the observed alteration in carnitine absorption. Intestinal segments from PF and short-term RF animals had significantly greater carnitine accumulation suggesting decreased release into the circulation. In the long-term RF setting, RF and PF animals demonstrated reduced carnitine absorption. When the data were expressed per unit of weight the absorption rates normalized, implicating reduced intestinal mass as the factor responsible for reduced carnitine absorption. No significant difference was found in the amount of residual carnitine suggesting amelioration of impaired transport out of the enterocyte in chronic RF.

Intestinal absorption of L-ascorbic acid in rats with renal failure.

We studied L-ascorbic acid absorption in rats subjected to subtotal nephrectomy (renal failure (RF) group) and compared the results with those obtained in sham-operated normal animals and those pair-fed with their azotemic counterparts (PF group). In vivo recirculating perfusion and in vitro everted sac techniques were employed. The in vitro experiments were repeated substituting buffer within the serosal compartment with pooled sera from uremic and normal individuals. L-Ascorbic acid absorption in vivo in the RF group was significantly lower than those found in normal control and PF groups. In contrast, the in vitro mucosal to serosal transport was increased in the RF and PF groups when compared with the normal control group, suggesting increased permeability to L-ascorbic acid in the former groups. The disparity between in vivo and in vitro results in the RF animals is indicative of some inhibitory influence present in the intact uremic animals. However, experiments comparing the effect of uremic with normal sera on in vitro transport failed to reveal any suppressive effect of uremic chemical environment. In addition, serum ascorbic acid was reduced in PF and RF groups when compared with the normal control animals, thereby excluding elevated blood level as a cause of impaired absorption in intact animals with RF. In conclusion, in vivo jejunal absorption of L-ascorbic acid is impaired in rats with RF despite evidence of increased in vitro permeability. The latter appears to be mediated by reduced nutrient intake and weight loss. The inhibitory influence present in vivo could not be reproduced by incubation with uremic sera in vitro.

[A case of pulmonary alveolar microlithiasis on the bone image of dual-energy subtraction radiography].

A 33-year-old female was evaluated for abnormality of her chest roentgenogram which showed diffuse fine granular micronodulation in the lower two thirds of both lung fields. The bone image (soft tissue cancelled image) of dual-energy subtraction radiography obtained with Fuji Computed Radiography (FCR) demonstrated that the abnormal shadows were characterized by calcified deposits.
