Glucocorticoid-induced alterations of monocyte defense mechanisms against Candida albicans.

The mechanisms of glucocorticoid-induced immunosuppression of human monocytes for the defense of Candida albicans were examined in this in vitro study. Dexamethasone at pharmacological concentrations (10(-7)-10(-5) M) dose-dependently attenuated growth inhibition of C. albicans by resident monocytes. In interferon-gamma (IFN-gamma)-primed monocytes, fungal growth inhibition was maximal and not altered by dexamethasone. Similarly, phagocytosis and killing of Candida by monocytes were not affected by steroids. To study the underlying mechanisms, we found that Candida-induced synthesis of tumor necrosis factor-alpha (TNF-alpha) by monocytes was completely abrogated by dexamethasone. Substitution of TNF-alpha to dexamethasone-treated monocytes fully reversed the alterations of growth inhibition of C. albicans induced by steroids. These findings suggest that glucocorticoids affect the growth control of Candida by monocytes indirectly via suppression of the formation of TNF-alpha, which is required as an autocrine cofactor for full monocyte activation.

Tropisetron (Navoban) in the prevention of chemotherapy-induced nausea and vomiting--the Nordic experience.

An open, noncomparative, Nordic multicenter study was carried out during 1991-1992 to evaluate the 5-HT3 receptor antagonist tropisetron (Navoban) as an antiemetic agent for various types of cancer chemotherapy. A total of 630 patients were recruited from 15 centers in Sweden, Denmark, and Finland. Gynecological cancers (60%), breast cancer (15%), and lung cancer (10%) were the main diagnoses. Prior experience of chemotherapy was documented in 338 patients (54%). In 260 patients (41%), cisplatin was part of the cytostatic regimen. Carboplatin (23%), doxorubicin (27%), and epidoxorubicin (24%) were also frequently included. In all, 23 cytostatic agents were used in various combinations. The mean number of courses studied was 4.6 (range 1-19). Altogether, 394 of 619 evaluable patients (64%) were completely protected from acute nausea and vomiting during the first course of chemotherapy. Delayed nausea and vomiting were completely prevented in 45%-73% (days 2-6) in the complete series. Treatment efficacy remained stable (60%-79%) during ten consecutive courses of chemotherapy. With noncisplatin regimens, complete protection from acute nausea and vomiting was achieved in 72% compared with 52% for cisplatin regimens (P < 0.0001). Patients without prior experience of chemotherapy had higher control rates of acute nausea and vomiting (72%) compared to patients treated before (57%) during the first course, but not later on. Descending neurons with dopamine-like or with substance P/FMRFamide-like immunoreactivity target the somata of olfactory interneurons in the brain of the spiny lobster, Panulirus argus.

Two sets of descending neurons primarily target the somata of neurons in the olfactory deutocerebrum of the spiny lobster, Panulirus argus. Hundreds to thousands of dopamine-like immunoreactive fibers originate in the lateral protocerebrum and terminate among the clustered somata of the olfactory deutocerebrum projection neurons (lateral soma cluster) and those of the olfactory deutocerebrum local interneurons (medial soma cluster). A pair of giant neurons with substance P- and FMRFamide-like immunoreactivity from the median protocerebrum terminate primarily in the lateral soma cluster, but also branch in the core of the olfactory lobe itself. Neurons of both types terminate in numerous bouton-like swellings. The terminals in the lateral cluster at least contain numerous, large, dense-core and small, clear vesicles. The terminals contact the somata and the primary neurites through both traditional chemical synapses and large zones of direct membrane appositions. In most instances, a vesicle-containing profile forms a triadic arrangement with a neurite and a soma the latter being frequently connected via large gap-junction-like structures. Rosette-like arrangements formed by a vesicle-containing profile surrounded by up to eight neurites are also common. Dissociated lateral cluster somata support both fast inward and sustained outward voltage-activated currents. Substance P, but not dopamine or FMRFamide-related peptides, alters the fast inward current. The somata of the olfactory projection neurons, and possibly those of the olfactory local interneurons, appear to serve an integrative, and not merely a supportive role in these invertebrate central neurons.

Efficacy and tolerability of tropisetron in comparison with a combination of tropisetron and dexamethasone in the control of nausea and vomiting induced by cisplatin-containing chemotherapy.

In a double-blind, randomised, multicentre study, the efficacy and tolerability of tropisetron and a combination of tropisetron and dexamethasone were compared for the control of nausea and vomiting induced by cisplatin in patients previously not entirely protected by tropisetron monotherapy. In all, 160 women with gynaecological cancers were studied during two consecutive courses of cisplatin-containing chemotherapy. During the first course (the screening course), all patients received tropisetron monotherapy [5 mg intravenous (i.v.) on day 1 and 5 mg orally on days 2-6] as antiemetic treatment. During the second course (the test course), tropisetron was compared with a combination of tropisetron and dexamethasone (20 mg i.v. on day 1 and 4.5 mg twice daily on days 2-6). This part of the study was double-blind, randomised and placebo-controlled. Candidates for randomisation were patients with partial control of nausea (< 12 h of nausea) or partial control of vomiting (1-4 episodes of vomiting) during the screening course. Patients with complete control of nausea and vomiting in the screening course continued with tropisetron monotherapy; patients with treatment failure received open rescue treatment in course 2. Total control of acute nausea was achieved in 37% of the tropisetron + placebo group and in 75% of the tropisetron + dexamethasone group (P = 0.001). Significantly more patients on tropisetron-dexamethasone than on tropisetron-placebo were also free of delayed nausea. Acute vomiting was prevented in 40% of the patients in the placebo group and in 75% in the dexamethasone group (P = 0.001). Delayed vomiting was also significantly less frequent in dexamethasone-treated patients than in placebo-treated patients. Tropisetron was well tolerated both as monotherapy and in combination with dexamethasone. The most frequent adverse events were headache (34%), constipation (12.5%) and fatigue (12.5%). Adding high doses of a corticosteroid did not induce further adverse events or disregulate concurrent diseases.

Urethral lengthening with anterior bladder wall flap for urinary incontinence: a new approach.

Urethral irregularity (impeding catheterization) and failure to achieve continence are common complications in surgery for urinary incontinence. We describe a surgical technique using an anterior bladder wall flap that is sutured to the posterior wall in an onlay fashion creating a flap valve mechanism. Experimental work in dogs demonstrated a significant increase in the leak point pressure in the surgical group when compared to controls (p = 0.019). Voiding cystourethrography and bladder inspection demonstrated an anterior flap valve with no fistula formation in all animals. Histological examination showed a viable anterior bladder wall flap in all cases. This technique was then applied to 6 patients with neurogenic bladder and low urethral resistance that failed to resolve with medical treatment. Urinary continence was achieved in 4 patients. In 1 patient a vesicourethral fistula developed 3 months postoperatively, since the mother failed to catheterize for 12 hours. This technique is a useful alternative in the treatment of urinary incontinence.

DNA ploidy in carcinoma of the gallbladder. Prognostic significance and comparison of flow and image cytometry on archival tumor material.

The aim of this investigation was to determine whether the cytometrically assessed DNA ploidy is a parameter of prognostic significance in gallbladder carcinomas. For this purpose the DNA content of tumor cells from archival tumor material from 80 patients with cholecystic cancer was analysed retrospectively by using comparatively different cytometrical methods, namely flow cytometry (FCM) and image cytometry, using tissue sections (ICM-S) and nuclear suspensions (ICM-N). Conventional tumor classifications (according to pTNM and Nevin) were able to predict the further clinical course. DNA ploidy showed a statistically significant correlation only to histological grade (P < 0.001), but not to tumor stage and survival. Only a trend towards a poorer outcome for patients harboring non-diploid tumors became evident especially when no residual tumor was present postoperatively (R 0-stage; P = 0.08). The different cytometrical methods discussed in detail, showed a highly significant concordance in the determination of DNA index values (DI) and in the classification of the tumors as either diploid or non-diploid. In conclusion, our results indicate that cytometrical determination of DNA ploidy provides no further prognostic informations in gallbladder carcinomas as compared to conventional tumor staging.

[Orbital metastasis of kidney carcinoma].

PURPOSE: The case report of a patient with renal cell carcinoma and orbital metastasis is presented and the clinical and histological characteristics are discussed. PATIENT: A 69-year-old man, status post nephrectomy secondary to a renal cell carcinoma, presented with a swelling of the lower lid, reduced visual acuity and visual field defects in the left eye. Ultrasonography and CAT scan revealed a well demarcated orbital tumor. THERAPY: The tumor was excized in toto and the diagnosis orbital metastasis of a renal cell carcinoma verified histologically. The patient showed good postoperative recovery with greatly reduced clinical symptoms. Two years later a recurrence of the orbital tumor was noticed, which was also removed in toto. CONCLUSION: The in toto excision of an orbital metastasis of a renal cell carcinoma is a valid therapeutic approach regarding the tumor morphology and the achievable postoperative symptomatic relief.

Human dopamine D4 receptor gene: frequent occurrence of a null allele and observation of homozygosity.

We report a null mutation in the first exon of the human dopamine D4 receptor (DRD4) gene. The mutation is predicted to result in a truncated non-functional protein and is the first natural nonsense mutation found in a human dopamine receptor gene. It occurs with a frequency of about 2% in the general population. The distribution of the mutation was found to be similar in healthy controls and patients suffering from psychiatric diseases which included schizophrenia, bipolar affective disorder and Tourette's syndrome, indicating that heterozygosity for this mutation in the DRD4 gene is not causally related to major psychiatric diseases. We also identified an adult male who is homozygous for this mutation. He shows no symptoms of major psychiatric illness, but he displays somatic ailments including acousticous neurinoma, obesity and some disturbances of the autonomic nervous system. Some of these symptoms might be related to the absence of functional DRD4 protein.

Biological assessment of aquatic pollution: a review, with emphasis on plants as biomonitors.

In a number of disciplines including ecology, ecotoxicology, water quality management, water resource management, fishery biology etc., there is significant interest in the testing of new materials, environmental samples (of water or sediments) and specific sites, in terms of their effects on biota. In the first instance, we consider various sources of aquatic pollution, sources typically associated with developed areas of the world. Historically, much water quality assessment has been performed by researchers with a background in chemistry or engineering, thus chemical analysis was a dominant form of assessment. However, chemical analyses, particularly of such materials as organochlorines and polyaromatic hydrocarbons can be expensive, and local environmental factors may cause the actual exposure of an organism to be little correlated with chemical concentrations in the surrounding water or sediments. To a large extent toxicity testing has proceeded independently of environmental quality assessment in situ, and the work has been done by different, and differently-trained researchers. Here we attempt to bring together the various forms of biological assessment of aquatic pollution, because in our opinion it is worth developing a coherent framework for the application of this powerful tool. Biotic assessment in its most primitive form involves the simple tracking of mortality in exposed organisms. However, in most natural environments it is extended, chronic exposure to contaminants that has the most wide-ranging and irreversible repercussions--thus measures of sub-lethal impairment are favoured. From an ecological standpoint, it is most valuable to assess ecological effects by direct study of in situ contaminant body burdens and impairment of growth and reproduction compared with 'clean' sites. A distinction is made here between bioindication and biomonitoring, and a case is made for including aquatic macrophytes (angiosperms) in studies of contaminant levels and effects in the biota. It is apparent that there is a concurrent need for laboratory-based testing of new industrial by-products before any are released in the environment, and such studies should aid the investigation of mechanisms and modes of toxicity, but environmental assessment, and tracking of improvements in environmental quality are most effectively achieved by active biomonitoring experiments.

Immunoaffinity purification of baculovirus-expressed rubella virus E1 for diagnostic purposes.

Three monoclonal antibodies, termed 4E10, 1E11:10, and 2D9:1, were generated against rubella virus. Immunoblot analysis with purified authentic rubella virus or recombinant baculovirus-expressed rubella virus structural proteins E1, E2, and C demonstrated that they were directed against the E1 envelope glycoprotein of the rubella virus particle. By using the yeast Ty virus-like particle system, it was possible to map the binding site of 1E11:10 within amino acids 236 to 286 of the E1 protein and the binding sites of 2D9:1 and 4E10 outside this region. Immunoaffinity purification with these monoclonal antibodies made it evident that they are useful for obtaining large quantities of pure baculovirus-expressed rubella virus envelope protein E1. The diagnostic potential of this immunoaffinity-purified recombinant rubella virus E1 protein compared with that of authentic rubella virus is demonstrated.

Effects of calcitriol on expression of triiodothyronine-stimulated renal tubular transport of p-aminohippurate in rats of different ages.

Following treatment of rats of various age groups with calcitriol (2 micrograms/kg b.w., 3 days, once daily) renal tubular transport of p-aminohippurate (PAH) is reduced in kidney cortical slices prepared from 5-, 15-, 33-, and 55-day-old rats. The decrease in renal excretion of PAH following such treatment has a longer degree and is demonstrable in 15- and 33-day-old rats only. Repeated treatment with calcitriol provokes a decrease in body weight, statistically significant in 5- and 55-day-old rats. In 33- and 55-day-old rats repeated treatment with calcitriol (2 micrograms/kg b.w., 3 days, once daily) causes an increase of kidney weight. Repeated treatment with calcitriol is connected with an increase in plasma level and in renal excretion of calcium, statistically significant in most age groups. In rats of various age groups, renal tubular transport of PAH can be stimulated by repeated administration of triiodothyronine (T3; 100 micrograms/kg b.w., 3 days, once daily). This increase in PAH transport, measured in kidney cortical slices, cannot be observed in rats simultaneously treated with calcitriol and T3. Obviously, the expression of higher transport rates for PAH can be prevented by renal effects of calcitriol. Simultaneous treatment with T3 and calcitriol causes an increase in kidney weight and a decrease in body weight in rats of various age groups.

How much do 'attention' tests tell us?

Twelve measures commonly used to assess attentional processes were examined in a sample of 120 outpatients referred for neuropsychological evaluation. A single factor solution emerged. A second analysis partially replicated factors derived by Shum. McFarland, Bain, and Humphreys (1990). Classification rates (impaired vs. unimpaired) for the measures are presented. The implications of these findings for clinical assessment of attention are discussed

[Comparative study of the extension of coronary arteriosclerosis with risk factors and changes in the retinal artery].

PURPOSE: To identify a possible relationship of the extent of coronary artery disease, with risk factors for coronary artery disease and the retinal arteriolar changes (evaluated by fundoscopic examination). METHODS: We studied 96 patients (66 males), 36 to 72 years of age. All patients were studied by selective cinecoronaryarteriography, fundoscopic examination, as well as evaluated for the common risk factors, cigarette smoking diabetes, dislipidemia, systemic hypertension, age, alcoholism, male sex, family history and obesity. RESULTS: Male (p < 0.001) and smoking (p < 0.01) were the statistically significant associations with the extent of coronary artery disease. No significant relationship with the extent of coronary artery disease was present for the other risk factor nor for changes in retinal arteriolar branches. CONCLUSION: A relationship between extent of coronary artery disease with male sex and a cigarette smoker is occurred. No relationship with other risk factors, nor with retinal arteriolar changes were present.

Costochondritis presenting as a peculiar skin lesion.

Costochondritis is an important complication of median sternotomy that can present months to years after surgery. Delay in diagnosis may result in more disability and increased medical costs. The varied clinical manifestations of costochondritis are reviewed and a case is presented.
