Colonic cleansing, fluid absorption, and discomfort following tap water and soapsuds enemas.

Tap water and soapsuds are common enema solutions, but little is known about their effectiveness. Consequently, nurses have no guidance when choosing the best enema for a given patient. In this study, liver transplant patients were given either a tap water or soapsuds enema preoperatively. Amount of enema instilled into the colon and output were measured. Soapsuds enemas produced significantly greater output than tap water and were equally well tolerated. Most subjects who received tap water enemas retained more fluid than was eliminated. Based on these findings, nurses should use caution when giving repeated enemas to patients sensitive to large fluid loads.

Turn your research wishes into action--and make it fun.

Recognizing that beginning researchers have difficulty starting a study and successfully completing it, the authors describe 10 guidelines for conducting a research study and making it fun. They describe strategies such as working with a partner, selecting an exciting topic to study, and using time effectively. They discuss ways to seek support from colleagues and friends and also provide information about writing grant proposals. The authors provide personal examples from their research on enema techniques to illustrate each guideline.

[Prolongation of a primary twin pregnancy to the 31st week of pregnancy by surgical extraction of the 1st twin in the 17th week of pregnancy for umbilical cord prolapse].

A 24-year-old woman with a twin pregnancy had a premature rupture of membranes (PROM) of the first foetus in the 16th gestational week (gw). After 9 days the umbilical cord prolapsed. In the 17th gw the foetus was extracted by vaginal hysterotomy, because of an inferior anterior wall placenta. The patient remained hospitalised receiving tocolysis, lung maturation induction and prophylactic antibiosis. The pregnancy could be prolonged for 109 days after PROM and 99 days after the extraction of the first twin. The second child was born in the 31st gw by Caesarean section, following uncontrollable labour.

A framework for planning public health nursing services to families.

We designed a model to assist public health nurses and nursing supervisory staff in planning, delivering, and evaluating their services to families. Specifically, the intent was to assist nurses to estimate the numbers and types of services required. The model includes a gradient of family health characteristics, an original delineation of nine basic public health nursing services, a coping index and scoring guidelines, and a methodology for integrating assessment of health needs and coping abilities.

The role of the tubulointerstitial changes in the prognosis of IgA glomerulonephritis.

The authors investigated the tubulointerstitial changes (atrophic tubuli, interstitial fibrosis and round-cell infiltration) by two methods in renal biopsies from 56 patients with IgA glomerulonephritis. A statistically significant correlation was found to exist between the tubulointerstitial changes and serum creatinine level at the time of renal biopsy. A statistical significant correlation was also established between serum creatinine at the end of a period of 5 years of follow-up and tubulointerstitial changes. Their results suggest that severe tubulointerstitial lesions in IgA glomerulonephritis carry a poor prognosis.

[Complication during the dipyridamole-test (author's transl)].

Performing the dipyridamole-test for evaluating coronary artery disease a man at the age of 57 years developed after application of 15 mg dipyridamole intravenously angina pectoris. For removal of angina pectoris 240 mg aminophylline were injected, after what bradycardia and finally cardiac arrest occurred. Resuscitation was successful. Possible reasons and some precautions were discussed.

Transverse diameter of chest and heart after birth asphyxia in the newborn infant.

The transverse diameter of the chest and heart was measured in 745 newborn infants after birth asphyxia. Both diameters showed a good correlation with birth weight. The great individual variations of the measurements were due to different degrees of birth asphyxia observed in the infants and to their different responses to compensate asphyxia. A wet lung and an increased transverse diameter of the chest (above the 90th percentile) indicate a good, and a decreased transverse diameter of the chest (bellow the 10th percentile) indicate a bad prognosis for survival. The cardiomegaly usually disappears after the first 12 hours of life, but in some cases it persists for more than 5 days, depending on the severity of birth asphyxia.

Transverse diameter of the chest and of the heart of infants in the course of physiological cardiorespiratory adaptation.

The morphology of the lungs and heart was analysed in 824 newborn infants with normal cardiorespiratory adaptation. Under normal conditions, the air content of the lungs became satisfactory in the first 6--12 hours and normal in all cases after 24 hours. The values for the transverse diameter of the chest and heart were brought into correlation with birth weight, body length and gestational age. The transverse diameter of the chest and heart did not change during the early postnatal period (between 6--12 hours and 5 days). Both diameters were correlated with birth weight. The closest correlation was found in newborns under 1500 g, a varying one in those between 1500 and 2000 g and a close correlation in the category over 2000 g. The correlation of the two diameters with both length was linear and close, without any difference between the values obtained at different times of examination. A rather loose correlation was found between the transverse diameter of the chest and heart, and gestational age.

Iron turnover in chronic hepatitis.

Radioisotope studies of iron kinetics carried out in patients with chronic hepatitis yielded the following results. Serum iron level and free iron binding capacity showed little difference from the normal mean value. All three types studied (chronic persistent hepatitis, chronic active hepatitis, chronic active hepatitis with cirrhosis) revealed an abnormal distribution of iron in the first 24 hours. Normalization of iron distribution ensued in persistent hepatitis and in chronic active hepatitis with cirrhosis, but in chronic active hepatitis the abnormal distribution persisted, as reflected by a decreased iron utilization and an increased iron storage in the liver. The cause of this is attributed to a transitory accumulation of ferritin in the liver.
