[Importance of the number of treatment sessions in the success of root canal therapy].

Root canal therapy is usually performed in single or multiple-visit treatment. The choice of single or multiple visit depend on dentist and patient available time. In general dentistry, multiple-visit is widely chosen by dentist whereas endodontic speciality recommend single-visit. In this study we analyzed 120 root canal therapy performed by students, to determine the number of visits needed to achieve treatment from endodontic-cavity-access to root canal filling and its possible, influence in the successful rate. Results showed that globally, 60% of treatments were performed in multiple-visit. Results showed also hat 89.3% of anteriors teeth were performed in single-visit but there is a higher rate of fail: 31.78%. Nevertheless, these results do not condemn endodontic single visit therapy.

[Incidence of postoperative pain in single session root canal therapy (study in black senegalese apropos of 96 cases)].

This prospective study was conducted during a period of 30 days in 96 patients to determine the incidence of postobturation pain after single-visit root canal treatment in black Senegalese people. The canals of all teeth were hand-prepared and filled with zinc-oxide-eugenol paste by a Lentulo spiral paste filler. Patients were asked to categorize their pain according to the following criteria: No pain, Slight pain, Moderate pain and Severe pain. The data were analysed statically with chi-square test to determine the relationship, if any, between the pain experienced and sex, age, tooth type, apical level of root filling and number of treatment performed in the single-visit. Results show that eighteen patients experienced pain (18.75%); our results are ranging between 10% and 25% described in the literature. No significant correlation was found between postobturation pain and any other factor with the exception of the apical length of canal filling. Characteristics of postobturation pain after root canal treatment performed in single-visit in Senegalese people presents the same features as described in international papers, that is, pain appears early and analgesic therapy is effective.

[Effect of the quality of endodontic treatment on the longevity of fixed prostheses. Apropos of 64 pulpectomies].

64 teeth with well-fitting post-retained crown were examined radio graphically and clinically and a number of failures were assessed including the peri-apical condition and the quality of root filling. All the teeth were vital and prosthetic was the only reason of endodontic treatment. Results have shown that 25% of the teeth had areas of radiolucency around the root apices. Various criteria parameters were reviewed for possible correlation to the failures. 15 out of the 16 cases of failures could be related to the endodontic therapy procedures: lack of using a rubber dam during the treatment and an unsatisfactory condensed and sealed root filling. Only one case was related to the deviation of post from the line of canal suggest a lateral perforation. To perform a post-retained crown the quality of endodontics must be considered: the operative procedures should be more controlled and enough time should be observed after root canal therapy to ensure its successful.

[Effect and mechanism of action of Tinospora bakis roots on biliary secretion using in vivo models].

The aim of this study is to determine the choleretic properties of lyophilized aqueous extract of Tinospora bakis roots. For thus, we have used healthy and cholestatic Wistar rats. After administration of the extract (4 mg/100 g of weight) biliary flow, biliary acids flow, biliary glutathione and glucose concentrations were measured by specific methods. Comparative results between treated rats and non treated rats show: For healthy rats: a significant increase of biliary flow, biliary acids flow in the group of treated rats versus non treated healthy rats. There were no significant difference in the glutathione and glucose concentrations. For cholestatic rats: we noted also significant increase of biliary flow, biliary acids flow and glutathione concentration in the treated rats group. These results suggest that Tinospora bakis has a choleretic effect in vivo which depends on biliary acids and/or glutathione concentration.

[Staphylococcus aureus purulent pleurisy in children. Experience of the Albert Royer Hospital for Children of the Fann University Hospital Center in Dakar].

The authors report here by a retrospective study 58 cases of Staphylococcus aureus empyema at Albert ROYER child hospital located in the Fann University Teaching Hospital of Fann between January 1st 1992 and December 31, 1995. In this study staphylococcus aureus is the bacterium involved in pleural effusions of the children (54%) a long way ahead Streptococcus pneumoniae (16%). Infant less than 30 month is more affected (86%). The average age of the patients is 16.8 month +/- 16.6. The resistance of the germ to usual antibiotics, the precariousness of the research field and mechanical complications linked to the outpouring explain their seriousness. The treatment lies upon an adapted antibiotic and bactericidal therapy associated to closed chest tube drainage.

[Long term evaluation of results of endodontic treatments of dental pulp necrosis (74 cases obturated by the monoconal technique].

Various factors may affect the outcome of root canal therapy. In this study 74 endodontocally treated pulpless teeth were evaluated: (43 with periapical radiolucency and 31 without periapical lesions). These 74 non surgical treatment were performed in 60 patients (26 men and 34 women) by undergraduated students. The overall success rate was 71.62%; the rate of success for cases with no periapical radiolucency exceeded 80% whereas only 62.8% of the cases with perapical radiolucency had undergone partial resolution or complete resolution. Only 17.1% of lesions demonstrated complete radiographic resolution 12 to 16 months after the endodontic treatment. Reasons that may explain the low success rate are lake of asepty, root canal preparation and root canal filling technique.

[Cardiac thyrotoxicosis and left ventricular thrombosis, a case report].

Authors report about a left ventricular thrombus as a complication of thyrotoxic heart disease, on a 45 years old woman. Clinical state featured signs of thyrotoxicosis, global cardiac failure and hypertension. The electrocardiogram showed a left axis deviation, the chest X-ray a cardiac enlargement (cardio-thoracic index = 0.55) on behalf of left movement of heart walls and severe left ventricule dysfunction (Ejection fraction about 18%). This exam also found a big thrombus at he left ventricule apex. The treatment disappearance of the thrombosis and improvement of cardiac failure signs and echographic parameters. Authors discuss circumstances leading to thrombosis in thyrotoxic heart disease, and the usefulness of anticoagulant drug therapy.

[Left ventricular aneurysm in human immunodeficiency virus infection: a case report].

The authors report the case of a 30 year old man with a left ventricular aneurysm who was seropositive to HIV 1 and HIV 2. The patient was stage IVC 1 (AIDS related complex) by the "Center for Disease Control" classification. The clinical presentation was pyrexia, loss of weight, micropolyadenopathy and cardiac failure. The electrocardiogramme showed low voltage in the peripheral leads with a QS morphology in S2, S3 and aVF and abrasion, of the R wave in the precordial leads. Doppler echocardiography demonstrated a large left ventricular aneurysm with a wide neck. Despite treatment with a diuretic, angiotensin converting enzyme inhibitor and anticoagulants, the patient died suddenly. Autopsy confirmed the wide necked left ventricular aneurysm. This would appear to be the first report of this form of cardiac disease during HIV infection. However, a simple coincidence of the two pathologies cannot be excluded.

[Echocardiographic parameters in hyperthyroidism with and without cardiothyreosis].

There has been little research done on the differences between hyperthyroidie patients who exhibit cardiothyrosis and those who do not. The objective of this research was to elucidate the variations in echocardiographic parameters that exist between these two groups, in order to determine practical implications. A prospective study on 37 subjects was performed: 20 in group I (without cardiothyreosis) and 17 in group II (exhibiting cardiothyreosis). In both groups, women were predominant. Left ventricular diameters and volumes were statistically higher in group II (p < 0.0007). The left ventricular end systölic stress was also higher (140.10(3) +/- 37 vs 131.10(3) +/- 51 dynes/cm2. p < 0.05). There was no significance between the two groups in shortening fraction, ejection fraction and mean rate of circumferential fiber shortening. The E/A ratio of the mitral flow was higher in group II (1.98 +/- 1.3 vs 1.3 +/- 0.7, p < 0.05), but the isovolumetric relaxation time and the deceleration time of the E wave were similar in both groups. Left ventricular systolic dysfunction was observed in 5 patients of each group. Relaxation abnormalities were found in 10% of the subjects in group 1 and 33.3% in group II. Significant valvular regurgitation was observed only in group II (8 cases of mitral, 1 case of tricuspid and I case of aortic regurgitation). Given that cardiovascular perturbations may be different during the stages of the hyperthyroidism, different therapeutic approaches may thus be considered, facilated by appropriate echocardiographic examination.

[Arterial pressure and body mass index of children and adolescents in a rural area of Thiadiaye, Senegal].

The purpose of this study is to assess the profile of blood pressure and body mass index of children and adolescents in a rural area in Africa. It is a prospective study concerning a sample of 465 children and adolescents in the district of Thiadiaye (Senegal). The ration boys/girls was 1.02. Blood pressure was higher in girls. The prevalence of hypertension was 5.1% for the children and 1.5% for adolescents. There was no significative variation in body mass index for children whereas we were noted an augmentation with age in adolescence. The parameters of corpulence are higher in girls in adolescence. The prevalence of obesity was 3% in children and 2.4% for adolescents. There was no correlation between blood pressure and body mass index. This study show many particularities in rural area as for as blood pressure and body mas index of children and adolescent are concerned.

[Survey of smoking in the rural area of Thiadiaye, Senegal].

The objective of this article is to study the place of tobacco addiction in a rural area in Senegal. We have performed a transversal and domicillary study on a sample of adults and adolescents of the district of Thiadiaye (Senegal). The mean age of the studied population is 39 +/- 8.5 years (extremes: 12 and 100 years). The sex ratio is 0.85%. The prevalence of tobacco addict was 24.07%, 9% of the teenagers and 32% of the adults were smokers. Tobacco use was more frequent among men (89% 5%) than women (10.5%) (p < 10(-7)). The most common form was traditional tobacco: 62.1%. Cigarette was smoked in 14.4% of cases. The mean duration of tobacco use was 13 +/- -12.2 years. The mean consumption per day of traditional tobacco was 5.1 +/- 4, whereas the mean number of cigarettes per day is 12.3 +/- -7. Our study shows that tobacco occupies an important place in rural area in Senegal where coexist with traditional habits, a modern tobacco use represented by cigarette smoking.

Trends in breast cancer screening in Missouri from 1987 to 1995, and predictions for the years 2000 and 2010.

BACKGROUND: Breast cancer is a major cause of morbidity and mortality in the United States (U.S.) and Missouri. In 1992, 3,915 new breast cancer cases were diagnosed and in 1995, 1,006 deaths from breast cancer were reported in Missouri. Although breast cancer incidence has increased in Missouri in the past 20 years, there are indications that early detection has also increased during the same period. Knowledge about which segments of the population have experienced the greatest increase in mammography screening rates helps in planning and implementation of breast cancer control programs at the state level. OBJECTIVES: Examine the prevalence and trends of lifetime mammography and 2-year mammography compliance in Missouri by age, race, and education from 1987 to 1995 and make predictions for the years 2000 and 2010. METHODS: We used data from the Missouri Behavioral Risk Factor Surveillance System (BRFSS), 1987 to 1995, to estimate the prevalence of ever having had a mammogram and compliance with mammography screening guidelines within two years by race, age, and education status among Missouri women over age 18. Using linear models, we regressed breast cancer screening prevalence estimates on time to obtain trends and predictions. RESULTS: Overall, African-American women were more likely to have had a lifetime mammogram than white women. However, we found a steady increase in the prevalence of ever having had a mammogram for all groups of women defined by age and education status, except among African Americans. Increase in the prevalence of ever having had a mammogram was much higher in women age 50 and older and slightly higher among women with a high school education or less. The average prevalence of 2-year mammography screening compliance was about 60% for all groups, a rate which did not significantly change between 1987 and 1995. By the year 2000, white women will have mammography rates equal to or higher than African-American women, and the majority of all women age 50 and older (98.3% to 100%) will have had a lifetime mammogram. CONCLUSION: Missouri target populations are predicted to attain Year 2000 National Health Objectives concerning lifetime mammography. Current efforts should be continued in order to maintain levels of mammography, particularly among African-American women.

[Cardiovascular manifestations in systemic scleroderma].

OBJECTIVES: Patients with systemic scleroderma often have latent heart disease which could play an important role in morbidity and mortality. We therefore conducted a prospective study of cardiovascular manifestations in patients with systemic scleroderma. PATIENTS AND METHODS: A prospective cross-sectional study included 29 patients with systemic scleroderma who underwent a complete cardiovascular work-up including physical examination, electrocardiogram, chest x-ray and Doppler-echocardiogram from July 1993 to February 1996. RESULTS: Hypertension was observed in 6 patients (20.7%) and was positively correlated with age (p = 0.007). Raynaud syndrome was also found in 6 patients (20.6%). Heart disease was observed in 14 patients (48.3%) and was positively correlated with age and lack of treatment for scleroderma (p = 0.008). Myocardial disease was the most frequent (11 patients, 37.9%), followed by pericardial disease and valve disease (4 cases each, 13.8%). Rhythm and conduction disorders were found in 2 (6.9%) and 8 (27.6%) of the patients. CONCLUSION: Cardiovascular manifestations are frequent but often latent in patients with systemic scleroderma. This finding emphasizes the importance of routine cardiovascular work-up in all patients with scleroderma.

[Echocardiographic parameters in normal pregnant women].

The authors report the results of a transverse prospective study of 80 normal women, divided into 4 groups of 20: control group, 1st trimester of pregnancy, 2nd trimester of pregnancy, 3rd trimester of pregnancy. These groups were comparable in terms of age, height and haemoglobin level. No significant difference was observed in terms of obstetric profile between the various groups of women. The results did not reveal any difference concerning the morphology of cardiac structures, the parameters of left ventricular diastolic filling and the frequency of valvular regurgitation. On the other hand, diameters and cavity volumes, left ventricular mass, left ventricular ejection fraction, and maximal antegrade aortic and pulmonary blood flow rates were higher during pregnancy; these increases were less marked when the measurements were performed in the dorsal supine position compared to those observed in the left lateral supine position.
