[An evaluation of the epidemiological surveillance system for infectious diseases in the Barcelona Olympic Games of 1992].

Due to the 1992 Barcelona Olympic Games some modifications were introduced in the epidemiologic surveillance system for infectious diseases in place in the city, in order to expand its coverage and shorten its timeliness in detecting outbreaks and investigating cases. These modifications were introduced for a group conditions (hepatitis, meningococcal disease, legionnaire's disease and food outbreaks), selected on the basis of incidence, time of the year, previous experience in other settings, and likelihood of outbreak occurrence. In the June-August 1992 period (Olympic period), no increases in the incidence of selected conditions were observed when compared to the same period in 1986 to 1991. Major changes were observed in the source of food outbreak reporting, with a large increase in outbreaks reported by emergency room departments. There was an increase in the number of domestic foodborne outbreaks and a reduction in those related with restaurants. Timeliness in the detection of cases was shortened. The use of similar modifications can be useful for epidemiologic surveillance systems in other comparable settings or occasions.

Influence of apolipoprotein-E phenotypes on postprandial lipoprotein metabolism after three different fat loads.

The aim of this study was to evaluate the postprandial response to three fat-loading tests in healthy subjects with different apolipoprotein E (apoE) phenotypes. Thirty-four subjects were studied: 15 with apoE3/3 (7 men and 8 women), 12 with apoE4/3 (5 men and 7 women), and 7 with apoE2/3 (4 men and 3 women). All received three oral fat loads at 1-wk intervals in meals rich in monounsaturated fatty acid, polyunsaturated fatty acid, and saturated fatty acid, with retinyl palmitate (60000 IU/m(2) of aqueous vitamin A) to quantify lipoproteins secreted by the intestine. No significant differences in postprandial lipoproteins were found between the three different fat loads. Peaks and incremental areas under the curve of retinyl palmitate in non-chylomicron fractions were higher in the apoE2/3- than in the apoE3/3- and apoE4/3-phenotype groups in meals rich in monounsaturated and polyunsaturated fatty acids (P < 0.05). When the three fat loads were analyzed together, the incremental area under the curve of retinyl palmitate was much higher in the apoE2/3- than in the other apoE-phenotype groups (P = 0.0004). In conclusion, the magnitude of intestinal lipoproteins after fat load, especially with monosaturated and polyunsaturated fatty acids, is higher in subjects with apoE2/3 than in those with apoE3/3 and apoE4/3 phenotypes.

Histo-pathologic characteristics of peri-implant soft tissues in Brånemark implants with 2 distinct clinical and radiological patterns.

This investigation is the result of a multicenter research project between the Catholic University of Leuven and the University of Madrid with the purpose of studying histometrically and ultrastructurally periimplant marginal tissue reactions in mendium-term Brånemark implants. 12 patients were selected and distributed according to predefined clinical and radiological parameters into a periimplant infection group (A) and a non-periimplant infection group (B). Biopsies were taken from these patients and analyzed by means of microscopy (LM), histometry (HM) and transmission electron microscopy (TEM). With LM group A showed pathological features consistent with an advanced gingivitis lesion, while group B showed signs of healthy gingiva in the connective tissue and vauolae formation in the epithelium. With HM, group A showed significantly higher transmigration of inflammatory cells in the epithelium than group B. In the connective tissue, group A showed a higher % of inflammatory infiltrate with significantly higher numbers of plasma cells and mononuclear cells than group B. With TEM, group A showed an ultrastructural picture consistent with plaque-induced periodontitis, while group B showed a rather healthy connective tissue and vacuolae formation in the epithelium. These results suggest that supracrestal marginal tissues around osseointegrated implants react to plaque bacteria similarly to periodontal tissues around teeth by chronic inflammation.

[Isolation and characterization of Shiga-toxin-producing Escherichia coli from frozen hamburgers and soft cheeses].

Shiga toxin producing-Escherichia coli (STEC), an important emerging foodborne pathogen, has been associated with bloody and non-bloody diarrhea, hemorrhagic colitis, hemolytic uremic syndrome (HUS) and thrombotic thrombocytopenic purpura. The cattle have been shown to be a major reservoir of STEC and raw foods such as ground beef and milk are the most common vehicles of infection. In the present study, the prevalence of STEC in 95 samples of frozen hamburgers and in 114 samples of soft cheese was established in 8.4% and 0.9%, respectively. The genotypic and phenotypic characteristics of the strains were determined. The virulence genes stx1, stx2, eaeA and EHEC-hlyA were identified by PCR and by colony blot hybridization assays. Serotyping, antimicrobial susceptibility and production of Stx using specific cytotoxicity assays on Vero cells were also determined. All STEC strains were characterized as eaeA-/EHEC-hlyA+. The stx2 genotype was prevalent (77.8%), and four different O:H serotypes were found, comprising: O8:H19 (5 strains), O113:H21 (1), O8:H16 (1), and O39:H49 (1). One STEC strain was nontypable. Although soft cheese complimented the microbiological quality controls for the coliform counts, the detection of STEC in one sample raises doubts concerning the effectiveness of the current quality controls. These data contribute to the implementation of strategies for the prevention and control of HUS.

[Crohn's disease].

A 48 years old male patient was admitted in the Risquez Hospital of Caracas, Venezuela presenting intestinal obstruction. An exploratory laparotomy was performed and two stenotic lesions in the small intestine were found. One of them in the proximal jejunum and the other in distal ileum. Both were resected and terminal jejunum-jejunum anastomosis and terminal jejunum-ileum anastomosis were performed. Pathological diagnosis was Crohn's Disease. Post operative evolution was satisfactory and four years after the patient is well.
