Postischemic liver damage in rats: amino acid analysis and morphometric studies.

Normothermic, temporary, total ischemia of the liver was produced for 60-225 min under transient portal decompression with a by-pass between the mesenteric and the femoral vein. Total amino acids in the liver tended to increase after an ischemic period of more than 120 min without reperfusion as compared with control with increasing trends in most of the individual amino acids. In a group undergoing 120 min of ischemia and 60 min of reperfusion, total amino acids and individual amino acids tended to decrease. Total plasma amino acids significantly increased after ischemia of more than 120 min. Without reperfusion, elevations of almost all amino acids except for branched chain amino acids were found, whereas after reperfusion most of the individual amino acids also increased including branched chain amino acids. Molar ratios of branched chain amino acids to tyrosine and phenylalanine decreased only after more than 120 min ischemia without reperfusion. Volume ratios of organelle disintegration on electron micrographs such as mitochondrial degradation and autophagic vacuoles were moderately increased after 90 min ischemia with a further steep rise after more than 120 min ischemia. The survival rates of the animals after 60, 90 and 120 min ischemia were 35% (6/17), 27% (3/11) and 25% (3/12), respectively. The following conclusions were obtained: 1) Pre-necrosis of the hepatocytes with simultaneous protein degradation started after ischemia for about 2 hr. The survival rates of the animals after 60 and 90 min ischemia were very low despite of mild necrosis of the liver. 2) Most of the amino acids in the liver were washed out into the plasma to cause an abnormal plasma amino acid pattern in the acute state. However, the molar ratio of branched chain amino acids to aromatic group was not reduced, in contrast to the ischemic group without reperfusion.

Postischemic liver damage in rats: effect of some therapeutic interventions on survival rate.

The aim of this paper is to elucidate the cause of death after 90 min of normothermic partial (2/3) ischemia of the liver and to examine the effects of glucagon, somatostatin, insulin, prednisolone and oral administration of polymyxin B (PB). The animals 24 hr after partial ischemia for 90 min were divided into two groups; namely, animals with normal appearance and those with moribund state. There were no significant differences in the plasma level of S-GOT, S-GPT, amino acids, NH3 or insulin, or in morphometrically estimated volume ratio of necrotic hepatocytes between the two groups of rats. The blood glucose level, however, was significantly decreased (31 +/- 28 mg/100 ml, n = 6) in the moribund rats with a higher incidence of positive Limulus gelation tests as compared with the rats with normal appearance (149 +/- 19, n = 5). The 1-day and 1-week survival rates of the animals were 42/62 (69%) and 32/61 (53%), respectively. A glucagon injection (1.5 mg/kg, after ischemia) was effective to elevate the 1-day survival rate (14/14), but failed to increase the 1-week survival rate (11/14). On the other hand, a somatostatin injection (100 micrograms/kg, after ischemia) or PB treatment (15 mg/kg/day x 5-9, before ischemia) succeeded to increase the 1-week survival rate (20/22 p less than 0.01 and 17/17 p less than 0.01, respectively), although no significant amelioration in transaminase levels or volume ratio of necrosis was demonstrated. It could be seen that a moribund state after partial ischemia was accompanied by severe hypoglycemic shock, and that the injection of somatostatin after ischemia or the annihilation of gram-negative bacteria by means of oral administration of polymyxin B before ischemia prevented the occurrence of the hypoglycemic shock.

Rotation-plasty for childhood osteosarcoma of the distal part of the femur.

A rotation-plasty procedure was used in four children with osteosarcoma of the distal part of the femur. The results showed that this procedure, combined with en bloc wide excision of the proximal part of the tibia and the distal part of the femur including the knee joint, appears to be an adequate surgical treatment for osteosarcoma of the distal part of the femur in children. None of our patients to date had either clinical or radiographic evidence of local recurrence during follow-up periods of between twenty-seven and fifty-eight months. One patient, however, died of widespread metastatic disease. Despite postoperative adjuvant chemotherapy with high doses of methotrexate, there have been no problems with wound-healing and early fitting of a prosthesis was possible. Because of the success in preventing local recurrence in these four patients, we now generally employ this method in the surgical management of osteosarcoma of the distal part of the femur in children.

Human serum inhibitors of collagenase as revealed by preparative isoelectric focusing.

Single step separation of pooled normal human serum by means of preparative isoelectric focusing in the range from pH 3.5--9.8 revealed more heterogeneous inhibition of collagenolytic activity than previously reported. Essentially three inhibition zones were resolved. According to their electrophoretic behaviour the respective serum fractions displaying inhibitory activity were designated alpha-, beta- and gamma-collagenase inhibitors. The main component responsible for collagenase inhibition in the alpha-zone was found to be alpha 2-macroglobulin. In the beta-zone inhibitory activity focused around pH 6.3. In the gamma-range a non-dialysable cationic component focusing at pH 9.2 was also able to decrease collagenolytic activity derived from rheumatoid arthritis synovial culture supernatant. These findings were supported by single step separation of serum on DEAE-anion exchange chromatography.

Treatment of osteosarcomata of the distal femur by rotation-plasty.

Rotation-plasty was carried out on fifteen patients--10 male and 5 female patients aged between 6 and 32--with an osteosarcoma of the distal femur following resection of the tumor. In the operation the lower leg was rotated through 180 degrees after resection of the distal femur and knee joint so that the ankle joint was effectively turned into a "knee joint". The indication for this operation was initially limited to children for whom the only possible therapeutic alternative was a high above-knee amputation. The surprisingly good functional rehabilitation justifies the indication of this method as an alternative to an endoprosthesis or arthrodesis even for young adults. In spite of the cosmetically very unusual position of the foot, none of the patients has so far experienced psychological problems thanks to suitable guidance. Both from an oncological and psychological point of view, this method represents an extremely responsible operation and should therefore only be carried out by hospital departments specializing in the therapy of bone tumors.

First clinical experience with heterologous cancellous bone grafting combined with the fibrin adhesive system (F.A.S.).

Heterologous bone grafts are mostly refused for bone transplantation because of the low osteogenic potency. The remodelling of the heterografts can be improved significantly by application of a fibrin-adhesive-system. Between 1977 and 1979 thirty-five bone defects caused by bone tumors resp. seven times by osteomyelitis were operated on and closed with cancellous Kiel-bone grafts (Braun, Melsungen) soaked thoroughly with fibrin-glue (Tissel/Tissucol TM, Immuno AG, Vienna). Postoperatively there were two infections, but none of them in our osteomyelitic cases. After an average follow-up period of 24.6 months (15-36), the highly satisfying, preliminary results were exactly analysed. Despite the relatively short postoperative time, in almost all cases the complete remodelling of the heterografts into new bone tissue could be observed. A formula to calculate the amount of fibrin-glue, necessary for the procedure, is given.

Experience with bioceramic implants in orthopaedic surgery.

In the period from 1974 to March 1980 bioceramic endoprostheses were implanted in 155 patients. 94 patients were suffering from degenerative hip diseases and 61 had been operated for malignant bone tumours. The majority of the total endoprostheses consisted of a ceramic socket and a metal shaft prosthesis with ceramic head attached to it. Bioceramic double-cup prostheses were implanted in 19 patients without using bone cement. Following resection of malignant bone tumours, the proximal humerus and the proximal femur were replaced by special bioceramic endoprostheses. The post-operation period after implantation has reached 70 months in the case of tumour endoprostheses (average 22 months) and up to 49 months in the case of hip joint endoprostheses (average 24 months). The analysis deals exclusively with complications specific to the ceramic material. These complications are: breakage of the ceramic head (8 X), loosening of the ceramic cup (13 X) and pseudarthrosis of the cone connection in tumour endoprostheses (9 X). Abrasion particles were found in all cases which we were able to follow up histologically.

A bioceramic endoprosthesis for the replacement of the proximal humerus.

In patients with malignant tumors in the region of the shoulder, radical resection can avoid amputation in most instances. To improve the function of the arm, endoprosthetic replacement of the defect is desirable. A three-component endoprosthesis made of a bioceramic material (aluminium oxide) was designed, implanted without bone cement. Fast anchorage to bone is achieved by using a conical sleeve, fixed upon the previously conically reamed humerus shaft. A stable primary fit is always feasible. Subsequent bone in growth into grooves inside the conical sleeve provides a permanent anchorage of the endoprosthesis. The authors experiences are based on implantations of 38 endoprostheses. The original diseases were primary malignant bone tumors in 19 patients, one case of "solitary" plasmocytoma and metastases into the proximal humerus in 16 patients. In two women, resection was made because of posttraumatic subcapital humeral pseudarthrosis. The follow-up study includes only those 27 cases operated on at least one year ago. 12 of the 14 patients with primary tumors have been surviving for 12-55 months (range 27.4) without signs of metastases or recurrent disease. Seven patients with metastases died of their original diseases after 7.7 months on the average. Owing to extensive resection of the shoulder musculature the mobility in the shoulder joint is considerably reduced. All the patients have good movement of the elbow joint and free function of the hand.

[The angiographic diagnosis and differential diagnosis of inflammatory disease of bones and soft tissues (a report concerning 40 cases) (author's transl)].

The angiographic appearances of inflammatory disease of bones and soft tissues was analysed in 40 patients. The angiographic findings in inflammation of bone (28 patients) differed greatly, from normal findings (14.2%), moderate angiographic changes corresponding to the severity of the inflammatory process (32.1%), marked changes in the arterial, capillary and venous beds (42.9%) and significant local staining with little peripheral reaction (10.7%). Soft tissue inflammation on the other hand showed increased vascularity in all cases. A typical angiogram was found in 50% of cases and shows the following characteristics: vascularity as compared with the surroundings is usually increased and may be extensive, far more so than is found surrounding vascular bone foci; the contrast staining is usually ill-defined and the veins in the late phase are increased in calibre. Early venous filling is common and should not be regarded as a sign of malignancy. In 17.5% of patients the angiogram simulated a tumour; in four patients (10%) a false positive diagnosis of tumour was made, in two (5%) a tumour could not be excluded and in one case a benign tumour was diagnosed because of localised staining. None of the angiograms which had been interpreted incorrectly showed typical pathological vessels, which are the most important tumour criteria.

[Endoprosthetic replacement following resections of malignant tumors of the extremities (author's transl)].

Due to advances in chemotherapy and endoprothetics, amputation is increasingly being replaced by local resectioning in malignant tumors. While suitable implantations for bridging defects in the proximal humerus and proximal femur are available, prostheses for resectioned tumors in the knee area have not yet been fully developed. From 1966 to 1978, 77 endoprostheses were implanted in 29 primary bone tumors, 3 malignant soft-tissue tumors and 45 bone metastases. There were 43 endoprostheses of the proximal femur, 31 endoprostheses of the proximal humerus and 3 special endoprostheses of the knee region. In most cases, eliminating the tumor radically was successful and a functional extremity was retained.

Extracortical attachment of bioceramic endoprostheses to long bones without bone cement.

Dense, highly pure A12O3 ceramic not only shows excellent wear resistance and tissue biocompatibility, but also has sufficient mechanical strength for loaded endoprostheses. The necessary diameter size of the material to sustain adequate shear stresses can be achieved, if the endoprosthesis is attached extracortically to the conically truncated long bone according to the principle of the conical sleeve. Extracortical attachment of the endoprosthesis causes minor nutritive damage since long bones derive their vascular supply mainly from the medullary cavity. In this way a primary stable and loadable connection is established, which is a prerequisite for permanent anchorage of the implant through new bone formation. After experiments in 12 dogs bioceramic endoprostheses were implanted in a total of 40 human patients using this technique of extracortical attachment (12 hip joints, 24 proximal humeri, 4 special constructions). Thirty-six of these implants were stable after observation periods ranging from one to 49 months. A stable anchorage of the prostheses to the pre-existing bone through new bone formation could be demonstrated histologically. The above preliminary results show that bioceramic endoprostheses can be "incorporated" in the skeleton and possibly also withstand the functional stresses.

[Surgical treatment of the congenital femural defect by means of rotation plasty (author's transl)].

Experiences made in 5 cases with Borggreve's rotation plasty have shown that in cases of congenital femural defects and exceptionally also in malignant bone tumors of the distal femur this method should be suggested to the patient as a possible therapy. By rotating and shortening of the leg, the ankle joint is converted to a new knee joint. With this method an active movement of the knee joint can be obtained, so that the fitting with a below knee prosthesis becomes possible.

[Complete resection of a vertebral body (author's transl)].

The resection of one or more vertebral bodies is technically difficult and quite demanding for the patient. In cases of solitary metastases to a vertebral body such a procedure may be considered when the general condition of the patient is satisfactory. To mobilise the patient quickly the stabilisation of the spine with bone cement and plates is mandatory. Three of four cases have been treated by two consecutive operations, one patient received a prefabricated ceramic implant. Mobilisation was maintained until four months after surgery. An analysis of previously published reports on complete vertebral body resection shows advantages and disadvantages of several methods.
