Two cases of red cell aldolase deficiency associated with hereditary hemolytic anemia in a Japanese family.

Two cases of red cell aldolase deficiency associated with congenital nonspherocytic hemolytic anemia are reported. The proband is a fourteen-month-old Japanese boy. Consanguineous marriage was not proven but probable in this family, as the parents were born in the same small island. The proband had moderate to mild anemia aggravated by upper respiratory infections, 1 cm hepatomegaly and 2.5 cm splenomegaly, but was unremarkable in other respects and has thus far not shown mental or growth retardation. He did not have dysmorphic features. The red cell aldolase activity was 6% of the normal mean. The enzyme was unstable with respect to heat, and Km for fructose 1,6-diphosphate (F-1,6-DP) was high. The parents and other heterozygotes showed intermediate activity between that of the proband and that of normal subjects. Red cell F-1,6-DP concentration in this case was remarkably increased. Red cell glucose consumption, and lactate formation, as well as hexose monophosphate shunt activity, were decreased as compared with a comparable reticulocyte-rich hereditary spherocytosis patient. Hexose monophosphate dehydrogenase by a high concentration of F-1,6-DP in his red cells. As a result of family study, another homozygous aldolase deficiency case associated with hemolytic anemia was found. He is 13 years old and a nephew of the proband's paternal grandmother. His hemolytic anemia also is moderate to mild and aggravated by upper respiratory infections. He does not seem to have mental or growth retardation, nor does he possess dysmorphic features.

Chromosomal findings and their correlation to prognosis in acute lymphocytic leukemia.

Sixteen of the 17 patients with acute lymphocytic leukemia (ALL) were revealed to have chromosome abnormalities in their leukemic cells. Of nine children, six had modal chromosome numbers between 50 and 59; these patients and one additional patient with a pseudodiploid karyotype have achieved and retained a complete remission. The only patient without chromosome abnormalities achieved a remission, but had a relapse and died. The only child who expired without achieving a remission had 47 chromosomes in his leukemic cells. Of eight adults, none had the hyperdiploidy seen in the children. The four with hypo-or pseudodiploidy failed to achieve a complete remission. One patient had 47 chromosomes in his leukemic cells, and this patient and the remaining three, all with ploidy abnormalities, achieved a complete remission, but a relapse occurred and they died. The chromosomal abnormalities were very extensive in most cases, many patients having marker chromosomes of an unknown origin. Four patients clearly exhibited karyotypic evolution when they relapsed. The patients with hyperdiploidy had a better prognosis, and those with pseudodiploidy had a poorer prognosis. The role of individual chromosome aberrations, however, remains to be elucidated.

Cryptic vascular malformation of the choroid plexus.

A 45-year-old man and a 47-year-old woman with cryptic vascular malformations of the choroid plexus in the lateral ventricle are reported. Each patient had an intraventricular hematoma, located in the right trigone and in the anterior part of the left lateral ventricle, respectively. The vascular lesion of one patient was angiographically visualized as find abnormal vessels communicating with the right anterior choroidal artery; in the other, angiography did not reveal the lesion. In the former patient, histological examination of the excised specimen led to a diagnosis of arteriovenous malformation; in the latter, a diagnosis of venous malformation was made. Both patients were successfully treated surgically.

Absence of HLA-DRW2 in Japanese pemphigus vulgaris.

17 Japanese patients with pemphigus vulgaris were examined by HLA typing. HLA-DRW2 was found in none of them as compared to 56 (39%) of 144 Japanese controls (p = 0.02). The association of HLA-DRW4 with pemphigus vulgaris was not recognized in our study. HLA-A, B and C typings were also performed, showing relatively high frequencies of the B15, BW35 and CW4. However, the occurrence of these specificities was not statistically significant. It is suggested that DRW2 plays an important role in the susceptibility to pemphigus vulgaris.

Intracranial germ cell tumor indicating a high level of alpha-fetoprotein (AFP).

An autopsy case of intracranial and extrapineal tumor is reported. The tumor, detected 2 years and 4 months before death, was an ectopic pinealoma (germ cell tumor) and showed elevated serum alpha-fetoprotein (AFP) of 48,000 ng/ml. Histologically, intra- and extracellular PAS-positive hyaline globules were observed in the tumor. The tumor tissues of the paraffin sections revealed a bright positive fluorescence for AFP with indirect immunofluorescence method. Besides, there were generalized metastases in this case.

Hepatocellular carcinoma combined with hepatic sarcoma.

An autopsy case of hepatocellular carcinoma combined with hepatic sarcoma was presented. There were three main tumor nodes in the liver. They showed necrosis due to therapeutic embolization of the hepatic artery. Histologically, one of the three was a hepatocellular carcinoma, and the other two were of spindle cell sarcomas. Extrahepatic metastases all showed characteristics of sarcoma. In the present case report, some problems concerning hepatic sarcoma and sarcomatous representation of hepatocellular carcinoma were proposed in reviewing the literatures.

Resection of leiomyosarcoma originating in internal iliac vein and extending into heart via inferior vena cava.

A patient's condition was initially diagnosed as constrictive pericarditis, but an echocardiogram revealed the presence of an intracardiac tumor. The tumor was a slow-growing leiomyosarcoma that originated in the left internal iliac vein, extended into the inferior vena cava, and reached the right ventricle. This tumor was considered to be a recurrent uterine leiomyosarcoma that had been resected 4 years before. After a second resection of this tumor, the patient was living a normal life 61/2 years postoperatively, in spite of recurrence in the pelvis and lung. We reviewed 12 cases of leiomyosarcoma in the world literature that reached the heart through the inferior vena cava. We added our case and evaluated the clinical features of all 13. This is the first successfully resected case of leiomyosarcoma of this nature so far as we could determine.

[Experimental teratoma derived from mouse yolk sac (author's transl)].

By using no carcinogens, well differentiated teratomas were induced by BALB/c nu/+, +/+ and ICR strain of mice. On the 11th to 18th day of gestation, mice were laparotomized and all fetuses were removed from uterus, leaving placentas in situ and fetal membranes were protruded into peritoneal cavity. After 5 to 139 days of operation, extrauterine tumors developed in twenty five mice of fifty fetectomized mice. In BALB/c nu/+ mice the tumor developed in six of ten, in +/+ mice eighteen of twenty five and in ICR mice only one of fifteen. The tumors were all histologically well differentiated teratomas, some contained tissues derived from one germ layer and other from two or three germ layers but no undifferentiated cells. The teratomas became cystic, so we call, endodermal cysts on histological examination during rather long term observations. We concluded that these induced tumors were not genuine tumors in real pathologic means but only well differentiated tissues derived from the yolk sac on developmental stage. Alpha-fetoprotein (AFP) in serum or in cystic contents of the tumor bearing mice were not detected by double immunodiffusion method.

[Laboratory and clinical studies of cefoxitin in pediatrics (author's transl)].

Laboratory and clinical studies of CFX were conducted on 30 pediatric inpatients at the Department of Pediatrics of Mie University. The results of the sensitivity evaluation conducted on 37 clinical isolates consisting of 16 species were in accordance with the findings reported hitherto in the literature, i.e., CFX was superior to CEZ and CET in terms of the growth inhibitory effect against Gram-negative rods. The serum peak level was obtained 5 minutes after an intravenous injection of 25 mg/kg, and 15 minutes after a drip infusion of 30 minutes using the same dose. The average terminal half life was 13 minutes 15 seconds for the former and 20 minutes for the latter. Clinical evaluation was made on a total of 22 eligible patients. The results were classified as follows: Excellent in 4 cases, good in 12, fair in 4 and poor in 2. The effective rate of CFX was 72.7%. Side effects observed were vascular pain, rash and vomiting, all of which were mild in nature and disappeared immediately after discontinuation of, or change in the routine of drug administration.
