Pathological study of small hepatocellular carcinoma: frequency of their invasion.

We studied the pathological findings in 28 small hepatocellular carcinomas (less than 5 cm in diameter), especially the mode of invasion. Seventeen of the 28 lesions were encapsulated. Capsular invasion was present in 14 of the 17 encapsulated tumours. Vascular invasion was found in 12 of the 28 tumours. The small hepatocellular carcinoma, even when encapsulated, frequently invaded blood vessels and adjacent liver tissue, in spite of frequent vascular invasion, distant metastasis was seen in only 1 of the 28 tumours.

Cerebrospinal fluid dynamics in traumatic intracerebral pneumocephalus evaluated with computed tomographic cisternography.

Cerebrospinal fluid dynamics in a case of traumatic intracerebral pneumocephalus are evaluated with metrizamide computed tomographic cisternography. Accumulation of metrizamide into the air cyst through the ventricular system is due to transependymal penetration, which can be explained by the mechanism of the pressure gradient from the ventricle towards the air cyst.

11;19 translocation in a congenital leukemia with two cell populations of lymphoblasts and monoblasts.

This report describes a case of a female infant of congenital leukemia with a chromosomal translocation t(11;19) (q23;p13) which presented initially with lymphoid features and at relapse with monocytic ones. The clinical course and the results of sequential cytochemical, cytogenetic and immunological studies are considered to be consistent with the interpretation of leukemogenesis of the myelo-monocytoid progenitor cell which still retains the capability of exhibiting lymphoid features to a limited extent. Although leukemia with t(11;19) has been classified as ANLL, most commonly M5 of FAB classification, the patient with this chromosomal abnormality may have a mixed leukemia in which cells with lymphoid features and those with monocytic ones exist.

Discrepancy between in-vitro and in-vivo inductions of differentiation by retinoids of human acute promyelocytic leukemia cells in relapse.

In-vitro studies of leukemic cells with retinoic acid and a therapeutic clinical trial with its derivative, etretinate, in a 58 yr-old male patient with 15;17 translocation-positive acute promyelocytic leukemia (APL) in relapse are reported. Actinomycin D was used in combination. Bone marrow promyelocytes from the patient prior to etretinate and actinomycin D matured morphologically in the liquid culture with retinoic acid; 98% were matured myeloid cells after 6 days at a concentration of 10(-6) M of retinoic acid as compared with 2% in the control culture. Positive NBT reactions were seen in none of the cells in the latter but in 95% of the cells in the former. Actinomycin D, when added alone, only induced NBT positivity, but, when used in combination with retinoic acid, increased both NBT positivity and morphologically matured cells. The patient was treated daily with 2 micrograms/kg of actinomycin D (or 20 mg/m2 or 33 mg/m2 cytosine arabinoside after the 6th day) in 24-h infusions and per oral 90 mg/body of etretinate. No effectiveness was observed both morphologically and clinically. The patient expired 15 days after the initiation of etretinate. Thus, a discrepancy existed in the response of leukemic cells from this relapsed patient with APL to the in-vitro and in-vivo attempts to include differentiation by retinoids and actinomycin D.

Enhanced production of intracellular ara-CTP after sequential use of methotrexate and 1-beta-D-arabinofuranosylcytosine.

Synergistic antiproliferative effect has been proven in vitro when mouse leukemic cells were sequentially treated with MTX and ara-C. The mechanism of this combination effect not well elucidated but the intracellular uptake of ara-C was higher when cells were pre-exposed to MTX. In this experiment, the intracellular ara-CTP was measured by HPLC after MTX and ara-C were sequentially administered to BDF1 mice bearing L1210 leukemic cell, either being sensitive or resistant to MTX. When MTX at the dose of 12 mg/kg was preceded 6 h and 3 h to ara-C at the dose of 25 mg/kg, the intracellular levels of ara-CTP were found to be significantly higher as compared with those of ara-C alone as control group. At 1 h after ara-C, ara-CTP was measured about 165 and 130% of the control levels, respectively, and at 12 h, ara-CTP was over 4 times higher of control level with group of mice to which MTX was preceded 6 h prior to MTX. On the contrary, the enhancement of ara-CTP production was definitely diminished with MTX-resistant cells in the same administrative model. From our present experiment, the time sequential modulation of intracellular ara-CTP production by MTX was reconfirmed in vivo, and this modulation might depend upon the sensitivity of MTX of leukemic cells.

Non-Hodgkin's lymphomas in Osaka, Japan.

Six hundred and eight-four cases with non-Hodgkin's lymphomas (NHL) in Osaka, Japan were reviewed, using current histopathological classification to obtain a general feature of Japanese lymphomas. There were no remarkable differences in the distribution of age, sex and the location of primary tumors between Japanese NHL and the disease in Western races. Histologically, however, several differences were present; in particular, the lower frequencies of nodular lymphomas of nodal origin and lymphocytic type of nodal and extranodal origin in the present cases. About 10% of the cases show a peculiar histology characteristic for adult T cell lymphomas. From this study, it appears that the ratio of low-grade malignant NHL in Japan is much lower than in the Western countries.

Primary gastric choriocarcinoma. Report of an autopsy case with immunohistochemical study.

An autopsy case of primary gastric choriocarcinoma of a 70-year-old Japanese female is presented. The tumor was initially diagnosed as poorly differentiated adenocarcinoma of the stomach by biopsy. Autopsy revealed co-existence of choriocarcinoma and adenocarcinoma in the stomach and a large amount of human chorionic gonadotropin in the serum. The choriocarcinoma metastasized to the liver, lung, and omentum, and the adenocarcinoma to lymph nodes. In the gastric tumor, immunohistochemical stains showed human chorionic gonadotropin in choriocarcinoma cells, alpha-fetoprotein and carcinoembryonic antigen in adenocarcinoma cells. The pathogenesis and pathological characteristics of primary gastric choriocarcinoma are discussed.

Effect of clenbuterol on peripheral airway obstruction in bronchial asthma.

A study was carried out in 6 patients with bronchial asthma to investigate the effects of clenbuterol, a beta 2-sympathomimetic bronchodilator, on peripheral airway obstruction. The basal lung functions of the patients were almost within normal range in both vital capacity (VC) and forced expiratory volume in 1 second (FEV1), but their maximal flow rates were lower in effort-independent phase of both maximal expiratory flow volume (MEFV) curve and partial expiratory flow volume (PEFV) curve. Furthermore, they demonstrated marked basal frequency dependence of dynamic compliance [CL,dyn]. Oral administration of clenbuterol (40 micrograms) produced a significant increase in the maximal flow in effort-independent phase of both MEFV and PEFV curves, and markedly decreased frequency dependence of CL,dyn in comparison with the baseline values, while it improved both VC and FEV1 to a lesser extent. These results suggest that clenbuterol preferentially reduced the peripheral airway obstruction in bronchial asthma.

Orcein-positive material in Kupffer cells from the liver.

Orcein-positive material in Kupffer cells obtained from 1,377 needle biopsies of the liver was studied for hepatitis B surface antigen (HBsAg). Fifty-five (57%) out of 96 cases with orcein-positive Kupffer cells had acute hepatitis, and 46 (84%) of these 55 were in the late stage of their illness. Orcein-positive material in Kupffer cells was not associated with HBsAg as evaluated by the immune peroxidase method. Orcein-positive material in Kupffer cells appears to consist mainly of lipofuscins developing in the course of acute hepatitis.

[Phase II trial of peplomycin in non-Hodgkin's lymphoma].

Seventeen patients with malignant lymphoma were entered into a phase II study of peplomycin (PEP) to determine the efficacy of the drug. There were 8 males and 9 females with a median age of 64 yrs (range 3-74 yrs) and a median PS 3 (range 2-4). Three of these were children. At first PEP was given intermittently and intramuscularly (8 cases) at a dose of 10 mg every one (3 cases) or two (5 cases) weeks, and then intravenously by 22-hr continuous infusion (9 cases) at a dose of 5 mg per day for 5 days. Mean cumulative dose was 78 mg. Objective responses were obtained in 6 patient (35%). CR lasting 4 weeks was obtained in one patient with diffuse mixed-type lymphoma. Five patients, one with diffuse medium-sized cell type and 3 with diffuse large cell type, had PR, lasting 6, 7, 7, 9, and 50+ weeks, respectively. Pulmonary fibrosis was found in two patients on autopsy and interstitial pneumonia in two patients clinically. Temporary high fever occurred in 7 patients, stomatitis in 3 patients and anorexia in 3 patients.

[Preoperative chemo-embolization for treatment of hepatocellular carcinoma].

A multidisciplinary treatment for hepatocellular carcinoma including preoperative chemo-embolization with resection treatment was introduced. Recent attempts at combining lipiodol with chemo-embolization have been quite promising. An ideal multidisciplinary treatment should include an effective device using the transportal approach.

Phase II study of vindesine in patients with non-small cell lung cancer.

A phase II study of vindesine (VDS) was carried out in 21 patients with non-small cell lung cancer (NSCLC). There were 13 and 8 patients with and without prior chemotherapy, respectively. VDS was administered at a weekly iv dose of 3 mg/m2. Partial response was observed in two of 15 adenocarcinomas and one of 2 adenosquamous cell carcinomas, and the overall response rate was 14.3% (3/21). Myelosuppression, especially leukopenia, was the most common dose-limiting side effect. Neurotoxicity was also a common side effect but the degree was mild. It was concluded that VDS at a dose of 3 mg/m2 every week seems to be active against NSCLC.
