[In vitro drug sensitivity test].

Human tumor clonogenic assay(HTCA) not only offers potential advantages for prediction of the sensitivity of individual patients, to anticancer drugs, but also for the development and preclinical testing of prospective new antineoplastic agents. However, HTCA has several theoretical and technical problems, such as a low success rate, the requirement of large numbers of tumor cells and the long time period necessary for evaluation. In order to solve these problems, the MINI-hybrid assay has been developed. We reviewed our experiences to date with chemosensitivity testing by MINI-hybrid assay. Twenty-two of 23 tumors gave evaluable chemosensitivity results (95.6%), and the range of thymidine incorporation for evaluable assays was 7 X 10(2)-1.2 X 10(5)cpm. In addition, the results of drug sensitivity testing of cultured cell lines by MINI-hybrid assay were well correlated with those obtained by HTCA. With its high evaluability rates, the need for fewer cells, the short duration (5 days) required and ease of quantitation, the MINI-hybrid assay is widely applicable to the chemosensitivity testing of human tumors.

[Experience of cefuzoname in pediatric patients. Fundamental and clinical investigations].

Cefuzoname (CZON) was administered to 50 pediatric patients with infections, and the efficacy was investigated in 49 patients. The efficacy rate was 83.7% and the drug was evaluated to be highly effective for diseases in the pediatric field. The activity of CZON was especially good against Staphylococcus aureus, and the efficacy rate for 13 clinical isolates was 69.2%, which was comparable to that of cefotiam or cefazolin. CZON appeared to be useable alone in Gram-positive infections. It might be very powerful in the control of infections in secondary immunodeficiency.

A case of adenocarcinoma of the lung which was successfully treated by chemotherapy with patient survival without recurrence for more than three years.

A 63-year-old man was admitted to the National Cancer Center Hospital on December 20, 1983 for a close examination of the abnormal shadows found in chest roentgenograms. Chest radiographs showed a massive right pleural effusion, a large tumor in the right hilar region and swelling of the left hilar nodes. Transcutaneous needle biopsy revealed the tumor's cytological type to be adenocarcinoma. The clinical stage was considered to be T3N2M1 (American joint committee (AJC) stage III M1), and his performance status was 4, so he was given etoposide (VP-16) alone as the initial chemotherapy treatment. He showed a partial response upon two courses of VP-16 therapy. Subsequently, one course of cisplatin (80 mg/m2) + vindesine (3.3 mg/m2), two courses of VP-16 (80 mg/m2 D1-4), five courses of cyclophosphamide (80 mg/m2) + adriamycin (50 mg/m2) + vincristine (1.4 mg/m2) and two courses of VP-16 (100 mg/m2 D1, 3, 5) were administered sequentially until May 31, 1985. No radiation therapy was given and, up to March 31, 1987, no sign of recurrence had been observed.

Myeloblastoma with an 8;21 chromosome translocation in acute myeloblastic leukemia.

An 8;21 chromosome translocation was found in cells from myeloblastomas of two acute myeloblastic leukemia (AML) patients. In one patient, a sudden onset of paraplegia, which was caused by an epidural myeloblastoma in the thoracic vertebral canal, was the initial symptom of the disease and that of the first relapse. In the other, myeloblastomas, which occurred in the breasts during a hematologic remission, heralded the first relapse. Karyotypes of tumor cells obtained from both patients showed the t(8;21) and additional chromosome abnormalities. These findings, together with earlier experience and reports in the literature, indicate that myeloblastoma may be unique to the 8;21 translocation, and that certain chromosomal abnormalities additional to the t(8;21) may be necessary for the tumor formation.

Predictable risk factors in children with acute lymphoblastic leukemia.

The predictable prognostic factors were analysed among 174 children with acute lymphoblastic leukemia who were treated during the last ten years under different protocols in one institute. It was confirmed that the children under 1 year of age, or with T-cell marker, had poor prognosis. The initial WBC of more than 50,000/mm3 was less significant as a predictable risk factor with chemotherapy of the newer protocols. Cell kinetic study was found to be of no more help than the initial WBC, but a more accurate prediction could be obtained by measuring glucocorticoid receptor of leukemic cells. The prognosis was poor among children with initial WBC of more than 50,000/mm3 and receptors of less than 20,000 sites per cell.

In vivo augmentation of the cytotoxicity of spleen lymphocytes against syngeneic B-16 melanoma cells and the suppression of the artificial metastases in C57BL/6 mice by subcutaneous multiple injections of high dose human recombinant interleukin-2 (rIL-2).

The cytotoxicity of spleen lymphocytes against YAC-1 cells, against syngeneic B-16 and F-10 melanoma cells was augmented not only by incubation of spleen lymphocytes with human recombinant interleukin-2 (rIL-2) in vitro, but also by injecting C57BL/6 mice with high dose rIL-2 for more than 3 consecutive days. In animals injected s.c. with multiple high dose rIL-2, the numbers of tumor nodules in the lung were significantly decreased 21 days after i.v. tumor inoculation. In addition, in these groups of animals no liver metastases were observed although liver metastases were detected in 6/11 control mice.

Fine-needle aspiration biopsy via percutaneous transhepatic catheterization: technique and clinical results.

Under fluoroscopy, we performed fine-needle aspiration biopsy of the pancreas and bile duct via percutaneous transhepatic catheterization in 24 patients with obstructive jaundice. The sensitivity, specificity, and overall accuracy of our technique were 67, 100, and 71%, respectively. This method of biopsy avoids malignant seeding of the tract and does not burden patients psychologically.

Active inflammatory change of the liver as a cause of postoperative hepatic failure.

Histopathology of cirrhosis was compared to the hepatic functional reserve and to the prognosis. One hundred and thirteen patients including 51 surgically treated for hepatocellular carcinoma (HCC) and 62 subjected to wedge biopsy of the liver during surgery for esophageal varices were studied. The type of cirrhosis associated with hepatocellular carcinoma was classified into 4 groups according to the degree of inflammation and the piecemeal necrosis. Of the 16 without cirrhosis, 13 (81 per cent) are living. Nine had an inactive cirrhosis and 5 (56 per cent) are living. Ten had a slightly active cirrhosis and 4 (40 per cent) are living. Sixteen had a fairly active cirrhosis and 7 (44 per cent) are alive. Immediate postoperative death due to acute hepatic failure occurred in 6, 4 of whom had a fairly active cirrhosis. In patients with active alcoholic hepatitis with numerous Mallory bodies and ballooning degeneration of liver cells, the prognosis was the poorest, if the liver was resected, even though the functional reserve seemed to be adequate. To prevent acute hepatic failure, liver histology during the surgery is predictive and wedge biopsy of the liver is recommended.

Surgical clinicopathologic study of malignant fibrous histiocytoma.

Fifteen men and six women, ranging in age from 20 to 79 years, with malignant fibrous histiocytoma comprised this series. Amputation of the right lower extremity was performed in one patient, wide resection in 7 and marginal excision in 9, respectively. In the other 4 patients, a non-curative resection was carried out. Adjuvant chemotherapy was prescribed post-operatively for 12 patients. Histologic grade of the surgical specimens was Stage I in 3, II in 6, III in 8 and IV in 4. Among fourteen patients living from 10 months to 9 years and 9 months after the operation, the 7 subjected to a wide resection are all disease free. Seven patients died of a local recurrence or a distant metastases. The survival rate of the patients with Stages I, II is significantly higher than those with Stages III, IV. These results show that a wide resection is to be favored and that staging is useful to estimate the prognosis.

An inhibitor of protein kinase C, 1-(5-isoquinolinylsulfonyl)-2-methylpiperazine(H-7) inhibits TPA-induced reduction of vincristine uptake from P388 murine leukemic cell.

The effects of protein kinase C inhibitor H-7 (1-(5-isoquinolinylsulfonyl)-2-methylpiperazine) on tumor-promoting phorbol ester induced inhibition of vincristine uptake in P388 murine leukemic cells were investigated with the objective of assessing the possible role of Ca2+-activated, phospholipid-dependent protein kinase (protein kinase C) in vincristine uptake. 12-O-Tetradecanoylphorbol-13-acetate (TPA) is a potent inhibitor at concentrations above 1 nM. Other phorbol esters also inhibited vincristine uptake in approximate proportion to their activity in competing for [20-3H]phorbol 12,13-dibutrate binding. TPA enhanced the Ca2+-activated, phospholipid-dependent phosphorylation of histone III-S by a soluble protein fraction of cells. Phosphorylation of various cell lysate proteins (p18, p21, p29, p34 and p45) were also stimulated by TPA. These TPA-induced stimulations were also inhibited dose-dependently by H-7. It is tentatively concluded that the phosphorylation of cell lysate protein substrates by protein kinase C may be an important mechanism linked to the regulation of vincristine uptake in leukemic cell.

Shedding of the common acute lymphoblastic leukemia antigen (CALLA) by lymphoblastoid cell lines.

The release of common acute lymphoblastic leukemia antigen (CALLA) into culture medium by lymphoblastoid cell lines was studied. CALLA release was detected using a solid phase indirect radioimmunometric assay which combines the specificity of lectins and monoclonal antibodies and using immunoadsorption of labeled CALLA in spent medium from cells incubated with 35S-methionine. No CALLA was present in the medium of cells pulse-labeled at 37 degrees C, when they were placed at 4 degrees C, thus this is an active process. Sequential analysis of soluble CALLA revealed shedding of CALLA to be associated with cell growth.

Use of bromphenol blue printing method for detecting sweat on the palm.

Sympathetic nerve fibres innervating the sweat glands in the skin are known to accompany sensory nerve fibres closely. Examination of sudorific function, therefore, is a useful aid in making a diagnosis of severed peripheral nerve and also provides valuable information on nerve function in the recovery stage following injury and surgery such as neurorrhaphy. Among the many methods which have been used clinically, the one employing bromphenol blue is thought to be the most simple and accurate.
