A case of familial ventricular tachycardia.

Ventricular tachycardia was noted in 4 members of the same family. One showed diffuse hypokinesis of the left ventricle by echocardiography and left ventriculography, 2 showed progressive left ventricular dysfunction and 1 showed regional perfusion defects of the left ventricle shown by thallium scintigraphy. One patient was diagnosed as dilated cardiomyopathy. Although no definitive cause of the left ventricular disturbance was identified in the 3 other patients, they may have all been dilated cardiomyopathy.

Characterization of chicken kit tyrosine kinase receptor in Cos cell transfectants and in chicken brain.

The Kit tyrosine kinase (Kit) encoded by the c-kit proto-oncogene is a receptor for stem cell factor (SCF). Kit proteins of mice and humans are expressed in various kinds of hematopoietic progenitor cells and are essential for the growth of these cells. Wild-type chKit (chKit+) and a mutant chKit (chKit42) that contained an amino acid change from Asp777 to Asn corresponding to that in Kit of the W42 mutant mice were produced in Cos-1 cells transfected with expression plasmids containing the chicken c-kit cDNA, and characterized using two kinds of anti-chKit antisera. The W42 mutant Kit has previously been shown to be defective for kinase activity. The chKit+ of 145 kilodalton (kDa) and 130 kDa with varying degrees of N-linked glycosylation were detected. Western blot analysis using an anti-phosphotyrosine monoclonal antibody showed that autophosphorylation of chKit+ was greatly enhanced upon chicken SCF induction. The chKit+ did not respond to mouse SCF. The kinase activity of chKit42 was abolished by the amino acid substitution, indicating the Asp777 residue was essential for the activity. In addition, 145 kDa chKit conjugated with sialic acid residue(s) was detected in chicken brain by immunoprecipitation using the antisera. An in vitro kinase assay showed the kinase activity of this protein. These structural and functional similarities of chKit to mammalian Kit proteins shown in this study implicate a possible role of chKit in chicken hematopoietic system.

Proximal tibiofibular joint: a radiographic and computed tomographic study.

In 671 knees of 351 patients, the proximal tibiofibular joint was radiologically investigated. To clearly detect the contour of the joint, radiographs were obtained under fluoroscopic control with the lower extremity internally rotated. The contour was classified into three types: the joint lines of both tibia and fibula are straight (A); the joint line of the fibula is convex (B); the joint line of the tibia is convex (C). The rotation and the inclination of the joint were measured. Of the 671 knees, CT examination was also carried out in 51 knees. The obliquity of the joint was measured and the location of the fibula as related to the tibia was evaluated by CT. We also examined the relationships between the contour and the rotation, inclination, obliquity of the joint and the location of the fibula, and determined the characteristics of type B and C as compared with type A. The characteristics of type B were as follows: (1) this type was mainly seen in younger patients, (2) the rotation was the lowest and the inclination was the highest among the three types, (3) in CT the joint line was more saggital and the fibula was located more anteriorly. The characteristics of type C were as follows: (1) this type was seen in older patients, (2) the rotation was the highest and the inclination was the lowest among the three types, (3) in CT the joint line was more frontal and the fibula was located more posteriorly.

Effects of fructose ingestion on carbohydrate and lipid metabolism during prolonged exercise in distance runners.

The effects of ingestion of fructose (F), as well as of arginine (A) and citric acid (C) on both carbohydrate metabolism and lipid metabolism during prolonged exercise in seven distance runners on treadmill running were investigated by comparing it with those of glucose + arginine + citric acid (GAC) and placebo. With the values of the plasma glucose, though FAC as well as placebo contrastively indicated almost a flat distribution, GAC increased from 30 min rest before the exercise (i.e., the time for 500 ml intake of GAC) to the beginning of the exercise. The plasma glucose for GAC showed significantly higher values (138 +/- 22 mg/dl) than that for FAC or placebo, whose values were 102 +/- 18 mg/dl and 93 +/- 8 mg/dl, respectively. The plasma glucose for GAC was likely to decrease gradually as the exercise progressed more. At the end of exercise, however, the plasma glucose for FAC indicated significantly higher values (100 +/- 12 mg/dl) than that for GAC, whose values were 89 +/- 12 mg/dl. Though the serum free fatty acid showed upward tendencies of values in all cases of FAC, placebo, and GAC as the exercise progressed, no significant difference of the values in those cases was observed (the following values were drawn at the end of the exercise; FAC:655 +/- 185 microEq/l, placebo:603 +/- 289 microEq/l, GAC:485 +/- 140 microEq/l).(ABSTRACT TRUNCATED AT 250 WORDS)

Combined cisternal drainage and intrathecal urokinase injection therapy for prevention of vasospasm in patients with aneurysmal subarachnoid hemorrhage.

The effect of cisternal drainage and intrathecal urokinase injection in preventing symptomatic vasospasm (SVS) after aneurysmal subarachnoid hemorrhage was studied in 60 patients with uniform background (Hunt & Kosnik grade III, younger than 70 yrs, undergoing surgery within 72 hrs after hemorrhage). The incidence of permanent neurological deficits caused by vasospasm was 5/16 without cisternal drainage, 5/34 with drainage alone, and 1/10 with drainage and urokinase injection. Analysis of patients without postoperative cisternal drainage showed the amount of subarachnoid clot on the initial computed tomographic scan was closely related to the occurrence of SVS (p < 0.05, unpaired t test). Analysis of patients with cisternal drainage showed the amount of bloody cerebrospinal fluid (CSF) drained during the 10 days after surgery and the duration of drainage placement were critical in preventing vasospasm (p < 0.05, unpaired t test). Greater CSF drainage significantly reduced the incidence of permanent neurological deficits caused by vasospasm (p < 0.01, chi 2), but significantly increased the incidence of hydrocephalus requiring shunt procedures (p < 0.01, chi 2). Urokinase injection via cisternal drainage achieved a further reduction in the occurrence of SVS. Intrathecal thrombolytic therapy after aneurysmal surgery is an effective method for SVS prophylaxis, and CSF drainage (> 1500 ml for 10 days) enhances the effect.

Immunocytochemical localization of estrogen and progesterone receptors (ER & PgR) in breast cancer.

The localization of estrogen receptors (ER) and progesterone receptors (PgR) in human breast cancer was examined light and electron microscopically by using specific monoclonal antibodies. Among the breast cancer patients, ER-positive (ER[+]) cases were shown by enzyme immunoassay (EIA) to account for 58.9% (63/107), and 63.5% of these cases were women in the postmenopausal state. The PgR-positive (PgR[+]) rate was 50.5% (54/107), with 44.4% of these positive cases being postmenopausal women. The rates of ER(+) and PgR(+), ER negative (ER[-]) and PgR negative (PgR[-]), and ER(+) and PgR(-) cases were 43.9% (47/107), 34.6% (37/107) and 15.0% (16/107), respectively. Although ER(-) and PgR(+) cases were in few number, they were found (6.5%, 7/107). These results correlated well with those obtained by the immunocytochemical method. In either case, i.e anti-ER or anti-PgR reaction, positive nuclei and negative nuclei were found intermingled with each other in a given visual field. In electron microscopy, both anti-ER and anti-PgR antibodies bound to sites in the euchromatin area of the nucleus.

Effects of UFT (mixed compound of tegafur and uracil) on cell kinetics and inhibition of thymidylate synthase in L1210 ascites tumor.

Previous work in our laboratory showed that UFT (mixed compound of tegafur and uracil, molar ratio 1:4, respectively) caused the prolonged reduction of dTTP in L1210 leukemia cells in comparison with 5-fluorouracil (5-FU). The purpose of this study was to assess the effect of UFT on cell cycle distribution and thymidylate synthase activity of a leukemia cell line as compared with 5-FU. UFT and 5-FU were orally given to BDF1 mice bearing L1210 ascites tumor on day 3 after the tumor inoculation. Cell cycle distribution patterns at 24 hr after the drug administration showed a higher percentage of S phase in tumor cells treated with UFT than in those treated with 5-FU. Until 6 hr after the oral administration of the drugs, UFT inhibited the incorporation of [3H] deoxyuridine into DNA more long than 5-FU did. These results indicated that UFT has longer and stronger inhibitory effects on DNA replication than 5-FU in vivo under the employed experimental conditions (i.e., low and single doses of these fluorinated pyrimidines).

[Bone mineral density of the radius in patients with rheumatoid arthritis measured by dual energy X-ray absorptiometry].

Bone mineral density (BMD) of the radius was measured in 181 female patients with rheumatoid arthritis (RA) and 255 control subjects using dual energy X-ray absorptiometry (DXA). The BMD of RA patients was assessed according to patients' age and menopause and was compared with age-matched controls for various activity factors of RA and duration of the disease that might correlate with BMD. At stages I and II of premenopausal RA, generalised osteoporosis was not observed. At stages III and IV, the BMD decreased considerably in premenopausal RA patients at age of 45 years or older. There were significant correlations between the BMD of postmenopausal RA and all factors examined, many of which did not affect premenopausal RA. These results suggest that osteoporosis of female patients with RA is influenced only slightly by the disease itself, but it is secondary osteoporosis induced by decreased activities of daily living due to RA.

Acute hemolysis during cyclosporine therapy successfully treated with vitamin E.

The immunosuppressant cyclosporine induces acute hemolysis (hemolytic uremic syndrome) in some patients after transplantation, possibly because of its effects on the vascular endothelium. We report a case of Coombs-negative, non-immune severe hemolytic anemia during cyclosporine therapy after allogeneic bone marrow transplantation for severe aplastic anemia. Uremia was not observed. Vitamin E was effective in treating the intravascular hemolysis.

[A case of adenomyelolipoma].

We report case of adenomyelolipoma. CT and MRI revealed a large, capsulated, septated adrenal mass with abundant fat tissue. However, enhancing components were demonstrated at the capsule and septations on angiography. On the pathological study, the capsule and septations consisted of adrenal adenoma and the tumor contained various forms of myelolipomatous tissues. These myelolipomatous tissues were classified into 4 groups. Type I: Scattering of fat cells and hematopoietic elements without coalescence. Type II: Collection of myelolipomatous tissue with unclear margin or small myelolipomatous tissue that cannot be classified as type I or III. Diameter of the lesion is less than 1 cm. Type III: Collection of myelolipomatous tissue with clear margin or replacement of cortical nodule. Diameter of the lesion is less than 1 cm. Type IV: Collection of myelolipomatous tissue. Diameter of the lesion is equal to or greater than 1 cm. We defined adenomyelolipoma as a lesion combining adrenal adenoma (or hyperplasia) and various forms of myelolipomatous tissues (type I-IV) in view of the strong relationship between adrenal adenoma (or hyperplasia) and myelolipomatous tissue.

[Ultrasonography of pediatric right lower abdominal pain: correlation with clinical and pathological results].

Right lower abdominal pain is a common problem in the pediatric population. Ultrasonography (US) is a well-accepted imaging modality in the clinical management of this problem. One hundred and six consecutive pediatric patients with right lower abdominal pain were prospectively evaluated. There were 54 boys and 52 girls, with ages ranging from 3 to 15 years (mean: 9.9 years). Final diagnoses were established based on pathological findings or clinical follow-up. US had sensitivity of 90%, specificity of 98%, and accuracy of 94% in the diagnosis of acute appendicitis. Among all true negative cases (n = 55), there was a variety of US findings other than appendiceal abnormality in 21 cases including intestinal and/or mesenteric (n = 19), gallbladder (n = 1), and ovarian (n = 1) abnormalities. All 21 cases were managed medically without complication. We conclude that US of pediatric right lower abdominal pain is a reliable imaging modality not only for the diagnosis of surgical cases but for eliminating the unexpected surgical intervention.

[The first case in Japan of unstable hemoglobin, Hb Buenos Aires [beta 85 (F1) Phe-->Ser], with parvovirus B19 infection (first case in Japan)].

A 9-year-old girl was admitted to Mie University Hospital June 25, 1992, complaining of fever, skin rash and pallor. Physical examination revealed anemia and hepatosplenomegaly. Laboratory findings showed normocytic normochromic anemia, increased reticulocyte counts and remarkably decreased haptoglobin level. Red blood cells had no morphological abnormality. Bone marrow examination showed erythroid hyperplasia without abnormal cells. Hemoglobin electrophoresis showed an abnormal band. The amino acid structure and sequence of the abnormal hemoglobin was determined to be an unstable hemoglobin, Hb Buenos Aires [beta 85 (F1) Phe-->Ser]. Sequence of genomic DNA and cDNA was compatible to Hb Buenos Aires. Parvovirus B19 infection was thought to have caused severe anemia due to hemolytic crisis in this patient because its IgM antibody was positive on admission. She recovered soon without any treatment and has been followed in the outpatient clinic. Her parents and brother showed no hemoglobin abnormality.

[Matrix metalloproteinase (MMP-9) in patients with rheumatoid arthritis].

MMP-9 degrades type IV collagen, gelatin, type V collagen, and type XI collagen. We measured proMMP-9 and proMMP-9/TIMP-1 complex in sera and joint fluids by sandwich ELISA, and examined immunohistochemically the expression of these proteins in joint tissue from patients with rheumatoid arthritis (RA). ProMMP-9 was purified by the three chromatographic steps from the culture medium of HT1080 cells. Purified proMMP-9 and activated MMP-9 by APMA showed a single band of Mr92000, Mr67000, resectively on gelatin-zymography. We raised two monoclonal antibody colones 2G9, 8G7 against proMMP-9 with BALB/c mice. 2G9 and 8G7 reacted with proMMP-9 on Western blotting, and these clones reacted proMMP-9 and proMMP-9/TIMP-1 complex on sandwich ELISA, specifically. ProMMP-9 concentration in 86 sera (749.4+/-940.2 ng/ml) and 54 joint fluids (4539.9+/-7681.5 ng/ml) from patients with RA was significantly higher than those of patients with osteoarthritis and control. Immunohistochemistry with 2G9 showed that in rheumatoid synovium proMMP-9 localized in neutrophils and monocytes diffusely infiltrated the sublining layer. In addition, osteoclasts in rheumatoid subchondral bone were intensively stained. ProMMP-9 concentration in joint fluids from 39 RA patients was not correlated to stage and class of Steinbrocker and to clinical laboratorial data. But, it was positively correlated to the number of proMMP-9 positive cells in RA synovium (r=0.607) and the score of Roony's diffuse infiltrates of lymphocytes (r=0.720). Our results suggest a role of MMP-9 in joint destraction of RA throughout neutrophils and monocytes, and a regulation of this enzyme by lymphocytes.
