[Clinical study of patients with hepatic disease at a city hospital in Japan--consideration on indications for liver transplantation].

One-hundred and ninety-one patients with liver diseases who were admitted to National Fukuoka Central hospital during a 3 years period between January 1, 1984 and December 31, 1986 were studied to assess the indications for liver transplantation with the use of a criteria by Dr. Van Thiel et al. The patients were divided into 2 groups; I--indication and II--no indication. Then group I was divided into 3 groups; A--no contraindication, B--relative-contraindication and C--contraindication. Of the 131 patients, group A and B which had the indication of liver transplantation without contraindication were 17%. Deaths by April 30, 1990 were as follows; I 62% (A 36%, B 86%, C 100%) and II 12%. Ten out of 13 deaths in Group I were all due to hepatic failure, while only 3 out of 8 deaths in Group II were due to hepatic failure. Of the other 60 patients with malignancy, 57 were hepatocellular carcinomas, and the other 3 were 2 metastatic hepatic cancers and cholangioma. Among the patients with hepatocellular carcinoma, 24 out of 46 died of cancer, while the other 19 patients died of hepatic failure. The criteria of the indication of liver transplantation seems acceptable for the evaluation of Japanese patients with non-malignant hepatic diseases. As to patients with hepatocellular carcinoma, a new and separate criteria is in need to evaluate them for liver transplantation.

Immunohistochemical expression of carbohydrate antigen 19-9 in gastric carcinoma.

Immunohistochemical localization for carbohydrate antigen 19-9 (CA19-9) was studied in the gastric tissues from 115 patients with advanced gastric carcinoma. We compared two groups: 57 patients who died of recurrence or metastases within 2 yr (group I) and 58 patients who survived 5 yr or more after resection (group II). The staining patterns were classified into negative, apical, and/or cytoplasmic type and stromal type. Thirty-three cases (58%) of group I showed a stromal type, whereas only 17 cases (30%) of group II showed the same type. In undifferentiated type carcinoma, the stromal type was seen in 20 cases (65%) of group I and seven cases (23%) of group II. However, in differentiated type carcinoma, there was only a small difference in the appearance of stromal type between group I (50%) and group II (37%). The immunohistochemical localization for CA19-9 in the tumor tissues, particularly in undifferentiated carcinoma, will be useful in predicting the prognosis of the patients with advanced gastric carcinoma.

[A solitary metastatic osteosarcoma of the lung with thin walled cavity: report of a case].

A 13-year-old girl had a history of wide resection of the left fibula under the diagnosis of osteosarcoma three years before. Postoperatively, a thin walled cystic lesion in the right upper lung field, which enlarged gradually with thickening of the wall was found on the chest roentgenogram. The right upper lobectomy was performed after clinical evaluation. The pathologic examination confirmed that the tumor was the metastatic osteosarcoma of the lung. Although metastatic osteosarcoma is common in the lung, very rare cases demonstrate thin walled cavity formation in chest roentgenogram. We reported herein a case of a solitary metastatic osteosarcoma of the lung with thin walled cavity considering with pathogenesis of the lung.

Transitional mucosa in human colorectal lesions.

Mucosa adjacent to colorectal disease was studied mucin-histochemically. Selected specimens were also studied immunohistochemically for carcinoembryonic antigen (CEA). Transitional mucosa, which showed elongation of crypts and marked sialomucin secretion, accompanied by a marked reduction in the normal sulfomucin content, was evident in 96 of 100 carcinomas (96 percent), 18 of 36 adenomas (50 percent), and 10 of 30 metaplastic polyps (33 percent). When considering the appearance of transitional mucosa, not only in the neoplastic lesions such as carcinoma or adenoma but also in the begin polyp, the transitional change adjacent to the carcinoma cannot be classified as a precancerous phenomenon; rather, it is a secondary one. The mucin-histochemical study disclosed transitional mucosa in all the 21 carcinomas less than 1 cm in diameter and immunohistochemical staining for CEA showed no remarkable change in the adjacent mucosa. Thus, it seems apparent that a change in mucous secretion precedes that of CEA expression in the mucosa adjacent to the carcinoma.

Treatment of symptomatic non-parasitic liver cysts--surgical treatment versus alcohol injection therapy.

Fourteen patients with benign symptomatic non-parasitic cysts of the liver were either surgically treated, had alcohol injected into the cysts, underwent deroofing of the cyst or in 5, a cystectomy was done. Alcohol was injected into 6 patients and there has been no recurrence for as long as 5 years and 8 months after the treatment. Liver dysfunction occurred in 3 patients given blood transfusion during the surgery and/or postoperative course, an elevated temperature (over 39 degrees C) occurred in one patient. Adverse effects of alcohol injections were minor and transient. Based on our experience, the injection of alcohol is an effective treatment for benign symptomatic cyst of the liver. When a malignancy is suspected on imaging and/or cytologic studies, or when alcohol administration is ineffective, then surgery is indicated.

[A case report of advanced gastric cancer remarkably responding to mitomycin C, aclacinomycin A, SF-SP and lentinan combination therapy].

A 63-year-old female diagnosed as inoperable gastric cancer was treated with combination immunochemotherapy of Mitomycin C, Aclacinomycin A, SF-SP and Lentinan. In this case, the tumor directly invaded the pancreas and the peritoneal dissemination, metastasis to the para-aortic lymph nodes and Virchow's metastasis were identified. As the result of this therapy, the primary tumor was remarkably reduced in size. The subjective symptoms and the metastasis of the para-aortic lymph nodes and Virchow's metastasis disappeared. The side effect was only mild thrombocytopenia.

Hypoxia enhances the lethality of mitomycin C and carboquone against human malignant tumor cells in vitro.

Solid tumors contain hypoxic cells which are relatively resistant to antineoplastic activity of radiation or to chemotherapeutic agents. We determined the increase in antineoplastic activities of mitomycin C (MMC) and carboquone (CQ) against human tumor cells under conditions of in vitro hypoxia, with the following results. (a) The HeLa cells had been exposed to various concentrations of MMC or CQ for 2 h under normally aerated (about 20%) or hypoxic conditions (5.0 and 0%) and were maintained under normally aerated conditions for 3 days. The succinate dehydrogenase activity was assayed, using the succinate dehydrogenase inhibition (SDI) test. Treatment with MMC or CQ reduced the SD activity, compared to findings in the control cells, under hypoxic conditions. (b) The cells were exposed to the drug, MMC or CQ, under normally aerated or anoxic conditions, for 15-45 min and the colonies counted on day 14. Treatment with 0.45 microM of MMC for 45 min inhibited the clonogenicity of HeLa cells by 25.4% of findings in the control cells under anoxia. Treatment with 0.03 microM of CQ for 30 min inhibited the clonogenicity by 41.5%. (c) The sensitivity of 14 human tumor tissues (6 gastric and 8 colorectal cancers) to MMC or CQ under hypoxic conditions was determined, using the SDI test. MMC and CQ showed hypoxic enhancement. Therefore, MMC and CQ have definite positive effects on hypoxic cells, and hence a higher therapeutic ratio for treating clinical solid tumors.

Very well differentiated adenocarcinoma of the colon: a case report.

A 47-year-old woman with chronic colitis was found to have a sessile tumor at the hepatic flexure of the colon. An endoscopic biopsy revealed no aytpia, but surgery was performed since, in view of the tumor's size, malignancy could not be ruled out. A tumor measuring 3 x 2 x 1.5 cm was found in the resected specimen. The tumor exhibited quite unusual histologic features, showing invasive, moderately atypical glands forming mucin-filled cysts, very similar to those seen in colitis cystica profunda, under pseudopolypoid mucosa covered by non-atypical glandular epithelial cells. The extension of glandular elements into the pericolonic fatty tissue, and the intermingling of anaplastic microglands led to a final diagnosis of adenocarcinoma. In a case such as this, it is very difficult to evaluate cancerous tissue by endoscopic biopsy, even with an amount of tissue adequate for histologic examination.

[Long-term chemotherapy in patients with stage IV gastric cancer after surgical treatment--a randomized comparative study of FT-207 and UFT].

A prospective randomized and comparative study of FT-207 and UFT was performed for cases of Stage IV gastric cancer involving long-term cancer chemotherapy after surgery. Immediately after the surgery, the subjects were randomly divided into two groups: A group treated by MMC and FT-207: and B group treated by MMC and UFT. From a total of 54 cases, 8 cases were excluded, so that A group consisted of 24 subjects and B group of 22 subjects. There were no differences in background factors between the two groups. In a comparison of all the cases, B group revealed a significantly higher survival rate than A group. These results indicated that the simultaneous use of MMC and UFT was effective as a long-term cancer chemotherapy after surgery for patients with Stage IV gastric cancer.

[Clinical application of the subrenal capsule assay (SRCA) and related problems].

Subrenal capsule assay was performed on 35 fresh cancer tissue samples (13 stomach, 7 colon, 8 breast and 7 others) against MMC, 5-FU, ADM, CPA, MTX and cDDP employing normal immunocompetent ddY mice following Bogden's original method. Evaluability was 86% (30/35). Sensitivity was between 32 and 46% for each of the drugs except cDDP. Some organ-specific variations were noted between gastro-colic cancers and breast cancers with regard to chemosensitivity spectrum and also individual cases of the same histological type did not show a uniform one. Gastric cancer had the highest activity. The assay/clinical correlations were evaluable in 10 cases where 6 cases on active agents achieved two PR, two NC and PD, while all 4 cases on non-active agents showed PD. Histological analysis of implants, however, disclosed the following inherent problems: First, there was heterogeneity in tissue fragments due to the variable amounts of stroma; Second, apparent host reactions were seen in the untreated groups on the 6th day, accompanying marked damage to cancer cells. In contrast, in drug-treated groups, host reactions in most of them were suppressed and cancer cells were seen frequently in considerable amounts. This fact implies the mechanism that SRCA is based on, in spite of the xenograft immune; Third, there was some discordance between the macroscopic and microscopic findings. SRCA is assumed to be appliable to individual clinical cases up to a certain level which should be determined by histological analysis as well as by accumulation of clinically correlated cases.

[Phase II study of (2''R)-4'-0-tetrahydropyranyladriamycin (THP) in gastrointestinal cancer].

A phase II study of a new anthracycline, (2''R)-4'-0-tetrahydropyranyladriamycin (THP) was performed on 37 patients with gastrointestinal cancer in 6 co-operative study institutions. Twenty-five patients out of 37 were evaluable for response according to the Koyama-Saito's criteria. THP was administered weekly at doses of 10 to 30 mg/body or every 3 to 4 weeks at doses of 40 to 60 mg/body intravenously. Of the 14 patients with gastric cancer, we obtained one complete response and 3 partial responses (response rate 28.6%), and of the 6 patients with rectal cancer, we obtained one partial response (16.7%). Leukopenia of less than 3 X 10(3)/mm3 and erythrocytopenia of less than 300 X 10(4)/mm3 were seen in 48% and 26% of cases. Neither cardiotoxicity nor hair loss were seen. These results suggest that THP is useful in the treatment of patients with gastrointestinal cancer.

Arteriovenous malformation of the ileum with chronic intestinal bleeding.

A 47 year old man had a ten year history of intestinal bleeding due to arteriovenous malformation of the distal ileum. Selective angiography is the most reliable technique for the diagnosis of the lesion and a correct diagnosis was not made until th selective superior mesenteric arteriography was done after repetitive laparotomy and intestinal resection. Transcatheter embolization was transiently successful. This case seemed to belong to type 2 according to Moore's classification and was probably of congenital origin.

Remnant liver function during surgery for extensive hepatic resection.

True functional reserve of remnant liver should be determined at the same condition that contemplated hepatectomy has been performed, and this condition can be achieved before resection by temporary clamping of the inflow vessels of hepatic lobes to be removed. Using indocyanine green (ICG), the remnant liver function (RLF) was evaluated whether or not the method can be a useful indicator for or against hepatomy. In the study of healthy dogs, the RLF proved to be a reliable indicator to prospect hepatic failure which may occur after extensive resection of the liver. Then, the method was employed in nine patients who underwent 40 to 70 per cent hepatectomy. The results indicated that the RLF with ICG during surgery, if performed under stable hemodynamics, is a beneficial tool to decide the resectability in equivocal cases.

Ethanol-induced microaggregate formation in pig and rabbit blood. An in vitro study.

Increased microaggregate formation was found after mixing ethanol with blood from pig and rabbit in vitro, measured with Swank's Screen Filtration pressure method. Final ethanol concentrations were in the range found in ethanol-intoxication in man. No rise in SFP was noticed when plasma with 10,000 platelets/microliter was used. It is therefore doubtful that altered plasma proteins caused the rise in SFP. It is suggested that if a direct effect of ethanol upon platelet aggregation exists in vivo, it may be of importance in acute ethanol intoxication and in chronic alcoholism.

Serum alpha-fetoprotein levels after hepatic artery ligation and postoperative chemotherapy: correlation with clinical status in patients with hepatocellular carcinoma.

Eight patients with hepatocellular carcinoma and positive serum alpha-fetoprotein (AFP) levels were treated by hepatic artery legation and postoperative chemotherapy. Three patterns of clinical response were discerned. First, a marked decrease in AFP levels was found in five patients following the treatment. Three of these patients are alive, all with lowered or normal serum AFP levels at 20, 22, and 60 weeks after operation. Second, two patients displayed only a transient decrease in AFP levels followed by a gradual increase. Third, there was a continuous increase in AFP levels in one patient who showed no clinical improvement. The serum AFP levels in each case appeared to correlate with the prognosis of these patients. Thus, serial measurements of AFP levels may provide an index to assess the clinical result of hepatic artery ligation in patients affected with hepatoma.

Successful treatment of early cancer of the liver and portal hypertension in patients presenting with bleeding oesophageal varices.

A small hepatoma was diagnosed by selective arteriography in 2 patients admitted for treatment of bleeding oesophageal varices. In both cases, hepatoma and variceal lesions were successfully treated. In the first case, left gastric vena caval shunt and local excision of the hepatoma were performed. In the second case, distal splenorenal shunt and local excision of the tumour were carried out. Although a combination of hepatoma and oesophageal varices may be an embarrassing problem, both lesions can be remedied provided that hepatoma is diagnosed at an early stage. It is stressed that arteriography is of value in assessment.
