Successful one-stage operation for completely obstructive colorectal carcinoma.

We describe herein a new and successful method of performing a safe and steady one-stage operation for completely obstructive colorectal carcinoma. First, a long ileus tube is utilized to decompress the dilated proximal bowel preoperatively and irrigate the feces-loaded colon intraoperatively. Following this procedure, a standard resection with radical lymph node dissection is carried out without a diverting colostomy. We performed this procedure successfully in seven patients, none of whom developed any anastomotic leakage.

Mastopathy of the accessory breast in the bilateral axillary regions occurring concurrently with advanced breast cancer.

We herein report a 41-year-old Japanese woman who demonstrated advanced cancer in the left breast occurring concurrently with mastopathy of the accessory breast tissue in the bilateral axillary regions, which appeared to be metastatic lymphadenopathy. A preoperative examination, including a mammogram, US, and CT, did not provide us with a definite diagnosis of the axillary masses: it was essential to diagnose the masses preoperatively since a bilateral mastectomy with nodal dissection is called for if the right axillary masses are metastatic from a cancer in the right breast. An intraoperative cytological examination from the bilateral axillary masses revealed adenosis with fibrocystic changes in the accessory breast tissue. We therefore performed a modified radical mastectomy only on the left side. The patient was thus saved from an unnecessary mastectomy of the right breast. Based on our experience, we wish to emphasize that the accessory breast tissue should be considered for a differential diagnosis when evaluating the axillary masses in order to avoid over-surgery, especially when a patient has been diagnosed to have massive breast cancer. An intraoperative cytological examination is strongly recommended to reach a final diagnosis in such confusing cases.

Pancreatic enlargement in obstructive jaundice. Effects of biliary stream diversion in humans.

To verify the influence of obstructive jaundice on pancreatic growth, the anteroposterior width of the pancreas was measured by computed tomography in 30 cholangiocarcinoma patients excluded patients with distal bile duct tumor (jaundice group) and 74 control subjects. Follow-up examinations were performed on 12 patients with and without internal biliary drainage to elucidate the temporal relationship between pancreatic enlargement and the diversion of the obstructed biliary stream. Histologic analysis on autopsy samples from 13 control and 10 jaundice cases also was performed. Mean pancreatic head and body widths in the jaundice group were 2.93 +/- 0.3 cm and 2.01 +/- 0.3 cm, respectively. These values were significantly greater than those of the controls (2.13 +/- 0.3 cm and 1.49 +/- 0.3 cm, P < 0.01). The glandular widths returned to their normal sizes following internal biliary drainage. No changes were seen in patients who underwent external drainage alone. Histologic examination revealed that enlargement of the acinar cells or of the islet of Langerhans was often seen in the jaundiced patients. Therefore obstructive jaundice is thought to cause pancreatic growth through a trophic effect by interrupting biliary circulation.

Immunohistochemical expression of sialyl Tn and sialyl Lewis(a) antigens in stromal tissue correlates with peritoneal dissemination in stage IV human gastric cancer.

Immunohistochemical expression of sialyl Tn antigen (TKH2 and B72.3), sialyl Lewis(a) antigen (CA19-9) and CEA was examined in 52 cases with Stage IV gastric cancer, the objective being to examine whether or not these tumour-associated antigens show different expression between peritoneal dissemination and liver metastasis. Stromal type for TKH2, B72.3, CA19-9 and CEA were seen in 14 (50%), 13 (46%), 18 (64%) and nine (32%) cases with peritoneal dissemination, respectively, and in four (17%), three (13%), four (17%) and four (17%) cases with liver metastasis, respectively. The stromal type correlated to peritoneal dissemination in TKH2 (P < 0.05) B72.3 (P < 0.05) and CA19-9 (P < 0.05). In other clinicopathological factors, the stromal type for TKH2, B72.3 and CA19-9 correlated to the undifferentiated type. However, even in the undifferentiated type group, the stromal type correlated to peritoneal dissemination in TKH2 (P < 0.05), B72.3 (P < 0.05) and CA19-9 (P < 0.05). Based on these results, the spread of sialyl-rich tumour-associated antigens into the surrounding stroma in the undifferentiated type of gastric cancer may be associated with peritoneal dissemination.

Improvements in diagnosis have changed the incidence of histological types in advanced gastric cancer.

The data on 912 patients with early cancer and 1245 with advanced cancer who were seen between 1971 and 1990 were compared. The incidence of undifferentiated-type cancer increased significantly in patients with advanced gastric cancer, but not in patients with early gastric cancer. When the histological types were compared with regard to sex, age and location in patients with early gastric cancer the undifferentiated type was found to increase only in males, while in patients with advanced gastric cancer the undifferentiated type increased in both sexes as well as in younger patients and in both the upper and middle third of the stomach. These differences in the trends between early and advanced cancers are probably due to the different degrees of diagnostic accuracy for the early detection of histological types.

Mortality in psychiatric patients, with a specific focus on cancer mortality associated with schizophrenia.

BACKGROUND: Higher mortality rates among psychiatric patients compared with the general population have been widely reported. On the other hand, lower cancer mortality for schizophrenics has been occasionally pointed out. Few studies from Japan have investigated mortality among psychiatric patients, and this study is the first large-scale follow-up in this country. METHODS: A total of 4980 patients admitted to a national mental hospital from 1948 through 1982 were followed up until 31 August 1985. The standardized mortality ratios (SMR) were calculated in comparison to the general population, using the person-years method. RESULTS: The SMR for total deaths and those for malignancy were as follows for males/females respectively: 2.55/3.02 and 0.84/1.37 for schizophrenia, 1.76/2.37 and 1.44/2.10 for depression, 2.45/3.04 and 1.18/1.82 for mania, 1.81/1.90 and 0.27/1.07 for neurosis, 5.55/4.33 and 1.85/3.34 for alcohol/drug abuse, and 3.65/3.57 and 1.01/0.72 for organic brain syndrome. CONCLUSIONS: The SMR for total deaths were significantly elevated in schizophrenia, depression, mania, neurosis, alcohol/drug abuse, and organic brain syndrome, respectively. The SMR for malignancy were not elevated nor lowered significantly in any of these disease categories. The SMR for stomach cancer in male schizophrenics was significantly lower (0.27; P < 0.05).

Increased incidence of undifferentiated type of gastric cancer with tumor progression in 912 patients with early gastric cancer and 1245 with advanced gastric cancer.

BACKGROUND: There is little information concerning alteration in histologic differentiation with progression of gastric cancers. METHODS: The authors analyzed data of 912 patients with early gastric cancer and 1245 with advanced gastric cancer. All patients underwent gastric resection. Special focus was placed on the grade of histologic differentiation. RESULTS: The undifferentiated type was found in 38% of early gastric cancers and 58% of advanced gastric cancers (P < 0.01). The undifferentiated type was found in 37% of gastric cancers limited to the mucosal layer, and increased with tumor progression into the deep layer to account for 65% of the cancers that invaded the gastric serosa. With regard to tumor location, the undifferentiated type in the gastric body or other locations was more common in patients with advanced gastric cancer (P < 0.01). CONCLUSIONS: One explanation for the histologic predominance of the undifferentiated type in advanced gastric cancer is that in some gastric cancers, the predominant histologic type may be altered from the differentiated to the undifferentiated type with progression of the tumor.

[Newest advances in clinical phytotherapy in Chinese psychiatry].

Based on a review of the literature contemporary psychiatric practice of herbal medicine in the Chinese People's Republic is re-evaluated. In many articles the results lack methodological precision. In some most trustworthy publications it is stated that for some psychiatric conditions certain herbal drugs are effective and that by combined treatment of modern drugs with phytotherapy a greater effectiveness can be achieved and the time needed for restoration of health can be reduced and the frequency of side-effect is smaller. To confirm the conclusions on the importance of phytotherapy in psychiatry, according to the authors, more elaborate investigations in psychiatry are needed.

[A case of nonresectable gastric cancer treated by sequential methotrexate and 5-fluorouracil].

An advanced gastric cancer patient with multiple retroperitoneal lymph node metastases and bone metastases was treated with sequential MTX and 5-FU. Complete response was obtained against both gastric primary lesion and retroperitoneal lymph nodes observed endoscopically and by computed tomography. Partial response was obtained against bone metastases observed by bone scintigraphy. Side effects of the chemotherapy were not observed.

Intraductal papillary neoplasm of the pancreas.

BACKGROUND: In 1989, Morohoshi et al. reported an intraductal papillary neoplasm of the pancreas (IPNP), which was a morphologically distinct, but rare tumor. METHODS: Two cases with IPNP were analyzed by immunohistochemical and DNA flow cytometric methods. RESULTS: The patients included a 67-year-old man and a 71-year-old woman. Both tumors were characterized by a well-defined papillary growth in the cystically dilated main pancreatic ducts, associated with papillary and nonpapillary hyperplasia. Immunohistochemically, the tumor cells of both cases were positive for the epithelial markers (AE1/AE3 and CAM 5.2), and in one of the two cases, the tumor cells and hyperplastic cells surrounding the tumor conspicuously revealed multiple hormonal markers such as serotonin, somatostatin, glucagon, gastrin, and pancreatic polypeptide. The nuclear DNA content of the tumor cells of the first case, which showed moderate cellular atypia, was considered to be diploid, whereas that of the second case, which revealed severe atypia, was aneuploid. CONCLUSIONS: These results suggested that these tumors arose from multipotential stem cells capable of epithelial and neuroendocrine differentiation, and results of the flow cytometric study was related to the degree of cellular atypia of the tumors.

Carcinoembryonic antigen (CEA) in stage IV gastric cancer as a risk factor for liver metastasis: a univariate and multivariate analysis.

Serum carcinoembryonic antigen (CEA) levels were determined in 68 patients with Stage IV gastric cancers, the objective being to examine the clinicopathological relationship between the metastatic patterns of gastric cancer and the serum CEA level. Of the 68 patients, 31 were diagnosed as cases of liver metastases and 37 as cases of peritoneal dissemination. Serum CEA levels were elevated in 21 of 31 patients (67.8%) with liver metastases and in 7 of 37 patients (18.9%) with peritoneal dissemination (P < 0.01). A univariate analysis showed that liver metastasis correlated with a young age (P < 0.01), the lower portion of stomach (P < 0.05), Borrmann types 1 and 2 (P < 0.01), differentiated type (P < 0.01), and nonserosal involvement (P < 0.05) more than did peritoneal dissemination. A multivariate analysis showed that in addition to Borrmann type 1 and 2, elevated CEA levels (> 2.5 ng/ml) is an independent risk factor involved in liver metastasis. Thus careful follow-up and postoperative adjuvant therapy are required for patients with elevated CEA levels, even with "curative" resection.

[A case of solitary plasmacytoma that originated in a rib].

A 52-year-old woman was admitted to our hospital with the chief complaint of left anterior chest pain. A chest X-ray showed a tumor shadow in the left 4th rib. No abnormalities were detected in peripheral blood and aspirated bone marrow samples. The tumor was surgically resected with the involved bone, the intercostal muscle and the parietal pleura. The resected specimen contained a 3 x 6 x 3 cm lesion, and histological investigation of the tumor revealed a plasmacytoma of the rib. It relapsed as a multiple myeloma within two months after surgery. Chemotherapy was administered and the bone lesions were observed to disappear. We believe that the best way to treat solitary plasmacytoma is to first resect the solitary tumor, and then to add cobalt irradiation and chemotherapy after surgery.

Relevance of ethology to psychiatry.

The interactions between ethology and psychiatry were reviewed. The ethological methods have provided more objective and theory-free observation techniques than the traditional psychiatric methods. The evolution-based ethological viewpoints afford some fundamentally new and more comprehensive approaches in understanding psychiatric conditions. The ethological theory has many concepts in common with various other schools of psychiatry, and thus is expected to act as a bridge to integrate them to lead to a more comprehensive psychiatric theory system. Ethology has not explored its potential in therapy, but may be useful, especially in terms of prevention and social psychiatry.

Comparison of R1 and R2 gastrectomy for gastric cancer in patients over 80 years of age.

To evaluate the benefit of R2 gastrectomy for gastric cancer in Japanese patients over 80 years of age, data on 93 patients who underwent gastrectomy between 1967 and 1989 were collected retrospectively from charts filed in the Kyushu University Hospital and two affiliated national hospitals. Of 93 patients, 62 had a localized tumor without evidence of metastatic spread. These 62 patients were classified into two groups according to the procedures performed: 35 underwent R1 gastrectomy and 27, R2 gastrectomy. The clinical and pathological characteristics of the patients in the two groups were comparable at the time of surgery, except that the group who underwent R1 gastrectomy was older. R2 gastrectomy involved a significantly longer operation time (P less than 0.01) and greater intraoperative blood loss (P less than 0.05) when compared to R1 gastrectomy, but no patient undergoing this extensive surgery died. The difference in the morbidity rate between the two groups was not statistically significant. The 5-year survival rate was 55.8% in the R1 group and 65.4% in the R2 group. When gastric cancer invaded the serosa and/or secondary nodes, a substantial increase in survival time was gained with R2 gastrectomy when compared to an R1 operation. These findings suggest that an R2 gastrectomy is feasible, even for patients over 80 years, but is indicated mainly when the carcinoma had invaded the serosa and/or secondary nodes and the patients are at good risk for major surgery.

The current clinical practice of herbal medicine in psychiatry in mainland China: a review of literature.

The current clinical psychiatric practice of herbal medicine in the People's Republic of China was explored by reviewing the literature. The results found in many of the articles were lacking methodological strictness. Some reliable articles reported that certain herbal medicines were effective for psychiatric conditions, and that a combination treatment of modern drugs with herbs was useful for the enhancement of the efficacy and the reduction of both recovery time and side effects. It is suggested that more sophisticated investigations are necessary to corroborate any conclusions concerning the value for herbal medicine in the psychiatric field.
