Real-time scanning was applied to the sonically guided puncture technique. A special puncture attachment connected to the ultrasound sector scanner was developed for this purpose. A puncture needle is introduced into the scanning plane through the canal in the attachment. Excellent real-time images of both the target lesion and the needle can be clearly seen on the oscilloscope during the puncture procedure. Puncture not only for cystic structures but also for solid masses can be made easily and safely by this new technique. Successful applications of the technique have been carried out for renal cyst puncture, percutaneous antegrade pyelography, percutaneous nephrostomy, percutaneous renal biopsy, and perineal prostatic biopsy.

Topical instillation of Adriamycin, 40 mg in 20 ml sterilized distilled water, was performed in 20 cases of bladder tumors every day for two weeks. In 8 cases (40%) the tumors disappeared completely, while in 5 cases (25%) tumors were reduced in size, but in 7 cases (35%) there was no effect. Therefore, the rate of effectiveness was 65%. The local bladder reaction and urethral pain were noted in all cases, as a result of which the therapy was interrupted in 2 cases. Abnormal values of red blood cells, white blood cells and platelets were not observed. In 3 cases the serum level of Adriamycin was only trace 1 or 2 hr after instillation. In conclusion, Mitomycin C was thought to be superior to Adriamycin as far as the effectiveness and irritability on the normal bladder epithelium were concerned.

Antitumor activity and lethality of cyclophosphamide alone and in combination with several drugs were investigated in male ddY mice. The antitumor activity was estimated by weighing the solid tumor on the 15th day after Ehrlich ascites cell inoculation. Pentobarbital induced sleeping time for monitoring the activity of hepatic drug-metabolizing enzymes was defined as the time between the loss and the recovery of the righting reflex. Consecutive administration of pentobarbital shortened the pentobarbital sleeping time and increased the antitumor activity after cyclophosphamide. On the contrary, a single administration of SKF 525A or cycloheximide prolonged the pentobarbital sleeping time significantly and decreased the antitumor activity after cyclophosphamide. Consecutive administration of aminopyrine, or chlorpromazine shortened the pentobarbital sleeping time and increased the antitumor activity after cyclophosphamide. These results indicate that aminopyrine and chlorpromazine may increase the levels of the hepatic drug-metabolizing components and may activate cyclophosphamide by conversion to an active form. Effect of a consecutive administration of morphine on the pentobarbital sleeping time and the antitumor activity was uncertain in individual cases. On the other hand, aminopyrine, chlorpromazine, or morphine in consecutive administration increased the lethality of cyclophosphamide.

A special chair-type apparatus for transrectal ultrasonotomography was developed in our laboratory 3 years ago. We did a mass screening program for prostatic disease, using this equipment as the primary study in the system. The 132 apparently normal men evaluated ranged in age from 40 to 76 years (mean 55.0). The 48 patients suspected of having prostatic disease underwent secondary studies, consisting of the usual urological examinations. Prostatic disease was detected in 24 cases, including 18 cases of benign prostatic hyperplasia. Thus, the model program revealed an extraordinarily high prevalence of latent prostatic disease among apparently normal older men.
To elucidate the mechanism of the reduction of embryonic loss due to an increase of dietary protein level in rats fed a high energy diet, the relationship between embryonic loss and progesterone metabolism was investigated in rats fed high energy diets (digestible energy: about 5 kcal/g) differing mainly in casein and starch contents. The embryonic loss was minimum in rats fed a relatively high protein-low starch diet (30% protein-19% starch) and maximum in rats fed a very high protein-very low starch diet (39.9% protein-2.5% starch). On the basis of measuring urinary pregnanediol and in vivo conversion from progesterone to urinary pregnanediol and in vivo conversion from progesterone to urinary pregnanediol in each group, the production of progesterone in the body was shown to vary. In vitro studies showed that the mechanism responsible may involve inhibition of progesterone synthesis by the ovary in the presence of a very high protein-very low starch diet.

Our brief experience of the use of ultrasonography and ultrasonic Doppler method for the diagnosis of intrascrotal contents were described. Five cases of testicular tumor, two cases of hydrocele and one case of epididymal tuberculosis were successfully diagnosed by ultrasonography. Only one case of chronic inflammation was misdiagnosed as a testicular tumor. Two cases of testicular torsion and two cases of acute epididymitis were easily differentiated by the ultrasonic Doppler method. We suggest that this is the most reliable technique for differentiation between these two diseases.

A new type of equipment for transrectal ultrasonotomography was described in a previous paper. This equipment was evaluated from the data obtained from 205 cases in which examinations were performed. Tomograms were successfully recorded in 94% of cases. Tomograms sufficiently useful for diagnosis and the measurement of organs were obtained in 89% of cases for the prostate, 78% for the urinary bladder, and 80% for the seminal vesicles. No basic problems in the procedure were found, although hypotension of unknown origin was observed during the examination in a few cases.
We successfully obtained ultrasonic Doppler signals, presumably caused by the renal vessels, through the body surface without invasion. An outline of the method and several results are presented in this preliminary report.
