Patients with hemoptysis examined by Tc-99m sulfur colloid and Tc-99m-labeled red blood cells: a preliminary appraisal.

A prospective study of 16 patients with hemoptysis was conducted over a 2 1/2-year period. Technetium-99m sulfur colloid (Tc-SC) and technetium-99m-labeled red blood cells (Tc-RBC) were sequentially used to identify the general area of bleeding. Tc-SC accurately located the bleeding area in 3 patients and Tc-RBC identified the bleeding area in 8; there were no false-positive studies. Tc-SC scans were positive at 6-10 minutes and all patients had positive Tc-RBC studies. In general, the scans accurately located the source of bleeding as determined by bronchoscopy and/or surgery. Because of the small sample, statistically significant differences between both techniques were not ascertained. Both Tc-SC and Tc-RBC may prove to be helpful, minimally invasive imaging procedures for determining the general area of hemorrhage in patients with clinically significant hemoptysis. Although a trend suggests that Tc-RBC is a more sensitive method, further comparison is needed for corroboration.

Observations of the use of 1 alpha hydroxycholecalciferol in the prevention of bovine parturient paresis.

Varying doses of 1 alpha-hydroxycholecalciferol (1 alpha-HCC) (50, 150, 250 and 350 micrograms) in propylene glycol were injected intramuscularly into 30 dry adult Israeli Friesian cows. Fourteen of these animals received a second dose; four were given 250 or 350 micrograms 48 hours after the first dose and 10 were given 350 micrograms 72 hours after the first dose. Plasma calcium rose after 24 hours at all dose levels except 50 micrograms. A dose-dependent peak in plasma calcium was reached after three to four days, followed by a return to baseline five days (150 micrograms) and eight days (250 and 350 micrograms) post injection respectively. Repeating the injection 48 or 72 hours later increased the time span by three and four days respectively. The effect of plasma inorganic phosphate was double that on plasma calcium. Plasma magnesium declined slightly three days post injection. High calcium feeding in conjunction with one or two injections of 350 micrograms 1 alpha-HCC did not modify the response of plasma calcium. An injection of 350 micrograms of 1 alpha-HCC was given once to 40 parturient paresis-prone cows of the same breed and twice at 72-hour intervals to 37 such cows. Six of the animals received 5 mg of flumethasone together with the second injection and 13 received it 48 hours later. This was to induce parturition, which occurred within 24 to 48 hours. None of the cows injected earlier than 24 hours prepartum developed parturient paresis in comparison with 22 out of 60 control animals which did. The results suggest that 1 alpha-HCC is useful in the prevention of bovine parturient paresis.

[Diagnostic significance of morphological changes in biopsies of the pelvic crest (author's transl)].

Pelvic crests of 265 autopsies were histologically investigated. In nearly all cases with chronic diseases a remodelling of bones and especially processes of bone resorption were found. A fibroosteoclastic bone resorption was evident in 28 of 40 cases with chronic renal failures, in 5 of 26 cases with chronic lung diseases and/or chronic insufficiency of the right heart ventricle, in 4 of 12 cases with chronic diseases of the gastro-intestinal tract, and in a case of lung cancer. Processes of bone formation were less prominent. The findings indicate that several chronic diseases caused by different overlapping pathogenetic mechanisms can principally lead to the same spectrum of alterations of bone structure. This and the aetiological ambiguity of typical patho-histological reaction patterns of bones require a critical reserve in the interpretation of adequate findings in bone biopsies. It has been pointed out that the use of morphometric methods and the use of slices of undecalcified bones are necessary to obtain a reliable diagnostic tool in bone biopsies.

Use of 1alpha-hydroxycholecalciferol in the prevention of bovine parturient paresis.

A synthetic biologically active derivative of vitamin D (350 microgram of 1alpha-hydroxycholecalciferol [1alpha(OH)CC]) was injected into 2 nonlactating 7-year-old Israeli-Friesian cows. Plasma calcium values increased after 24 hours, peaked at 48 hours, and returned to base-line values 120 hours after injection. An injection of 350 microgram of 1alpha(OH)CC was given to 23 parturient-paresis-prone Israeli-Friesian cows from 7 days to 6 hours prepartum; 13 cows were injected once, 6 were injected twice, and 4 were injected 3 times, all at 48-hour intervals. Parturient-paresis-prone cows (n = 23) of the same breed were used as controls. Within 0 to 36 hours postpartum, plasma calcium concentrations were found to be higher in cows injected with 1alpha(OH)CC than in the control cows. The increase was highly significant (P less than 0.01) in cows injected at least twice. None of the cows injected with 1alpha(OH)CC, within 72 to 24 hours prior to calving developed parturient paresis; but 9 of 23 control cows developed parturient paresis. Prior to calving, none of the injected cows developed hypercalcemia and there was no local or systemic clinically detectable signs of toxiosis. When given at the right time prepartum, 1alpha(OH)CC is considered to be an improvement over previous methods of preventing bovine parturient paresis.

Mondini defect in association with multiple congenital anomalies.

A case of bilaterally symmetrical genetic aplasia conforming to Mondini type of congenital deformity in a 12-day-old child is presented with the help of temporal bone sections. Cochlear changes include a stunted modiolus, deficient interscalar septum between the middle and upper coils forming a scala communis cochleae, a degenerated organ of Corti and reduced spiral ganglion cells and dendrites. The vestibule is malformed, with membranous labyrinth being deficient. The utricle and semicircular canals are absent. There is no oval window or stapedial footplate, and the facial nerve is hypoplastic. An interesting feature is the unusual association of bilateral bony choanal atresia, atrial septal defect, cleft lip, absence of olfactory bulbs in the brain, and congenital ophthalmic anomalies.
