Tumour-associated eosinophilia: a review.

In a recent study of cervical carcinoma, 13 cases with a marked eosinophil infiltrate around the tumour were found. The histological appearance of the tumours was distinctive and suggested a specific response, similar to the lymphocyte infiltration in medullary carcinoma of the breast and seminoma. A review of published reports shows that tumour-associated tissue eosinophilia (TATE) and tumour-associated blood eosinophilia (TABE) may be seen in tumours of different histological types from different anatomical sites, and may occur together or separately. Tumours with TATE alone appear to have a better prognosis that those without, while TABE is associated with tumor spread and a poor prognosis.

Tyrosine crystals in salivary gland tumours.

Tyrosine crystals were seen in 24 of 113 salivary gland pleomorphic adenomas from Malawi and one of six carcinomas arising in a pleomorphic adenoma; they were present in tumours from all common anatomical sites. They were not found in 71 other salivary tumours of varying histological type. Similar crystals were found in one out of 56 pleomorphic adenomas from patients operated on at St Thomas's Hospital. No excess tyrosine was demonstrated in a tumour containing many crystals. Their great frequency in black patients is confirmed but an adequate explanation for this phenomenon has not been found.

Salivary gland tumors in Malawi.

A study of 190 tumors of major and minor salivary glands seen in Malawi over a nine-year period showed an overall incidence rate similar to that in Western countries. However, submandibular and palatal tumors are relatively more common, parotid tumors more likely to be malignant, and submandibular tumors less likely to be malignant than in Western countries. No adenolymphomas were seen. Despite the long history given by many patients with pleomorphic adenomas, the rate of malignant change in these tumors was not high; the majority of carcinomas (16% of all tumors) appeared to rise de novo. Histologic features were similar to those of tumors from other countries but the occurrence of tyrosine crystals in 20% of the pleomorphic adenomas was unusual. Melanocyte colonization of a mucoepidermoid tumor and a carcinoma, both in the primary site and in metastases of a salivary gland tumor, has not been previously reported.

Intussusception due to inflammatory fibroid polyps of the ileum: a report of 12 cases from Africa.

Eleven cases of inflammatory fibroid polyp of the ileum from Malawi and 1 from Sierra Leone are described. All 12 patients presented with acute intestinal obstruction as a result of an intussusception caused by the polyp. The lesions are characterized by a variable proliferation of fibroblasts and small vessels which may involve the whole thickness of the bowel wall. Tissue mast cells and eosinophils were noted in the polyps. The aetiology of these lesions remains obscure but they appear to be reactive rather than neoplastic. The condition appears to occur more commonly in Malawi than in the United Kingdom or North America and is an important cause of small bowel obstruction in that part of Africa.

Massive splenomegaly in Northern Zambia. I. Analysis of 344 cases.

Massive splenomegaly is common in Northern Zambia. Nearly 40% of 344 patients were diagnosed as Tropical Splenomegaly Syndrome of malarial origin. Portal hypertension, lymphoreticular and haemopoietic malignancies, and schistosomiasis accounted for another 46%. The common causes of splenomegaly show differences of frequency within Zambia and also between Zambia and other African countries.

Massive splenomegaly in Northern Zambia. II. Schistosomal splenomegaly and elevated IgG.

Twenty-three patients from Northern Zambia with massive splenomegaly due to chronic schistosomiasis were studied. The mean serum IgG levels were found to be extremely high compared with local blood donors and local patients with Tropical Splenomegaly Syndrome (TSS). Mean serum IgG levels also appear to be higher in Zambians with schistosomal splenomegaly than in those reported in patients from most other areas. Mean IgM levels of patients with schistosomal splenomegaly were higher than those of local blood donors, but not as high as in the TSS. IgG and IgM estimations are sometimes of value in the differential diagnosis of splenomegaly in the tropics.

Unusual presentation of patients with myelomatosis in Malawi.

Malignant myeloma is not rare in tropical Africa. Its mode of presentation in Malawi is most often that of a tumour mass in bone. The types of immunoglobulin heavy and light chains in the tumour cells occur with a frequency similar to that observed in temperate countries.

Rhinoscleroma in Malawi and Eastern Zaire.

The first ten cases of rhinoscleroma from Malawi and six from Eastern Zaire are reported. Most of the patients were aged between 20 and 40 years (age range 3-60) and presented with either a nasal "tumour' or abnormalities of the nasal septum. Histological examination of biopsy material revealed features diagnostic of rhinoscleroma - a mixture of mature plasma cells and lareg foamy macrophages (Mikulicz cells) containing intra-cellular Gram negative bacilli. The organisms were best seen in sections stained by the Warthin-Starry technique. This study suggests that rhinosceleroma is perhaps more common in Malawi and Zaire than has been realised.

Prevalence of gall stones at necropsy in nine British towns: a collaborative study.

A prospective study of the prevalence of gall stones at necropsy in nine towns in England and Wales showed considerable geographical variations. The age- and sex-standardised prevalence ranged from 20.6% in Ipswich to 9.2% in Wakefield. The distribution of gall stones differed from that of all-cause mortality and was negatively correlated with that of mortality from ischaemic heart disease. Socioeconomic influences related to affluence do not appear to be major determinants of the distribution of gall stones.

Experimental Trypanosoma cruzi infection in rhesus monkeys 111. Electrocardiographic and histopathological findings.

In five rhesus monkeys surviving 'Peru strain' or 'strain 7' Trypanosoma cruzi infection for six to eight years, positive xenodiagnosis results and high indirect fluorescent antibody titres (4096 - 65536) persisted until the animals were killed. Abnormal electrocardiograph patterns in two monkeys (H and K) were possibly compatible with myocardial damage. Histopathological changes attributable to T. cruzi infection were minor in four monkeys but severe in one (R). In this animal, infected with what was judged previously to be the less virulent of the two T. cruzi stocks used ('strain 7'), there was severe myocarditis, with myofibre degeneration, and lesions of the oesophagus. Elevated serum levels of five enzymes were not detected in any of the chronically infected monkeys.
