An open, randomised, parallel group study of lactulose versus ispaghula in the treatment of chronic constipation in adults.

The efficacy, tolerance and acceptability of lactulose (Duphalac) and ispaghula (Fybogel Orange) were assessed in the treatment of chronic constipation in adults. In an open, prospectively randomised, parallel group study, 124 patients with a history of constipation for more than three weeks were treated with either 15 ml bd of lactulose (increasing to 60 ml daily if necessary) or one sachet bd of ispaghula. Over the four-week treatment period both treatments were shown to be effective, and numbers of concurrent effects were similar between the two groups. Differences were demonstrated with regard to acceptability in favour of lactulose.

Bicarbonates

Ciprofloxacin therapy of experimental endocarditis caused by methicillin-susceptible or methicillin-resistant Staphylococcus aureus.

Ciprofloxacin was more effective (P less than 0.01) than either imipenem or nafcillin therapy of experimental methicillin-susceptible Staphylococcus aureus endocarditis in rabbits after 2 or 3 days of treatment. There was no significant difference between results of treatment of methicillin-susceptible S. aureus experimental endocarditis with ciprofloxacin and results with the combination of nafcillin and gentamicin. Ciprofloxacin was more effective (P less than 0.01) than vancomycin therapy of experimental methicillin-resistant S. aureus endocarditis after 3 days of treatment. After 5 days of treatment, there was no significant difference between the results of treatment of experimental methicillin-resistant S. aureus endocarditis with ciprofloxacin and results with vancomycin.

Animals

Changes in cerebral hemodynamics in preterm infants with and without patent ductus arteriosus.

We compared anterior cerebral pulsatility index (ACPI), anterior cerebral mean flow velocity (ACMFV), common carotid pulsatility index (CPI) and common carotid mean flow velocity (CMFV) in three groups of preterm infants with birthweights less than 1500 grams: 6 without evidence of PDA (group A), 6 with PDA treated with fluid restriction, diuretics or digoxin (group B) and 6 with surgical ligation of PDA (group C). Infants were assessed in three time periods: the first four days of life, five days before surgical ligation and five days post-ligation. Analyses of variance showed no significant differences in the three groups for the four measures in time 1. In time 2, ACPI was 0.61 for group A, 0.58 for group B, 0.78 for group C (p less than 0.01). ACMFV was 9.22 for group A, 7.71 for group B, 6.37 for group C (p less than 0.05). CPI was 0.84 for group A, 0.83 for group B, 0.90 for group C (NS); CMFV was 7.80 for group A, 5.63 for group B, 4.28 for group C (p less than 0.01). In time 3, significant differences (p less than 0.01) were found only for CMFV.

Blood Flow Velocity

Referral recommendations for binocular function and/or developmental perceptual deficiencies.

The complexity of the area of binocular function and perceptual development has resulted in the necessity of intraprofessional referrals. We have divided this specialty into systems. These include the accommodative, vergence, ocular motor, sensory and perceptual systems. In each system we have provided symptoms and clinical findings, which can be utilized for tentative diagnostic purposes. It is incumbent on those practitioners whose interests do not lie in this area to be sufficiently knowledgeable relative to both the patient's difficulty, and to the referral source who might best serve the patient's needs.

Social adjustment and alcoholism among Chinese immigrants in New York City.

Based upon the patient files (N = 132) maintained by the Chinatown Alcoholism Services (CAS), interviews with CAS staff, and the clinical experience of the authors with Chinese alcoholics, this paper describes the social contexts of alcoholism among a group of working-class Chinese males in New York City. It focuses on the relationship between social adjustment and drinking behaviors, and analyzes the meaning of alcohol use and abuse within the ethnic community. The paper also examines the role of significant others in the alcoholics' drinking and help-seeking behaviors. Finally, treatment and research issues pertaining to alcoholism among the Chinese are discussed.
