[Traumatic rupture of the cervical trachea. Emergency diagnosis and treatment].

The authors report 3 cases of traumatic rupture of the cervical trachea including one associated with total division of the oesophagus. They were impressed by the lack of precision and rapidity of the first surgical measures. They then give an account of the therapeutic gestures which are possible in a non-specialised unit.

Pharmacologic regulation of alveolar surfactant in fetal and newborn rabbits: ACTH, heroin, isoxsuprine, and glucocorticoid.

The evidence that pharmacologic doses of glucocorticoids induce lung maturation and diminish risk of respiratory distress syndrome has been reviewed briefly. Studies with heroin corroborate that pharmacologic induction of lung maturation is possible in the human fetus. Endogenous fetal glucocorticoids may serve as a physiologic mechanism that prepares the immature lungs for birth. Catecholamines may induce release of stored surfactant from type 2 cells into alveolar spaces.

[Pre-, per-, and postoperative resuscitation in thoracic surgery in newborn infants].

The authors report their five year-experience in reanimation during thoracic surgery in the new-born. The report concerns 66 cases (43 atresias of the oesophagus and 23 diaphragmatic hernias). Prognosonis depends on: 1 -- A permanent temam of suitably qualified doctors ready to carry out at any time the required therapy on the infant. 2 -- Free air way (kinesitherapy, continous and prolonged suction of oesophageal atresias and bronchial suction). 3 -- Ventilation (surgery on clean lungs in atresias, diaphragmatic prosthesis and suturing of skin in hernias). Respect of these rules in diaphragmatic hernias saved 12 lives out of 23; in oesophageal atresias, 16 survived out of 16 cases classified as good cases, and the total survival rate was 73%.
