[Heroin dependence as a rare cause of symmetrical globus pallidus necrosis].

Long-term survival after carbon monoxide intoxication has been accepted for a long time as pathognomonic for elective cerebral tissue damage, especially in cases of isolated symmetrical necrosis of the globus pallidus. The results of old bilateral necrosis in the globus pallidus are described in a case of an acute heroin death after long-term drug abuse. The evaluation of such cerebral changes is discussed as they have to be interpreted as hypoxic or oligemic tissue damage. The different causes of symmetrical necrosis of the globus pallidus that are relevant for legal medicine are compiled and discussed.

[Assessment of damage to the child in acute CO poisoning in pregnancy].

Diaplacental transition of CO is discussed in two cases of lethal CO intoxication of pregnant women. Foetal CO-Hb has a time lag of several hours against maternal CO-Hb. In highly acute CO intoxications noxious foetal CO intoxication may not occur in spite of high maternal CO concentrations. However, there is a possibility of foetal hypoxia based on maternal hypoxaemia. In cases of subacute CO exposure for several hours, higher CO concentrations may be found in the foetal blood than in the maternal blood. Oxygen therapy of such CO intoxication cases must continue for a certain time after the mother had been detoxicated, because foetal CO elimination lags behind that of the mother. Subacute CO intoxications with sublethal CO-Hb concentrations may cause foetal damage and intra-uterine death as a consequence of CO-Hb, cellular toxic CO-effects and the hypoxia caused by maternal hypoxaemia. Medical decision for interruption of pregnancy as a consequence of foetal indication should be taken after due consideration of the legal problems involved in such action.

[Forensic medicine aspects of fatal coronary insufficiency with normal coronary arteries].

Clinical experiences give examples for the existence of various courses of anginal symptomatology even with cases of sudden heart death demonstrating angiographically normal coronary arteries. Pathogenetically may be considered spasms of regular or little changed coronary arteries, coronary muscle bridges and acute arrhythmias. In cases of recurrent myocardial ischemias an interstitial fibrosis and endocardial fibrosis can be proved histologically in the myocardial supply area. However an acute coronary insufficiency based on rheological and metabolic etiology cannot be found with morphological methods. The results are discussed, considering forensic aspects in cases of competitive causes of death.

[Acute and subacute fetal CO-poisoning].

The main principles of placental CO exchange in cases of CO intoxications of pregnant women are shown. Fetal COHb lags behind maternal COHb by several hours depending on the exposure pattern. During CO elimination the fetal COHb levels again lag behind the mother's. A case of a lethal CO intoxication of a pregnant woman, mens IX, with exposure time of 10-12 h and COHb of 75% for the mother and 46% for the fetus allows to make conclusions about process of intoxication and about different times of death for mother and fetus. In highly acute lethal intoxications of the mother COHb in fetal blood is low or negative. A vital danger of the fetus is not probable when mother survived a short CO exposure.

[Quantitative morphological examinations of coronary muscle bridges (author's transl)].

The coronary muscle bridge which was found in non-selected autopsy material with a frequency of 24% meets with increasing interest in the discussion on the causes of sudden cardiac death and within the scope of coronary bypass-surgery. Twenty morphometrically examined coronary arteries have been used to demonstrate that the coronary muscle bridge provide local protection from atherosclerosis. Pathomorphological and functional effects of coronary muscle bridges are discussed.

[Morphometrical examinations in cases of stenosing coronary sclerosis and their importance for evaluation of competing causes of death (author's transl)].

Coronary sclerosis is generally supposed to be the most important factor for coronary thrombosis, myocardial infarction and coronary heart death. Stenosing coronary sclerosis may be postmortally documented by angiography and morphometry. It is possible to obtain sufficient morphological data to suggest acute cardiac insufficiency, if the maximum grade of stenoses as a functional parameter and the heart weight are regarded in addition to the quantitative results of the three main branches of the coronary arteries (lumen and intima areas). Generally the cardiac results of autopsy are used individually and subjectively for the explanation of the cause of death. The conclusiveness of these results of autopsy depends on the circumstances of death and the existence of further pathological findings. Competing causes of death may exist in the form of illnesses, injuries, alcoholic and drug effects, physical strain and emotional stress or medical provisions. The quantitative valuation of the cardiac findings with a critical limit for an acute coronary death permits a more exact interpretation of such competing causes of death. This method of examination may also reveal an unpresumed competing cause of death, for example an intoxication. This was demonstrated by four autopsy cases.

[Criminal process record Winckelmann (Triest, 1768). Comments on the criminal process dealing with the murder of Johann Joachim Winckelmann from the forensic historical and legal medicine viewpoint].

Johann Joachim Winckelmann, German historian of ancient art and archaeologist, was born on 9 December 1717 in Stendal, a town in Saxony-Anhalt. At the age of 50 he was murdered on 8 June 1768 in a Trieste hotel. The voluminous original record of the criminal proceedings against his murderer, Francesco Arcangeli, was presumed lost for about 150 years. A new edition in the wording of the original text appeared in 1964. This long sought historical document gives cause for forensic-historical reflections under consideration of the autopsy protocol about Winckelmann, which is likewise a historical document. A considerable change of paradigm in comparison to current autopsy protocols is observed with regard to the evaluation of injuries and the circumstances of death.

[Satellite dish as source of electric current. Case report of a fatal and nearly fatal accident at the work site].

The authors report on an initially unrecognized fatal accident caused by electric current at the place of work. The source of the current was a satellite dish which had been improperly connected to an old music box with subsequent connection to the household electrocution system (220 V, 50 Hz). Only when a second nearly fatal accident occurred, proper diagnosis and detection of the uncommon source of the electric current was made.

[Child abuse with fatal sequelae in an Essen autopsy sample].

In the Institute of Forensic Medicine in Essen/Germany 24 cases of lethal child battering or neglect have been observed over 17 years from 1973 to 1989. The medicolegal and morphologic findings in these cases are presented and compared to the literature. The patterns of abuse/neglect in Essen compare to those frequently reported in the literature: skull/brain traumas caused by blunt impact, multiple hematomas, bone fractures, and symptoms of malnutrition, vitamin deficiency, and general neglect. In many cases the abuse could be shown to have been chronic. In the seven years 1983-1989 the cases of lethal child mistreatment and neglect amounted to 0.18 percent of all autopsies. Five cases of lethal sexual assault in children during the same period are compared to the mistreated cases.

[Case report of pathologic and criminal self injury].

A case of morbid self-violence (Münchhausen Syndrome) leading to death and a case of criminal self-violence (cutaneous emphysema) are reported. The issue of such cases is discussed with regard to the consequences concerning judicial procedure of punishment and insurance.

[Irrational behavior in exposure to cold. Paradoxical undressing in hypothermia].

One December morning the naked bodies of two alcoholics were found on an open place in the center of the city of Essen. The phenomenon of paradoxical undressing due to vital hypothermia is described and the significant influence of alcohol on hypothermia and confusion in connection with hypothermia is pointed out. Compared to the results of other authors, external and internal signs of lethal hypothermia were analysed in a retrospective series of 30 cases (autopsy material in the region of Essen, investigation period from 1979-1989).

[The hydrodynamic effect of gunshots in cranial explosive shots].

Case report on five fatal gunshot injuries in which discharge of the cranial contents had occurred owing to a skull blast. Despite an appreciable explosive action with destruction of the bony cranium, the brains were not completely destroyed. In analogy to the "Krönlein shot", comparative reflections were made concerning wound ballistics. The case reports show that injury patterns resembling "Krönlein shot" may arise in cranial blast shots in rare cases despite destroyed projectile parts and even in a mouth shot.

[Fatal violence by adolescents toward children--sexual motivation is in the foreground].

In seven cases, obtained from the autopsy material of two German institutes of forensic medicine (Hamburg and Berlin-East) the circumstances of manslaughter committed by youth versus children are presented. There is a tendency of sexual background in such an offender-victim-constellation. The cause of death was strangulation in most cases; the perpetrators intended to conceal the sexual misuse. Such cases are very rare; epidemiological conclusions concerning escalating violence of the youth cannot be drawn.
