[Injury pattern and identification after airplane catastrophies. Cooperation between forensic medicine and federal criminal investigations. An airplane accident in Mühlheim/Ruhr 8 February 1988].

On February 8th 1988, a two-motor passenger aircraft of Metroliner type with 21 people on board entered a front of heavy weather at an altitude of 900 m and crashed after being struck by lightning which led to complete breakdown of the electrical systems on board. The site of the crash was in the marshy Ruhr meadows. The formation of the terrain enabled a subdivision into plan squares for rescue. The identification of the 21 bodies was carried out in the Essen Institute of Forensic Medicine in collaboration with the identification commission of the Federal Criminal Investigation Office. The experience and recommendations for future (possibly larger-scale) disasters derived from this are described. Furthermore, the accident pattern in the casualties typical for this air crash is discussed.

[Agonal and postmortem movement sequences in hanging].

The agonal and supravital movement sequences documented are evaluated on the basis of a case of hanging which was recorded with a video camera. This is evidently an autoerotic accident which was initially misinterpreted by the police as suicidal hanging. The process of hanging documented in the film allows inferences with regard to the genesis of concomitant injuries during the various movement phases. In contrast to observations communicated so far in the literature, the inspiratory phase has particular significance. Moreover, the time course underscores the well-known loss of consciousness within seconds.

[Quantitative morphologic studies of so-called dynamic, eccentric coronary stenoses in cases of acute coronary death].

The residual normal wall segment of an eccentric stenosis of the coronary artery can reduce the residual lumen by spastic contraction of the media. Such dynamic coronary artery stenoses may consequently cause or intensify myocardial ischemia or may lead to sudden coronary death due to dysrhythmia. In 39 cases of sudden coronary death, the histological cross section of extramural coronary arteries were measured by planimetry after postmortem coronary angiography. Of a total of 382 stenoses wit more than 45% narrowing, 178 (48.6%) were of the eccentric kind. These eccentric stenoses of the coronary artery were localized in the RIVA in 48%. On average, 4.5 eccentric stenoses were found in each case of coronary death. Eccentric stenoses with round (49.4) or elliptically situated (35,3%) restenoses was on average 30.5% of the residual lumen wall. The quantitative findings show that the high percentage of eccentric stenoses with a larger normal wall segment is of functional significance with regard to acute coronary deficiency. The morphological description and evaluation of the coronary sclerosis should also take into consideration the kind of stenosis.

[Hemosiderin findings in the liver, spleen and lung in newborn infants and infants].

The content and distribution of hemosiderin in the liver, spleen and lungs of stillborn children, newborns and infants were determined histologically by Perl's method (Prussian blue reaction). We describe a study of 59 deaths up to the age of 2 years. The hemosiderin content depends on age and decreases with increasing age. No relevant differences were found in cases of sudden infant death syndrome (SIDS). In the liver hemosiderin is localized preponderantly in the periphery of the lobules. The spleen shows a diffuse siderosis of the red pulp. Minimal intrapleural, septal and perivasal depositions of hemosiderin can be found in the lung tissue. The significance of these findings is discussed.

[Lipomatosis cordis as a cause of ventricular perforation in implantation of a pacemaker probe].

A case report is given on lethal perforation of the right ventricle during emergency implantation of a pacemaker probe. The forensic autopsy revealed extensive lipomatous infiltration of the right ventricle in the rupture area. Lethal complications occur occasionally in spite of the standardization of implantation techniques, especially as a result of reanimation attempts. Such cases are discussed with regard to the forensic aspects with reference to the literature. Lipomatosis cordis as "locus minoris resistentiae" has special significance for ventricular perforation.

[New diagnostic possibilities of postmortam coronary angiography by digital subtraction angiography].

Postmortal digital subtraction angiography permits vessels filled with a contrast agent to be visualized selectively in a video film shot and the contrast agent flow to be observed directly in the coronary arteries. Compared to conventional static methods of postmortal coronary angiography, the judgment on the hemodynamic effects of stenosis and occlusion, as well as of collaterals and anastomosis, will be improved and therefore the diagnosis of acute coronary death. The diagnostic possibilities and advantages of DSA are demonstrated in two cases.

[A rare cause of 2 unclear fatalities: undiagnosed acute and chronic amebiasis].

Two cases are reported of unexpected and unclarified death caused by an infection with Entamoeba histolytica. The first case concerned a German whose disease was not diagnosed during the asymptomatic phase (2 years' duration) or in the acute phase during the last 3 weeks before death. This is typical of the disease. In the final phase, he developed a hepatic abscess. Death occurred as a consequence of bilateral apoplexia in the adrenal glands. In the second case a Turk fell ill with gastralgia and diarrhea. Developing hepatic insufficiency with comsumptive coagulopathy, he died after 6 days. Intoxication and/or intolerance of fructose were assumed as tentative diagnoses. These two cases show clearly that even outside an endemic area, one has to reckon with the possibility of amebiasis.

[Comparative histologic and hormonal studies of the thyroid gland with special reference to sudden infant death (SIDS)].

In 53 cases of death - including 12 cases of sudden infant death syndrome (SIDS) - where blood samples could be taken within 18 h postmortem, the thyroxines T4, FT4, T3 and FT3 were determined (ELISA and RIA). These hormone values were compared with the corresponding histological thyroid findings in 43 cases (11 SIDS, 32 controls). Nearly identical T4 and FT4 mean values were found in both groups which were within the norms. In contrast to the average values of the control group, the T3 and FT3 concentrations of the SIDS group showed an increase of 3.7-fold and 1.9-fold. Accordingly, histological examination of the SIDS group showed highly activated and extensively released follicles whereas normal colloidal-containing follicle structures were observed in nearly all control cases. The present findings indicate that neither postmortem T4 T3 conversion nor intensified agonal hormone secretion is likely to be the only cause of the increasing T3 and FT3 values. In SIDS cases chronic or recurring chronic stress situations are supposed to be the cause for the hormonal and histological thyroid findings. Some differential diagnoses are discussed. Within 18 h after death, increased concentrations of T3 and FT3, together with simultaneous colloid release, represent a diagnosis of SIDS.

[Progressive muscular dystrophy of the Duchenne type].

A case of sudden death is reported that occurred as a result of Duchenne type progressive muscular dystrophy with extreme myogenic changes. These changes consisted of extensive muscular lipomatous transformation, typical of the advanced stage of the disease. The heart showed myocardial fibrosis, the respiratory system acute tracheobronchitis with early signs of bronchopneumonia. The pattern of distribution of the changes in the musculature is discussed with regard to the sudden death as well as to the possibility that medical malpractice may have occurred.

[Coronary muscle bridge and its relations to local coronary sclerosis, regional myocardial ischemia and coronary spasm. A morphometric study].

Morphometrical investigations on 35 coronary arteries with muscle bridges prove their local protection from atherosclerosis. The average values of the intima thickness in the artery proximally situated to the coronary muscle bridge are 406.6 microns, in the region of the bridge 66.3 microns and distal to the bridge 103.4 microns. The correspondent values of the media are not significant. Stenoses situated proximally to the coronary muscle bridges are almost completely of the eccentric kind and must be considered as potentially dynamic stenoses. The morphological and angiographical findings in cases of coronary muscle bridges, together with clinical observations, indicate a connection between coronary muscle bridges and coronary spasm, at least under pathophysiological conditions.

[Histologic thyroid gland findings in the newborn infant and infant, with special reference to sudden infant death].

The morphological picture of the thyroid gland, the only endocrine organ with a follicle structure, allows a limited conclusion to be drawn with respect to the functional state in spite of any physiological variability. The thyroid of the newborn shows total colloid release and collapse of the follicles. The regular structure of the thyroid will be rebuilt within several weeks after birth. Total colloid absorption can be found in cases of stress-activated thyroids as well as in cases of death due to freezing. A histological examination was done on 88 thyroids of fetuses, newborns and infants. About 70% of the results on 27 cases of sudden infant death syndrome (SIDS) may be interpreted as a morphological correlate of a premortal chronic or recurrent stress reaction. The value of these results is discussed.
