[Computer tomographic appearances of intra-abdominal abscesses (author's transl)].

CT diagnosis of intraabdominal abscess was confirmed in 18 patients by laparatomy and in two by autopsy. In CT abscess is characterized by an expansive soft-tissue lesion with different density values, mostly with a central fluid collection and a thickened wall. The peripheral inflammatory reaction leads to obliteration of fat planes surrounding the adjacent organs. Further symptoms are contrast enhancement of the wall and extraluminal gas collection. The combination of all these signs together with a suitable history and clinical findings allow a reliable diagnosis of abscess formation. When only isolated signs are found, differentiation from tumor, pseudocyst or hematoma may be difficult.

[Endoscopic retrograde cholangiography (ERC) in congenital cystic biliary anomalies (author's transl)].

Cystic anomalies of the bile duct are rare conditions found mostly in the younger age groups and four times more frequently in women than in men. The introduction of retrograde contrast filling of the bile ducts has led to a marked improvement in the pre-operative diagnosis. Precise demonstration of the anatomical relationships is of the utmost importance for surgical intervention. A number of variants of cystic bile duct anomalies must be differentiated from double gall bladders, bile duct fistulas, pancreatic cysts and pseudocysts; small cysts (type IV) must be distinguished from the autoimmune form of cholangitis-pericholangitis and from acquired generalised bile duct dilatation. The prognosis of cystic biliary anomalies is determined by the complications of these conditions. These include ascending cholangitis, stone formation and malignant change.

[CT and lymphography correlations in the evaluation of patients with testicular tumours (author's transl)].

27 patients with testicular tumours were examined by lymphography and computerized tomography for evaluation of metastatic spread in the retroperitoneal lymphnodes. A good correlation of results was found in 23 patients. In 4 cases lymphography failed to show metastatic disease, which was easily recognized by CT. In account of the lymphatic spread pathways of testicular tumours leading mainly to the lymphnodes close to the kidney pedicle CT appears to be method of choice in the staging of these patients, as the lymphnodes in this area are often not demonstrated by lymphography. Computerized tomography allows the exact localisation and extension of neoplastic lymphnodes and at the same time a good evaluation of the surrounding organs.

[The value of xerotomography of the lung and mediastinum in routine diagnosis (author's transl)].

Using controlled case material (n = 55), the weighted value of xerotomography is compared with that of conventional film tomography for the examination of mediastinum and lung. The validity of information of both methods for the final diagnosis is compared. In the visualisation of bronchial units and vascular structures, xerotomography is slightly superior to tomography with silver halogenide films, due to high degree of sharpness of detail, great object range and amplification of the marginal areas. Nodular hilar processes which are in spatial relationship to plane, pulmonary infiltrations or are overshadowed by the cardiac shadow, can be better clarified via conventional technique because of the differences in film density. Overshadowing of linear structures can simulate calcifications in the xeroradiographic image. In our opinion, a combination of film tomography in two planes with xerotomography in an optimal sectional plane can be recommended as a suitable procedure.

[The radiological findings in septic Candida arthritis (author's transl)].

There are very few reports of bacteriologically proven Candida arthritis. With the increasing number of cases of immune deficient infants being treated now, one must expect an increase in frequency of fungus infections, including Candida arthritis. We have observed three cases of fungus arthritis in infants with reduced resistance. The radiological changes were indistinguishable from bacterial arthritides. During the acute phase the accumulation of pus and exudate showed widening of the joint space on the radiograph, with increase of the peri-articular soft tissues. In two patients a central bone abscess was seen in the metaphysis which later showed a sclerotic margin. In one patient with a gonarthritis there was complete healing, but in two fungus infections of the hip there was persistent late demage. One patient developed a deformed hip joint with subluxation, which had to be treated surgically after ineffective medical treatment. In the other case there was considerable flattening of the femoral head.

[Carotid-cavernous fistula. Diagnosis by angiography and scintillation camera (author's transl)].

Four patients with post-traumatic and one with a spontaneous carotid-cavernous fistula are described. Perfusion scintigraphy made the diagnosis in three patients and in the other two strongly suggested it. Exposures in ventral and lateral projections are able to make the correct diagnosis. Sequential scintigraphy is also valuable for post-operative follow-up. Cerebral angiography can localise the fistula and the adequacy of the circle of Willis can be determined pre-operatively. In addition to the major venous flow from the cavernous sinus into the superior ophthalmic vein, other characteristic pathways may be demonstrated. Knowledge of the pathological and anatomical situation is a prerequisite for treatment planning.

[Transvenous xerotom-angiography of the pulmonary hilus (author's transl)].

Normal-size lymph-nodes and air-filled bronchi do not contribute significantly to the shadow of the normal hilus. This consists mainly in pulmonary arteries and veins of the upper lobes. Any enlargement beyond the vascular shadow has to be considered as pathologic. Xerotom-angiography following injection of contrast medium into the cubital vein shows the pulmonary vessels near the hilus more clearly. Pulmonary arteries towards the truncus are better shown. This improves the clarity of the hilus. Coincident demonstration of the superior vena cava may show pathologic processes in the upper mediastinum.

[The effect of natural and synthetic secretin on insulin secretion (author's transl)].

The effect of natural secretin (G.I.H.) and synthetic secretin (Wünsch) on the secretion of insulin was investigated. Natural as well as synthetic secretin causes a short uniphasic release of insulin without influence on the concentration of glucose in the serum. Since the insulin releasing effect on synthetic secretin was demonstrated, it is clear, that the release of insulin by preparations of natural secretin mentioned in medical literature is not due to contamination by other peptides.
