Cyclical changes in nasal airway resistance and middle ear pressures.

The relationship between unilateral changes in nasal airway resistance and the ipsilateral middle ear pressure were investigated in 8 otologically and rhinologically normal adults over the course of 7 1/2 h under laboratory conditions. Despite mean changes in unilateral nasal resistance of 0.75 Pa/cm3/s associated with the nasal cycle, no correlation existed with changes in ipsilateral Eustachian tube function, as judged by serial middle ear pressure recordings (r = 0.06, r2 = 0.00, p = 0.461). Middle ear pressure in healthy adults in a controlled environment remained very constant (mean = -6.71 +/- 0.52 daPa). 69% of middle ear pressures were equal to or above 0 daPa, which represents evidence in favour of a net positive production of middle ear gas.

Neurological manifestations of Lyme disease.

Neurological disorder may be the initial manifestation of Lyme disease. Six cases of neurological Lyme disease have been seen in the years 1986-89, five of whom contracted the disease in the West of Ireland. Three presented with a radiculoneuropathy, one with myalgia/fatigue and one with bilateral sixth nerve palsies and ataxia. These cases indicate the spectrum of neurological involvement of Lyme disease in Ireland which reflects that seen in Europe. They also highlight some of the problems in diagnosis which sometimes necessitate treatment while awaiting serological studies. We feel even in the absence of a history of tick-bite or rash, Lyme disease should be considered in the differential diagnosis of many neurological disorders, especially in patients from the West of Ireland.

Scanning electron microscopy of the jejunal and ileal mucosa of rabbits infected with Trichostrongylus colubriformis.

The jejunum and ileum of rabbits infected by immature stages of Trichostrongylus colubriformis were examined with the scanning electron microscope. In the heavily parasitised jejunum, the normally leaf-shaped villi often showed abnormal shapes. Surface epithelial cells became rounded and dome-shaped, and apical microvilli appeared irregular. In the ileum, where the density of worms was much lower than in the jejunum, the main difference observed was the irregular arrangement of the microvilli. The changes observed in rabbits parasitised with immature T colubriformis were compared with previous scanning electron microscope observations of lambs and rats infected with adult intestinal nematodes and the origin and functional significance of the lesions were discussed.

The use of postoperative suction drainage in total knee arthroplasty.

A retrospective review of 299 total knee arthroplasties performed between 1973 and 1983 revealed 170 knees in which postoperative suction drainage was used and 129 knees in which drains were not used. Comparison between these two groups revealed no statistically significant difference in wound problems, postoperative temperatures, or resulting range of motion. However, blood transfusions were given more than twice as often to the patients whose knees were drained (39% compared to 16%, p less than .01). The drained group also had a greater decrease in hemoglobin than the nondrained group (3.1 gm compared to 2.6 gm, p less than .01). In a review of these patients, no advantage was found for the use of postoperative suction drainage in the uncomplicated total knee arthroplasty.

Tartrate-resistant acid phosphatase staining of monocytes in Gaucher disease.

Cytochemical studies were performed on peripheral blood from 30 patients with type 1 Gaucher disease. In 29 of the patients, peripheral blood monocytes stained positively for tartrate-resistant acid phosphatase, whereas monocytes from 18 normal individuals and 14 patients with monocytosis did not. In the Gaucher patients, the percentage of monocyte positivity for tartrate-resistant acid phosphatase ranged from 2 to 97. There was no correlation between the percent monocyte staining and the degree of disease severity, as measured by hepatosplenomegaly, pancytopenia, or extent of bone disease, for the group as a whole. In Gaucher patients who had not undergone splenectomy, however, there was a significant correlation between percent monocyte staining and the degree of hepatosplenomegaly, anemia, and thrombocytopenia. The presence of tartrate-resistant acid phosphatase may be secondary to the lysosomal accumulation of glucosyl ceramide within these monocytes, although this remains to be confirmed. If so, these circulating cells may represent precursors of the Gaucher cells in tissues. Tartrate-resistant acid phosphatase staining of peripheral blood monocytes may be useful as a diagnostic marker for Gaucher type 1 disease and for further studies on the pathogenesis of the disease.

Spiritual and religious concerns of the hospitalized adolescent.

One of the tasks of adolescence is the development of a value system. The way in which these values are expressed in relationship to illness in adolescence, however, has not been studied. The authors conducted a social science survey to study the spiritual concerns of hospitalized adolescents. One hundred and fourteen hospitalized adolescents, ranging in age from 11 to 19, answered a Likert scale questionnaire on Spiritual and Religious Concerns (SRQ). Analysis of variance was performed to correlate responses with sex, race, religion, type of school, and severity of illness. A subgroup of hospitalized adolescents, those with more serious disease, experienced intensified spiritual and religious concerns. In response to the questionnaire, over 15% of the patients requested further help. Our findings suggest that training in adolescent health care and the provision of services to teenagers ought to include teaching in the area of spiritual and religious values of teenagers, with emphasis on the hospitalized adolescent.

Beta-adrenergic and muscarinic cholinergic receptors: overlap in their genetic determination.

Significant positive correlation between the ligand binding values for beta-adrenergic receptors and those for muscarinic cholinergic receptors was found in five inbred strains of mice (r = 0.84) and in different vertebrate species (r = 0.99). Comparative analysis of analogous receptor studies done on various brain regions in other laboratories revealed high positive correlation between regional binding values for these two receptors. In conjunction with the work presented here, an overlap in the genetic determination of beta-adrenergic and muscarinic cholinergic receptors is suggested.

Use of isotope bone scans and skeletal survey X-rays in the follow-up of patients with thyroid carcinoma.

We have evaluated the utility of isotope bone scans and skeletal survey X-rays in the periodic evaluation of patients known to have had thyroid carcinoma. Patient data were evaluated in 29 patients with known metastatic disease and 79 patients without known metastatic disease who were followed over a period of 3-12 years and who had coincident 131Iodide whole body scans, technetium pyrophosphate bone scans, chest X-rays, and skeletal surveys, at periodic intervals. The information gained for each patient, at a time when two or three of the major survey procedures were conducted coincidentally, was evaluated retrospectively for its impact on classification and management of the patients. Metastatic tumor was detected in most instances by 131I RAIU in the lungs or by chest X-rays. Six patients had proven bone metastasis. Four of these patients had positive 131Iodide RAIU in bones, and two were positive and two negative by pyrophosphate bone scan. Only one was positive by skeletal survey X-rays at the time metastases were first detected by bone scans. Nine of the patients with metastatic disease displayed abnormal bone scans, but six of these were determined to be false positives. Bone scans detected one poorly differentiated metastatic tumor which did not accumulate 131I. Bone scans were more sensitive than skeletal survey X-rays. Physical exam, chest X-ray, and I131 whole body scans detected all instances of metastatic carcinoma.(ABSTRACT TRUNCATED AT 250 WORDS)

Renal cadmium content in the West of Scotland.

The trace element content of kidneys has been documented in very few international centres. Significant differences can be demonstrated between the cortical and medullary levels of calcium, zinc and cadmium. For the West of Scotland the values for the latter are similar to those found in Scandinavia. There are significantly higher cadmium contents in subjects who smoke and who have evidence of ischaemic heart disease.

The properties of human fetal bone marrow stromal and hemopoietic cells.

The capacity of fetal bone marrow to form stromal cell colonies, granulocyte-macrophage colonies, stromal cell monolayers and to produce granulocyte-macrophage colony stimulating activity from these monolayers was evaluated in comparison to adult bone marrow. Granulocyte-macrophage colony formation on routine feeder layers was approximately equal in both cases. The fetal bone marrow colonies were larger in size than those from adult precursors. There were at least ten times more stromal cell precursors in fetal bone marrow than in adult bone marrow. The fetal bone marrow stromal cell monolayer formed within 7-12 days whereas adult stromal monolayer formation required 3-4 weeks. Fetal bone marrow stromal cell monolayer produced granulocyte-macrophage colony-stimulating activity (GM-CSA) effective only for fetal GM precursors, whereas adult bone marrow produces GM-CSA effective for fetal and adult normal and CML bone marrow.
