Glucose and ketone bodies production in hepatocytes isolated from fetuses at term--I. Effect of maternal fasting.

The effect produced by maternal fasting on glucose and ketone bodies production has been studied in hepatocytes isolated from fetal rat. Maternal fasting produces a decrease in the weight of fetal liver. Maternal fasting produces a decrease in glucose production, both from endogenous substrates and adding lactate (10 mM) to the incubation medium. Maternal fasting produces an increase in ketone bodies production, both from endogenous substrates and adding acetate (5 mM) to the incubation medium.

Glucose and ketone bodies production in hepatocytes isolated from fetuses at term--II. Effect of maternal adrenalectomy.

Glucose and ketone bodies production has been studied in hepatocytes isolated from fetuses at term of fed and fasted adrenalectomized mothers. Maternal adrenalectomy diminishes the fetal liver weight. This effect is increased when the adrenalectomized pregnant rat is fasted for the last 2 days of gestation. Maternal adrenalectomy diminishes glucose production in hepatocytes isolated from fetuses at term. This diminution is markedly greater when the adrenalectomized pregnant rat is fasted for the last 48 hr of gestation. Maternal adrenalectomy diminishes ketone bodies production in hepatocytes isolated from fetuses at term.

Laboratory control of continuous intravenous heparin therapy with Howel time and activated partial thromboplastin time. A prospective, randomized and blind study.

Although heparin has been used extensively to treat Deep Venous Thrombosis (DVT) and arterial ischemia (AI), controversy still exists regarding optimal dosage and the need for monitoring. Different authors have employed various test with variable results. Others, however, persist in giving heparin without laboratory control. This study was made in order to compare, in a prospective, randomized and blind manner, two coagulation tests, namely: Howel Time (HT) and Activated Partial Thromboplastin Time (APTT), in controlling the dose of heparin given by continuous intravenous infusion in DVT and AI. Our results show no significant difference in complications and failures of the therapy with either test, although significantly higher doses of heparin were needed to maintain APTT within therapeutic range than those needed to keep HT within a similar range.

Standard and higher doses of bromocriptine for severe chronic portal-systemic encephalopathy.

In a single-blind, cross-over fashion we assessed the therapeutic value of the dopamine agonist bromocriptine at doses of 15 and 20 mg/day plus cleansing enemas, versus the vigorous standard treatment of neomycin 6 g/day plus cleansing enemas. We studied four patients with severe chronic portal systemic encephalopathy. All the patients presented elevated prolactin levels. During the study the following parameters were assessed: mental state, number connection test times, frequency of asterixis, EEG, blood ammonia, and serum prolactin levels. In all but one patient, portal systemic encephalopathy parameters improved during the standard therapy, as opposed to both bromocriptine periods. During the period with 30 mg/day of bromocriptine all four patients developed precoma, although later one patient improved. Serum prolactin levels were rapidly suppressed during both bromocriptine periods. Prolactin levels did not correlate with changes in mental state. In patients with severe portal systemic encephalopathy, treatments with 15 and 30 mg of bromocriptine plus cleansing enemas were no better than the standard therapy.

Action of bromazepam on sleep of children with night terrors. I. Sleep organization and heart rate.

Bromazepam was administered in a single dose of 1.5 mg one-half hour before bedtime to study its short-term action and the effect of its discontinuation on the sleep of 6 children suffering from night terrors. On the third night of the drug's administration, a statistically significant reduction in slow-wave sleep was observed which was maintained after the drug had been discontinued. The other sleep parameters were not significantly affected, although a slight increase in REM sleep was seen after discontinuation of bromazepam, with a statistically significant increase in the second third of the night on the first 2 nights 'off medication'. A comparison of the individual nights yielded no significant change in heart rate, although certain internal changes were observed during the second night 'on medication' and during the nights 'off medication'. These changes can be explained by the changing relationship between cardiac variability and sleep organization. There were insufficient episodes of night terror during the short duration of the study to allow any conclusions to be drawn on the effect of the drug on this aspect.

[Pulmonary valve atresia with intact interventricular septum in the grown child and the adult].

Atresia of the pulmonary valve with an intact interventricular septum is a rare cyanotic congenital cardiac malformation. Rare at birth, it is exceptionally rare for cases to survive over 5 years. Three cases of porlonged survival aged eleven nineteen and thirty-four years are reported. In the latter case, the abnormality was probably aquired. Prolonged survival is related to a collateral pulmonary circulation. Surgical treatment is discussed with respect to the anatomical features. Good results were obtained in the two operated cases.
