Histocompatibility antigens and humoral immunity to Epstein-Barr virus.

To explore possible genetic determinants of immunity to Epstein-Barr (EB) virus infection, prevalence rates and geometric mean titers (GMT) of antibody against EB viral capsid antigen (VCA) were measured in 422 adults of known HLA and ABO histocompatibility types. Deviation from the overall prevalence (87%) of anti-VCA titers > 10 was limited to subjects with HLA-A10 (95%) and those with a "blank" HLA-A locus (78%). The latter deviation was largely accounted for by the 13 subjects of HLA-A1/blank constitution (46%). Deviations from the overall mean anti-VCA titer (GMT 131.5) were found in seropositive subjects with blood group 0 (GMT 153.3) and tissue antigens HLA-A3 (GMT 150.8), HLA-B7 (GMT 152.1), HA-Bw15 (GMT 97.0), and HLA-B27 (GMT 86.4). Separate reanalysis of the data for male and female subjects verified the association of blood group 0 with elevated and HLA-B27 with reduced mean levels of anti-VCA antibody.

Metaproterenol and theophylline in asthmatic children.

Metaproterenol (M) and theophylline (T) were compared in an eight-week randomized, double-blind, crossover study in 20 moderately severe asthmatic children. All subjects were clinically maintained on both M and T. T was superior when evaluating cough, attacks, extra medicines, and PEFR values. Diary analysis revealed T better in 11, M in six and M/T equal in three subjects. Side effects were more common with T. M might be an adequate alternative to T in the treatment of asthmatic children requiring daily bronchodilators.

A simple, rapid micro-latex fixation test.

A micro-latex fixation test (LFT) for the determination of rheumatoid factor (RF) is presented. Its advantages compared to similar tests are greater precision, simplicity, increased sensitivity, lower cost, reproducibility and adaptibility to large-scale testing. Micro-LFT titres are presented from a wide range of sample populations. The majority of normal samples show measureable titres whereas rheumatoid patients show high titres. A large sampling of pre- and post-transplant sera from kidney patients was studied and the micro-LFT titres were in the range of normal persons. The transplant sera were tested for lymphocytotoxic antibodies and no correlation was observed with the micro-LFT titres.

Effect of sensorineural hearing loss on loudness discomfort level and most comfortable loudness judgments.

A simple up-down adaptive procedure was used to estimate the 50% point on the psychometric function for loudness discomfort level (LDL) and the two functions describing the range of most comfortable loudness (MCL) for listeners with sensorineural hearing impairment. For pure tone and speech stimuli, median LDL and MCL levels were observed at relatively constant SPLs for subjects with hearing loss less than or equal to dB HL and at progressively higher SPLs with further increase in hearing loss. Correlation analysis verified a statistically significant relationship between LDL and magnitude of hearing loss. The nonlinear relationship between LDL and hearing loss together with the large intersubject variability in the data suggest that prediction of LDL from hearing threshold would often be highly inaccurate. These results also demonstrate that averaging LDL data across a group of subjects with a wide range of hearing loss may lead to inaccurate conclusions regarding the effects of sensorineural hearing loss on LDL.

Association of HLA antigen a9 with progressive systemic sclerosis (scleroderma).

Upon evaluation of 40 subjects with progressive systemic sclerosis (PSS), a significant association of HLA antigens A9 and Aw24 (a subgroup of A9) was found with diffuse scleroderma. HLA-Aw23 (a second subgroup of A9) was also increased in the patients, however, this was not statistically significant. Diffuse scleroderma appears to be one of the few diseases that shows an association with the HLA-A locus.

HLA association studies in black sarcoidosis patients.

The frequencies of HLA antigens were determined in 32 black sarcoidosis patients and in 89 normal black controls. Aw23 was found in 23.6% of controls as compared with a lower frequency (6.2%) in patients (P = 0.06). No other associations were found.

HLA frequencies in cancer: a third study.

Frequencies of 25 HLA antigens in 526 Caucasian patients were compared to those in 629 healthy controls who were HLA-typed between September 1975 and February 1977. Haplotypes were compared for 711 patients and 549 controls typed between September 1974 and December 1976. Frequency deviations were found in those with ALL, AML, breast cancer, lymphoma and ovarian cancer, but only the increase in A29 in AML patients was statistically significant when corrected for the number of specificities. Interesting associations, when compared with earlier studies, include elevation of AW24 in both ALL and AML patients and increased B27 in ALL patients. Significant haplotype differences were increased A3-B8 and absence of A1-BW17 in ALL patients and increased A11-B5 and A2-BW40 as well as absence of A2-B5 in AML patients.

A microassay for leukocyte migration: analysis of its reproducibility.

A reliable microassay for human leukocyte migration is described by which 50 to 100 assays can be performed each day in quadruplicate with the number of indicator cells obtainable from 10 to 20 ml of peripheral blood. The reproducibility of this method is demonstrated with respect to the variability among replicate test wells (including reading), the variability among different test readers, the variability among replicate cultures, and the variability of using indicator cells from different subjects. A microculture system is described that requires only 75,000 mononuclear cells to consistently produce detectable polymorphonuclear leukocyte migration inhibition factor in response to PPD. The reproducibility of this culture system is demonstrated with respect to the variability of lymphocyte responsiveness on repetitive testing in the same individuals.

Specificities in human cell-mediated cytotoxicity.

Specificity of natural cell-mediated cytotoxicity was investigated through selective reactions detected by direct cell-mediated cytotoxicity and by inhibition of cytotoxicity through competition. Assuming that target cells reacting alike in direct cytotoxicity shared common antigens, we classified 10 target cells into three groups by target antigens: TA (target antigen) 1, 2, and 3. Partial confirmation of the three groups was achieved in the cross-competition assay. The distinction of TA 1 as a group was clear but some cross-reactivity existed between TA 2 and TA 3 cells.

HLA matching and cadaver kidney transplant survival in North America: influence of center variation and presensitization.

In an analysis of 4,851 first cadaver kidney transplants, we found a statistically highly significant correlation between the number of HLA antigens mismatched and graft survival (P less than 0.0005 at 1 year). The difference in the survival rates of grafts with no HLA mismatch compared with grafts with four mismatches was 11 to 12%, similar to results of previous analyses. HLA-A locus antigens had a slightly stronger effect than B locus antigens. The correlation of HLA matching with graft survival was most significant at centers with poor overall transplant outcome, and there was no correlation at centers with very good overall results. Presensitization also had the strongest effect at centers with poor overall graft survival.
