Intermediate filaments connect z-discs in adult chicken muscle.

When adult chicken skeletal myofibrils are treated with a myosin-extracting solution, the Z-discs with attached actin filaments retain their linear connections with one another in the extracted myofibril. The sarcomere length increases in the extracted myofibrils from a control lenght of 2.5 micrometer up to 6 micrometer. In a sarcomere, eight to fifty 10 nm filaments can be seen in parallel array in the H-zone. The 10 nm-wide filaments do not bind heavy meromyosin and are two to four micrometers in length. These intermediate filaments are postulated to be an integral part of the sarcomere, connecting Z-bands along the length of the myofibril.

Evaluation and characteristics of currently available inulin.

Inulin was not available for clinical testing in the United States between February 1973 and July 1975 because the raw material received in the United States differed from previously manufactured lots in molecular weight, glucose content, optical rotation, melting point, and water solubility. The comparability of the "new" and "old" inulin preparations was assessed in 12 adult male spinal cord injury patients, seven with normal renal function and five with compromised function. The "new" inulin proved to be as accurate and safe an agent for the determination of glomerular filtration rate as the previously available material in both groups of patients, and was made available for general clinical use in the United States in July 1975.

Bacteriuria in patients with spinal cord lesions: its relationship to urinary drainage appliances.

Urine cultures from spinal cord injuried patients demonstrate a high incidence of bacteriuria, with patterns of bacteriuria varying with the mode of drainage. The highest incidence of positive cultures occurs in patients with indwelling or suprapubic catheters, and the lowest in patients who are appliance-free. The organisms occurring most frequently differ from those of the general hospital population as reported by Needham, Smith and Matsen. There is an increase in the incidence of Pseudomonas, Proteus and Serratia and a decrease in the occurrence of E coli and Klebsiella. Sixty-four percent of the organisms occurred in concentrations of 100,000 col/ml or greater, and 65% of all positive cultures contained multiple species of organisms. Cultures taken from urinary collecting appliances and hands demonstrate possible sources of bacteriuria and the need for careful training and education of patients and their attendants and constant monitoring of urinary tract appliance care.

A general practitioner in an ophthalmology accident and emergency department.

After a short period of intensive training, a general practitioner successfully replaced a senior house officer (SHO) in the accident and emergency department of an eye hospital on one morning a week for a year. An unbiased observer compared the performance of the general practitioner after one year with that of a full-time SHO who had had 17 months' experience; their performances were about equal. Although a sessional general practitioner costs about 28% more than an SHO, the real cost is much less because undue length of service as an SHO or change to another specialty (because of the SHO surplus) delays achievement of a permanent grade. Continuity is a great advantage of the general practitioner. Replacement of some SHOs by general practitioners would reduce the surplus of SHOs with poor promotion prospects. The commonest diagnoses were Meibomian cysts (18%), corneal foreign bodies (20%), corneal abrasions (12%), and conjunctivitis (8%).

A double blind comparison of buprenorphine and morphine in conscious subjects following administration by the intramuscular route.

Buprenorphine was studied to determine the respiratory depressant effect in a double blind comparison with morphine. Buprenorphine produced a more prolonged displacement of the carbon dioxide response curve than morphine. Because of the marked difference in the time effect curves the relative potency was determined at the time of peak effect--3 hours post-administration for buprenorphine (0.15 and 0.3 mg) and 1 hour post administration for morphine (5 and 10 mg). The data from the subjects who received higher doses of buprenorphine (0.6 and 1.2 mg) showed that there was a linear relationship over the dose range 0.15-1.2 mg in terms of respiratory depression.

Renal function in patients with spinal cord injury: the eighth year of a ten-year continuing study.

In the eighth year of a continuing study of renal function in patients with spinal cord injury, 78 percent had good function, 13 percent mild deterioration, 4 percent moderate deterioration and 5 percent severe deterioration of kidney function. No single factor was shown to be a dominant cause of decreased function. The factors most frequently associated with renal deterioration were vesicoureteral reflux, renal calculi, recurrent pyelonephritis as demonstrated by calyceal blunting, and recurrent decubitus ulcers, the latter usually in combination with other factors. Most of these factors could be prevented by good medical supervision and meticulous self-care.

Teaching nursing management by programmed instruction.

Student and faculty dissatisfaction with lectures prompted several revisions of teaching methods for nursing management. Revisions centered around a programmed text for business plus various methods of application to nursing. One part of management (control) was also programmed in nursing terms. Data collected revealed that students who used the programmed business text and participated in small group discussions achieved significantly higher scores than either students who attended lectures or used the programmed business text in conjunction with a tutorial system. Data also demonstrated that students who used the unit programmed specifically for nursing management achieved significantly higher scores than students who used the programmed text for business management. This data suggest students level of achievement is increased when the transfer step from business to nursing is eliminated. For this reason, two faculty members completed the writing of a programmed text for nursing management. This text has been tested over the past two years and will be published in the near future. We were unable to collect objective data regarding the actual application of management principles to nursing practice. Subjectively, we felt the students had a stronger framework of principles unique to nursing management and were able to transfer this knowledge to leading a team with greater ease than previously. Without student dissatisfaction originally, and student acceptance and participation throughout the changes, this study would not have evolved. It was student encouragement which prompted the program specific to nursing.
