BACKGROUND: Occasional case reports describe urinary incontinence in patients taking the selective serotonin and norepinephrine reuptake inhibitor antidepressant venlafaxine. OBJECTIVE: In this study the authors investigated the possible effect of venlafaxine on urinary function in a series of 9 patients with urinary retention resulting from spinal cord lesions. They primarily sought to understand whether the reported venlafaxine-induced urinary incontinence was a specific drug-induced effect and, if so, whether venlafaxine might be an effective treatment of urinary retention. METHODS: During a 1-week baseline period, patients measured postvoiding residual volume through a catheter and recorded the number of micturitions within 24 hours. At the end of the baseline period, venlafaxine 75 mg extended-release on a once-daily evening administration schedule was added to their therapy for 1 week. RESULTS: None of the patients reported severe/uncontrollable side effects while taking venlafaxine. Extended-release venlafaxine (75 mg/day) significantly reduced the postvoiding residual volume and increased the micturition rate; the volume diminished on the first day of treatment and remained stable over the ensuing days. CONCLUSION: These findings suggest that venlafaxine could be useful to improve voiding in patients with spinal cord disease.

A total of 462 patients (mean age 52 years) affected by reversible focal ischemic attacks (RIAs) were followed prospectively in 8 neurologic institutions in Italy for 4 years. All cases were evaluated with a cerebral angiography and 21% of angiograms were normal. At the end of the follow-up period the cumulated probability for death, stroke, cardiac event and new RIA was respectively 7%, 8%, 3% and 36%. The predictive value of the baseline characteristics of this series was evaluated by a multifactorial analysis which showed that RIA and stroke (specific cerebrovascular risk) were more likely to develop in patients with a history of more than one RIA and in those in whom multiple vascular territories were involved. Moreover, previous myocardial infarction, intermittent claudication, angina pectoris, time elapsed since the first attack, and duration and severity of the attack itself were independently associated with general cardiovascular risk (death, stroke and myocardial infarction). We conclude that predictive factors, and thus also pathogenetic mechanisms, may be different for general cardiovascular risk and specific cerebrovascular risk in RIA patients.

The number of ultrasonography expert consultant Nephrologists is more and more increasing thanks to the contribution that this methodology has brought in both clinical and treatment fields. Up to now, a database of the ultrasonography benefits, as well as the main criteria for the interpretation of the urinary tract echographic examination has not been compiled, yet. We have therefore drawn up and distributed a questionnaire to the consultant nephrologists of the urinary tract echography study group (Apulo-Lucano division). This questionnaire is made up of 27 ultrasonography application and interpretation issues; it aims mainly at creating a common 'language' to reduce the variety of 'descriptors' currently employed by all different specialists and centres involved. 60 consultant nephrologists participated in the study, from the 29 Nephrology and Dialysis O.U.s of Puglia and Basilicata regions, where there is an active echographic service. Data collected show the key role of ultrasonography investigation for all nephrology patients, as high quality and cost efficient test procedure. Moreover, despite the fact that there are differences in echographic examination performance and interpretation, literature data show clearly that it is fundamental to follow general shared principles. The responsibility and task of those specializing in this discipline should be to allow reproduction and comparison of ultrasonographies, also among different operators and centres, and meta studies, i.e. 'a series of comparative studies', which are still very few in number.
It is well recognized that in subjects with cerebrovascular disease (CVD), changes in hemorheological properties may occur in about 50% of the patients. This paper concerns a study on microhemorheological perturbations of the erythrocyte membranes in a group of 20 RIA's patients, treated with Pentoxifylline. At the admission of the subjects to the study a trend to an increased whole blood viscosity and erythrocyte aggregation was associated to alterations of membrane fluidity analyzed by means of the fluorescent probes in the patients with respect to the controls. At the end of pharmacological treatment and wash-out, our data indicate a clear modification in erythrocyte membrane which results improved. Our results underline how the improvement of the erythrocyte membrane fluidity, induced by pentoxifylline, is a basic factor of blood flow in the microcirculation.
Clinical and histopathological data of a patient affected by myotonic dystrophy (Steinert Disease) and multiple pilomatrixomas (calcifying epithelioma of Malherbe) are reported. This association has been previously reported in other 16 cases. The prevalence of pilomatricoma in myotonic dystrophy results much greater than in general population. This association affects females as well as males. This tumour, when associated with myotonic dystrophy, is more frequently multiple than single.
In a previously work we have reported that T8+ lymphocytes decrease can be obtained by incubation of lymphocytes from normal blood donors with serum taken from patients in the acute phase of the disease. In this paper is described a partial characterization of the masking serum factor present in the acute phase of the disease. It displays a m.w. of approximately 640,000 d and its effect disappears either after heating 60 min. at 37 degrees C or 30 min. at 56 degrees C, after dialysis against 1M NaCl or lyophilization. Another serum factor, insoluble at 50% ammonium sulphate final saturation, resistant to heating and lyophilization, able to remove (in vitro) the former masking factor from the T8+ cells, is present in the remission phase.
Clinical data of 233 patients (202 definite and 31 probable MS) have been examined in relation to the following prognostic indicators: sex, age at onset, poussè frequency, interval between the first two relapses, initial symptomatology. Regression analysis technique and covariate analysis have been used versus the latest Expanded Disability Status Scale of each patients. Onset at younger age and a longer interval between the first two episodes have resulted statistically significant for a more favourable prognosis. The shift from a remitting toward a progressive course appears to happen within the first five years in 60% of the cases.
