Segmentary analysis of the coronary artery distribution in the left ventricle.

In a series of 90 human hearts, from individuals aged between 4 days and 94 years (X = 61.09 +/- 21.96) the coronary arterial distribution of the left ventricle (LV) was studied using Selvester's system of segmentation[24]. Postmortem angiographies and microdissection techniques were used. The analysis of the 12 segments of the LV showed that the three septal segments (basal, mesial and apical) present a type of irrigation which is practically constant and of a dual type. The superoapical, posterobasal and posteromesial segments present an irrigation with a high index of exclusivity for the anterior interventricular and circumflex arteries. In the rest of the segments the vascularization is of a mixed type, although the apical segments reach an important degree of arterial exclusivity. The analysis by segments of the LV allows us to conclude that the arterial vascularization of the left ventricle depends fundamentally on the anterior interventricular artery and the left circumflex artery, which are those that present the greatest extension, together with a small contribution by the right coronary artery, which is always minor with respect to the left coronary artery.

Relationship between depressive and positive symptoms in schizophrenia.

The aim of this study was to investigate the relationship between depressive and positive symptoms, two positive symptom using scales (PANSS and SAPS) in samples defined by three schizophrenic diagnostic systems (DSMIII-R, ICD9 and Langfeldt) at difference phases of the illness and in taking into account the negative and extrapyramidal symptoms and the doses of neuroleptics. With both scales, correlations between depressive and positive symptoms were significantly negative in two diagnostic subgroups (DSMIII-R and Langfeldt) in the acute phase. These correlations were also significant when negative symptoms, subjective extrapyramidal signs and the doses of neuroleptics were partialled out. Only the extrapyramidal physician's score was intercorrelated with positive and negative symptoms. Among the positive symptoms, 'conceptual disorganization' (or 'positive formal thought disorder') and 'suspiciousness/persecution' were especially correlated negatively with depression. At the post-acute phase or at the residual phase, no significant correlation between depressive and positive symptoms was found in any diagnostic subgroup. These results show the necessity of taking into account the phase of illness and the diagnostic criteria in order to study depression in schizophrenia.

Response of genetically obese Zucker rats to ciprofibrate, a hypolipidemic agent, with peroxisome proliferation activity as compared to Zucker lean and Sprague-Dawley rats.

Genetically obese Zucker (fa/fa) rats were used as an experimental model to study the effects of hypolipidemic agents on peroxisome proliferation; comparison was made with Zucker lean phenotype (Fa/-) and Sprague-Dawley strain/phenotype. The pharmacokinetics of a single administration of ciprofibrate (1 or 3 mg/kg), appeared to be similar in all strains/phenotypes. After a 2-week oral administration at the same dosages, there were dosage-related increases in hepatocellular peroxisomal yield and in the hepatic enzymes' cyanide-insensitive acyl-CoA oxidase and catalase. The peroxisomal yield was less increased in Zucker than in Sprague-Dawley rats, while the enzyme activities were similarly increased. Although the absolute specific activity of microsomal omega-lauryl hydroxylase (cytochrome P4504A1) was lower in Zucker rats, it was increased more in this strain than in Sprague-Dawley rats in response to drug exposure. The hypolipidemic effect (cholesterol and triglyceride reduction) was more pronounced in Zucker obese rats. Based on biochemical and morphological results, no major differences between strains/phenotypes in terms of peroxisome proliferation were observed following a 2-week administration of ciprofibrate.

Maintenance requirements in metabolizable energy of adult, nonpregnant, nonlactating Charolais cows.

The objective of this study was to determine the maintenance requirements in metabolizable energy (MEm) of adult, nonlactating, nonpregnant Charolais cows. A feeding trial was conducted using 12 cows fed at one of two feeding levels (75 [L] and 113 [H] kcal of ME.kg BW-.75.d-1) for 116 d. Body composition was estimated from subcutaneous adipocyte diameter. Body weight changes averaged -468 and +46 g/d, respectively. Diet DM digestibility averaged .496. The L cows spent less time eating and ruminating but had other behavioral characteristics similar to those of H cows. Estimates of MEm were calculated from BW or body composition changes and amounted to 109 and 124 kcal of ME.kg BW-.75.d-1, respectively. Heat production (HP) was then measured over 67 d in a second trial on two L and two H cows from the feeding trial and planes of feeding were switched after 14 d. Estimates of MEm varied from 112 to 105 kcal.kg BW-.75.d-1. Within animal, day-to-day variations in heat production were high (4.6% on average) and prevented the detection of any precise changes of HP with time on treatment. None of the trials showed any significant effect of level of feeding on MEm.

[The usefulness of adenosine triphosphate in supraventricular paroxysmal tachycardias].

With the objective to evaluate adenosine triphosphate (ATP) usefulness in supraventricular paroxysmal tachycardia (SPT), doses of 0.25 mg/kg, of ATP are administered fastly i.v. in bolus, to fifty consecutive patients with SPT resistant to vagal stimulation maneuvers. The effect achieved allowed to identify two groups: a) 39 patients in whom tachycardia was interrupted; b) 11 patients in whom tachycardia did not cease, but the induced transient provoked atrial-ventricular (A-V) blockage allowed to identify the underlying mechanism of tachycardia (in 6 of them atrial tachycardia and in 5 atrial flutter). In both groups the ATP effect was present in less than 30 seconds. In seven patients (five from group (a) and two from group b) the QRS duration was over 0.12 s. Six patients had a left ventricular ejection fraction below 30%. Side effects were frequent, but always short. ATP could become the drug of choice in the treatment of SPT without response to vagal maneuvers, due to the fact that, to its therapeutic activity, it must be added its diagnostic usefulness, its fast action and metabolization, together with the fact that it does not induce severe side effects.

Amisulpride versus bromocriptine in infantile autism: a controlled crossover comparative study of two drugs with opposite effects on dopaminergic function.

An alteration of dopaminergic (DA) function much more complex than simple hyperactivity has been evoked in infantile autism. We therefore compared the clinical efficacy of a DA antagonist (amisulpride) and a DA agonist (bromocriptine) in a randomized, double-blind, crossover trial in 9 children with autism, likely severely mentally retarded. Amisulpride acts preferentially on specific autistic symptoms whereas bromocriptine acts more on motor hyperactivity and attention symptoms. These findings raise the specificity of these two drugs which appear to act preferentially on some target symptoms and are consistent with some clinical and pharmacological observations showing a sedative effect with low doses of DA agonists and a stimulant effect with low doses of DA antagonists such as the benzamides.

[A critical study of conditions for prescription and evaluation criteria of neuroleptic treatment in resistant schizophrenia].

From the study of Kane et al. (1988), devoted to clozapine, a critical analysis of criteria of assessments about studies in treatment of resistant schizophrenic patients was drawn up. Therefore, among the inclusion criteria, the authors strengthen the necessity of a very long past neuroleptic treatment (beyond six months) before diagnosing a resistance, the "drug-free improvers" characterized by improvement when patients had been treated by a placebo, the necessity of a very long placebo wash-out (beyond six weeks), and the improvement by a second treatment after a first ineffective treatment. Moreover, the doses of neuroleptics opposed to clozapine are often too high leading to adverse effects and so decreasing the positive benefits. For instance, the dose of chlorpromazine is often increased to 1,800 mg/day whereas the doses required should be only 600 mg/day in equivalence to 500 mg/day of clozapine. Lastly, the scales more specific of the symptomatology of schizophrenia such as SANS-SAPS or PANSS should be used in the clinical trials whereas until now, all the studies were made by standard and global evaluations with BPRS, CPRS and AMDP.

[Lipomatous hypertrophy of the interatrial septum. Report of 2 cases diagnosed in vivo].

Lipomatous hypertrophy of intervenous tubercle is a clinicopathological entity characterized by an accumulation of not capsulated fatty tissue, with its thickness exceeding 15 mm, in the atrial septum, above the fossa ovalis, at the intervenous level and protruding into the right atrium. It can be clinically associated with atrial electric abnormalities such as disorders of the atrial conduction and supraventricular arrhythmias, difficulty of the venous return and sudden death. Nowadays its diagnosis is not difficult using non invasive image techniques. We present two cases of lipomatous hypertrophy of intervenous tubercle which were suspected by echocardiography. In one of them we used transthoracic echocardiography and in the other one transesophageal echocardiography. The diagnostic was latter confirmed by magnetic nuclear resonance and axial computerized tomography.

[A protracted course in Cardiobacterium hominis endocarditis].

A 69-year-old man without previous cardiac disease was found over the last 9 months to have a markedly elevated erythrocyte sedimentation rate (ESR: 120 mm/1. h), haemolytic anaemia (haemoglobin 8.2 g/dl, lactate dehydrogenase 304 U/l), markedly reduced exercise tolerance, backache and weight loss of 5 kg. Radiological, biochemical and endoscopic examinations failed to provide a diagnosis. Nine blood cultures grew, at normal body temperature, Cardiobacterium hominis, a rare Gram-negative organism which can cause endocarditis. Echocardiography revealed endocarditis of the aortic valve with regurgitation. Despite protracted and high-dosage antibiotics (4 times daily 10 million U penicillin G for 6 days, followed by four times 5 million U penicillin G for 6 days, followed by four times 5 million U daily for five weeks, and three times daily 60 mg gentamycin for 10 days), as well as treatment of extensive chronic parodontitis, anaemia, haemolysis and increased ESR have now persisted for over a year, with negative blood cultures. Immune-complex phenomena are thought to be the reason for the persistence of signs of infection.

[Biological treatments of schizophrenia].

Biological treatments of schizophrenia primarily consist of neuroleptics. These drugs can be prescribed in a relatively standardized manner (chlorpromazine equivalent), but neuroleptics will be less than effective on the type of schizophrenia with negative symptoms. Treatment can also be performed by utilizing the bipolarity of some drugs, and it is only with these drugs that resistant schizophrenia can be mobilized. Consideration is also given to other biological treatments and new prospects in chemotherapy.

[Lipomatous hypertrophy of the interatrial septum versus lipomatous hypertrophy of the intervenous tubercle].

The lipomatous hypertrophy of the interatrial septum is a clinicopathological entity characterized by an accumulation of fat, not encapsulated but circumscribed in the atrial septum, over the fossa ovalis, and protruding into the right atrium; its thickness exceeding 15 mm. It is clinically associated with atrial electric abnormalities such as disorders of the atrial conduction and supraventricular arrhythmias, difficulty of the venous return and sudden death. In 1669 Lower described at the entrance to the right atrium, between the venae cavae, there is a tubercle formed by an accumulation of fat, covered over with muscular fibres that protrudes into the right atrium, which be called intervenous tubercle. The aim of this work is to find out the normal morphology of the intervenous tubercle and to find out whether or not lipomatous hypertrophy of interatrial septum is related to it in some way, because both coincides anatomically.

The intervenous tubercle (of lower): morphological characteristics and changes in relation to age.

We have studied the morphological, macroscopic and structural characteristics of the intervenous tubercle (of Lower) in 100 human hearts, relating these to age and to the thickness of the epicardial fat. The morphological characteristics vary depending on age, these changes being attributable to a deposition of fat following the deposit of adipose tissue occurring in the epicardium.

[Isolated congenital diverticulum of the left ventricle].

Congenital diverticulum of the left ventricle is an uncommon cardiac malformation. We present a 45 years old woman, previously asymptomatic. The symptoms and electrocardiogramas suggested ischemic heart disease. The left ventriculogram demonstrated an isolated apical diverticulum, although the coronary arteries were normal. We will consider the etiopathogeny, classification, clinical history, diagnosis and treatment of the isolated apical diverticulum.
