Parasympathetic overactivity and its evaluation in patients with sinus nodal dysfunction.

In 17 controls, and 17 patients with sinus nodal dysfunction, an electrophysiologic study was made of sinus nodal function and atrioventricular nodal conduction in the basal state. The study was then repeated in all patients after atropine. The heart rate, mean sinus cycle length, variations of sinus cycle length, sinus node recovery times, sinuatrial conduction time, AH interval, and atrioventricular nodal Wenckebach threshold were significantly different in patients from those of controls. All these parameters changed significantly in patients after atropine, and were comparable to those of controls except for the atrioventricular nodal Wenckebach threshold. Atropine failed to increase the heart rate beyond 90 beats per minute in 10 of 17 patients (sensitivity of 59%) or by at least 30% above the resting heart rate only in 4 of them (sensitivity of 24%). The variations of sinus cycle length, and their standardized value, could detect sinus nodal dysfunction with sensitivities of 59 and 47%, respectively. From our results, we conclude that there is parasympathetic overactivity in patients with sinus nodal dysfunction. Because of their very low sensitivities, the atropine test and variations of sinus cycle length were not useful in identifying sinus nodal dysfunction noninvasively. The normal response of the heart rate to atropine does not exclude sinus nodal dysfunction, but atropine may help to differentiate abnormalities intrinsic and extrinsic to the sinus node during the electrophysiologic study.

Neutralization of the activity of vipera ammodytes ammodytes snake venom on myocardium of rats by antitoxinum viperinum: a histopathological study.

Antitoxinum viperinum was tested for its ability to prevent alteration of the myocardium induced by Vipera ammodytes ammodytes venom. Antivenom was injected intraperitoneally either immediately, 30 min or 2 hr after the intraperitoneal injection of venom. The light microscopic examination showed that the antiserum neutralized the effects of venom and antivenom might be useful in treating V.a. ammodytes venom poisoning.

[Pseudoobstruction syndrome of the large intestine (endoscopic therapy)].

The authors present an account on the successful treatment of pseudoobstructive syndrome of the large intestine (Ogilvie syndrome) by decompression, which was achieved by inserting a catheter by means of a coloscope into the transverse colon or caecum. The authors used this method in two patients.

[Ventricular tachycardia in patients with arrhythmogenic right ventricular dysplasia. Electrophysiologic characteristics and testing of the effectiveness of anti-arrhythmia therapy].

The authors present a clinical and electrophysiological characteristic in two cases of symptomatic sustained ventricular tachycardia (VT) with a QRS morphology of left bundle branch block, refractory to common antiarrhythmic treatment. It was confirmed that the mechanism of this arrhythmia associated with a high risk of sudden death is the reentry phenomenon and the authors analyzed its relationship to arrhythmogenic dysplasia of the right ventricle. In a way uncommon in our literature the authors demonstrate the importance of comprehensive electrophysiological examination in these serious cases and its usefulness in confirming the origin of VT in the right ventricle, its importance for mapping the accurate localization of tachycardia and the investigation of effectiveness of drugs and the evaluation of aptness of non-pharmacological treatment.

Prediction of "hot spots" in interleukin-2 based on informational spectrum characteristics of growth-regulating factors. Comparison with experimental data.

The Resonant Recognition Model (RRM) is a theoretical method for analysis of protein and nucleotide sequences, based on the Fourier transform of the numerical representation of sequences. The amplitude spectrum of this transform is designated Informational Spectrum (IS). There are certain common frequencies in IS of growth-regulating factors. These characteristic frequencies may correlate with their roles in cell proliferation and metabolism, and in antitumor activity. IS of IL-2 has prominent characteristics in the main frequency domain of growth factors, frequency domain of antitumor factors, and frequency domain characteristic for IL-2-alpha receptor. By means of the inverse method for these 3 domains, the amino acids in the sequence of human IL-2 that may be relevant to its biological function, the so-called "hot spots", were predicted. The most probable hot spots, obtained in this way, are in the potential binding site of IL-2 to its receptor, which agrees with experimental data.

The relationship of the resonant recognition model to effects of low-intensity light on cell growth.

The resonant recognition model (RRM) is a model of protein-protein and protein-DNA interaction based on a significant correlation between spectra of numerical presentation of the amino acid or nucleotide sequences and their biological activity. Having compared absorption characteristics of photosensitive proteins with their RRM spectral characteristics we obtained a linear correlation between the RRM frequency space and real frequency space, which determines a scaling factor between these two frequency spaces. On applying the RRM model to several groups of peptide growth factors, characteristic RRM frequencies were revealed and the corresponding real characteristic frequencies for these groups of growth factors were calculated using the scaling factor previously obtained. The real frequency characteristics of growth factors obtained in this way correspond with maxima in the action spectra of low-intensity light irradiation effects on cell proliferation.

Comparison of indirect and direct methods for determination of sinoatrial conduction time.

We have performed an electrophysiologic study (EPS) in 18 subjects (mean age 21 +/- 17 years) with normal sinus node function (group A) and in 15 patients (mean age 43 +/- 26 years) with sinus node dysfunction (group B). Three extrastimulus methods (both Strauss and Kirkorian methods and our modification of the extrastimulus method) were carried out in 31 patients of both groups and sinoatrial conduction times (SACTSTR, SACTKIR,SACTMOD) could be estimated in 27 of them. SACT by the Narula method (SACTNAR) was assessed in 25 patients of both groups. Sinus node electrograms (SNEs) were attempted in 22 patients and were obtained in 17 of them. Among the indirect methods, by comparison with SACTSTR in group A the best correlation was for SACTMOD (r = 0.81) and the weakest for SACTKIR (r = 0.67); there was no significant difference between SACTs estimated by Strauss and other methods. In group B the weakest correlation was also for SACTKIR (r = 0.68), whereas the correlations for both SACTMOD and SACTNAR were similar (r = 0.72 and r = 0.74, respectively); in this group SACT was significantly underestimated only by the Kirkorian method (P less than 0.01; sensitivity of 45%). By our method sensitivity was 82%, similar to both the Strauss and Narula methods (92 and 92%, respectively). By comparison of indirect methods with the direct measurement of SACT we have found similar good correlations for the Strauss, our own, and the Narula methods in group A (r = 0.81, r = 0.75, r = 0.79, respectively) and in group B (r = 0.81, r = 0.86, and r = 0.72, respectively); here too the weakest correlation was, in both groups, for the Kirkorian method (r = 0.67, and r = 0.68, respectively). The SACT in group B was the more significantly underestimated (P less than 0.001). Significant underestimation of SACT occurred also by our method and the Strauss method (P less than 0.01, and P less than 0.001, respectively), whereas the Narula method did not significantly underestimate the directly measured SACT. From the SNEs after atrial pacing we have shown the limitation of the Kirkorian method and observed the conspicuous beat-to-beat variability of SACTdirect' as the possible explanation of chaotic pattern of return cycles by the extrastimulus methods in some patients of group B.(ABSTRACT TRUNCATED AT 400 WORDS)

[The role of internal medicine and hospitals in preventive services].

Preventive care is an integral part of the integrated, differentiated, modern therapeutic and preventive doctrine which in the present period of ideas on health and disease holds the most important position in ensuring active health, so-called superhealth. The effectiveness of a wide complex of preventive measures can be ensured only if the entire society participates. Health workers whose profession it is to protect health must all participate in prevention at all work places incl. hospitals. Without the participation of in-patient departments the health services cannot fulfill their preventive role in the health protection of individuals and society. The tasks of internal medicine, incl. internal in-patient departments, ensue from the contemporary main tasks of this basic medical discipline. The authors give an account of the main forms of preventive activities in hospitals and draw attention to the complexity of implementation of clinical prevention at the present time. The implementation of preventive activities in clinical departments and the participation of hospital staff in prevention should not be impeded by any administrative or conceptual barriers or regulations for the activities of health institutions.

[Is Campylobacter pylori the pathogenic agent in gastritis and peptic ulcer?].

The authors analyzed a group of 164 subjects where they collected during gastroduodenoscopy specimens of the gastric mucosa for histological and microbiological examination. Campylobacter pylori (CP) was detected in 37 of 41 subjects with gastritis type B (90.2%), in 36 of 45 patients with duodenal ulcers (80%) and in 15 of 24 patients with gastric ulcers (62.5%) and in one of 15 subjects with gastritis type A. In 8 patients with drug induced and reflux changes the presence of CP was not detected. In the control group of 31 subjects CP was found in 4 (13%). Gastritis type B was diagnosed in 91% of the patients with gastric ulcers and in 93% of patients with duodenal ulcers. Elimination of CP by amoxicillin was successful in 71% of the patients with duodenal ulcers, 67.5% patients with gastric ulcers and in 45% of subjects with gastritis type B. After elimination of CP in all subjects rapid regression or complete disappearance of inflammatory changes was recorded. Within 12 months after elimination of CP and healing of that ulcer a relapse was recorded in 20% of the patients. Based on their observations the authors conclude that CP appears to be the causal factor of gastritis type B and one of the causal factors of peptic ulcers. Elimination of CP improves the course of both diseases.

[Diagnosis and classification of tubulointerstitial nephritis].

To make a correct diagnosis of tubulointerstitial nephritis and to make it in time calls for knowledge of its clinical symptomatology (it is usually very marked in acute forms and in chronic forms it is frequently inapparent) and it is essential to know the main causes which produce this type of renal damage. In some instances the already developed disease is diagnosed and not infrequently even renal failure. Recognition of the aetiological and pathogenetic multiplicity of the disease, as made possible by the submitted classification of the WHO expert commission, is one of the prerequisites of the diagnosis and treatment in time or of prevention by effective provisions.

[Morphology of tubulointerstitial nephritis].

The morphological picture of interstitial (tubulointerstitial) nephritis is not rare when renal biopsies or necropsies are evaluated. According to experience assembled during examinations of renal biopsies (a group of 1484 biopsies) in particular biopsies with morphological changes of the character of interstitial nephritis (144 in the group) and according to findings reported in the available literature, the authors submit an account of changes encountered in a large group of interstitial nephrites. The need of such an account ensues in particular from clinical practice where the morphological picture can be decisive for establishing the correct diagnosis in cases where other, in particular clinical and biochemical examination procedures, do not suffice.

The importance of immunohistochemical examination of glomerulonephritis.

There were 942 cases of glomerulonephritis classified according to WHO out of 1499 renal biopsies. Immunoglobulins IgG, IgA, IgM, C3 and fibrinogen were examined immunohistochemically in a standard way and IgD, IgE, albumin, C4 and HBsAg in a non-standard way. The results were compared with a control group. The results show that the excluding value of the negative immunohistochemical examination in glomerulonephritis is higher than the proving value of the positive examination.
