New trends in cesarean section.

The Authors studied the frequency of Cesarean section in relation to deliveries in the last trimester of pregnancy, the frequency of repetitive Cesarean section, age and parity of the patients, indication for cesarean section, week of delivery, and perinatal and maternal morbidity and mortality at the First Clinic of Obstetrics and Gynecology, University of Catania, Catania, Italy, from 1972 to 1974 and from 1983 to 1985. The frequency of Cesarean section has increased from 12.07% (735 cases out 6,086 deliveries) to 16.89% (837 cases out of 4,955 deliveries). There was a decrease in frequency of repetitive Cesarean section from 30.47% (224 cases) to 27.95% (234 cases) and in perinatal mortality rate from 4.35% (32 cases out of 735) to 3.19% (27 cases out of 845 newborn). There were no cases of maternal death, while the most common cause of maternal morbidity was uterine atonia. From 1972 to 1974 mechanical causes were the most frequent indication for Cesarean section (28.16% -207 cases), while from 1983 to 1985 previous hysterotomy (32.73% -274 cases) was the main indications for Cesarean section.

Parity and sexual behavior in pregnancy.

Correlation between parity and sexual behavior during pregnancy was studied in 106 pluriparous and 69 primiparous patients. Sexual desire, frequency of coitus, frequency of orgasm, which partner initiated sexual activity, and level of sexual satisfaction were analyzed in the year before pregnancy and during pregnancy. Sexual desire, frequency of coitus, and sexual satisfaction were qualitatively equal in both groups. The frequency of coitus in particular decreased progressively in the three trimesters of pregnancy and more evidently in the pluriparous. The partner who initiated sexual activity most frequently was the male in the pluriparous group.

Paraovarian tumors.

Between 1965 and 1985 59 paraovarian cystic tumors, were treated at the First Clinic of Obstetrics and Gynecology, Catania University, Catania (Italy). In 45.76% of cases (27 cases) the cyst was localized on the right and in 40.67% (24 cases) on the left parametrium, bilaterally in 6.77% (4 cases) and multiple on the same side in 6.77% (4 cases). In 91.52% (54 cases) the patients were in fertile age and 8.48% (5 cases) were in menopause. The dimensions of the cysts varied between 2 and 20 cms, with higher incidence between 6 and 10 cms (49.16%). In 1.69% there was benign neoplastic degeneration (1 case of cystadenoma). In 6.77% (4 cases) there were no symptoms, while pelvic pain and menstrual disorders occurred in 57.62% (34 cases) and 35.59% (21 cases) of cases respectively. In 5.08% (3 cases) there was torsion of the cyst. Enucleation of the cyst was performed in 44.06% (26 cases), and in the other cases therapy was adopted according to the patient's age, parity, associated gynecological pathology and neoplastic degeneration.

Dermoid cysts of the ovary.

From 1965 to 1984, 105 cases of ovarian dermoid cysts have been diagnosed and treated at the First Clinic of Obstetrics and Gynecology, Catania University, Catania, Italy. The patients were aged between 9 and 70 years, with a higher frequency of dermoid cysts between 26 and 30 years (21.91%, 23 cases). In 0.95% (1 case) the patient was premenarchal, 84.76% (89 cases) in fertile age, and in 14.29 (15 cases) in menopause. 35.22% (37 cases) were nulliparous with an average of 31 years, and 64.76% (68 cases) pluriparous with an average age of 38.9 years. In 4.76% (5 cases) the patients had already been treated for dermoid cysts. The asymptomatic form occurred in 13.33% (14 cases), while in 9.52% (10 cases) of cases there was torsion of the cyst. In 89.53% (95 cases) the cyst was monolateral, double in the same ovary in 0.95% (1 cases), and bilateral in 9.52% (10 cases). Epithelial cancerous degeneration occurred in 1.95% of cases (2 cases). In 11.43% (12 cases) enucleation of the cyst was performed while in the other cases therapy was chosen in relation to age, parity, and associated gynecological pathology of the patients. The Authors compare their data with that in literature.

Pure dysgerminoma of the ovary.

Five cases of dysgerminoma are reported. All patients underwent unilateral salpingo-oophorectomy for FIGO Stage 1 dysgerminoma; five-year follow-up showed no recurrence of illness. A review of current work-up for staging, prognosis, and controversial therapeutic modalities for FIGO Stage 1 is presented.

A clinical and epidemiological study on eclampsia in the obstetrics and gynecology department of Catania in the years 1978-1983.

From January 1978 to July 1983 22 patients have recovered from eclampsia in the Obstetrics and Gynecology Department - Vitt. Emanuele II Hospital - Catania University. 64% of the patients had hypertension in pregnancy combined with edema in 92.85% of the cases (13 cases) and with proteinuria in 18% of the cases (4 cases). Eclampsia manifested itself before labour in 28.57% (6 cases), during labour in 19.05% (4 cases), in postpartum in 19.05% (4 cases) and in the successive three days after delivery in 33.33% (7 cases). Perinatal death rate was 11.28% and in all the cases it was due to intrauterine fetal death. In 22 patients treated there was a case of acute pulmonary edema and a case of maternal death.

Ultrasounds examination during polychemotherapy for ovarian cancer.

The usefulness of ultrasound diagnosis during polychemotherapy for ovarian cancer is valued. Between September 1980 and March 1983, 32 patients with ovarian cancer underwent polychemotherapy, had an ultrasound follow-up. Ultrasounds are very important for selecting the patients who must undergo second look.

Serum aldosterone and plasma renin activity in normal and toxaemic pregnancies.

The present study was performed to examine serum aldosterone and plasma renin activity variations in normal and toxaemic pregnancy. Blood samples were collected by 166 normal pregnant women and 196 toxaemic women. Results don't show important differences between normal and toxaemic pregnancies concerning either serum aldosterone or plasma renin activity.

Polychemotherapeutic treatment for ovarian carcinoma (our experience).

Since September 1980 to June 1983 we have treated 32 patients with ovarian cancer. Most patients were submitted to time polychemotherapy cyclophosphamide, adriamycin and cis-platinum. The maximum of survival time was 24 months; six patients died; the period of survival of the remaining 24 patients is between 2 and months.

Experimental study of the effects of cortical erosion on the resistance to compression of diaphyseal compact bone.

The authors investigated the variations in breakage under comparable compression loads induced by concentric erosion of diaphyseal bone in experimental animals. The results varied according to whether the internal or external cortex had been eroded, and whether the proximal or distal segments of the diaphysis were involved. Our findings appear to have some practical relevance regarding the etiopathogenesis of pathological fractures.

Polyamine levels in human semen of unfertile patients: effect of S-adenosylmethionine.

Polyamine content has been determined in the seminal plasma of unfertile men before and after S-adenosylmethionine treatment. Spermine and spermidine levels in azoospermic patients were found very low (80 and 24 microgram/ml) with respect to normospermic (1500 and 60 microgram/ml): S-adenosylmethionine treatment enhanced polyamine content (50-300%) in oligospermic patients. Furthermore, an increase of sperm count and motility has been observed in these patients.

Clinical efficacy of Apomorphine SL in erectile dysfunction of diabetic men.

Although subgroup analyses from large randomised premarketing studies have shown that Apomorphine SL enhances the percentage of erections firm enough for sexual intercourse in diabetic men, the clinical role of the drug in this patient population remains to be elucidated. The aim of the present study was to assess the efficacy of Apomorphine SL in diabetic males with erectile dysfunction (ED) and to identify factors predicting those who may benefit from the treatment. A total of 130 diabetic patients were randomised to receive either four tablets of 3 mg Apomorphine or a matching placebo. Assessments of efficacy comprised the erectile function (EF) domain of the International Index of Erectile Function (IIEF) and the one-item global efficacy question (GEQ). Patients with both a positive response to the GEQ and an improvement of at least 5 points in the EF domain of the IIEF were considered responders and subanalysed by several parameters indicative of the severity of both ED and diabetes. Response rate was 17% after placebo and 22% after Apomorphine SL. The EF domain of the IIEF and both questions 3 and 4 scores did not significantly improve in either of the two arms over the baseline. A younger age and a lower Hb1Ac were significantly linked to the status of responder in the Apomorphine arm. Apomorphine SL failed to show a statistically significant benefit over a placebo, but 22% of patients had a clinically significant erectile response. These figures seem to suggest that the drug has a limited use for ED diabetic patients.

Levels of anxiety and pain during chemotherapy with peripheral versus central vascular access: an experimental evaluation.

PURPOSE: The purpose of this research was to evaluate pain and anxiety levels in patients undergoing chemotherapy via central venous catheter (CVC) and peripheral venous access (PVA). METHOD: The sample consisted of 56 cancer patients undergoing at least one chemotherapy treatment cycle. They were divided into two groups based on the administration method of the chemotherapeutic drug: the experimental group (patients with CVC) and the control group (patients with PVA). The instruments used were the following: for multidimensional pain evaluation the Italian Pain Questionnaire (QUID, devised by De Benedettis et al), and the Visual Analogue Scale (VAS) Chart of Pain. To evaluate the state and anxiety traits, the State-Trait Anxiety Inventory (STAI questionnaire, by D. Spielberg) was used, and we used structured interviews for the statistical survey. RESULTS: Analyzing the data from the STAI questionnaire, it appeared that the state-trait anxiety levels of the experimental group and the control group were not significantly different (analysis of the mean difference made using the Student's t-test). The average scores of the STAI questionnaire for both groups fell within the normal range. This indicated that the anxiety level was unaffected by the introduction of the device (the CVC). The data collected with the QUID questionnaire and subsequent statistical analysis demonstrated that the difference between the means (Student's t-test) of the experimental group and the control group was significantly lower, as regards perception of the sensorial and evaluative quality of pain (p=0.05) in the experimental group. From the evaluation of the VAS method, it appeared that the difference between the means was not significant in either group, although the distribution of frequency tended towards lower values in the experimental group with respect to the control group. This confirms previous QUID research where a lower pain level is recorded. CONCLUSIONS: Analysis of the data collected via the structured interview revealed that most of the CVC sample did not respond to catheter insertion as a sign of a worsening of their illness.
