Ovarian pregnancy and IUD.

Two cases of ovarian pregnancy associated with an IUD in situ are described. One of the cases documents the first reported case of ovarian pregnancy coexisting with a Copper-T device; thus the range of types of IUDs combined with ovarian pregnancy is further extended. The consciousness of the physician of the coexistence, in combination with the wide use of updated diagnostic means, especially estimations of low values of HCG and laparoscopy, should ensure early diagnosis of ectopic pregnancies, including ovarian ones.

Quality of semen obtained from 627 fertile men.

Six hundred and twenty seven men, whose wives conceived within 6 month of their semen analysis, were included in this study. Attention was focused on 516 men where conception occured within 3 months of semen analysis. The quality of the seminal plasma (volume pH, fructose and calcium concentrations) and the spermatozoal characterization (concentration, motility, vitality and morphology) are given in details. It was concluded that spermatozoal motility and morpholoy are the most crucial factors in determining its fertilizing capacity. Measurement of vitality is important only in hypokinetic specimens. Sperm density is a limiting factor only below 10 million/ml. A widely scattered range of sperm concentration was found among the fathered men which may indicate that the incidence of pregnancy is a multifactorial statistical probability which increases when the quality of the semen is high.

Varicocelectomy and male fertility: comparison of semen quality and recurrence of varicocele following varicocelectomy by two techniques.

Two hundred thirty-eight infertile men aged between 20 and 50 years suffering from left varicocele underwent ligation of the internal spermatic vein. The patients were operated on using Ivanissevich technique IT (210 patients) or the Bernardi technique BT (28 patients). In the BT group recurrences were found in 7.1% as against 28% in the IT group. The low rate of recurrence found in the BT group might be due to the fact that the patients were under local anaesthesia and collaborated with the surgeon. Varicocelectomy was followed by significant improvement in sperm quality 3 to 9 months after operation. Improvement among the fertile men was more rapid than among the infertile men. Sperm concentration improved significantly in 50% of the cases. Sperm motility improved in 37% of the cases, while morphology improved only in 29% of the cases. Sperm vitality was identical in response to that of motility. Varioocelectomy did not cause any change in the seminal plasma constituents (volume, pH, fructose). Improvement in sperm quality was identical in the IT and BT groups, as was the impregnation rate. 94 couples reported pregnancies 39.5%), which occurred within 2 years after operation (12 +/- 1.5 months mean +/- SEM). No correlation was found between size of varicocele and improvement in sperm quality and fertilizing capacity.

Relation between semen quality and fate of pregnancy: retrospective study on 534 pregnancies.

Full semen analysis including sperm and seminal plasma (SP) was performed on 534 men, within 3 months of their wives conception, 65% of the pregnant wives eventually delivered, while the remainder aborted. The abortions were divided into five groups: early abortion (6%), late abortion (3%), repeated abortion (14%), and habitual abortion (12%). SP quality of the husbands of the various groups were identical. The motility and vitality of spermatozoa of the various groups were in the same range. Sperm concentration was significantly higher in the repeated and habitual abortion groups with a tendency to polyzoospermia. The morphology of spermatozoa was much better in these two groups. Generally, sperm quality of the men whose wives were repeated or habitual aborters was better than in the other groups. We could not find any evidence in the routine semen analysis indicating that sperm quality was responsible for the abortions. The reason for the abortions seems to be a female factor or chromosomal aberrations.

Papillary cystadenofibroma of the endometrium.

Six cases of papillary cystadenofibroma of the endometrium were found in he medical literature. A seventh case of this kind of tumor is now described clinico-pathologically and the literature pertinent to this particular type of tumor reviewed.

Cervico-isthmic pregnancy carried to term.

A case of term pregnancy of cervical origin, defined as cervico-isthmic pregnancy, is described. Review of the literature suggested that similar cases originally described as term cervical pregnancies were actually cervico-isthmic. The occurrence of these rare cases is explained by successive thinning and unfolding of the isthmus from the direction of the cervix up to the closed, resisting anatomic internal os. The isthmus is thus incorporated into the cervical cavity; in a few exceptional cases this allows the development of the pregnancy to term. The most common clinical feature of these cases is the appearance of the vaginal portion of the cervix; it is shortened or effaced and softened, and the inferior segment bulges into the fornices.

Sperm penetration in vitro: correlations between parameters of sperm quality and the penetration capactiy.

A nonlinear regression analysis was used in order to fit a logistic model to 200 runs of human ejaculated spermatozoa penetrating cervical mucus of good quality. The data revealed excellent correlation between the number of sperm penetrating (sigma penetration, SP) and the motility (r = 0.776) and vitality (r = 0.534) of the spermatozoa. The percentage of abnormal spermatozoa found in the ejaculate was negatively correlated (r = -0.649), while sperm concentration showed a poor correlation with SP (r = 0.327). Furthermore, concentration was shown to have no effect in samples containing over 5 million sperm/ml. The conclusion of this study is that the two major parameters of sperm quality determining the capacity of spermatozoa to penetrate cervical mucus are motility and the presence of normal forms of spermatozoa.

Combined tubal and multiple intrauterine pregnancies following ovulation induction.

An infertile woman who conceived after human menopasual gonadotropin-induced ovulation experienced ovarian hyperstimulation and a quintuplet gestation--an ectopic and a quadruplet intrauterine pregnancy. The intrauterine pregnancy was confirmed by B-scan ultrasonography about the time that the tubal pregnancy was ruptured. Salpingectomy did not interfere with the intrauterine pregnancy, but in spite of a cervical cerclage the patient developed premature labor after about 26 weeks of gestation.

Ultrastructural features in normal and hyperplastic postmenopausal endometrium.

Seven samples of postmenopausal endometrium were studied by electron microscopy. Four samples were diagnosed as adenomatous hyperplasia (2 of which were atypical) and 3 as normal postmenopausal endometrium. The most striking ultrastructural features of hyperplastic endometrium were: numerous nucleoli, deep nuclear membrane infoldings, increased nucleocytoplasmic ratio, prominent and enlarged RER closely associated with mitochondria and nuclear membrane, abundant free ribosomes and marked network microfilaments. In the case of atypical adenomatous hyperplasia, the protruded intraglandular proliferating epithelial cells exhibited more atypical features than the epithelial cells of the glandular lining, suggesting a more advanced degree of anaplastic change. The characteristic features of the normal postmenopausal endometrium were: paucity and random distribution of the cytoplasmic organelles, the presence of large cytoplasmic vacuoles and short, blunt microvilli. Secretory vacuoles opening into the lumen of the gland were found in one case of cystic atrophy of a normal postmenopausal endometrium. Collagenization was found in the stroma of both groups, although predominantly in the normal postmenopausal endometrium. In 2 cases of adenomatous hyperplasia stromal cells with vacular cytoplasm were found. The significance of these findings, as related to their importance as precursor stages of endometrial cancer (in the cases of adenomatous and atypical adenomatous hyperplasia); as involutional manifestations (in the cases of normal postmenopausal endometrium); and as related to the absence of cyclic activity (in both groups) is briefly discussed.

Effect of caffeine on sperm penetration rate into cervical mucus in-vitro.

A system for determination of sperm penetration into cervical mucus in a glass capillary tube is described. Repeated counts are made of the number of sperm at fixed horizontal distances from the semen reservoir. The yields indices for total numbers of spermatozoa entering the mucus per unit time and the rate migration of the sperm through the mucus. It was found that the rate of entry was sensitive to changes in motility such as those induced by caffeine, and differences in cervical mucus quality. The system allowed quantitation of these differences.

Gonadotrophin release in ovariectomized patients. I. Suppression by clomiphene or low doses of ethinyl oestradiol.

Administration of clomiphene citrate (150 mg/day) for 5 days to twenty-four ovariectomized patients and seven normal female patients evoked a significant release of FSH and LH in the normal control group and suppressed the gonadotrophin secretion in the castrated patients. A similar suppressive effect on gonadotrophin secretion was noted in eight ovariectomized patients treated for 10 days with low doses (50 microgram/day) of ethinyl oestradiol. It is suggested that in the ovariectomized hypoestrogenic patients, clomiphene acted as an oestrogen, suppressing by a negative feedback action gonadotrophin release in a way similar to ethinyl oestradiol. In the normal control group with an adequate steroid environment, clomiphene acted (probably at the hypothalamic level) as an oestrogen antagonist and stimulated gonadotrophin secretion. In view of our findings, it seems as if the ability of anovulatory patients to respond to clomiphene treatment by increased gonadotrophin secretion depends upon the absolute concentration of the compound in the different organs and by the quantitative relation of clomiphene to the endogenous oestrogens.
