In vivo and in vitro evaluation of marginal integrity in ceramic inlays.

The marginal integrity is an important factor for the long-term success of ceramic inlays. The long term clinical performance of porcelain inlays depends on a number of factors of which the marginal adaptation is of significant interest. The aim of this study was to determine the margin quality of adhesively luted sintered porcelain inlays both in vivo and in vitro. MOD cavities without bevels were prepared on 10 extracted human mandibular molar teeth. Using the Ducera inlay system, inlays were fired on refractory dies and luted with a dual-curing composite resin. After polishing, each, tooth was sectioned in buccal/lingual and mesial/distal directions and marginal adaptation was assessed microscopically. The mean marginal gap of 78.77 +/- 14.85 microns recorded for occlusal margins was significantly smaller than that of 128.85 +/- 34.34 microns seen at the approximal margins. For in vivo evaluation, 25 fired porcelain inlays, including 7 onlays, were placed in Class II cavities. The assessment of the marginal adaptation of inlays was made according to the scaling system used by Aberg et al. (Acta Odontol Scand 1994; 52:140-149). In 19 of the clinical cases, the restoration was contiguous with the existing anatomic form. Both in vivo and in vitro evaluations showed the margin quality of porcelain inlays to be high.

Urethral reconstruction with autologous vein graft: an experimental study.

There is no universally accepted material for urethral reconstruction. This study presents the results of segmental urethral replacement with a free graft of jugular vein in rabbits. Histological examination showed ingrowth of normal transitional epithelium into the venous endothelium. Retrograde urethrograms revealed an excellent result up to 300 days. Fistulas and infection occurred in 4/44 rabbits; these settled spontaneously. No structures or papillary hypertrophy were noted. Segmental urethral reconstruction with autologous vein graft is a simple technique with few complications and appears suitable for use in clinical cases.

[Therapy of pilonidal sinus with the Limberg transposition flap].

Pilonidal sinus can not be mentioned as a minor surgery procedure because of high recurrence rate, high morbidity and long disability period. In our clinic between the years of 1988-1991, 92 pilonidal sinus cases were treated primarily with Limberg's flap technique. Only in two cases, late wound healing was detected, no other complications and no recurrences were seen. Compared with other primary treatment methods, Limberg's flap technique was observed to be much more convenient for pilonidal sinus cases.

Free arterialized venous flap for the reconstruction of defects of the hand: new modifications.

Arterialized venous flaps were used in 13 cases for reconstruction of defects in the hand. The flap was used as a fascial venous flap in three cases, and as a sensory venous flap in three cases. In three cases, the flap was designed as an ipsilateral pedicled flap, similar to a standard free flap, instead of as a contralateral pedicled type. Ten flaps survived completely. Two flaps underwent superficial necrosis, with viable subcutaneous-tissue layers.

Peripheral T cell receptors alpha beta and gamma delta in patients with Hodgkin's lymphoma.

Antigen recognition by T cells is determined by an antigen specific T cell receptor (TCR). Two heterodimeric TCR structures associated with CD3 have been defined: TCR alpha beta and TCR gamma delta. TCR alpha beta and its function are well described but the role of TCR gamma delta in normal and lymphoproliferative disorders is not well established. In newly diagnosed or relapsed/refractory Hodgkin's disease (HD), a disease associated with defective T cell functions and increased sIL-2R, we determined levels of seven TCR alpha beta variable regions [beta V5(a), beta V5(b), beta V6(a), beta V12(a), alpha beta V(a), alpha V2(a)] and TCR gamma delta by using monoclonal antibodies (MCA). TCR gamma delta levels did not show any difference, but several variable regions of the TCR alpha beta differed when groups are compared with each other and the control group.

Bacteriology of antrum in adults with chronic maxillary sinusitis.

Swabs of 126 chronically inflamed maxillary sinuses were processed for aerobic and anaerobic bacteria during endoscopy. Bacterial growth was present in 113 of 126 specimens (90%). Anaerobic bacteria were isolated in 100 of 113 culture-positive specimens (88%). Anaerobes only were recovered in 59 cases (52%) and, in 41 (36%), they were mixed with aerobic or facultative bacteria. Aerobic or facultative bacteria were present in 13 cases (12%). A total of 323 isolates (2.8 per specimen), 228 (2.3 per specimen) anaerobes and 95 (1.7 per specimen) aerobes or facultatives, were isolated. The predominant anaerobic organisms were anaerobic cocci and Bacteroides species; the predominant aerobes or facultative bacteria were Streptococcus species and Staphylococcus aureus. These findings indicate the major role of anaerobic organisms in chronic sinusitis.

Effect of hydroquinone (HQ) on the development of chick embryos.

The developmental toxic effect of hydroquinone (HQ) was evaluated in chick embryos after 72 and 96 hour of incubation. HQ was injected into the air sacs of the eggs at doses ranging from 0.0625 to 40 micrograms per egg. On the 15th day of incubation, live embryos including controls were removed from the eggs and examined for gross malformations and body hemorrhage. The LD50 values for HQ at 72 and 96 hours of incubation were 8.59 and 15.63 micrograms per egg, respectively. HQ produced gross malformations such as body hemorrhage, curled claws, everted viscera, defective beak, exencephaly and monophtalmia in developing chick embryos at dosages ranging from 20 to 0.0625 micrograms per egg in both groups. The various types of malformations and their incidences in both groups were higher in comparison with the controls, but were not statistically significant. At no concentration did malformations occur greater, than 35 percent for both 72 and 96 hours of incubation. The results of this study reveal that HQ is embryo toxic at higher doses and that teratogenic potential of HQ is unremarkable.

The beneficial effect of cyclosporin-A on the no-reflow phenomenon in rat skin island flaps.

The no-reflow phenomenon is one of the factors that increase morbidity in flap and replantation surgery. Prevention and treatment of the phenomenon is an area of intense current research. This study investigated the possible effect of cyclosporin administered systemically on survival of skin flaps subjected to ischaemia-reperfusion injury. Cyclosporin treated flaps showed a statistically significant increase in survival areas regardless of the time of infusion (p < 0.01). These findings suggest that cyclosporin could be valuable in preventing or treating no-reflow in critical flaps. Possible mechanisms of action are discussed.

A case of ventricular tachycardia refractory to medical treatment related to a cystic mass in left ventricle--a case report.

This is a case report of a twenty-two-year-old woman who had attacks of ventricular tachycardia that could be prevented by the use of various antiarrhythmic drugs for three months. Electrophysiologic study showed that the mechanism of the tachycardia was "reentry." In cardiac catheterization a non-contractile mass having calcification was seen on the apical portion of the left ventricle. Angiographically, the mass resembled a diverticulum. Recurrence of attacks was terminated by resection of this mass, which had a cystic appearance.
