Intrarenal satellites of renal cell carcinoma: histopathologic manifestation and clinical implication.

OBJECTIVES: This study was designed to gain more insight into the incidence of satellites of renal cell carcinoma related to the pathologic stage and grade of the primary tumor. METHODS: One hundred eight nephrectomized kidneys with renal cell carcinoma were sectioned at 3-mm intervals and inspected microscopically for satellite carcinomas. The number, site, size, and distance of satellite carcinomas found were recorded in relation to the primary carcinoma. RESULTS: The overall incidence of satellite carcinoma was 6.5% (7 of 108). The incidence of satellite carcinoma in relation to the pathologic staging of the primary tumor was 7.1% (1 of 14) in pT1, 3.0% (2 of 66) in pT2, and 14.3% (4 of 28) in pT3. The incidence was higher in patients with N1 disease (25%) than in those with NO (5.0%), and higher in M1 disease (25%) than in those in MO (5.77%). There was no relationship between the grade and multicentricity. CONCLUSIONS: The incidence of satellite carcinoma was higher in cases with high-stage primary carcinoma. However, even in localized diseases (PT1, pT2), satellite carcinomas were found at at 3.75% incidence. When nephron-sparing surgery is indicated in low-stage cases, precise clinical staging using a combination of various imaging modalities is mandatory, with intraoperative ultrasonography being one possible modality for detecting a concomitant satellite carcinoma.

Retroperitoneal Ewing's sarcoma.

We report a case of extraskeletal Ewing's sarcoma arising in the retroperitoneal cavity. The tumor was excised en bloc with the left kidney and infiltrated portion of the left ureter. Histological diagnosis was confirmed by both electron micrography and immunohistochemical staining using a newly developed mouse monoclonal antibody 5C11, which specifically reacts with Ewing's sarcoma. An adjuvant chemotherapy T-11 regimen has been successfully preventing the recurrence.

[Light and electron microscopic examination compared to evaluation by tumor size in subrenal capsule assay for chemosensitivity testing in esophageal cancer].

Specimens from six clinical cases of esophageal cancer were transplanted under the renal capsule of AF nude mice, and chemosensitivity to anticancer drugs (UFT, CDDP) was evaluated by measuring tumor size (SRC). Histological analysis of the xenografts by light (LM) and electron microscopy (EM) was also performed to confirm the precision of this original method used for SRC. Two-dimensional morphometry by EM was also performed in four cases. With the exception of one case, the same results were obtained by graft measurement analysis and light microscopic observation, which allowed easy evaluation by observing cancer pearl formation, prominent proliferation of tumor cells or total cell keratinization. However, there was no false-positive sensitivity in the former method. EM observation did not reveal any special findings with regard to chemotherapy response, but increased numbers of cytoplasmic vacuoles and desmosomes, cytoplasmic swelling, a reduction of the N/C ratio and nuclear deformity implied common changes to cell necrosis induced by the anticancer drugs. For clinical use of SRC against esophageal cancer, it is supposed to be best to compare LM histological evaluation, which is too complicated and time consuming. For this reason, it is potentially useful to perform SRC by evaluating tumor size, because another goal of chemosensitivity testing involves determination of anticancer drugs without effectiveness.

Architecture and function of the extravascular fluid pathway: special reference to the macula cribriformis in the diaphragm.

Silver staining, transmission electron microscopy (TEM) and scanning electron microscopy (SEM) have been used to delineate the structure and function of the extravascular fluid pathway of the diaphragm of monkeys, rats, mice and rabbits. The submesothelial connective tissue was examined by SEM using the NaOH maceration method. A sieve-like structure, the macula cribriformis, was found in the submesothelial connective tissue on the peritoneal side of both the muscular and tendinous portions. The macula cribriformis, which consisted of densely-packed collagen fibrils, was oval, spindle and polygonal in shape and ranged in diameter from 20 to 300 microns with 5-300 foramina (3-15 microns). The macula cribriformis was located between peritoneal mesothelial cells with stomata and subperitoneal lymphatic capillaries. Intraperitoneally-injected latex particles were carried into the diaphragmatic lymphatic capillaries via the peritoneal stomata and the foramina of the macula cribriformis. These results support the hypothesis proposed by Kihara (Okajimas Fol Anat Jpn 28: 601-621, 1956) that the macula cribriformis is a pre-lymphocapillary pathway, one of the extravascular fluid pathways.

ITI (inter-alpha trypsin inhibitor) phenotyping by immunogold staining.

Polymorphism of Inter-alpha-Trypsin Inhibitor (ITI) was investigated in 685 unrelated Japanese individuals by isoelectric focusing followed by electroblotting and immunogold silver staining. The allele frequency estimates were ITI*1 = 0.455 +/- 0.013, ITI*2 = 0.516 +/- 0.013, ITI*3 = 0.028 +/- 0.0045 and ITI*4 = 0.001 +/- 0.001 and the phenotype frequencies fitted the Hardy-Weinberg equilibrium. The ITI system could be regarded as a potential genetic marker in paternity testing and anthropological studies.

Long-term follow-up study of patients with gastric adenomas with malignant transformation. An immunohistochemical and histochemical analysis.

BACKGROUND: Little is known about the changes in the immunohistochemical and histochemical characteristics that take place during the malignant transformation of gastric adenoma (GA). METHODS: Nine patients with GA who developed carcinoma (Group A) and 33 who did not (Group B) during an equivalent follow-up period (mean, 76.4 months; range, 38-166 months) were studied. Tissue sections from these patients were stained for p53, c-erbB-2, bcl-2, carbohydrate antigen 19-9 (CA 19-9), carcinoembryonic antigen (CEA), and proliferating cell nuclear antigen (PCNA), and by high iron diamine-alcian blue staining. The gastric or intestinal phenotypes of the GA cells were evaluated histochemically by paradoxical concanavalin A and galactose oxidase-Schiff reactions. RESULTS: In Group A, the following were more frequent; severe dysplasia (77.8% vs. 0%, P < 0.001), villous structures (66.7% vs. 9.1%, P < 0.05), sulfomucin secretion (88.9% vs. 39.4%, P < 0.05) and mixed gastric-intestinal phenotype (77.8% vs. 9.1%, P < 0.001). The development of carcinoma in Group A positively correlated with an increase in p53 expression and PCNA-labeling index. None of the adenomas was positive for c-erbB-2, but four of nine carcinomas showed positive reactions. CONCLUSIONS: Mucin histochemistry of GA may be of some value in assessing the potential for subsequent carcinoma. Immunoexpression of c-erbB-2, bcl-2, CA 19-9, and CEA have only limited value. In contrast to p53, the immunoexpression of c-erbB-2 is a late event in the malignant transformation of GA.

Laparoscopic surgical correction of circumcaval ureter.

Laparoscopic transposition and reanastomosis of a circumcaval ureter were performed in a 52-year-old man with right flank pain. A preoperative perfusion pressure study revealed abnormally high intrapelvic pressure. Under laparoscopy, the renal pelvis was divided above the ureteropelvic junction and the ureter was relocated from behind the vena cava. A 5 cm segment of redundant ureter containing the postcaval segment was resected and the ureteral end and renal pelvis were reapproximated with interrupted sutures by intracorporeal knot typing. The postoperative convalescence was uneventful, not necessitating the administration of analgesics. The patient resumed full activities 3 weeks later. The intravenous urogram and renogram obtained 2 months after the operation revealed remarkable improvement in the ureteral obstruction.

Antitumor effect of interleukin-2 entrapped in liposomes on murine renal cell carcinoma.

Antitumor effects of interleukin-2 entrapped in liposomes (IL-2 liposomes) on Renca, murine renal cell carcinoma of spontaneous origin, were evaluated by in vivo and in vitro experiments. As a local treatment model, IL-2 liposomes at a dose of 1.0 x 10(4) units were for 7 days injected adjacent to the Renca tumors inoculated in the back of BALB/c mice. The treatment inhibited the tumor growth and prolonged the survival time of mice significantly compared to the control (p < 0.01). Serum IL-2 levels after subcutaneous administration of IL-2 liposomes confirmed its slow releasing mechanism into blood circulation. The spleen cells from mice following the IL-2 liposome treatment showed higher cytotoxicity in vitro than that of IL-2 alone using Renca, YAC-1, and P-815 cells, which was determined by 51Cr-release assay. Indirect immunoperoxidase staining of tumor-infiltrating lymphocytes (TILs) showed that accumulation of Lyt-2+ and L3T4+ cells were seen in the tumor treated with IL-2 liposomes. These results indicate that IL-2 liposomes have a long-acting cytotoxicity against renal cell carcinoma caused by a slow release mechanism generated through the efficient local immune response mediated by TILs.

[The heterogeneity of periodic acid-thionin schiff/KOH/PAS staining property and the histological grading of atypia in colorectal carcinoma and adenoma].

To assess the mucin changes associated with the development of colorectal cancer, the periodic acid-thionin schiff (PAT)/KOH/PAS staining properties of 38 advanced carcinomas, 8 submucosal invasive carcinoma, 29 mucosal carcinomas (adenomas with severe atypia), 19 adenomas with moderate atypia and 38 adenomas with mild atypia were studied with special attention to the heterogeneity of staining property within each lesion. 6 submucosal carcinomas and 28 mucosal carcinomas were judged to have benign adenomatous portion using hematoxilin-eosin staining. The more advanced the carcinomas, or the higher the grade of atypia of adenomas, the fewer the lesions of which goblet cell mucus (GCM) and the mucus at the luminal surface or that in the luminal accumulation (SC) stained red. SC frequently stained bluer than GCM in highly atypical glands. The staining properties of benign adenomatous portions concomitant with submucosal or mucosal carcinomas were similar to those of benign adenomas, but not to their carcinomatous foci. These findings suggest that the changes in the PAT/KOH/PAS staining property associated with the development of colorectal carcinoma mainly occur in SC, and that the majority of submucosal and mucosal carcinomas were 'carcinoma in adenoma'.

Polymorphism of factor H (beta 1H globulin) in Japanese: description of a new allele.

Polymorphism of factor H (HF) was investigated in 1060 unrelated Japanese individuals using isoelectric focusing and immunoblotting. Besides 6 different HF types a null type and an unusual type were observed. The family analysis suggested the hereditary occurrence of a new variant allele HF*C. The population data fitted the Hardy-Weinberg equilibrium, assuming that the null allele HF*QO occurs commonly. The allele frequencies were HF*A = 0.407 +/- 0.011, HF*B = 0.491 +/- 0.011, HF*A1 = 0.011 +/- 0.002 and HF*QO = 0.091 +/- 0.006. The HF polymorphism in Japanese was shown to be controlled by the above 4 common alleles.

GC subtyping in dental pulps.

By means of non-immobilized isoelectric focusing followed by immunoblotting, group-specific component (GC) subtypes were detected in dental pulp tissues. The amount of GC in dental pulp was quantitated using rocket immunoelectrophoresis and the minimum amount of the protein necessary for the typing of GC was estimated to be about 0.02 microgram in 10 microliters of the sample. The results of GC typing in dental pulp tissues were in agreement with the phenotypes obtained from serum samples of the same individuals. Subtyping by the present method was also possible in dental pulp tissues obtained from the teeth stored for 4 weeks at room temperature. The GC subtyping by non-immobilized isoelectric focusing is of practical use for medicolegal investigation of teeth.

Randomized trial comparing three forms of pelvic floor repair for neuropathic faecal incontinence.

A randomized controlled trial in women with neuropathic faecal incontinence compared total pelvic floor repair (n = 12) with anterior levatorplasty and sphincter plication alone (n = 12) and postanal repair alone (n = 12). Review at 6 and 24 months indicated that results were significantly better for total pelvic floor repair than either of the other procedures. Complete continence was achieved in eight of the 12 patients 2 years after total pelvic floor repair. Only total repair significantly elongated the anal canal. Both total pelvic floor repair and anterior levatorplasty improved sensation in the upper anal canal.

Biodegradation of random copolypeptide membranes consisting of N-hydroxyalkyl L-glutamine as one component.

Two component random copolypeptide membranes, consisting of N-hydroxyalkyl L-glutamine and L-alanine or L-leucine were prepared by carrying out aminolysis reactions with 2-amino-1-ethanol (E) or 5-amino-1-pentanol (Pe), together with a cross-linking reaction with 1,8-octamethylenediamine (OMDA) on membranes of the starting copolymers consisting of gamma-benzyl L-glutamate (B) and L-alanine (A) or L-leucine (L). The relationships between their bulk structure and membrane properties were investigated, such as the swelling ratio in water, aqueous vapour permeability, tensile properties and enzymatic degradation behaviour of the membranes in a pseudo-extracellular fluid (PECF). The tensile properties of the hydrophilic membranes were highly dependent on the swelling ratio of PECF, and the hydrophobicity of the side chains, whose behaviour was typical of an elastomer. We showed that a common relation was obtained between the rate of water vapour permeability and the swelling ratio of membranes in PECF despite the difference of the nature of the side chains. Biodegradation of these membranes in vitro by bromelain indicated that the degradation was a bulk rather than a surface phenomenon, and that the rate of degradation was also highly dependent on the swelling ratio of samples and on the hydrophobicity of the side chains of samples.

Functional morphology of the lymphatic system in the monkey diaphragm.

A morphological study of the lymphatic system in the diaphragm of adult Japanese monkeys was carried out using intraperitoneal injection with India ink, light microscopy, transmission electron microscopy (TEM) and scanning electron microscopy (SEM) techniques. For SEM examination of the subperitoneal connective tissue, the peritoneal mesothelium was stripped by either the NaOH/sonication microdissection techniques or the NaOH digestion method. Light microscopy of 1 micron Epon sections and TEM of thin sections demonstrated that lymphatic capillaries were distributed in the subperitoneal connective tissue of the muscular and tendinous portions. Gaps (stomata) between neighboring cells of the peritoneal mesothelium were occasionally encountered. In SEM observations, stomata with oval shapes and various sizes were abundantly seen in the peritoneal mesothelium, but were absent from the pleural mesothelium. It deserves particular note that a large number of foramina were detected in the subperitoneal connective tissue. The cluster of the foramina was similar in appearance to the "macula cribriformis" depicted by KIHARA (1956). Carbon particles injected into the monkey peritoneal cavity passed through the peritoneal stomata and then the macula cribriformis, ultimately to enter the lymphatic capillaries. This paper also discusses the functional implication of the lymphatic system in the diaphragm.

Transferrin subtyping in dental pulps.

Serum transferrin (TF) subtypes were also found in dental pulps by isoelectric focusing and immunoblotting. The types observed in dental pulps completely agreed with those in serum samples from the same individuals. The allele frequencies in 105 samples were TF*C1 = 0.757 and TF*C2 = 0.243. Reliable subtyping was possible for 4 weeks following extraction of the teeth. The TF system can provide a useful genetic marker for the medicolegal individualization of teeth.
