A new type of metal bracket was prepared to minimize the destruction at the bracket-resin interface and to suppress resin from remaining on the tooth surface in debonding. The brackets were made of stainless steel and had a modified cylindrical outward shape (phi 3.8 mm x 1.8 mm), twin-type wings, and a 0.018 inch (0.457 mm) slot. Retentive forces and debonding aspects of residual resin for the brackets were studied using three representative adhesives. The mean tensile strengths at the bracket-resin interface were over 80 kgf/cm2 for all three resins. As the quantitative evaluation for the residual resin condition, a resin remaining ratio (RRR) was adopted. The aspects of the residual resin differed according to the type of adhesive resin used. Almost no resin was detected on the tooth surface just below the central part of the bracket, and even for the peripheral part which did not have an effective resin retention form, the RRR was under 30% for the heavily filled resin.
Titanium is strong, resists corrosion and has a low density and excellent biocompatibility. Conventional ceramic-metal restorations have been extensively used in dentistry because of their esthetic appearance and good mechanical properties. This study investigates oxidation effects on the porcelain-titanium interface reactions and bond strength. Pure titanium was treated in a porcelain furnace at temperatures of 600 to 1000 degrees C under either vacuum or air. X-ray diffraction analysis of the surface of pure titanium revealed that the relative peak intensity of alpha-Ti decreased and that of TiO2 increased, with increasing firing temperature. The Vickers hardness number of titanium increased with temperature especially over 900 degrees C, and was harder in air than in vacuum. The tension-shear bond strength of the porcelain-titanium system was the highest in the green stage and lowest after 900 degrees C treatment. Metallographic microscopy of the porcelain-titanium interface revealed a thick band-like zone in the sample treated over 900 degrees C. The excess thick layer of TiO2 apparently weakened the bond strength of porcelain-titanium. Unlike the conventional ceramic-gold alloy system the recommended degassing procedure was not suitable for porcelain-pure titanium restoration.
Between January 1977 and December 1989, 140 patients of Stage III breast cancer were treated in Sapporo Medical College. Sixty-six of these patients received intra-arterial infusion chemotherapy. The anticancer drugs were mainly given by two routes, infusion into the internal mammary artery and the subclavian artery. Continuous infusion of 5-FU and intermittent injections of ADR, MMC, 4'-epi-ADR or THP-ADR were jointly or individually made in each artery. The 5-and 10-year overall survival rates were: infusion group 49.2% and 49.2%, non-infusion group 64.0% and 45.0%, respectively. Intra-arterial infusion chemotherapy seems to be useful because non-infusion group contained mostly Stage IIIa and conversely the infusion group contained mostly Stage IIIb. A significant difference was seen between 5-FU infusion group and MMC.ADR group (p=0.026). MMC group, MMC + ADR combination group and 4'-epi-ADR group were marginally significantly different in terms of survival rates of the anticancer drugs of Stage IIIb.
A new type of ceramic bracket was prepared to minimize the destruction at the bracket-resin interface and to suppress resin remaining on the tooth surface in debonding. The brackets were of a chemical bonding type, and had a box-formed cavity in the bracket base to hold resin, a 0.018 inch slot, and twin-type wings. Impact strengths and debonding aspects of the residual resin were studied. The impact strengths of the brackets seemed to exceed the desired level, compared to other control ceramic brackets or metal brackets. A resin remaining ratio (RRR) was adopted as the quantitative evaluation for the residual resin condition. Mean RRR's of the brackets were about 10%, which was a very low value in comparison to the other brackets.
Porcelain-fused-to metal systems are popular in restorative dentistry. Residual stress caused by a mismatch of the thermal expansion coefficient of metal and porcelain induce clinical failure of the restoration. Titanium and its alloys have been gradually used in dentistry for their adequate strength, good corrosion resistance and excellent biocompatibility. Their oxidation characteristics and low thermal expansion coefficients capacitate for matching the low fusing porcelains. This investigation examined thermal compatibility of pure titanium with two low fusing porcelains and one high fusing porcelain. Thermal expansion coefficients (alpha) of pure titanium and porcelains were measured from room temperature to 600 degrees C at a mean heating rate of 5 degrees C/min. Difference in thermal expansion values (delta alpha) was also calculated to present the residual stress in each titanium-porcelain pairs. The bonding patterns of titanium to each opaque was demonstrated similar results from the predictions of alpha and delta alpha values.
Fe-containing hydroxyapatites and fluoridated hydroxyapatites were synthesized at 80 degrees C and pH 7.4. Fe-containing hydroxyapatites became poorly crystallized with the increase of Fe2+ ions in the solution and a- and c-axis dimensions seemed to decrease slightly. The apparent solubility of Fe-containing hydroxyapatites decreased greatly with the increase of iron content in spite of the decrease in crystallinity. Fe-containing fluoridated apatites were less well crystallized than Fe-free fluoridated apatites. The Fe2+ uptake of fluoridated apatites was independent of fluoride concentration in the solution. a-Axis dimensions of Fe-containing fluoridated apatites decreased with the degree of fluoridation in addition to the decrease related to the substitution of Fe2+ ions. The apparent solubility of Fe-containing fluoridated apatites decreased more than that of Fe-free fluoridated apatites especially at a low fluoride content.
The spontaneous rupture of hepatocellular carcinoma (HCC) constitutes usually critical and life threatening condition because of hypovolemic shock due to massive hemorrhage, underlying liver cirrhosis and extensive tumor growth. Recently, transcatheter arterial embolization (TAE) has been used for controlling arterial bleeding of spontaneous rupture of HCC. We report a long surviving case (1 year 9 months) with stage IV ruptured HCC treated by emergency TAE.
Stereotactic aspiration for intracerebral hematoma has become widely used due to improvements in computed tomography (CT). CT-guided stereotactic aspiration for hypertensive intracranial hematomas was performed in 46 cases. Stereotactic aspiration with coordinate software was carried out among 16 cases. We compared the clinical results of conventional stereotactic aspiration with results of the new method using coordinate software. Using coordinate software, we can examine the route for the target point, as well as the target point of the hematoma before surgery. Therefore we can avoid obstacles such as ventricles and internal capsules. Further, we can aspirate hematoma by using only this one procedure. Mean hematoma aspiration percentage has risen from 44.9 to 87.2% by using coordinate software. Rate of drainage tube insertion after operation has decreased from 90 to 50%. The period of placement of the drainage tube and the frequency of infection due to the drainage tube has become lower, too. Therefore we can say that the outcome of treatment of hypertensive intracranial hematomas is improved by using coordinate software.
A 67-year-old female was admitted to our hospital, because of high fever and dry cough. She had undergone semiradical hysterectomy and radiation therapy for carcinoma of the uterine cervix one year previously. Her chest roentgenograms on admission showed lobar consolidation of the left upper lobe. Antibiotics were administered but her general condition and pulmonary consolidation did not improve. As Legionellosis was highly suspected, we performed bronchoscopic examinations. Bronchial mucosa was almost normal with no secretion, and Legionella was isolated from the specimen obtained bronchoscopically. Six days later, L. pneumophila serogroup 2 was isolated and identified from an intratracheal aspiration, and serological diagnosis was made by indirect immunofluorescence antibody. We could also detect the bacteria in the BALF by immunofluorescence microscopy and in the tissue of the TBLB specimen with the ABC method.
To evaluate acute lung injuries and their persistence in patients with summer-type hypersensitivity pneumonitis, repeated bronchoalveolar lavage (BAL) and pulmonary function tests were performed. BAL was performed on 36 occasions in 17 patients with hypersensitivity pneumonitis. Nineteen BAL procedures in 16 cases were done during the active phase within the three hospital day and BAL was repeated in 8 cases during inactive phase. Anti-Trichosporon cutaneum antibodies were detected in all of 15 cases examined using the Ouchterlony method and indirect immunofluorescent methods. The number of total BAL cells, lymphocyte and neutrophils were increased in the active phase, and OKT4/OKT8 was quite low (0.39). As the disease became inactive, the number of total BAL cells, lymphocytes and neutrophils decreased. On the other hand OKT4/OKT8 increased quickly. Significantly negative correlations were recognized between the number of BAL lymphocytes and %VC, and lymphocytes and %DLCO4. More improvements in BAL findings, %VC and %DLCO one year after acute episodes were seen in cases that moved house than cases that did not move, and these often relapsed. We concluded that complete clearing of the patient's house or moving out, if necessary, were needed to avoid relapse and persistent lung injuries.
Hemobilia occurred in a 70 year old female from the left hepatic artery pseudoaneurysm induced by PTBD. A Tracker-18 catheter introduced into the left lateral posterior hepatic artery superselectively and embolization was performed with both sponge gel and Ivalon. This new coaxial catheter system allowed selective occlusion of the hepatic artery pseudoaneurysms with minimal loss of liver function.
Spinal instrumentation surgery has come into wide use recently, especially for tumor of the spine. It is very effective for fixing the spine for relieving the patient from pain. But the decision about the indication of instrumentation surgery for tumor of the spine must be made carefully. In cases of double malignant tumors including spinal tumor, it is hard to decide how to treat tumor of the spine. We report a case of 78-year-old male with multiple myeloma of the cervical spine and cancer of the prostate. The patient had been in good health until one year previously, when he started to complain of nuchal pain. He became bedridden with loss of voluntary control of the right upper extremity for the last two months. Then he was admitted to our hospital and radiological examination was carried out. It revealed tumor of the cervical spine and it seemed to be bone metastasis. Further examination revealed that the patient was suffering from multiple myeloma and cancer of the prostate. The tumor of the spine was diagnosed as either multiple myeloma or metastasis of cancer of the prostate. Spinal instrumentation surgery using Luque-rod was carried out as one of the comprehensive treatment plans for double malignant tumors. The instrumentation surgery was effective for fixing the spine and improving the patient's quality of life. In operative procedures, intraoperative spinal cord monitoring was carried out. Intraoperative spinal cord monitoring was indispensable for preventing spinal cord injury. The tumor of the spine was diagnosed as multiple myeloma by pathological examination.(ABSTRACT TRUNCATED AT 250 WORDS)

We present 5 patients with hemobilia treated successfully by emergency transcatheter hepatic artery embolization (TAE). All patients had severe associated illnesses and were very poor candidates for operative treatment. In 3 patients, hemorrhage was completely controlled by TAE using sponge alone, however in 2 patients with hepatic artery pseudoaneurysm, additional TAE using coil was necessary for hemostasis. No major complication was encountered after TAE. Since angiography can offer both diagnostic and therapeutic benefits, we feel that angiography should be considered for exact localization of hemobilia and possible transcatheter therapeutic embolization for control.
Emergency transcatheter arterial embolization was performed in 6 patients with massive duodenal bleeding. Three out of 6 patients were bleeding from duodenal ulcer and erosion, 2 from aneurysm of the gastroduodenal artery (GDA) and 1 from duodenal tumor. In 4 cases, complete and permanent cessation of bleeding was obtained by GDA embolization alone and additional pancreatico-duodenal artery embolization were necessary for hemostasis in 2 patients with GDA pseudoaneurysm. One patient maintained by hemodialysis had complication of duodenal infarction and pancreas necrosis following this procedure. Emergency embolization represents a useful alternative to the operative management of massive duodenal bleeding and should be the initial treatment of choice in those patients.
A case of 22-year-old female with mediastinal tuberculous lymphadenitis and pericostal tuberculosis was reported. Her complaint was right chest pain and subcutaneous mass on the right chest wall. Chest contrast CT showed right paratracheal lymph node swelling with central low density area and surrounding rim enhancement, which has been reported as typical characteristics of mediastinal tuberculous lymphadenitis. Pigeon-egg sized subcutaneous mass with fluctuation was palpable on the right sternal border and the smear of its content showed acid-fast bacilli. In spite of two months therapy with antituberculous drugs, both masses were unchanged in size. The lesions resected surgically, were both encapsulated abscesses containing yellowish pus, and microscopic examination of these specimen disclosed the finding of tuberculosis. Mycobacterium tuberculosis was cultured from contents of both masses. After nine months of anti-tuberculous therapy, no sign of recurrence is observed until now. Both masses were discontinuous and the possibility of lymphangitic spread of organism was speculated as its etiology.
Emergency TAE was employed in 17 patients with massive hemorrhage from gastric ulcer. All patients were considered unacceptable or extremely high risk candidates for emergency surgical control of hemorrhage. In 12 patients, hemorrhage was completely controlled and their prognosis has been fair. Partial control in 4 patients were related to underlying severe coagulopathy and arterio-sclerosis. 1 case with ulcer which received a dual blood supply from left gastric artery (LGA) and small LGA originated from the splenic artery failed to stop bleeding. No major complication was noted after these procedures. Emergency TAE should be the initial treatment of choice in patients with massive gastric bleeding.
