Between January 1977 and March 1991, 86 patients of Stage III and Stage IV breast cancer were treated by intra-arterial infusion chemotherapy (IAIC) in Sapporo Medical College. The 5- and 10-year overall survival rates of Stage III b were: infusion group 53.1% and 53.1%, non infusion group 50.1% and 16.7%, respectively. A significant difference was seen in the 10-year survival rate (p less than 0.05). The 5- and 10-year overall survival rate of Stage III b histologically effective group was 67.3% and non-effective group 31.8%. A significant difference was seen between these two groups (p = 0.0241). A significant difference was seen between n2 positive Stage III b infusion group and non-infusion group in 7-year survival rate (p less than 0.01). The ischemic effect on tumor cells by degeneration changes of the blood vessels was important for the histological effect. Pre-operative angiography helped in the choice of anticancer agents. The sufficient adjuvant chemo-endocrine therapy will assure a good prognosis for IAIC-treated Stage III b breast cancer patients.
The usefulness of emergent embolotherapy was evaluated in 17 patients with life-threatening hemorrhage from 18 ruptured splanchnic artery pseudoaneurysms. Complete hemostasis was obtained in 16 out of 17 patients by embolotherapy. Of the 12 initial embolization procedures with permanent embolic materials including stainless steel coils, microcoils, and Ivalon, complete cessation of bleeding was obtained in 11. On the other hand, 6 out of 9 initial embolization procedures with Gelfoam particles failed to halt bleeding, and additional embolization with permanent embolic materials was required. Emergent embolization with permanent embolic materials using superselective catheterization should be considered the initial treatment of choice for ruptured splanchnic artery pseudoaneurysms.
Hematomas of the basal ganglia in head injury have long been recognized by pathologists with an interest in head injury but their mechanism has not been revealed clearly. We report two cases of bilateral traumatic hemorrhage in the basal ganglia. Case #1, a 17-year-old male was admitted to our hospital immediately after a traffic accident. Neurological examination revealed that the patient was comatose and had right hemiparesis. CT scan showed bilateral hemorrhage of the basal ganglia and subarachnoid hemorrhage in the perimesencephalic cistern. MRI showed high signal intensity areas in the bilateral basal ganglia, perimesencephalic cistern, cerebral white matter and corpus callosum. The patient was diagnosed as having diffuse axonal injury coinciding with bilateral hemorrhage of the basal ganglia. Stereotactic aspiration for the hematoma of the left basal ganglia was carried out. Case #2, a 75-year-old male was admitted immediately after falling from the roof of his house. Neurological examination revealed no neurological deficit except for headache and nausea. CT scan on the day of injury revealed no abnormality. But CT scan 12 hours following the injury showed bilateral hemorrhage of the basal ganglia. Blood pressure of the patient was within normal range and he was diagnosed as having traumatic bilateral intracerebral hematoma. Conservative treatment was carried out and the patient was discharged 7 days after injury with no neurological deficit. The mechanism of traumatic hemorrhage of the basal ganglia has not been clear. In case #1, diffuse axonal injury (DAI) may have played an important role in the bilateral hemorrhage. But in case #2, non-DAI factor such as vasoparalysis syndrome may have existed.(ABSTRACT TRUNCATED AT 250 WORDS)
Binding sites of atrial natriuretic peptide in the cerebral microvessels and choroid plexus of spontaneously hypertensive rats (SHR) and Wistar Kyoto control rats (WKY) were measured. In the microvessels, the number of ANP binding sites was lower in SHR than WKY, but there was no difference in affinity of binding sites for the ligand between SHR and WKY. In the choroid plexus, the number of ANP binding sites was lower and the affinity for the ligand was higher in SHR than in WKY. These results suggest that the physiological function regulated by the ANP receptors in the cerebral microvessels and choroid plexus were altered in hypertension and that ANP receptors in the cerebral microvessels and choroid plexus were differentially regulated.
Subtypes of protein kinase C were determined in tissues derived from neural crest. The bovine adrenal medulla and rat superior cervical ganglia contained type III enzyme as the major subtype with a small amount of type II enzyme. In PC12 cells, treated with or without nerve growth factor, type III was the major subtype but a minor peak which is distinct from types II and III was observed. These results show that the type III enzyme is prevalent in various differentiation stages of neural crest and suggest that the enzyme relevant to the regulation of catecholamine synthesis and release in these cells is type III.

The gene for multiple endocrine neoplasia type 2A (MEN 2A) is closely linked to RBP3 (retinol-binding protein 3, interstitial, probe IRBP.H4) and the DNA marker D10S15 (probe pMCK2), which have been assigned to the proximal long arm of chromosome 10 by linkage analysis both in Caucasian and Japanese populations. We have constructed a rare-cutting restriction map around the RBP3 and D10S15 loci by pulsed-field gel electrophoresis (PFGE). The RBP3 and D10S15 loci appeared to be within a single 160 kb MluI fragment. In 5 patients with MEN 2A, gene rearrangements, such as a gross deletion, were not found in the 880 kb NruI fragment which covered the closest region to the MEN-2A locus from the RBP3 and D10S15 loci.
Omental pedicle flaps were used in the treatment of patients with acute and chronic empyema with bronchopleural fistula. In 5 patients (group 1) with postoperative acute empyema owing to bronchial stump fistula, an omental covering was applied as a reinforcement to close the fistula. Six patients in group 2 with chronic tuberculous or Aspergillus empyema with multiple fistulas initially underwent open-window thoracostomy or cavernostomy and secondarily had omental transposition. In all patients, the right gastroepiploic vessels were used to provide the blood supply for the flap. Successful closure of the bronchial stump was obtained in all patients in group 1, but 2 of them died of recurrence of their underlying lung carcinoma within 1 year. Five of the 7 patients in group 2 had a favorable outcome, but 2 patients had partial recurrence after omental plombage. From our experience with these patients, we believe that the omental flap is effective for closing fistulas due to postoperative or chronic empyema but has only limited success in patients whose lungs are severely damaged by persistent infection.
To examine the response to biological hard tissues, a carbonate-containing hydroxyapatite with chemical composition and crystallinity similar to those of bone was synthesized at pH 7.4 and 60 degrees C. The apatite powder was mixed with collagen solution, whose antigenicity had been removed by enzymatic treatment, and formed into apatite-collagen pellets. After insolubilization by UV-irradiation for 4 h, the composites showed remarkably reduced disintegration and maintained their shape under 3.6 MPa of stress after 1 wk incubation in 0.9% NaCl solution. They showed good biocompatibility when implanted beneath the periosteum cranii of rats. The UV-irradiated sample kept its features well and was packed with newly created material 3 wk after implantation.
Hydroxyapatite (HA) and beta-tricalcium phosphate (TCP) were studied by computer graphics with a personal computer. Crystal models of fluoridated hydroxyapatite and fluorapatite, in which F- ions are substituted into some or all OH- positions, were also drawn. Data on the structural coordinates of the hydroxyapatite and beta-TCP crystals were put into a protein graphics program in Angstrom units. The connection of each element, with front and rear, was displayed by the shade-line erasing method, and the solidity of the image was expressed by the degree of lightness from the light source. The colour tint was obtained by the Raster graphics method. A fine picture can be obtained with the photo program. Rotating the graphics freely around the X, Y, and Z axes gives a view from any direction. The computer graphics demonstrated visually that F- ions contribute to stabilize the hydroxyapatite crystal and that beta-TCP has an unstable structure due to 0.5 probability of Ca2+ ions and [PO4-Ca-Ca-Ca-PO4] zigzag chain.
The activity of phospholipase D from the Savoy cabbage on alkyl phosphorylcholines containing alkyl groups of C14 to C22 was examined. Apparent Km and maximal velocity values in the presence of diethyl ester were 3.6-8.3 mM and 0.14-0.29 mumol/(min.mg protein), respectively. The maximal velocities for alkyl phosphorylcholine homologues were lower than the value for pig phosphatidylcholines, but higher than those for lysophosphatidylcholines and synthetic phosphatidylcholine with saturated acyl chains. Results suggest that solubilities of the substrates in the organic solvent play an important role in activity observed with phospholipase D.
The effects of single and multiple moxibustions on platelet function, blood coagulation and fibrinolytic activity in ddY mice were studied. The increase in platelet aggregation and ATP-release after a single moxibustion was dependent on moxa weight and the kind of platelet stimulus. Blood coagulative activity tended to increase in the early phase after a single moxibustion. However, multiple moxibustions maintained the homeostasis on blood coagulation and fibrinolytic activity. This investigation suggests that the effects of moxibustion on platelet functions and coagulative and fibrinolytic activities cause an enhancement of the phagocytic activity in the host defense mechanism.
The effects of a single cone moxibustion on cutaneous blood vessel and microvascular permeability in ddY mice have been studied by microscopical observation and by the dye leakage technique. In our results, moxibustion induced some kind of inflammatory response such as the blood vessel reaction with leukocytes exudation and the enhancement of microvascular permeability, which is considered one of the host defense mechanisms.
Changes in coagulative and fibrinolytic activities were studied in rats with hyperlipidemia induced by Triton WR-1339 (T-WR). After intravenous injection of T-WR (150, 200 or 300 mg/kg) into S.D. rats, dose-related increases in plasma lipids (total cholesterol, triglyceride, free cholesterol and phospholipid) were observed. In hyperlipidemic rats that received 300 mg/kg of T-WR, decreases in red blood cell count and Hb value were found. Significant increases in the ma value of the thromboelastogram and the fibrinogen level were observed in these T-WR treated rats. The alpha 2-plasmin inhibitor activity was found to decrease dose-relatedly. These results indicate that T-WR induced hyperlipidemia in rats is accompanied with an increase in coagulative activity and an indirect enhancement of fibrinolytic activity.
Brain lipid composition was studied in thiamine deficient rats treated with thiamine antimetabolites (oxythiamine: OT, and pyrithiamine: PT) and thiamine deficient diet (TDD). After intraperitoneal injection of OT (40 mg/kg/day) or TDD feeding for 6 days, body weight gain decreased. However, the PT (500 micrograms/kg/day) treated rats or the pair fed control (PFC: TDD + thiamine of 5 mg/kg, i.p.) showed no decrease in body weight gain compared with the regular diet control (C). Brain lipid levels (total lipid, total cholesterol, triglyceride, phospholipid, sphingomyelin and cerebroside) were examined in four brain regions (cerebral cortex, subcortical structure, brain stem and cerebellum). Total lipid level increased in four regions in OT or TDD treated rats, but total lipid level in the cerebellum in PT treated rats decreased. Total cholesterol level increased in all treated rats, while the triglyceride level in the brain stem decreased dramatically in OT or TDD treated rats. Cerebroside levels of four regions in the PT, OT or TDD group remarkably decreased, and PFC rats showed a significant improvement of the decrease in cerebroside level. It is conceivable that these changes in brain lipid composition provided some clues for the histological and morphological changes of the brain as manifested by the myelin degradation in acute thiamine deficiency.
Distal extention denture has intrinsic characteristics of cantilever and inevitably rotates around abutment tooth due to the difference of force transmissive characteristics between abutment tooth and alveolar mucosa. This study was undertaken to figure out the possible forms of advanced unilateral clasp denture and the principle to design them in the co-operation of direct and indirect retainer, and telescope retainer. The results were as follows: 1. Splinting of abutment resulted in much more efficient dispersion of load and in reduction displacement of abutment and denture base. 2. Generally the distal extention denture without indirect retainer exhibited considerable amount of displacement of abutment and denture base. Therefore this type of denture could not be an option upon designing unilateral denture. 3. Adequate organization of splinting of abutment, guide plane, and retainer with millinged abutment on clasp design denture was shown to improve retaining power as high as that of telescopic design denture. 4. Telescopic design denture with multiple abutment was much more preferable than that with single abutment. The amount of displacement of abutment and denture base could be greatly reduced by addition of positive-type indirect retainer to telescopic design denture with multiple abutment.
