[Predictable factors in healing of gastric ulcer, comparative analysis of local morphologic factor of gastric ulcer and suppression of intragastric pH].

Predictable factors in healing of 48 patients with gastric ulcer by PPI or H2-RA on 4 and 8 weeks of treatment were investigated. We evaluated the following factors such as patient's profile, local morphologic factors of gastric ulcer and suppression rate of 24 hr intragastric pH between healed and unhealed patients using single and multiple variable analysis. Results obtained from multiple variable analysis, the significant and independent factors of intractable gastric ulcers on the 4th week of treatment were ulcer size in over 20 mm and solitary ulcer. On the contrary, those of 8th week of treatment were shortage of holding time of intragastric pH over pH4 (less than 16.7 hrs) during treatment, the site of ulcer at lesser curvature of gastric angle and the shape of ulceration which showed mucosal overhanging. Considering with single and multiple variable analysis, we concluded that, in early phase, local shapes of the ulceration were important predictors in the healing of gastric ulcer, and the after that, the inhibition of intragastric acidity were significant factors in healing process of gastric ulcer.

[Effect of reimplantation response on recipient pulmonary hemodynamics in single lung transplantation].

Until now, reimplantation response (RR) has been thought to be a transient dysfunction of the transplanted lung in single lung transplantation. How does RR influence on the native lung in single lung transplantation? We studied the influence of RR to the native lung with 20 dogs which were divided separated into 3 groups. In group I (6 dogs) and II (6), left lung allotransplantation without immunosuppression was performed. In group II, PGI2 was administered to the donors at 1 microgram/kg/min for 60 minutes iv drip infusion during lung extraction and super oxide dismutase (SOD) was administered to the recipients at 20 mg/kg just before reperfusion of the transplanted lung. In group III (8), allotransplantation with immunosuppression (CYA 20 mg/kg, AZ 4 mg/kg) was done using PGI2 and SOD in the same way as group II. In all groups doppler flow probes were attached to the ascending aorta and left pulmonary artery. For over 1 week, pulmonary hemodynamics was evaluated. In group I pulmonary arterial pressure increased up to 38.7 +/- 19.7 mmHg on 3 postoperative day, which showed significant difference (p < 0.05) from group II. An increase in vascular resistance of the transplanted lung was recognized right after transplantation in the both groups (0.212 +/- 0.058 mmHg/ml/kg to 0.487 +/- 0.114 in group I, 0.207 +/- 0.072 to 0.407 +/- 0.172 in group II). However, vascular resistance of the right lung increased in group I against group II right after transplantation (0.182 +/- 0.116 to 0.367 +/- 0.195 in group I, 0.183 +/- 0.116 to 0.214 +/- 0.149 in group II), which showed significant difference (p < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Long-term follow-up study of Crohn's disease in Japan. The Research Committee of Inflammatory Bowel Disease in Japan.

Two hundred and three patients with Crohn's disease seen at Kyushu University, Fukuoka University, and their affiliated centers from 1973 to 1988 were followed for 4.4 +/- 3.2 years (mean +/- SD), to evaluate the prognosis relative to life span and surgery. Cumulative survival rate and cumulative operation rate were calculated by the life table method. Cumulative survival rates 5 and 10 years after diagnosis were 98.9% and 98.9%, respectively; these figures were not different from the expected survival rates in the sex-and age-matched general population. Cumulative operation rates 5 and 10 years after the onset of symptoms were 16.2% and 39.1%, respectively, In a second study, we investigated 419 patients with Crohn's disease, diagnosed at nine institutions from 1975 to 1990. The follow-up period was 6.3 +/- 3.9 years and the information was obtained from mailed questionnaires. Cumulative survival rates 5 and 10 years after diagnosis were 99.2% and 96.9%, respectively. Cumulative operation rates 5 and 10 years after the onset of symptoms were 30.3% and 70.8%, respectively. These results indicate that the prognosis of Japanese patients with Crohn's disease may be superior to that reported in Western countries.

Dental health indicator based on a questionnaire.

Dentistry has lacked an effective indicator of the impact of dental problems on a person's daily life. Subjective factors in dental health need to be included in order to improve current indicators. The purpose of this research was to develop a new type of indicator recorded from a questionnaire which takes account of subjective factors in dental health. The indicator we constructed has the following advantages. 1. Calculation process is simple in practice. 2. The indicator values range from 0 to 100, with values closer to 100 indicating a more favorable condition for the individual. 3. The distribution of indicator values is similar to the normal distribution. 4. The score of each item is reflected on the indicator, suggesting the individual's characteristics in terms of dental health. 5. Although it is principally an indicator for individuals, it can also be applied to groups. This study will also provide a model for the preparation of a dental health indicator.

Isolation of mouse complement component C7.

Mouse complement component C7 was purified from serum by a sequential procedure of fractionation precipitation by ammonium sulfate, followed by DE-52 anion exchange chromatography. Protein G affinity column chromatography, Mono S cation exchange chromatography and Superdex 200 gel filtration. The final product contained a highly purified mouse C7 component showing a single band on SDS-PAGE at the apparent Mrs of 90 kDa and 100 kDa under non-reduced and reduced conditions respectively. The yield of C7, which was measured by the biological activity, was 7.0%

Antihypertensive effects of BQ-123, a selective endothelin ETA receptor antagonist, in spontaneously hypertensive rats treated with DOCA-salt.

We determined the antihypertensive effects of BQ-123 (cyclo(D-Trp-D-Asp-L-Pro-D-Val-L-Leu-), sodium salt), a selective endothelin ETA receptor antagonist, in spontaneously hypertensive rats treated with deoxycorticosterone acetate-salt (DOCA-salt SHR). BQ-123 (1-30 mg/kg/h) decreased blood pressure in DOCA-salt SHR in a dose-dependent manner, although plasma immunoreactive endothelin-1 did not significantly increase and the maximal contractile response to endothelin-1 in the aorta significantly decreased as compared with values observed in age-matched SHR. These results suggest that endogenous endothelin-1 is involved in the maintenance of hypertension in DOCA-salt SHR, and that circulating endothelin-1 is not sufficient to reflect the physiological role of endothelin-1.

Analysis of the binding of the Src homology 2 domain of Csk to tyrosine-phosphorylated proteins in the suppression and mitotic activation of c-Src.

Csk (C-terminal Src kinase), a protein-tyrosine kinase, bearing the Src homology 2 and 3 (SH2 and SH3) domains, has been implicated in phosphorylation of c-Src Tyr-527, resulting in suppression of c-Src kinase activity. We found that mutations in the SH2 or SH3 domain of Csk, though they did not affect its kinase activity, resulted in a loss of suppression of c-Src activity in fibroblasts. In normal fibroblasts, tyrosine-phosphorylated paxillin and focal adhesion kinase pp125FAK, which colocalize at focal adhesion plaques, were the major proteins to which the Csk SH2 domain bound. Loss of binding to these proteins by the Csk SH2 mutants correlated with loss of the activity to suppress c-Src. Consistent with this observation, the levels of tyrosine phosphorylation of paxillin and pp125FAK were greatly reduced during mitosis, whereas the kinase activity of c-Src was elevated. We suggest that the SH2 domain is required for Csk to suppress c-Src, perhaps in combination with the SH3 domain, by anchoring Csk to a particular subcellular location where c-Src may exist. Our data also indicate that a certain fraction of the Csk and Src family kinases function at the focal adhesion plaques. The activity of the c-Src kinase localized at the focal adhesion plaques appears to be regulated by cell adhesion to the extracellular matrix.

[Calcified false channel wall in aortic dissection].

Chest radiographs and computed tomographic images of 110 patients with chronic aortic dissection were retrospectively reviewed. Among these, calcification of false channel wall was observed in 12 cases (about 10%). Stanford type A dissection was noted in 5 cases and type B in 7 cases. Calcified false channel wall may closely simulate intimal calcification of atherosclerotic aneurysm on chest radiographs and sometimes even on CT. Careful observation of CT images with and without contrast enhancement is helpful to correctly identify the site of calcifications in the false channel wall, true channel wall, and intimal flap in the individual cases. In 3 cases calcification was present mainly in the false channel wall with no or only minimal calcification in the true channel wall. These cases may suggest that the endothelialized false channel lumen may develop atheromatous changes much more rapidly than the true lumen. According to advances in surgical and medical treatment of acute aortic dissection, radiologists may encounter cases of chronic aortic dissection with increasing frequency. Full recognition of this potential pitfall in the diagnosis of aortic dissection is important to avoid confusion.

Differential expression of D type cyclins during neuronal maturation.

The multiple cyclins play essential roles in eucaryotic cell cycle. We previously reported that the expression of a D type G1 cyclin (p36cyclinD1) in neurons was increased at the onset of brain maturation. During neuronal differentiation of PC12h cells induced by nerve growth factor (NGF), expression of p36cyclinD1 and p46cyclinX was enhanced concomitant with neuronal maturation. In contrast, p34cyclinD2, which was highly expressed in undifferentiated cells, decreased as the cells matured. In situ hybridization and Western blot analyses demonstrated that embryonic rat brain strongly expressed cyclin D2 gene, but its expression was dramatically repressed in matured brain. These data suggest differential roles of the D type cyclins in the process of neuronal differentiation and/or function.

6-(1H-imidazol-1-yl)-7-nitro-2,3(1H,4H)-quinoxalinedione hydrochloride (YM90K) and related compounds: structure-activity relationships for the AMPA-type non-NMDA receptor.

A novel series of quinoxalinediones possessing imidazolyl and related heteroaromatic substituents was synthesized and evaluated for their activity to inhibit [3H]AMPA binding from rat whole brain. From the structure-activity relationships, it was found that the 1H-imidazol-1-yl moiety could function as a bioisostere for the cyano and nitro groups, and that 6-(1H-imidazol-1-yl)-7-nitro-2,3(1H,4H)-quinoxalinedione (11) showed the most potent activity for the AMPA receptor. Compound 11 was evaluated for selectivity versus other excitatory amino acid receptors, and its action against AMPA at its receptor in the rat striatum was characterized. These data showed that compound 11 was a selective antagonist for the AMPA receptor with a Ki value of 0.084 microM, being approximately equipotent with 2,3-dihydro-6-nitro-7-sulfamoylbenzo(f)quinoxaline (3) (NBQX; Ki = 0.060 microM). Compound 11 was also found to give protection against sound-induced seizure on DBA/2 mice at the minimum effective dose of 3 mg/kg ip (3; 10 mg/kg ip).

Effect of vitamin B6 deficiency on the expression of glycogen phosphorylase mRNA in rat liver and skeletal muscle.

The effect of vitamin B6 deficiency on the expression of glycogen phosphorylase mRNA in rat liver and skeletal muscle was investigated. The level of phosphorylase mRNA in the muscle of vitamin B6-deficient rats was reduced to 40% of that in the control rats. By contrast, the phosphorylase mRNA level was increased 5-fold in the liver of the deficient animals. It was also found that the expression of the beta-actin gene, generally regarded as a 'housekeeping' gene, was unaffected by B6 deficiency in the muscle but was enhanced in the liver of the deficient animals. These observations suggest that vitamin B6 may modulate the transcriptional activation of the phosphorylase gene in a tissue-specific manner.

Light and electron microscopic analysis of synaptic development in Macaca monkey retina as detected by immunocytochemical labeling for the synaptic vesicle protein, SV2.

The development of synapses has been followed in Macaca monkey fetal and infant retina using immunocytochemical labeling for the transmembrane synaptic vesicle glycoprotein, SV2. Electron microscopy (EM) was used to verify the presence of morphological synapses at selected ages. EM immunocytochemical labeling in adult retina showed that all synaptic types contained SV2 in inner (IPL) and outer (OPL) plexiform layers. In fetal retina, SV2 expression and the appearance of morphological synapses were closely related in time, demonstrating that SV2 is a reliable marker for synaptogenesis. SV2 expression appears along a foveal to peripheral gradient. Both SV2 and synapses appear in the foveal IPL at Fd50-55, and reach the retinal edge by Fd90-103. Cone ribbon synapses and SV2 labeling are not present in the foveal OPL until Fd60. Photoreceptors in the far periphery contain SV2 by Fd119-125. This pattern demonstrates an "inner to outer" direction of synaptogenesis. Cones show SV2 labeling before rods at the same retinal eccentricity. In the cone-dominated fovea, SV2 labeling and bipolar cell ribbon-containing terminals are present at Fd55 when amacrine cell conventional terminals are very scarce, indicating that bipolar synapses precede amacrine synapses in monkey foveal IPL. SV2 labeling and bipolar terminals appear first in the outer IPL which contains "OFF" ganglion and bipolar processes in the adult, suggesting that "OFF" midget bipolar cells may form the first synapses. Both SV2 immunocytochemical labeling and EM morphology find that monkey retina follows a generalized inner before outer, and cone before rod synaptic developmental pattern, similar to that in other mammals. The cone-dominated fovea initiates synaptogenesis, and shows a different synaptic sequence from rod-dominated peripheral retina.

Cellular localization of inflammatory cytokines in human glomerulonephritis.

We evaluated the expression of inflammatory cytokines in renal tissues obtained from 45 patients with several types of glomerulonephritis. Immunofluorescence studies with specific antibodies to interleukin (IL)-1 alpha, IL-1 beta, IL-6, tumour necrosis factor (TNF)-alpha, and TNF-beta showed intense cytoplasmic staining in the glomeruli and interstitium. Cells positive for these cytokines were found frequently in tissue from patients with lupus nephritis (WHO Class IV) and membranoproliferative glomerulonephritis, and, to a lesser extent, in tissue from patients with mesangial proliferative glomerulonephritis, Henoch-Schönlein purpura nephritis, and minimal change nephrotic syndrome. Most of these cells were dual-stained with a monoclonal antibody to monocytes-macrophages. In situ hybridization for cytokine mRNA, combined with immunoperoxidase staining for monocytes-macrophages, detected IL-1 alpha, IL-6, and TNF-alpha mRNA in monocytes-macrophages infiltrating the glomeruli and interstitium. Occasionally, there was weak or moderate immunostaining for IL-1 alpha, IL-6, and TNF-alpha in the glomerular mesangial and epithelial cells, but in situ hybridization signals were rarely found in these loci. These findings suggest that infiltrating monocytes-macrophages, rather than resident glomerular cells, are the major source of inflammatory cytokines in human glomerulonephritis.
