[Congenital aplastic anemia, type I].

Case report of a 7 year old girl, anaemic since birth. The anaemia, of variable intensity, is associated with a moderate reticulocytosis. Bone marrow films, show a marked erythroblastosis with conspicuous morphological abnormalities and internuclear chromatin bridges. The erythrocinetic pattern is that of ineffective erythropoiesis. The morphologic features are those of Heimpel and Wendt's type I congenital dyserythropoiesis. The erythrocytes are lysed by some acidified sera, thus showing a membrane anomaly which is unusual in type I. However the characteristics of the lysis are different from those of type II (HEMPAS) erythrocytes.

Transplacental transport of netobimin metabolites in ewes.

Neither netobimin (NTB) nor its metabolite albendazole (ABZ) were found in plasma after an oral administration of 20 mg/kg of NTB to pregnant ewes during the last third of gestation. ABZ metabolites, albendazole sulphoxide (ABZSO) and albendazole sulphone (ABZSO2) were found in plasma 30 min and 2 h, respectively, after administration. The maximal plasma concentration (Cmax) of ABZSO was detected at 11.6 +/- 1.0 h and for ABZSO2 at 16.5 +/- 2.3 h. The plasma levels of the latter remained constant for 36 h, and decreased as ABZSO was removed from the blood. Jugular plasma levels of both metabolites did not differ significantly from those observed in the ovarian vein, suggesting that there were no exchanges between foetal and placental tissues. Both metabolite concentrations were similar in the umbilical vein and artery and in the amniotic and allantoic fluids, their values were half the maternal plasma concentration, leading to the conclusion that there was transplacental movement of metabolites. Both metabolites reached the foetus and could be responsible for the teratogenicity of NTB in sheep.

Massive intrahepatic extramedullary hematopoiesis in myelofibrosis.

We describe the sonographic (US) and computed tomographic appearances of a large, solitary tumor in the liver produced by extramedullary hematopoiesis in an 82-year-old patient with agnogenic myeloid metaplasia. Confirmation of this diagnosis was made by US-guided fine-needle aspiration biopsy.

Protocol core needle biopsy and histological chronic allograft damage index as surrogate endpoint for Long-Term graft survival.

Following encouraging results from several single-center studies showing that early histological manifestations of chronic rejection are seen in the graft before a decline in transplant function, we tested this concept in a multicenter study and investigated whether protocol needle biopsy may be used as a surrogate to late graft survival in multicenter renal transplantation trials. During two mycophenolate mofetil trials, 621 representative protocol biopsies were obtained at baseline, 1 year, and 3 years. The samples were coded and evaluated blindly by two pathologists and a Chronic Allograft Damage Index (CADI) score was constructed. At 1 year only 20% of patients had elevated (>1.5 mg/100 mL) serum creatinine, whereas 60% of the biopsies demonstrated an elevated (>2.0) CADI score. The mean CADI score at baseline, 1.3 +/- 1.1, increased to 3.3 +/- 1.8 at 1 year and to 4.1 +/- 2.2 at 3 years. The patients at 1 year were divided into 3 groups, those with CADI <2, between 2 and 3.9, and >4.0, the first two groups having normal (1.4 +/- 0.3 and 1.5 +/- 0.6 mg/dL) and the third group pathological (1.9 +/- 0.8 mg/dL) levels of serum creatinine. At 3 years there were no lost grafts in the "low" CADI group, six lost grafts (4.6%) in the "elevated" CADI group, and 17 lost grafts (16.7%) in the "high" CADI group (P <.001). One-year histological CADI score predicts graft survival even when the graft function is still normal. This observation makes it possible to use CADI as a surrogate endpoint in prevention trials and to identify the patients at risk for intervention trials.

Comparative pharmacokinetics of netobimin metabolites in pregnant ewes.

The pharmacokinetics of netobimin (NTB) metabolites has been investigated in ewes. Non-pregnant ewes and ewes in the first and last third of pregnancy were dosed orally with 20 mg kg bodyweight of NTB. Blood samples were collected from the jugular vein from 30 minutes to 72 hours after administration and plasma samples were analysed by high performance liquid chromatography. Neither NTB nor albendazole (ABZ) were detected in any of the samples analysed. No statistically significant differences were found between the pharmacokinetic parameters of albendazole suphoxide (ABZSO) and albendazole sulphone (ABZSO2) among the three groups of ewes. The peak plasma concentrations (Cmax) ABZSO and ABZSO2 were reached about 10 and 20 hours respectively after administration in all three groups. The ratios of ABZSO/ABZSO2 for Cmax and the areas under the curve (AUCzero-infinity) were 6 and 3, respectively, in each group and suggest a low rate of oxidation of sulfoxide to sulphone.

Lateral parabrachial lesions impair taste aversion learning induced by blood-borne visceral stimuli.

The lateral parabrachial area (LPB), main relay from the area postrema (AP), plays a role in processing visceral information and is thus of potential importance in taste aversion learning (TAL). This study used a lesion approach to address whether LPB functional relevance depends upon the features of toxins that serves as visceral stimuli in TAL. In addition, we explored whether LPB involvement in TAL is restricted to those toxic events detected by the AP or whether it has a more general role. Results showed that LPB-lesioned animals were disrupted in acquiring a TAL induced by blood-borne AP-dependent aversive stimuli (intraperitoneal methylscopolamine) and by AP-independent stimulus (intraperitoneal ethanol), but still, clearly developed strong aversions when intragastric hypertonic sodium chloride, a vagally processed aversive stimulus, served as the aversive stimulus. These findings suggest that the LPB plays a critical role in TAL induced by blood-borne toxins, such as methylscopolamine or ethanol, but is not necessary for vagally mediated stimulus, such as sodium chloride. The present results are discussed in the context of the hypothesis holding separable and independent neural systems underlying TAL.

[Safety of meningococcal A and C vaccine. Data from the Spanish drug surveillance system. Meningococcal Vaccine Research Group of the Spanish System of Drug Surveillance].

OBJECTIVE: Data on meningococcal vaccines safety are scanty. In 1997 several vaccination campaign took place in Spain. Thus, this situation was used to improve our knowledge about the safety profile of this vaccine. METHODS: An inquiry was carried out to the Regional Centers of the Spanish Pharmacovigilance System to know the number of vaccinated people and the type and number of suspected cases of adverse reactions. RESULTS: There were 133 identified cases of suspected adverse reactions associated with meningococcal A and C vaccine until June 1st, 1998. Most of them affected the skin (25,3%) or nervous system (similar proportion). Those of allergic reactions accounted for 35,2%. Two cases were considered as severe, although they were resolved without secuelae. CONCLUSIONS: Serious risks were not detected. The Spanish Pharmacosurveillance System as an epidemiological surveillance resource has been useful to know the safety problems associated with antimeningococcal vaccine in the community.

Developmental toxicity in rat fetuses exposed to the benzimidazole netobimin.

Netobimin (NTB) is a prodrug of albendazole (ABZ) and is used as a broad-spectrum anthelmintic both in human and veterinary medicine. Pregnant Sprague-Dawley rats were treated po with 50, 59.5 and 70.7 mg/kg of NTB on Gestational Day (GD) 10. The results, observed on GD 20, demonstrated that NTB induced a significant increase of resorptions. Moreover, decreased fetal body weight and an increase in skeletal malformations were observed in treated groups. We report the first study in which vascular malformations are described in rats after the administration of a benzimidazole compound. An interesting relationship between intercostal vessel and rib malformations was found.

Determination of selenium levels in dairy products and drinks by hydride generation atomic absorption spectrometry: correlation with daily dietary intake.

The selenium concentrations in different dairy products and drinks in the Mediterranean coastal area of southeastern Spain have been determined by hydride generation atomic absorption spectrometry. Mineralization was carried out with an HNO3-HClO4 mixture (4:1) in a thermostatted sand bath. Selenium determination was carried out by the standard addition method. Mean concentrations obtained in diary products varied from 6.87 ng/g (rice pudding) to 178.90 ng/g (caramel custard), and those in drinks from 0.256 micrograms/l (wine) to 2.879 micrograms/l (pineapple juice). Repeatability, expressed as relative standard deviation, ranged from 5.06% to 13.25%. The results obtained from the recovery studies were proximate to 100%. Considering the mean daily individual consumption of these foods in Andalusia (southern Spain), the daily dietary intake of selenium supplied by this source is 5.869 micrograms per person per day.
