Ifosfamide + mitoxantrone in advanced breast cancer previously treated with anthracyclines.

We treated 19 patients previously exposed to anthracyclines (progression during therapy or after discontinuation of therapy) with ifosfamide (IFO) 2 g/m2 i.v. in a 1-h infusion daily for 3 days with mesna uroprotection and mitoxantrone (MZT) 12 mg/m2 on day 1 of every 3-week cycle. The response rate was assessed after two cycles. A response to treatment was observed in 6 of 15 evaluable patients (40%), with no complete remission and 6 partial remissions. The median duration of response was 6+ months (3+ to 12+). The toxicity was acceptable, with three episodes of grade 3-4 myelosuppression (8%) and one case of congestive heart failure resulting from a fluid overload that responded to medical treatment. The IFO-MZT combination is an effective second-line regimen in advanced breast cancer previously treated with anthracyclines.

Anesthesia and immunosuppression in an experimental model.

The purpose of the present work was to study: (1) if anesthesia induces changes in the helper/cytotoxic/suppressor T cell ratio in the rat spleen, and (2) if there is any difference between two anesthetic methods usually employed in rat experimentation. 50 Fisher 344 rats were allotted into 5 groups. In group A (control), a splenectomy was performed in an average time of 3 min under ether anesthesia. In groups B1 and B2, the rats anesthetized for 1 h with ether (group B1) or with one single dose of ketamine + atropine + diazepam (group B2) and then underwent splenectomy. Groups C1 and C2 were similar to groups B1 and B2 but splenectomy was performed 24 h later, under ether anesthesia during 3 min. T helper (Th) and T cytotoxic-suppressor (Ts-c) cells were identified in periarterial and follicular spleen tissue by means of an immunohistologic analysis using an immunoperoxidase technique with monoclonal antibodies. In periarterial tissue the Th/Ts-c ratio (mean +/- SE) was: group A, 1.61 +/- 0.11; group B1, 1.10 +/- 0.1; group B2, 1.28 +/- 0.11; group C1, 0.85 +/- 0.08, and group C2, 1.23 +/- 0.12. The differences between groups A and B1, C1 and C2 were statistically significant (t = 2.2833) as well as the difference between groups C1 and C2 (t = 2.5223). The Th/Ts-c ratio was also compared when equal to 1 or less than 1. The difference between groups A and C1 was statistically significant (p less than 0.05) as well. In follicular tissue the ratio was 1 in all the groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[Hemorrhagic cardiac tamponade without heart rupture during a myocardial infarct. A clinical case].

Cardiac tamponade during acute myocardial infarction is a life-threatening complication that can be confounded with right ventricular infarction. The most frequent cause of this complication is cardiac rupture. We report here a patient with acute myocardial infarction that developed cardiac tamponade on day 7, after receiving late systemic thrombolysis. The diagnosis was suspected with echocardiography and confirmed with hemodynamic measurements. The tamponade was partially relieved with pericardiocentesis but afterwards required emergency surgery. No cardiac rupture was found but an hemorrhagic infarction. We conclude that in this case the hemorrhagic tamponade was probably related both to late thrombolysis and to post infarction pericarditis.

[Toxic megacolon and intestinal perforation caused by Salmonella enteritidis].

Two cases of colitis due to Salmonella enteriditis which later developed a toxic megacolon with intestinal perforation are presented and the probable pathogenesis is discussed. This exceptional clinical course which has not been previously described forces to perform a differential diagnosis with chronic intestinal inflammatory disease which must be based on microbiologic, serologic and/or histologic criteria. Moreover, the authors warn about the unsuitability of using anticholigernic drugs in gastroenteritis since they could be related to the ethiopathogenical basis of the disease's unfavorable course in the described patients.

Idiopathic myelofibrosis terminating in erythroleukemia.

A 55-year-old woman with a one-year history of idiopathic myelofibrosis progressed to erythroleukemia. This is the first reported occurrence of erythroleukemia progression from idiopathic myelofibrosis. Certain patterns of leukemia transformation from myeloproliferative disorders are favored: Myeloblastic or myelomonocytic. The rare incidence of erythroblastic transformation is discussed.

Fatal septic thrombophlebitis due to Salmonella enteritidis.

The case is reported of a patient with gastric adenocarcinoma with metastases who developed a septic focus in a saphenous vein. Salmonella enteritidis was cultured from blood, sputum and stool specimens, and from a thrombus removed from a varicosity of the saphenous vein. Extraintestinal infections caused by non-typhoid salmonella usually afflict debilitated and immunocompromised patients. Metastatic septic foci may appear anywhere in the organism, including the vascular system, but involvement of the venous system has not previously been reported.

Mitomycin-C and vinblastine in advanced breast cancer.

Forty-two patients with metastatic breast cancer refractory to first-line therapies were treated with combination chemotherapy with mitomycin-C and vinblastine. A response to treatment was observed in 11 of 34 evaluable patients (32.3%), with 3 complete remissions (8.8%) and 8 partial remissions (23.5%). The median duration of response was 185+ days. The 12-month survival was 78% for responders, 48% for patients with stable disease and 0% for patients with progressive disease. The toxicity was acceptable with 20 episodes of moderate myelosuppression (58.8%) and 2 cases with congestive heart failure that responded to medical treatment. The MMC-VBL combination is an active regime for advanced breast cancer previously treated with antracyclines. This combination may be regarded as a standard second-line treatment for this type of tumor.

[Posterior urethral valves: prognosis related to the initial surgical treatment].

Long-term outcome of 32 boys with severe posterior urethral valves is reviewed. Diagnosis was established during the first month of life in 14 patients, along the first year in another 14 and later in five. Surgical management was, initially, bilateral upper urinary tract diversion (cutaneous ureterostomy or pyelostomy) in 17 patients (Group A), and valve ablation alone or vesicostomy in 15 children (Group B). Eight months was the mean age of patients at initial surgery (40% before the first month of life). The average age for closure of the temporary urinary diversion was 17 months. Eleven patients underwent later unilateral nephrectomy and eighteen antireflux surgical procedures. Follow-up ranged from 1 to 15 years (mean 6 years). Fifteen patients have a Glomerular Filtration Rate (G.F.R.) of more than 80 ml./min./1.73 m2. Eleven (73%) of them underwent early upper urinary tract diversion, and the remaining four (27%) underwent valve ablation or vesicostomy. Two children among the 32 have developed end stage renal disease. Average height and weight posttreatment values were higher in those patients treated with ureterostomy or pyelostomy. The results of our study suggest that renal function improved in patients with early upper urinary tract diversion.

Epithelioid hemangioendothelioma of the liver with myocardial metastases.

Epithelioid hemangioendothelioma is an unusual type of endothelial-derived vascular tumor located mainly in the soft tissues of the limbs. The present report describes a 48-year-old woman with an epithelioid hemangioendothelioma located in the liver with very unusual metastases to the myocardium. While the disease itself was controlled by adriamycin the patient died as a consequence of portal hypertension due to the abundant fibrotic component distinctive of this type of tumor. An epithelioid appearance can sometimes lead to a mistaken diagnosis of metastatic carcinoma, as happened initially in our case. The endothelial origin of the tumor must be confirmed by electron microscope identification of Weibel-Palade bodies or by immunohistochemical staining of factor VIII-related antigen. To the authors knowledge, this is the first case of myocardial metastases from an epithelioid hemangioendothelioma of the liver to be reported in English in the medical literature.

Palmar-plantar erythrodysesthesia syndrome associated with short-term continuous infusion (5 days) of 5-fluorouracil.

A case of palmar-plantar erythrodysesthesia syndrome (PPES) observed during a 120-h infusion of 5-fluorouracil (5-FU) is presented. This syndrome has been described in the literature after protracted infusion chemotherapy of over 30 days. The agent most frequently associated with this syndrome was 5-FU. A 53-year-old man was admitted to the hospital with a well-differentiated squamous cell carcinoma of the retromolar trigone. The patient received 100 mg/m2 of cisplatin on day 1 and 120-h continuous infusion of 1000 mg/m2 of 5-FU every 3 weeks. After the second course, the patient developed clinical features consistent with PPES. This side effect has not been previously reported with short-term (5-day or 120-h) continuous infusion of 5-FU. Less frequently, the syndrome has also been described with 10-day continuous infusion. The etiopathogenesis of PPES is unclear, but it seems to be dose-dependent and probably related to cutaneous drug accumulation.
