Simultaneous graft replacement of the ascending aorta and total aortic arch for type A aortic dissection.

We performed simultaneous graft replacement of the total aortic arch and ascending aorta for type A aortic dissection with a patent false lumen extending through the arch into the descending or abdominal aorta. During the past 7 years, this procedure was performed in 42 patients (28 men and 14 women), aged 20 to 72 years (mean age, 50 years). Nineteen patients underwent the procedure during the acute period, and 23 during the chronic period. The site of the initial intimal tear was the ascending aorta in 17 patients and the transverse aortic arch in 25 patients. Artificial graft replacement was initially accomplished by proximal anastomosis, followed by open distal anastomosis, and finally by anastomosis of each of the three arch vessels. There were 3 hospital deaths (7.1%), 1 resulting from acute dissection (5.3%) and 2 from chronic dissection (8.7%). Among the type A dissections, total arch graft replacement has been indicated in the setting of rupture of the aortic arch, arch dissection, and Marfan's syndrome. However, with increasing experience in arch reconstructions and improvement in outcome, the indications could be expanded to include all type A aortic dissections with a patent false lumen in the descending aorta.

Myosin subfragment-1 isoforms having different heavy chain structures from fast skeletal muscle of thermally acclimated carp.

Three heavy chain isoforms of chymotryptic myosin subfragment-1 (S1) with different molecular sizes of 96 kDa (H1), 94 kDa (H2), and 92 kDa (H3), were detected in the fast skeletal muscle from thermally acclimated carp. In total, six S1 isoforms were present, including two S1 isoforms for each heavy chain due to associated A1 and A2 light chains. H1 heavy chain was dominant in the 10 degrees C-acclimated carp and responsible for high acto-S1 Mg(2+)-ATPase activity and low thermostability. In contrast, H3 heavy chain predominating in the 30 degrees C-acclimated carp showed low acto-S1 Mg(2+)-ATPase activity and high thermostability. H2 heavy chain was found in the 10- and 20 degrees C-acclimated fish. H3 heavy chain featured three tryptic fragments with normal molecular masses of 25, 50, and 20 kDa in order from the N-terminus. However, H1 heavy chain contained an unusual, longer "20 kDa" peptide whose molecular size was estimated to be about 23 kDa.

Epidemiological survey of dementia and depression among the aged living in the community in Japan.

The authors surveyed the prevalence of depression and dementia in the elderly in Ohira town in Japan from 1989 to 1990. The total population of this town was 26,712, with 2,778 people aged 65 and above, constituting 10.4% of the total population. The prevalence of dementia (n = 128) was 6.1% and that of major depression (n = 9) was 0.4%, according to the DSM-III-R criteria. The prevalence of a depressive state which did not fulfill the criteria for major depression (n = 55) was 2.4%. The patients with multi-infarct dementia (n = 49) suffered from depression (42.8%) more frequently than those with dementia of the Alzheimer type (11.1%). The rate of depression coexisting with dementia increased with aging, while the rate of depression without dementia did not change in all the age groups.

Effects of exercise on rats subjected to tail suspension.

Deterioration of vital functions is observed in living organisms in microgravity. However, this deterioration can be arrested by adequate exercises. In this study, utilizing tail suspension in rats to simulate microgravity, we examined the influence of running exercise every other day on blood constituents, visceral parameters, and muscles.

[Responsiveness of lymphocytes from patients with M. avium-intracellulare complex (MAC) infection to PPDs as measured by IFN-gamma production].

We investigated the responsiveness of peripheral blood lymphocytes (PBLs) from patients with M. avium-intracellulare complex (MAC) infection to the stimulation with PPDs by measuring their IFN-gamma producing ability. PBLs were obtained from MAC patients at active stage (culture-positive after three-month chemotherapy), those at inactive stage (culture-negative for three months after or during chemotherapy), and healthy donors. PPDs used were PPD-S prepared from M. tuberculosis, PPD-B from M. intracellulare, and PPD-Y from M. kansasii. PBLs from active MAC patients did not produce IFN-gamma to a significant extent by stimulation with any of three PPDs, while PBLs from inactive MAC patients showed higher responses to each PPD compared to those from active patients. In inactive MAC patients, the maximal response was observed to PPD-B among three PPDs. On the other hand, PBLs from healthy controls produced different levels of IFN-gamma in response to three different PPDs, and their response was most remarkable to PPD-S. These results indicated that the responsiveness of patients' PBLs to PPDs was impaired during active stage of MAC infection and restored on recovery from the disease.

[Tumor necrosis factor alpha in bronchoalveolar lavage fluid and its production by alveolar macrophages with asbestos exposure].

It is generally accepted that asbestos exposure causes pulmonary fibrosis and carcinogenesis. Several studies have suggested that TNF alpha is produced principally by mononuclear phagocytes and that it may play a role in inflammation, fibrosis and anti-tumor activity. We studied TNF alpha production by alveolar macrophages and the TNF alpha concentration in bronchoalveolar lavage fluid harvested from asbestos-exposed subjects and healthy controls. TNF alpha production by alveolar macrophages was significantly higher in the asbestos-exposed subjects than in healthy controls (3696 +/- 1606, 1938 +/- 753 pg/ml; p < 0.01). The period from first exposure correlated inversely with TNF alpha production (r = -0.6; p < 0.05). The TNF alpha concentration in bronchoalveolar lavage fluid was also significantly higher in the asbestos-exposed subjects than in healthy controls. These results suggest that abnormal TNF alpha production by alveolar macrophages may be related to fibrosis and carcinogenesis due to asbestos.

[Relationship between nutritional depletion and cell-mediated immune function in active pulmonary tuberculosis].

A survey on the nutritional status and cell-mediated immune function of 47 hospitalized patients with active pulmonary tuberculosis and healthy controls was conducted. In the patients group: 1) Anthropometric measurements, such as %ideal body weight (%IBW), %arm circumference (%AC), %arm muscle circumference (%AMC) and %triceps skin fold (%TSF), were significantly reduced. 2) Visceral proteins including serum albumin (Alb), transferrin (Tf), prealbumin (PA) and retinol binding protein (RBP) were significantly reduced. 3) The imbalance of plasma amino acids, which was characterized by the depression of Fischer ratio, a molar ratio of branched chain amino acids (BCAA) to aromatic amino acids (AAA), was observed. Fischer ratio was significantly correlated with anthropometric measurements (%IBW, %AC and %AMC). Delayed-type hypersensitivity to DNCB (2,4-dinitrochlorobenzene) and lymphocyte transformation to phytohemagglutinin (PHA) and concanavalin A (Con A) were significantly impaired in the patients group, whereas NK cell activity was higher than that of controls. Alb, PA, RBP and Fischer ratio were significantly lower in the patients with reduced DNCB reaction than in those with normal responses. Lymphocyte transformation was significantly correlated with Fischer ratio, and NK cell activity was significantly correlated with Alb, PA, RBP. These data may suggest that the imbalance of plasma amino acids represented by the reduction of Fischer ratio and the depletion of visceral proteins are closely related to the impairment of lymphocyte function in the patients with active pulmonary tuberculosis.

[Inhaled nitric oxide after Fontan type operation].

The indication of Fontan type operation has been extending, but some cases on the boundary of the indication resulted in low cardiac output syndrome (LOS) postoperatively. Recently the inhalation of nitric oxide (NO) has been revealed to produce selective pulmonary vasodilatation, and it has come to be applied in the clinical setting. We experienced a case of 6-year-old boy with complex cardiac anomaly in whom the inhalation of nitric oxide was remarkably effective for the LOS caused by increased pulmonary vascular resistance after Fontan type operation. In the case various conventional treatments failed to improve his LOS during the weaning from a respirator on the second postoperative day. After the initiation of the inhalation of nitric oxide at a concentration of 6ppm, a rapid decreasing of pulmonary artery pressure concomitant with an improvement of hemodynamics were obtained. We concluded that the inhalation of nitric oxide is an effective strategy for the increased pulmonary resistance after Fontan type operation.

The modality of reconstruction in 'immodithymia'.

We studied the modality of reconstruction in the depressive patients whose premorbid personality presents 'immodithymia'. The patients try to pursue the reconstruction limitlessly in obsessive ways. That is, there is no discharge from their reconstruction, and the limitless succession of reconstruction supports the patients' existence. The hindrance of working for limitless succession provides the soil from which depression arises. The limitless reconstruction reflects an aspect of the deterioration of the depressive existence that is the loss of temporality or the dehistoricalization. On the other hand, when we consider the basic structure of manic-depressive psychosis under the concept of the excessive identification of the inner attitude, we can position the deterioration of our patients' existence between that of melancholics and that of manics.

On the delusional experience of controlling other persons in acute schizophrenics.

I present cases in which patients feel that they are able to control other persons, and consider the features of this delusional experience and note some points concerning its treatment. This delusional experience does not continue for a long period and is accompanied by a suicidal tendency, a pathological consciousness of the disappearance of the self, and some catatonic experiences. These features are not accidental, but result from the essentially solipsistic quality of this delusional experience. So, from the symptomatic viewpoint, this experience can be considered as belonging to catatonic ones in the acute phase of schizophrenia. From the therapeutic point of view, the occurrence of this experience should be carefully examined, bearing in mind its short duration. And when we encounter this experience, we should make provision against its suicidal tendency that often inevitably impels us to carry out electroconvulsive therapy.

Relationship between hind-limb muscle atrophy and serum enzymes in tail suspended rats.

Many studies on the effects of gravity have been reported using animals or human being since the gravity influences the biological body in space. Especially suspension techniques for rat have been used as a microgravity simulation. This study is a report on the relation between serum enzymes such as CK and muscle atrophy, caused by tail suspension in rats.

[Surgical results in atherosclerotic distal aortic arch aneurysm].

Results of surgical treatment of atherosclerotic aneurysm of the distal aortic arch, reported to date, are not satisfactory. We recently analyzed the surgical results in 119 patients with this condition who were treated at our department. There were 104 males and 15 females, ranging in age from 38 to 81 years (mean: 66 years). The arch was reconstructed by total replacement in 29 cases, partial replacement in 49 cases, patch reconstruction in 34 cases and extra-anatomic bypass in 7 cases. The lesion was approached by median sternotomy in 75 cases, median sternotomy+left thoracotomy in 9 cases and left thoracotomy in 35 cases. In cases where median sternotomy was used, selective cerebral perfusion was generally employed as an auxiliary procedure. After surgery, cerebral complications were observed in 19 cases (16.0%). In 15 of these 19 cases, postoperative cerebral damage was attributed to atheromatous emboli. Twenty-six patients (21.8%) died in the hospital. Major causes of death included respiratory insufficiency, multiple organ failure and low cardiac output syndrome. To improve surgical results for this type of aneurysm, the operative procedure needs to be modified so that these postoperative complications are prevented. Especially it is essential to prevent intraoperative embolisms of brain and coronary arteries, and a reconstruction by total arch replacement with median sternotomy has improved recent surgical results.

[A case of constrictive pericarditis following graft replacement of the descending aorta under left ventricular bypass using a centrifugal pump].

We experienced a case of constrictive pericarditis following replacement of the descending aorta. It has been pointed out that constrictive pericarditis can be developed following LV bypass using centrifugal pump due to the cardiotomy. But no actual case has reported so far. The case was a 61-year-old man for whom graft replacement of the descending aorta through left thoracotomy was performed. Fever and heart failure was elicited 3 weeks after the operation, and he was diagnosed as constrictive pericarditis by chest CT and echocardiography. The reoperation was performed 39 days after the graft replacement. Anterior wall of the both ventricle and postero-lateral wall of the left ventricle were strongly adhered to the pericardium. The adhesion was entirely dissected and the thickened pericardium was resected. The post-operative course was uneventful and the patient was discharged 42 days after the reoperation.

[Immunohistological evaluation of parathyroid hormone-related protein in breast cancer with and without calcification on mammography].

PTHrP is the main factor of humoral hypercalcemia of malignancy. PTHrP produces the same effect on renal and osteoblast membrane in vitro and on calcium and inorganic phosphate fluxes in vivo as human PTH. Physiologically, PTHrP is produced by rat mammary tissue during lactation. It is suggested that PTHrP correlates to the metabolism of calcium in the mammary gland. Using anti-PHTrP (1-34) monoclonal antibody 4B3, we investigated the correlation of PTHrP and calcification in breast cancer. All the breast cancers with calcification on mammography showed positive immunostaining. Most cases without calcification, on the other hand, showed negative. Positive PTHrP staining was not related to the any clinical parameter. It is suspected that PTHrP is also one of the main factors of calcification in breast cancer.

[A case of primary cardiac hemangiosarcoma].

A case of primary hemangiosarcoma of right atrium was reported. The patient was 43 years old female. She had the symptoms of tamponade due to massive bloody pericardial effusion. Echocardiography, computed tomography, NMR imaging and angiography revealed the precise delineation and extension of tumor, and also allowed diagnosis of hemangiosarcoma. Extirpation of tumor was performed under the cardiopulmonary bypass. The patient was died 3 months later after the operation for liver metastasis. The bloody pericardial effusion seems to be an important symptom of the primary malignant cardiac tumor, and early and precise diagnosis may allow successful operation and long survival of the patients.

[Surgical management of tetralogy of Fallot with absent pulmonary valve in infancy].

Between November, 1987 and May, 1990, definitive operation was performed in 7 infants for tetralogy of Fallot with absent pulmonary valve. Preoperative respiratory failure was serious in 5 patients and semi-emergency operation was done. Surgical procedure consisted of closure of ventricular septal defect, reconstruction of right ventricular outflow tract using heterograft valved pericardial roll and plication of aneurysmally dilated pulmonary artery with or without pulmonary artery suspension in 5 infants with severe respiratory distress and heart failure, operated on 26 days to 5 months of age. In the other two less symptomatic infants, operated on at 13 months of age, reconstruction of right ventricular outflow tract was performed by transannular bovine pericardial patch with monocusp. There were no operative death and six infants are doing well 6 months to 3 years postoperatively, but one patient died of prolonged respiratory infection and respiratory failure one year after operation. Postoperative pulmonary arteriogram revealed a 42 to 61 (mean 54) percent reduction in right and left pulmonary artery size in 3 patients who underwent extensive plication of pulmonary artery. We suggest that definitive plication with or without suspension of pulmonary artery, together with closure of ventricular septal defect and reconstruction of right ventricular outflow tract is supposed to eliminate air way obstruction.
