Mucinous colorectal carcinoma: clinical pathology and prognosis.

Mucinous carcinomas accounted for 37 (6.4%) of 540 cases of colorectal carcinoma. The clinical and pathological features of these mucinous carcinomas were compared with those of the 510 well or moderately differentiated adenocarcinomas. Mucinous carcinoma was more common in the patients 39 years of age or under (P less than 0.05) and was more frequent in the female patients. A large number of mucinous carcinomas were located in the rectum, followed by the right colon. However, the right colon showed a higher relative incidence (40.5% vs 12.5%, P less than 0.005). Mucinous carcinoma was characterized by infiltration of the surrounding tissues (24.3% vs 7.8%, P less than 0.005), positive lymph node involvement (75.7% vs 48.6%, P less than 0.005), and peritoneal implant (21.6% vs 4.1%, P less than 0.005). The cumulative five and ten year survival rates after resection of mucinous carcinoma were 45.5 per cent and 39.8 per cent, respectively, and those after curative resection, 72.4 per cent and 63.5 per cent, respectively. These survival rates were lower, without significant differences, than those for the well or moderately differentiated adenocarcinomas. The results suggest the need for aggressive lymph node dissection and wide excision of the surrounding tissues for mucinous carcinoma, with special attention paid to local recurrence.

[Postoperative total parenteral nutrition with fat emulsion for patients with esophageal varices].

The administration of fat emulsion and total parenteral nutrition (TPN) was evaluated in 68 patients with liver disorders who underwent surgical treatment for esophageal varices. The subjects were divided into two groups, fat group (28 cases) and non-fat group (40 cases) according to with or without the administration of fat emulsion during the period of postoperative TPN. The results of liver function tests, blood glucose levels, intravenous fat tolerance tests, serum lipid levels, fatty acid composition in serum total lipids, nitrogen balances and body weight changes during the period of postoperative TPN were compared between both groups. Conclusions as follows; 1. The administration of fat emulsion during the period of postoperative TPN did not worsen the results of liver function tests, and relatively low levels of blood glucose were retained. 2. The removal rates of fat emulsion from the blood (K2 values) during the period of postoperative TPN were significantly higher than in those preoperative period, and changes in serum lipid level revealed no tendency toward retention of fat emulsion administered intravenously. 3. Cumulative nitrogen balance were almost similar in both groups. 4. The administration of fat emulsion the period of during postoperative TPN corrected the abnormalities of fatty acid composition in serum total lipid. Further, it was suggested that approximately more than 5 ml/kg/day of 10% fat emulsion would be advisable to prevent the decrease of linoleic acid. 5. These results suggested that the administration of fat emulsion was useful for the patients with esophageal varices during the period of postoperative TPN.

Colorectal carcinoma in young adults.

Charts of 570 patients with colorectal carcinoma surgically treated in one institution during a 13 year period were retrospectively reviewed to evaluate clinicopathologic patterns and surgical results of those 39 years of age or under. Of the 570 patients, 57 (10 percent) were 39 years of age or under, and this group included more female patients than the older control group. Mucinous carcinoma was frequently found on histologic examination, with significantly high incidences of lymph node involvement, peritoneal dissemination, and advanced tumor stages by Dukes' classification in the young patients. The overall cumulative 5 year survival rate in the younger group was significantly lower than that in the control group (41 percent and 55.9 percent, respectively), whereas the difference in rates among the two groups for curative resection was not statistically significant (71.6 percent and 76.3 percent, respectively). Early diagnosis and attempts at curative resection are of utmost importance in the treatment of young patients with colorectal carcinoma.

Serum fatty acid concentrations in patients on postoperative parenteral nutrition with and without fat.

To examine changes in fatty acid composition of serum lipids, sixteen patients with gastric cancer were maintained on total parenteral nutrition (TPN) or intravenous feeding immediately after total gastrectomy. Subjects receiving no fat showed decreases in linoleic acid, linolenic acid and arachidonic acid and increases in palmitoleic acid and oleic acid, whereas those receiving fat emulsion showed no detectable changes. Decrease in the linoleic acid content was greater in subjects on a higher carbohydrate intake, but less in those on a higher fat intake. Multiple regression analysis of the relationships among carbohydrate intake (X1) and fat intake (X2) and changes in the linoleic acid percentage of total serum fatty acids (Y) in each case yielded an equation: Y =-4.75 X1 + 69.0 X2 - 27.9 (R = 0.885, p less than 0.05). Approximately 1 g/kg/day of fat provided in 40-50 kcal/kg/day of nonprotein energy intake was estimated to prevent decrease in the linoleic acid content in the serum fatty acid pattern during the postoperative catabolic stage.

Postoperative total parenteral nutrition in patients with liver disorders.

Sixty-one patients with liver disorders receiving total parenteral nutrition (TPN) for about 14 postoperative days were divided into three groups based on the parenteral nutritional regimen. The influence of these TPN solutions on the liver function tests and the nutritional assessments, and the availability of the specially formulated amino acid solution were studied. Glucose alone as energy source was infused in Group Ia. The mixture of glucose and fructose was infused in Group Ib. In these patients (Group I), a commercially available amino acid solution was administered simultaneously. A specially formulated amino acid, rich in branched-chain amino acids but poor in aromatic amino acids was infused with the mixture of glucose and fructose in Group II. There was no remarkable elevation of blood glucose and lactate levels in all patients. Blood glucose levels in Group Ib were maintained lower than that in Group Ia. Except for serum alkaline phosphatase, no remarkable abnormality was observed in liver function tests. Body weight changes were less than 5% in each group. Average nitrogen balances were -44.5 mg/kg/day in Group Ia, -5.5 mg/kg/day in Group Ib, -51.5 mg/kg/day in Group II. While the abnormalities in serum amino acid pattern and molar ratio of leucine, isoleucine, and valine to phenylalanine and tyrosine tend to be more enhanced in Group I, these abnormalities returned to near normal in Group II during TPN. By multiple linear regression analyses, 45 kcal/kg/day of energy intake would be required to maintain nitrogen equilibrium and zero body weight change. And when nitrogen intakes were 159 mg/kg/day in Group Ia, 114 mg/kg/day in Group Ib, and 189 mg/kg/day in Group II at 45 kcal/kg/day in energy intake, nitrogen balances were expected to be equivalent. These results suggest that postoperative TPN is good for nutritional support in patients with liver disorders. And also, the combination of glucose and fructose has better effect on nitrogen balance. The postoperative TPN with a specially formulated amino acid solution may be a valuable way of maintaining the nutritional status as well as normal serum amino acid pattern in patients with liver disorders.

Endochondral ossification of the condyle in rats on a strontium or low-calcium diet.

The mechanism of abnormal endochondral ossification induced by administration of strontium salts was studied in the mandibular condyles of rats by radiographic, histologic, and histochemical methods. It was shown by radiographic and histologic findings that ossification of the mandibular ramus was clearly inhibited in rats fed a low-calcium diet and rats fed a strontium diet. The change in appearance of the mandibular condyles of the rats fed strontium was more severe than that in those fed low amounts of calcium. From the histochemical findings, it was suggested that the metabolic dysfunction of chondroitin sulfate, periodic acid-Schiff positive materials, and collagen in the hypertrophic zone of the condylar cartilage (or in the part corresponding to trabecular bone of the mandibular ramus) was one factor inhibiting normal endochondral ossification.

The pectoralis major myocutaneous flap for intraoral reconstruction: surgical complications and their treatment.

The pectoralis major myocutaneous (PM-MC) flap was used for intra-oral reconstruction in 7 patients. Major necrosis did not occur in any of the cases, but minor necrosis did in two cases. In 2 cases an oro-cutaneous fistula was observed. Furthermore, in 2 cases severe facial oedema was experienced, which seemed attributable to circulatory disturbance in the part of the face where the flap was inserted. Muscle atrophy of the flap was observed in greater or lesser degree in all cases. In all cases the pectoralis major myocutaneous flap was used for the purpose of intraoral reconstruction. Intraoral reconstruction involves considerations different from transplantation in other sites. It is the purpose of this paper to present an updated review of our experience with 7 consecutive pectoralis major myocutaneous flaps, including the complications and their treatment.

Mouse bone collagenase inhibitor: purification and partial characterization of the inhibitor from mouse calvaria cultures.

The organ culture of neonatal mouse calvaria produced both collagenase and collagenase inhibitor. The inhibitor was purified by a series of column chromatographies: DEAE-cellulose and CM-cellulose ion-exchange chromatography, concanavalin A-Sepharose and heparin-Sepharose affinity chromatography, and finally by Sephacryl S-200 gel filtration. The purified inhibitor migrated as a single band on sodium dodecyl sulfate-polyacrylamide gel electrophoresis and had a molecular mass of 28,000. The inhibitor was purified 140-fold to a specific activity of 163 units/mg with a yield of 18% over the first step of the purification by DEAE-cellulose chromatography. The inhibitor stained positively for carbohydrate with periodic acid-Schiff's reagent indicating, in conjunction with its affinity to concanavalin A, that the inhibitor is a glycoprotein. In addition to mouse bone collagenase, this inhibitor also inhibited chick bone, rat bone, rabbit corneal, and human gingival collagenase, but did not inhibit bacterial collagenase.

Nonfunctioning islet cell carcinoma presenting bleeding gastric varices and splenomegaly.

This is a report of a 63-year-old Japanese woman with a nonfunctioning islet cell carcinoma of the pancreas presenting bleeding gastric varices and splenomegaly. These manifestations are extremely rare in patients with nonfunctioning islet cell tumor. The tumor originated in the tail of the pancreas and grew mainly within the spleen. The gastric varices due to increased blood flow to the tumor and arteriovenous fistulas within the tumor were confirmed by angiography and operation. The tumor was resected and she is in a good health for 14-months after the operation.

Liposarcoma of the cheek in an infant.

A case of liposarcoma of the cheek in a 6-month-old infant has been presented. This tumor, one of the most common malignant tumors of soft tissue in the body generally, is exceedingly rare in the head and neck regions, having been reported in this location on only a few occasions. A review of the literature reveals that this patient was one of the youngest so affected. The postoperative course was uneventful, and after three years no evidence of recurrence or metastasis has been observed.
