Standing views to differentiate gallbladder or bile leak from duodenal activity on cholescintigrams.

Confusing a gallbladder or bile leak with duodenal activity is a common source of error in the interpretation of cholescintigrams. Techniques such as supplementary oblique views, delayed views, the "water test," dynamic display and combined cholescintigraphy and ultrasound have been used with varying degrees of success. As illustrated in two cases, supplementary standing views provide a simple alternative method to differentiate gallbladder from small bowel activity.

Rotaviral enteritis in a raccoon.

A hand-reared raccoon (Procyon lotor) kit had severe diarrhea and died within 24 hr. Gross and histopathologic findings were compatible with a diagnosis of viral enteritis. The immunoperoxidase test revealed rotavirus group A antigen in the intestinal mucosa. This is the first record of rotaviral enteritis in a raccoon.

[Carcinogenic effects of low-dose gamma ray irradiation].

For a dose of 3 Gy delivered by cobalt 60 gamma rays on rats, a reduction of the dose rate by a factor of 60 decreased the carcinogenic effectiveness by a factor of about 5. This decreased effect was essentially observed for carcinomas.

Porcine group C rotavirus as a cause of neonatal diarrhea in a Quebec swine herd.

A porcine group C rotavirus was found to be the unique cause of a problem of enzootic neonatal diarrhea in a minimal disease herd composed of 190 sows on a continuous farrowing program. During the outbreaks of diarrhea, 10 to 80% of the litters were affected with a morbidity rate of 100% and case fatality rates of 5 to 10%. Clinical signs began 24 to 48 h after birth and were characterized by a profuse yellow diarrhea lasting a few days. Piglets from different outbreaks of diarrhea were necropsied. They had multifocal villous atrophy in the small intestine, especially in the ileum. Group C rotavirus was demonstrated by direct immunofluorescent staining of frozen intestinal sections and by polyacrylamide gel electrophoresis of viral RNA extracted from the intestinal contents of diarrheic piglets. The infection with clinical illness and lesions was reproduced experimentally in newborn piglets by oral inoculation of a suspension prepared from a pool of intestinal contents from diarrheic piglets.

[Metastasis of breast cancer in frontal meningioma].

The authors report a case of metastasis from a breast cancer located in a frontal meningioma. A review of the literature revealed 43 cases of primary intracranial tumours containing a secondary from an unrelated extracranial primary malignant tumour; in 34 of these cases the intracranial tumour invaded was a meningioma.

Lung carcinomas in Sprague-Dawley rats after exposure to low doses of radon daughters, fission neutrons, or gamma rays.

The effectiveness of radon-daughter inhalation and irradiation with fission neutrons and gamma rays in the induction of lung carcinomas in Sprague-Dawley rats at low doses is compared. Earlier reports which compared radon-daughter inhalations and neutron irradiations over a wider range of doses were based on dosimetry for the radon-daughter inhalations which has recently been found to be faulty. In the present analysis, low-dose experiments were designed to derive revised equivalence ratios between radon-daughter exposures, and fission neutron or gamma irradiations. The equivalence is approximately 15 working level months (WLM) of radon daughters to 10 mGy of neutrons (the earlier value was 30 WLM to 10 mGy). The relative biological effectiveness (RBE) of neutrons is 50 or more at a gamma-ray dose of 1 Gy. In these experiments with low doses and exposures, the lifetime incidences can be estimated from the raw incidences, while the derivation of the time dependence of the prevalence is essential for the estimation of RBE values and equivalence ratios.

Translocation of subcutaneously injected chrysotile fibres: potential cocarcinogenic effect on lung cancer induced in rats by inhalation of radon and its daughters.

Exposure to radon 222 and its daughters has been shown to induce lung cancer in rats. The cocarcinogenic effect of intrapleurally injected mineral fibres in rats which have previously inhaled radon has also been established. The aim of this work was to establish whether a similar process could be induced at a distance from the lungs by subcutaneous injection of chrysotile fibres. Three groups of animals were used: (1) 109 rats which inhaled radon only (dose: 1600 working-level months (WLM]; (2) 109 rats given a subcutaneous injection in the sacrococcygeal region of 20 mg of chrysotile fibres after inhalation of the same dose of radon; and (3) 105 rats injected with fibres only. No mesotheliomas occurred in any of the 3 groups. The incidence of lung cancer was 55% in group 2, 49% in group 1 and 1% in group 3. Statistical analysis using Pike's model showed that the carcinogenic insult was slightly higher in group 2 than in group 1. Electron microscopy analysis of fibre translocation from the injection site showed that less than 1% of injected fibres migrated to the regional lymph-nodes and only about 0.01% to the lungs. After injection, the mean length of the fibres recovered in lung parenchyma increased with time, suggesting that short fibres are cleared by pulmonary macrophages whereas long fibres are trapped in the alveolar walls. Although the high tumour incidence observed in group 1 might have masked the cocarcinogenic effect induced by the fibres, it is possible that this effect can occur only at short distances.(ABSTRACT TRUNCATED AT 250 WORDS)

[The association of 2 histologically different neuroepithelial tumors].

The association of two primary brain tumours is seen in phacomatoses. Tumours may also be induced by radiotherapy or a primary tumour may be associated with a secondary tumour. The presence of two histologically different primary neuro-epithelial tumours in a subject with no particular medical history appears to be very rare with a undefinable frequency.

The progesterone antagonist RU 486. A potential new contraceptive agent.

Since progesterone supports endometrial nidation of the fertilized ovum, a progesterone antagonist would theoretically block this process and thus have contraceptive potential. We have explored the ability of RU 486, a newly developed competitive progesterone antagonist, to function as a contraceptive agent. A single oral dose of 10 mg per kilogram of body weight given in the midluteal phase consistently induced menses within 72 hours in women with normal cycles and no risk of pregnancy. Bleeding was not prevented by administration of human chorionic gonadotropin in the midluteal phase. This suggested that giving a single dose of RU 486 late in the menstrual cycle might be an effective contraceptive strategy. This concept was tested in monkeys. When given to rhesus females on day 25 of the cycle, a single intramuscular dose of RU 486 (5 mg per kilogram) prevented pregnancy. The vehicle-treated control animals had a 28 percent pregnancy rate (P less than 0.05 by chi-square analysis). No side effects were noted in women or monkeys. These data suggest that a progesterone antagonist such as RU 486 has the potential to be an effective, safe, and convenient contraceptive agent. Further work will be necessary to assess the safety of long-term monthly administration and to define the optimal dose and time of administration in women.

Severe and transient pancytopenia associated with a chronic arenavirus infection.

A so-called 'docile' strain of Lymphocytic Choriomeningitis Virus (LCMV) lacks the ability to cause the fatal central nervous system syndrome, commonly associated with most other strains of this virus, in C3HeB/FeJ mice. Hematological evaluation during a 5 week period revealed that every mouse experienced a pancytopenia which was the most severe around three weeks post-infection. The abnormal red blood cell (RBC) morphology seen in the peripheral blood along with the increased reticulocyte count and marked erythroid hyperplasia in the bone marrow indicated peripheral destruction, rather than stem cell inhibition, as the mechanism causing the anemia. An increase in the 59Fe uptake into the blood confirmed the fact that there was no loss in the erythropoietic capabilities in these mice at this time. Although it was clear that the RBCs were being destroyed in the periphery, there was no evidence of a microangiopathic hemolytic anemia nor of a direct viral infection of these cells. Cyclophosphamide treatment, however, prevented the phenomenon. Thus, it seemed likely that the virus-induced hemolytic anemia in these mice was immune-mediated. The late, but not the early drop in the white blood cell counts and the thrombocytopenia, on the other hand, could be traced to granulocyte and megakaryocyte inhibition in the bone marrow.

[Spontaneous rupture of intracranial dermoid cyst. Clinical and radiological study].

A 22 year old woman had a seizure, and the skull X-ray showed an intracranial arched calcification. CT scan permitted diagnosis of dermoid cyst. Rupture of the cyst was testified by low density areas disseminated on the cortical surface and intra-ventricular fat level. Surgical removal confirmed dermoid cyst with rupture and fat fluid inside. Authors underline infrequency of such intra-ventricular fat level. CT scan, with low density of lipidic origin promote considerably such diagnosis both of the real nature of the cyst and of its rupture.

[Di George's syndrome complicated by graft versus host reaction].

On the occasion of a case of Di George syndrome, complicated by a post-transfusion lethal graft versus host reaction, the authors review its main clinical and biologic criteria. This case also shows that immune deficiencies should be diagnosed early in order to adopt preventive measures before transferring the patient in a specialized unit, where immunorestitution will be discussed.

Fluid therapy trials in neonatal piglets infected with transmissible gastroenteritis virus.

The main purpose of this study was to evaluate the effectiveness of an oral fluid therapy alone or combined with parenteral administration of a 5% dextrose solution to attenuate the clinical signs and the pathophysiological consequences of transmissible gastroenteritis in neonatal piglets. Eighteen two day old conventional piglets were infected with transmissible gastroenteritis virus while six others were used as controls (Group 1). At the onset of diarrhea, infected piglets were divided into three groups of six (Groups 2, 3 and 4). Piglets in group 2 were not treated and were fed a milk replacer ad libitum. Piglets in group 3 were removed from the milk replacer and placed on an oral glucose-glycine-electrolyte solution ad libitum. Those in group 4 were placed on oral fluid therapy and received a 5% dextrose solution intraperitoneally at the rate of 25 mL/kg of body weight once a day. Blood samples were collected in heparin within minutes after the infected piglets became comatose and from the controls at four or five days of age. The following variables were measured: packed red cell volume, blood pH, total plasma protein and bicarbonate, blood urea nitrogen, and plasma glucose, creatinine, chloride, inorganic phosphorus, sodium, potassium, magnesium and calcium. Vomiting and diarrhea appeared 12 to 24 hours postinoculation in the infected piglets. There was a sudden and rapid progression into a comatose and moribund state one or two days later whether the infected piglets were treated or not.(ABSTRACT TRUNCATED AT 250 WORDS)

Erythrocytic pyruvate kinase deficiency and hemolytic anemia inherited as a dominant trait.

A nonspherocytic hemolytic anemia, associated with a pyruvate kinase (PK) deficiency apparently inherited as a dominant trait has been identified in a family. In the affected members, the residual PK activities were 20% that of normal controls, an unusually low level for heterozygous subjects. The anemia was mild except in the proband, a 2-year-old boy who suffered from a severe anemia. The PKs of the proband and of his both parents have been characterized as kinetically and electrophoretically normal enzymes. Immunoprecipitation tests indicated a large amount of L-type inactive protein in the proband and his father. Moreover, an M2 type PK was present in the father's hemolysate, suggesting the existence of a compensatory process that derepressed the corresponding structural gene. We suggest that the presence of one or more mutated subunits in the tetrameric forms of L-type PK leads to the inactivation of these tetramers. This hypothesis accounts for the low residual activity in affected heterozygous members of this family. The severity of the hematological symptoms in the proband, in comparison with the mild hemolysis observed in the other heterozygous members of the family points to the existence of a large spectrum of pathologic expression for an identical PK defect present in a family.
