Studies on lipid and carbohydrate metabolism during surface-induced deep hypothermia with circulatory arrest for open-heart surgery.

Plasma lipids, blood glucose, plasma insulin (IRI) and serum dopamine-beta-hydroxylase (DBH) were measured in 30 subjects undergoing surface-induced deep hypothermia with circulatory arrest for open-heart surgery. Non-esterified fatty acid (NEFA) in the plasma rapidly increased at the lowest temperature (23 degrees C) reached and other lipids in the plasma decreased during the cooling period. An increase of NEFA and a decrease of triglyceride have been attributed to the action of lipoprotein lipase activity stimulated by heparin. It is also likely that the decrease of other lipids and beta-lipoprotein in the plasma results from the transient hypofunction of the liver due to hypothermia. Blood glucose increased during the cooling period, while plasma insulin showed no significant change. Serum DBH reflecting catecholamine also showed no significant change during the cooling or rewarming periods. Therefore, hyperglycemia in hypothermic open-heart surgery may result from the decrease of peripheral utilization of glucose and from the inhibition of insulin secretion due to the transient pancreatic hypofunction.

Experimental studies on myocardial metabolism of carbohydrate and lipid in surface-induced deep hypothermia.

In surface-induced deep hypothermia, metabolic acidosis resulting from lactacidemia was observed. In the aspect of myocardial metabolism, the rate of reduction in coronary A-V difference ratio of lactate, pyruvate and NEFA was less than that of coronary flow and myocardial oxygen consumption in the hypothermic heart. Namely, it seems that lactate, pyruvate and NEFA play an important role as energy fuel in the hypothermic heart. On the other hand, myocardial metabolism of glucose was reduced in the hypothermic heart. Moreover, it seems that exogenous corticosteroid and ATP do not influence on the myocardial metabolism of carbohydrate and lipid in the hypothermic heart.

[Cystic meningioma--report of two cases (author's transl)].

Cystic changes are rare in meningioma. The authors reported two cases of the cystic meningioma. 1) The first case was 56-year-old female, whose complaints were motor weakness and hypesthesia on the right side. At the operation, a hen egg-sized tumor with a large cyst was removed totally from the left frontoparietal mid-convexity. Multiple cystic cavities were contained in the tumor. Histologically the tumor was compatible with meningocytic meningioma with angiomatous component and showed numerous myxomatous degeneration and swollen vessel-walls. 2) The second case was 17-year-girl complaining of headache, blurred vision, right hemiparesis and episodes of Jacksonian seizure. At the operation, a goose egg-sized tumor in the left parietal lobe was removed and the tumor contained a large cyst. Histologically the tumor was a malignant meningioma, associating with relatively fresh necroses adjacent the cyst. 3) The pathogenesis of the cystic changes in meningioma was discussed.

Effects of chlorpromazine and diazepam on neuronal activities of the lateral vestibular nucleus in cats.

The effects of chlorpromazine and diazepam on vestibular neuronal activity were studied in gallamine immobilized cats. Chlorpromazine depressed all the P, N1 and N2-waves of field potentials in the lateral vestibular nucleus evoked by electrical stimulation of the vestibular nerve. Diazepam also reduced the amplitude of postsynaptic components (N1 and N2-waves), while the presynaptic one (P-wave) was slightly increased. Chlorpromazine suppressed the spontaneous as well as the increased vestibular neuronal firing following stimulation of the vestibular nerve. Diazepam enhanced the increased vestibular neuronal firing excited by the nerve stimulation and had no effect on the spontaneous unitary discharges.

Metabolic changes in surface-induced deep hypothermia combined with cardiopulmonary bypass for cardiac surgery.

Surface induced deep hypothermia combined with cardiopulmonary bypass (combined method) is excellent in open heart surgery of infants which combines each advantage of cardiopulmonary bypass and hypothermia. The present study was performed to investigate pathophysiological changes, especially metabolic changes, in combined method induced by two stressors such as cardiopulmonary bypass and hypothermia. In general, metabolic changes during hypothermia are temporarily enhanced immediately after the start of cardiopulmonary bypass. Lactacidemia and metabolic acidosis are rapidly improved after the start of perfusion rewarming, but abnormality of ceruloplasmin and cholesterol metabolism in the liver and hypopotassemia are not rapidly improved. Therefore, the treatment for these metabolic abnormalities seems to be necessary during and after the procedure.
