[Studies on the relationship between quantitative histometry and obstetric complications].

Quantitative histometry of the placenta seems to make it possible to correlate the histopathological changes with various obstetric complications and disorders more precisely. In the present paper, the relationship between the histopathological changes in the placenta and the obstetric complications was investigated by quantitative histometry based on the standardized categories with uncomplicated normal cases as the controls. Among the obstetric complications, special emphasis has been placed on the pregnancies complicated by toxemia of pregnancy and diabetes mellitus. The obstetric disorders of postmature pregnancy and prolonged delivery have also been related to the quantitative histometry. It was revealed that the cytotrophoblast proliferation associated with the formation of syncytial knots was a very common finding both in toxemia and in these obstetric disorders. However, there was a great difference between the two in that impairment of the vasculature was highly specific in the placentae in the case of toxemia. The most conspicuous histopathological change recognized in the placentae due to diabetes was villous edema, which would be due to the osmotic imbalance. However, it is so unusual to find cases complicated by severe diabetes that typical villous edema is rarely observed. The quantitative approaches in the field of histopathology may leave some problems in that the specimens taken from the organ could not always be representative of the whole organ. However, quantitative histometry based on the fairy standardized categories has been shown to offer more promising information on the relationship between the histopathological changes in the placentae and the obstetric disorders.

Antiulcer actions in rats of new synthetic polyisoprenoids.

The effects of 3-N,N-dimethylaminopropyl farnesyl acetate (DMFA), N'-geranylpiperazinyl farnesyl acetamide (GPFA) and N'-methylpiperazinyl geranylgeranyl acetamide (MPGA) were studied on gastric secretion, gastric bleeding and gastric mucosubstances in rats. DMFA lowered gastric juice and gastric bleeding, and GPFA increased gastric mucosubstances and decreased gastric bleeding. MPGA showed remarkable effects for all of these tests. Consequently it might be said that MPGA is a useful agent against both aggressive and resistant factors on gastric functions.

Verrucous carcinoma of the uterine cervix.

Three cases of verrucous carcinoma arising at the uterine cervix were presented with regard to their clinical courses, histological and ultrastructural findings, referring specially to the problems of diagnosis and treatment, and the relationship between viral infection and the disease. In order to obtain a correct diagnosis of verrucous carcinoma, not only repeated punch biopsies, but also wide resection of the tumor by incision biopsy are indispensable, aiming at an accordance between clinical and pathological diagnosis. In treating verrucous carcinoma, which is a slowly progressive nonmetastasizing tumor with strong local invasion, surgery should be the primary choice. Radiotherapy including postoperative irradiation is contraindicated for the reason that not only is verrucous carcinoma radioresistant, but also because anaplastic change following radiation often occurs. Even though in our survey using electronmicroscopy, no virus-like particles were observed, there still remains a strong possibility of the participation of some infections in verrucous carcinoma. This is because all of the primary lesions in our cases, like the other reports in the literature, were located at the posterior lip of the portio .

Plasma 11-deoxycortisol in normal and abnormal human pregnancy.

11-Deoxycortisol was assayed in maternal and cord plasma. The concentrations in maternal plasma increased as pregnancy progressed and reached a peak at 39 weeks. The levels decreased very rapidly in the postpartum period. In cases of intrauterine fetal death and anencephalic fetuses, the values were apparently lower than normal. In women who had undergone bilateral adrenalectomy, the maternal plasma levels were almost normal. The maternal levels were high in cases of twin pregnancies. Therefore, it is assumed that the fetus or the fetoplacental unit is the source of the increased 11-deoxycortisol in maternal plasma during late pregnancy.

[Viability of human chorionic villi in organ culture].

The method and technique of organ culture of human chorionic villi was elaborated in our laboratory. In this report, placental specimens obtained at 15 to 18 weeks of gestation were studied in organ culture for 7 days in terms of the maintenance of morphological integrity and the preservation of functions. The morphological aspect of the viability of the various villous elements with special emphasis on the trophoblast cells was described histologically and ultrastructurally. The functional aspect of the viability was discussed by analysis of the suppressive effect of the cultivated villi on plasminogen activator (PA) secretion by OK-432 elicited mouse peritoneal macrophages, by analyses of the activity of delta 5-3 beta-hydroxysteroid dehydrogenase coupled with delta 5-delta 4-isomerase (HSD) and of the radioactivity of [125I]-Iododeoxyuridine ( [125I]-IUdR) retained in the DNA of the trophoblast cells. Specimens of normal placenta were obtained at the time of induced abortion. The gestational ages were 15, 16 and 18 weeks. Specimens for organ culture were prepared under sterile conditions within one hour after expulsion of the placenta. Through gross dissection, the villi were isolated and minced into fragments of approximately 1 to 2 mm3. Incubation was carried out at 37 degrees C in a conventional static chamber with a gas mixture of 95% air, 5% CO2. The culture dishes and culture media were renewed every day. Data are the mean values of the duplicate incubations. In the first series of organ culture, placental fragments were removed at the end of each day of incubation, washed thoroughly and transferred to the new dishes with a culture medium freed from fetal bovine serum, where further incubation was performed for 24 hours. After this, placental fragments were fixed in 4% neutral buffered formalin, processed through paraffin embedding, serially sectioned at 5 micron and stained by periodic-acid Schiff (PAS) procedure. Culture medium obtained in this series at each end of incubation was put into a "macrophage plate" with the addition of plasminogen in the concentration of 2 IU/m1, in which OK-432 elicited mouse peritoneal macrophages with the capacity to secrete PA were cultured. Reaction was terminated at 24 hours unless otherwise indicated. Ten microliters of the medium in the "macrophage plate" was applied to the fibrin plate, and reaction was performed for 18 hours. PA activity was expressed by plasmin activity converted from plasminogen measured by the single radial immuno-diffusion method.(ABSTRACT TRUNCATED AT 400 WORDS)

Long-chain betulaprenol-type polyprenols from the leaves of Ginkgo biloba.

A long-chain betulaprenol-type polyprenol mixture was isolated from the leaves of Ginkgo biloba mainly as acetate. The structure was determined by mass spectroscopy, 1H-n.m.r. spectroscopy and 13C-n.m.r. spectroscopy. The mixture contained polyprenols-14-22, predominantly polyprenols-17, -18 and -19, and consisted of the dimethylallyl terminal unit (omega-terminal), two trans-isoprene residues, a sequence of 11-19 cis-isoprene residues and a terminal hydroxylated isoprene unit (alpha-terminal) aligned in that order. The concentration of these polyprenols in leaves increased from 0.04 to 2.0% of dry wt. with maturing of the leaves, though the content of total lipids was constant. The distribution of chain length in these polyprenols showed little variation throughout the whole life of the leaves.

Increase in osmotic fragility of bovine erythrocytes induced by bacterial phospholipases C.

Bovine erythrocytes were treated with each of three bacterial phospholipases C; phosphatidylcholine-hydrolyzing phospholipase C (PCase) of Clostridium perfringens, sphingomyelinase C (SMase) of Bacillus cereus and phosphatidylinositol-specific phospholipase C (PIase) of Bacillus thuringiensis. An increase in osmotic fragility was detected by means of a coil planet centrifugation (CPC) apparatus (Biomedical Systems Co., Tokyo) after the treatment with these enzymes. The peak of hemolysis normally observed in the untreated erythrocytes at the range between 50 and 100 mOsM shifted to 160 to 200 mOsM with the progress of sphingomyelin hydrolysis by phospholipase C of C. perfringens. Sphingomyelinase C of B. cereus showed two different effects on bovine erythrocytes: In the absence of divalent cations or in the presence of Ca2+ alone, the peak of hemolysis shifted to the region from 130 to 160 mOsM, without appreciable hydrolysis of sphingomyelin, while in the presence of Mg2+ or Mg2+ plus Ca2+, the peak of hemolysis further shifted to the region from 160 to 200 mOsM with the hydrolysis of sphingomyelin. Abrupt shift in osmotic fragility to a much higher region around 250 mOsM was produced by treatment with increasing amounts of phosphatidylinositol-specific phospholipase C. In this case, a significant amount of acetylcholinesterase was released from the erythrocyte membrane without hot or hot-cold hemolysis. The mechanism of alteration of osmotic fragility of bovine erythrocytes by treatment with phospholipases C seems to differ from case to case, depending upon the specific action of each enzyme toward the membrane phospholipids.

Metabolism of 14C-pregnenolone in the placenta throughout pregnancy in organ culture.

The present study was undertaken to assess the metabolism of radiolabeled pregnenolone in the placenta throughout pregnancy in organ culture. The product generated from pregnenolone was only progesterone regardless of the weeks of gestation. The amount of progesterone formed by the placenta was 5.60 +/- 0.15 x 10(-11) moles/mg protein/hour in the 1st trimester (n = 4), 6.47 +/- 0.09 x 10(-11) moles/mg protein/hour in the 2nd trimester (n = 3) and 7.72 +/- 0.25 x 10(-11) moles/mg protein/hour in the 3rd trimester (n = 6). The increase in the formation of progesterone as gestation advanced was statistically significant. The increase in the placental weight throughout pregnancy was in proportion to the rise in maternal plasma levels of progesterone. These data indicate that the manifold increase in progesterone in the maternal circulation as gestation advances mainly reflects the increase in the functional mass of the placenta rather than the increased rate of production of progesterone in the placenta.

Oculomotor characteristics of parkinsonism in comparison with those of cerebellar ataxia.

Oculomotor characteristics of patients with Parkinsonism have been analyzed. 1. The Parkinsonian patients showed relatively high frequency, but mild degree of abnormalities in various oculomotor functions such as pursuit eye movements, saccade and optokinetic nystagmus and frequently had positional and positioning nystagmus, although all of them might not relate to the pathology of parkinsonism. 2. When a visual target was presented laterally to a subject and head was free to move, the Parkinsonian patients showed relatively good eye-head coordination for gaze shift, resulting in accurate and stable gaze. However, the reaction times of head and eyes were prolonged and overt head movement was observed. 3. Ocular dyskinesias, characterized by smooth, slow, to-and-fro deviations with large amplitude and prominent in darkness were observed in 8 patients out of 10 with bodily dyskinesia while taking levodopa. 4. Oculogyric crises, paralysis of downward gaze, prolonged reaction time of saccade and poor optokinetic nystagmus were seen in a patient with juvenile Parkinsonism, whose neuropathological examination revealed the most striking changes in the substantia nigra, central grey and midline portion, and ventral and caudal to the central grey.

[Open heart surgery for removal of secondary cardiac tumors].

Recent remarkable development of the non-invasive diagnostic apparatus such as two dimensional echocardiography, computed tomography and RI angiography improves early diagnosis for secondary cardiac tumors and promotes surgical treatments for them. Five cases of secondary intracardiac tumors underwent open heart surgery using cardiopulmonary bypass with successful removal. Although a case of direct invasion of a malignant thymoma into the pericardium, RA and SVC was lost at ICU after surgery because of massive hemorrhage followed by LOS, another 4 cases, i.e., a 27 year-old female with implantation of LA myxoma into the RA and RV, a case with intracavitary extension of leiomyosarcoma of the IVC, a case of intravenous extension of the uterine leiomyomatosis through the IVC to the pulmonary artery and a case of intravenous extension of chondrosarcoma of the left fibula to the heart are alive actively. Open heart surgery has become acceptable treatment for cases with secondary intracavitary cardiac tumors in order to rescue emergency cases with circulatory obstruction by the tumor mass, to make sure a pathological diagnosis of unknown origin and to promote further chemotherapy and/or radiation therapy after surgery. Our surgical results may support these active treatments for secondary cardiac tumors.

Full and empty particles of hepatitis B virus in hepatocytes from patients with HBsAg-positive chronic active hepatitis.

Complete and defective hepatitis B virus (HBV) particles in sera and hepatocytes were observed by electron microscopy for an understanding of the maturation process of hepatitis B virus. To distinguish Dane particles with or without DNA on the basis of staining density with uranyl acetate, Dane particles, purified from sera of asymptomatic carriers, and Dane particle cores, separated by ultracentrifugation in a metrizamide density gradient, were observed by electron microscopy. Complete cores at a low density (1.19 to 1.23 gm/cm3) were electron dense and incomplete cores at a high density (1.23 to 1.27 gm/cm3) were partially electron dense or empty. These findings demonstrated that the presence or absence of DNA is reflected by the electron density of the core. Our result, in which less than 10% serum Dane particles have full cores in ultrathin sections of the pellet, is in agreement with Gerin's finding that defective Dane particles are predominent in sera. Naked core particles and Dane particles in two biopsy specimens from patients with hepatitis B surface antigen, hepatitis B e antigen, and DNA polymerase-positive, chronic active hepatitis were classified into complete versus incomplete particles on the basis of electron density. Nine hundred and fourteen naked core particles were detected in nuclei and cytosol of 68 hepatocytes. One hundred and five core particles (11.5%) were electron dense and 809 core particles (88.5%) were partially electron dense or empty. Furthermore, 488 Dane particles were observed in the cisternae of the endoplasmic reticulum of these hepatocytes. Fifty Dane particles (10.2%) had full cores, and 438 Dane particles (89.8%) had partially full or empty cores. These findings suggest that DNA may be incorporated into about 1 to 10% of core particles when they are assembled in nuclei of hepatocytes. Morphologic differences in damage to hepatocytes containing various frequencies of full Dane particles were also studied, but no significant correlation was found between damage in hepatocytes and frequency of full Dane particles.
