[Intraparenchymal granulocytic sarcoma in acute monocytic leukemia; case report].

Granulocytic sarcoma of the parenchyma of the brain present in a patient with acute monocytic leukemia, and its unusual course during treatment, is described. Four years after diagnosis of acute monocytic leukemia, a 24-year-old man developed severe headache during its remission period. The CT scan showed large intraparencymal mass in the right frontal lobe, which was partially removed and diagnosed as granulocytic sarcoma. Following the operation, radiation in total dose of 35.5 Gy was given to the whole brain, and there was also left intraventricular administration of methotrexate (MTX) and cytosine arabinoside (ara-C). The treatment resulted in the complete disappearance of the intraparenchymal mass apart from small calcifications. Five months later, the patient redeveloped severe headache with consciousness disturbance. CT scan revealed marked swelling in the left cerebral hemisphere with irregular contrast-enhanced areas. The patient died of brain herniation in spite of conservative therapy. Photomicroscopic findings of the left cerebral hemisphere proved the presence of "disseminated leukoencephalopathy" and the absence of tumor cells. On the other hand, the right frontal lesion consisted of no tumor cells but scar tissues. This unusual feature of the CT scan in the terminal stage might be caused by combination with the effect of highly concentrated MTX in the left cerebral hemisphere because of the increased permeability of the ependym and the relatively high radiosensitivity in the non-affected left cerebral hemisphere.

[Clinical course in poor grade patients with ruptured intracranial aneurysms and extensive subarachnoidal clot].

Three hundred and eight cases with ruptured intracranial aneurysms and variable amounts of subarachnoidal clot were evaluated clinically with special emphasis on the natural course of poor grade patients. Clinical severity was classified according to the Glasgow Coma Scale (GCS); severe cases (score 3-8) and mild cases (score 9-15). Patients were further divided into four groups based on the preoperative clinical course, and on whether radical surgery was performed or not. Group A, severe cases without radical surgery (17 patients, mean GCS score 4.2); group B, severe cases (which had undergone) radical surgery (24 cases, mean GCS score 5.5); group C, patients considered as severe cases on admission that had improved to mild cases before radical surgery (3 patients, mean GCS score 11.7); group D, mild cases which had undergone radical surgery (254 cases, mean GCS score 14.3). CT findings after their last bleeding episode were evaluated in each group using subarachnoid hemorrhage (SAH) score from "0"-"3" according to the severity of SAH. In addition, the SAH-B (brainstem) score ("0"-"9") was also evaluated. In this score, the amount of SAH in each perimesencephalic cistern (a. prepontine or interpeduncular cistern, b. ambient cistern, c. quadrigeminal cistern) was considered with a possible score of "0"-"3" for each cistern. (No patient had a concurrent intracerebral or intraventricular hematoma causing a mass effect on CT scan). All cases in group A died except one patient that remained in a vegetative state. All of them had severe subarachnoidal clot (mean SAH score 2.9, SAH-B score 8.2). In group B, in patients with same scores (mean SAH score 2.8, mean SAH-B score 7.3), the outcome was as follows: 6 patients (33.3%) had full recovery or were capable of self-management, 6 patients (33.3%) were partially or fully dependent, and 12 patients (50%) were either in vegetative state or died. Patients in groups C and D with good outcome had significantly less amount of SAH, especially in the perimesencephalic cistern (group C--mean SAH score 2.3, mean SAH-B score 3.7, group D--mean SAH score 1.9, mean SAH-B score 3.3). It is concluded that duration and level of unconsciousness in the cases of SAH without concurrent hematoma causing mass effect, has a good correlation with the severity of SAH in the perimesencephalic cisterns.(ABSTRACT TRUNCATED AT 400 WORDS)

Detection of HTLV-I antigen in peripheral and cord blood lymphocytes from carrier mothers.

Vertical transmission of human T-cell lymphotropic virus type-I (HTLV-I) from an antibody-positive carrier mother to her infant has been reported, but it is not clear when and by what route such transmission occurs. Peripheral blood and cord blood lymphocytes from 40 antibody-positive carrier mothers were cultured for 2 months and HTLV-I antigen was sought by indirect immunofluorescence. Viral antigen was detected in 28 (70%) samples of peripheral blood lymphocytes and in 2 of cord blood lymphocytes, both from mothers positive for antigen in peripheral blood. From these data it is estimated that transplacental infection occurs at a rate of about 7%.

Characteristic expression of cholesterol sulfate in rabbit endometrium during the implantation period.

Sialic acids and sulfate residues are the major negative-charged cellular components, which are considered to be crucial in embryonal adhesion to the endometrium. To explore the mechanism of implantation, we examined the change in the amounts of these substances in the rabbit endometrium during the implantation period. Gangliosides and sulfatides were present in very small quantity in the endometrium irrespective of the reproductive stage. Though the content of cholesterol sulfate was relatively low in the nonpregnant endometrium, it abruptly increased at day 5 of pregnancy, i.e. at the beginning of implantation, followed by a gradual decline toward day 9. Cholesterol sulfate level in the inter-implantation sites was about twice as much as that in the implantation sites and was comparable with that in the pseudopregnant endometrium. These results demonstrate that cholesterol sulfate is a major negative-charged lipid in the peri-implantation endometrium in rabbits. We further point to the difference in the concentration of cholesterol sulfate between implantation and interimplantation sites, thus suggesting cholesterol sulfate as a major participant in the process of implantation.

Ca(2+)-independent phospholipase A2 activity associated with secretory granular membranes in rat parotid gland.

Phospholipase A2 activity was detected in a secretory granular fraction (SG) purified by Percoll gradient centrifugation from rat parotid gland using [3H]phosphatidylcholine (PC) as a substrate. High activity of this enzyme was observed at neutral pH. The enzyme was activated by Triton X-100 and did not require Ca2+ for its activity. In the absence of Ca2+, its apparent Km for exogenous PC was 28 microM while it was slightly increased by adding 5 mM CaCl2 (73 microM). Furthermore, the enzyme was located essentially in a granular membrane fraction separated from granular lysate. The deacylation activities were also detected in other subcellular fractions, which showed a different detergent-susceptibility or pH-dependency from that in SG. These results suggest that secretory granules have membrane-bound phospholipase A2 which has properties different from that found in other organelles.

Tracking of fetal breathing movements by pulsed Doppler ultrasound.

Fetal breathing movements form a useful means of monitoring fetal conditions and functional development. Precise quantitative evaluation of fetal breathing movements requires the development of a continuous automated technique. A preliminary study of Doppler signals due to fetal breathing movements was carried out using a cardiotocograph in which the normal high-pass 'thump' filter had been removed. A pulsed Doppler system was then modified to produce five simultaneous outputs representing movement at depths separated by 1.5-cm intervals. These outputs represent the displacement inside tissue at the various depths. Signal processing was executed on a 32-bit computer with an analog-to-digital converter. On-line processing using a zero-crossing method and an off-line high-accuracy displacement estimation technique using an arc-tangent method were applied. Using computerized signal processing, it was possible to detect movements of a few hundred microns representing fetal breathing movements while rejecting gross movements due to maternal breathing, etc. Although further studies are required before clinical application, it appears that quantitative assessments of fetal breathing movements should be possible with this technique.

Exaggerated prolactin response to thyrotropin-releasing hormone in infertile women with the luteinized unruptured follicle syndrome.

Ten cases of luteinized unruptured follicle (LUF) syndrome out of 250 women with unexplained infertility were detected on ultrasonography, giving a frequency of 4%. Hormonal analysis revealed lower serum progesterone levels at mid-luteal phase in LUF cases, suggesting a link between LUF syndrome and inadequate luteal phase. Prolactin response to thyrotropin-releasing hormone was exaggerated in LUF cases as compared with ovulatory cases. Aberrant prolactin release may be a contributory factor in the pathophysiology of the LUF syndrome.

Two flavonol glycosides, hexandrasides C and D, from the underground parts of Vancouveria hexandra.

In addition to four known glycosides, icariin, epimedin B, epimedosides A and E, two new glycosides of a flavonol with a gamma,gamma-dimethylallyl group were isolated from the underground parts of Vancouveria hexandra. The structures were determined to be des-O-methyl-anhydroicaritin 3-O-beta-D-xylopyranosyl(1----2)-alpha-L-rhamnopyranoside 7-O-beta-D-glucopyranosyl(1----2)-beta-D-glucopyranoside and anhydroicaritin 3-O-alpha-L-rhamnopyranosyl(1----3)-alpha-L-rhamnopyranoside 7-O-beta-D-glucopyranoside by means of spectral analysis.

Intracranial metastasis of a choroid plexus papilloma originating in the cerebellopontine angle region: a case report.

The authors present a rare case in which a choroid plexus papilloma originating in the left cerebellopontine angle region metastasized and formed a space-occupying lesion in the right temporal region. A 46-year-old woman with choked disks presented with two separate mass lesions in the right temporal and the left cerebellopontine angle regions. Magnetic resonance imaging exhibited the relationship between the tumors and the surrounding structures. To our knowledge, no case has been reported in which a choroid plexus papilloma originating in the cerebellopontine angle region formed a space-occupying lesion in the right temporal region.

Identification of the rat bone 60K acidic glycoprotein as alpha 2HS-glycoprotein.

Previous reports have described an Mr 60,000-64,000 glycoprotein present in guanidium chloride (GdmCl)/EDTA extracts of bovine and rat bone. We have purified this protein from the long bones of rats and have raised polyclonal antibodies to the purified protein. The 60K glycoprotein has amino acid and carbohydrate compositions that are similar to those reported for the 60-64K protein(s). Several lines of evidence indicate that the 60K bone glycoprotein is the rat homologue of human alpha 2HS-glycoprotein. First, immunochemical data demonstrated that the 60K bone glycoprotein was present in serum as well as in EDTA/GdmCl extracts of bone. Second, immunolocalization and metabolic labelling experiments showed that the 60K protein is synthesized in liver and not in bone cells, although it is sequestered in vascularized regions of bone matrix. Finally, the NH2-terminal sequence for the rat 60K bone glycoprotein was highly similar to that of the human alpha 2HS-glycoprotein A chain. A surprising finding was that small amounts of contaminating 60K/alpha 2HS-glycoprotein were found in several protein fractions purified by ion-exchange chromatography of bone EDTA/GdmCl extracts. Because this protein was found to be highly immunogenic, the presence of anti-60K antibodies in anti-sera prepared against purified bone proteins should be considered as a potential problem.

Evidence for direct regulation of epidermal growth factor receptors by steroid hormones in human endometrial cells.

Recent evidence suggests that epidermal growth factor (EGF) may serve as a paracrine and/or autocrine mediator of oestrogen action in uterine growth. In this study, the effects of various steroid hormones on EGF receptors in primary cultures of human endometrial cells were examined. Human endometrial cells exhibited a single class of high-affinity binding sites for EGF (Kd: 0.14 nM) with approximately 3600 receptors/cell. The addition of progesterone increased the EGF binding without affecting the Kd value. Cortisol also increased EGF binding and acted additively with a relatively low concentration of progesterone (10(-10) M). Oestradiol alone had no effect on EGF binding. However, oestradiol in combination with progesterone and cortisol further increased EGF binding. These results present evidence for the direct regulation of EGF receptors by steroid hormones in human endometrial cells, and raise the possibility that steroid hormones may act on these cells in part by modulating EGF receptors.

Detection and typing of multiple genital human papillomaviruses by DNA amplification with consensus primers.

Many types of human papillomavirus (HPV) are associated with genital lesions. In order to develop simple and sensitive diagnostic procedures for HPV infection, we took advantage of the polymerase chain reaction (PCR). We compared the published nucleotide sequences of the L1 region from six genital HPV types and designed a pair of consensus primers for L1 region. The PCR with the consensus primers for L1 region (L1-PCR) could amplify at least nine genital HPV types, 6, 11, 16, 18, 31, 33, 42, 52 and 58, and the amplified HPV DNA could be typed by subsequent restriction mapping. L1-PCR was compared to Southern blot analysis and also to the consensus primer-mediated PCR for E6 region (E6-PCR) described before. Although both our PCR systems are nonradioactive, PCR for E6 region (E6-PCR) described before. Although both our PCR systems are nonradioactive, the sensitivity in detecting HPV DNA was even better than that obtained by using Southern blot analysis. By means of the PCR systems we detected HPV DNA in 100% of cervical condylomas (10/10), 92% of cervical intraepithelial neoplasias (33/36) and 96% of invasive cervical carcinomas (53/55), while we detected HPV DNA in 12% of normal cervices (12/102).

Clinicostatistical study of low potential malignancy ovarian cystadenoma (borderline cases).

The number of ovarian cystadenoma of low potential malignancy (abbreviation: LPM) was 31 (5.7%) among common epithelial tumors [benign cystadenoma = 400 (74.0%), carcinoma = 110 (20.3%)]. Their mean ages, bilaterlity, frequency of ascites and nulligravidity were situated intermediately between those of benign cystadenoma and carcinoma. Abdominal tumor, distention, pain and metromenorrhagia were the main symptoms, although gastrointestinal complaints were rare in LPM cystadenoma. With LPM cystadenoma, 19 (61.3%) patients were at stage I, 7 (22.6%) at stage II and 5 (16.1%) at stage III, but with carcinoma, 44 (40.0%) were at stage III-IV [33 (30.0%) at stage I, and 31 (28.2%) at stage II]. The 5-year actuarial survival rate was 90% for LPM cystadenoma, but 43% for carcinoma. These results suggest that the clinical and biological features as well as the histological definition of LPM cystadenoma are situated intermediately between those of benign cystadenoma and carcinoma.

Clinicostatistical study of ovarian tumors of germ cell origin.

Ovarian tumors of germ cell origin consisted of 440 dermoid cysts, 5 struma ovariis, 3 immature teratomas (G1, G2), 2 carcinoids, 8 dysgerminoma, 3 yolk sac tumors, one choriocarcinoma, 4 dermoid cysts with malignant transformation, and one immature teratoma (G3) of a total of 1,387 ovarian tumors. The patients' mean ages ranged between 17.3 years to 31.9 years, but the mean ages of those with carcinoid and dermoid cyst with malignant transformation were 46.0 and 53.0 years, respectively. In dermoid cyst bilaterality was 17.3% and occurrence during pregnancy or puerperium was 19.6%. In benign tumors, torsion of pedicle and association with pregnancy or puerperium were frequent unlike common epithelial carcinoma, but ascites was rare. Abdominal tumor, pain and metromenorrphagia were the main symptoms, but abdominal distension was scarce with dermoid cysts. The rate of malignant transformation of dermoid cysts was 0.9% overall but 4.3% in patients over 40 years. The prognosis for borderline germ cell tumors was good.

EGF rapidly stimulates tyrosine phosphorylation in cultured endometrial cells.

Recent increasing evidence suggests that EGF has a role in modulating the differentiated functions of human endometrial cells in an autocrine/paracrine fashion. To explore the signal transduction pathway of EGF in endometrial cells, we used cultured human endometrial cells to examine whether EGF induces tyrosine-phosphorylation. EGF phosphorylated the 175 kDa protein on tyrosine residues within 10 seconds of stimulation. EGF induced tyrosine phosphorylation at as low as 0.1 ng/ml with the maximal effect occurring at 10 ng/ml. Estradiol was shown to enhance the phosphorylation by EGF in this system. These results thus suggest that tyrosine-phosphorylation might be an important step in the signal transduction of EGF in human endometrial cells. Furthermore, the observed stimulatory action of estradiol on tyrosine-phosphorylation by EGF might provide a clue in the elucidation of the cellular mechanism of estrogen action in endometrium.
