[Historical and herbalogical studies on coloring crude drug (Part 1): "Langbacao"].

The name of Langbacao was described for the first time in Kai Bao Ben Cao in 10th century, in which Langbacao was introduced as an herb medicine for scours. While, as the name of Qu, Wujie, it was first described in Er Ya and as the name of Langyecao, it was first described in Ben Cao Shi Yi. Many investigators have confirmed that the original plant of Langbacao is to be Bidens tripartita L. In Japan, Langbacao is generally recognized as ta-u-ko-gi, while some investigators suggested that it is unlikely that ta-u-ko-gi is identical to Langbacao. In the old Chinese literature, Langbacao had been mentioned as a dye which could make hair dark. But in modern Chinese and Japanese literature, few papers reported the use of Bidens tripartita L. as a dye. The characteristics of the shapes of Langbacao between old and modern literature seem not to be consistent. Other names of Langbacao are confused even in the modern literature. It seems that Langbacao in the old literature includes species of the genus Bidens other than B. tripartita.

Cytotoxicity of human beta-interferon produced in human glioma cells transfected with its gene by means of liposomes.

The cytotoxicity of human beta-interferon (HuIFN-beta) produced in human glioma cells was examined by use of our liposomes entrapping two plasmids, pSV2neo and pSVMTV-IFN-beta. After the cells had been transfected with these genes by means of the liposomes, neomycin-resistant cells were selected. When the selected cells were subjected to a single exposure to dexamethasone, all of the cells were found to produce HuIFN-beta and were eliminated by 8 days. Accordingly, the effect of HuIFN-beta produced in human glioma cells is considered to be cytocidal.

Butylated hydroxyanisole produces both mutagenic and desmutagenic derivatives under gastric conditions.

Dietary butylated hydroxyanisole (BHA) has been known to have inconsistent functions on carcinogenesis, both prevention and initiation. We assumed that both functions of BHA were introduced by the derivatives formed after the reaction with gastric components such as nitrite in the stomach. We then identified the derivatives produced by incubating BHA with sodium nitrite at pH 2.0 or pH 5.0. Eight derivatives were detected; 2-tert.-butyl-p-quinone (BQ), 3,3'-di-tert.-butyl-biphenyldiquinone-(2,5,2',5') (BBDQ), 2,6-di-tert.-butyl-8-hydroxy-dibenzofuran-1,4-quinone (BHDQ), 6-nitro-BHA, 2,2'-dihydroxy-3,3'-di-tert.-butyl-5,5'-dimethoxy-biphenyl (di-BHA), an oxidized product of di-BHA, and two unstable reaction intermediates. BQ was a major final product at pH 2.0, but not at pH 5.0. 6-Nitro-BHA and the oxidized products of di-BHA were also the final products. BBDQ was formed from di-BHA and easily converted to BHDQ. Their mutagenicity and desmutagenicity were assayed using Salmonella typhimurium strains. BQ and BBDQ were the mutagens of base-substitution type, BHDQ was the potent desmutagen against a mutagenicity of Trp-P-2, and the others had neither of the two activities. Thus, BHA was found to produce both the mutagen and desmutagen under the gastric conditions. BQ has been previously reported to be easily detoxified by glutathione, and BHA itself is well known to prevent carcinogenesis. In the assessment of dietary BHA on carcinogenesis, since one of the mutagens from BHA is easily detoxified in our bodies and another is converted to a desmutagen, BHA appears to be one of the favourable chemicals for us.

[Development of a fetal breathing movement tracking system using pulsed Doppler ultrasound].

Fetal breathing movements (FBM) form a useful means of monitoring fetal conditions and functional development. To develop a continuous and quantitative FBM monitoring system for clinical use, a non-imaging method using pulsed Doppler was devised. A pulsed Doppler cardiotocograph module was extended to obtain low frequency Doppler signals, by the addition of a 90 degree phase shifter, analog multipliers, quadrature detector, sample and hold circuits and low pass filters, to produce five simultaneous outputs representing movement at depths separated by 1.5cm intervals. These outputs represent the displacement inside tissue at the various depths. Signal processing was executed on a 32-bit computer with an A/D converter. On-line processing by means of a zero-crossing method and off-line arc tangent method with high accuracy displacement estimation were applied for calculation. With this system, FBM was detected as cyclic and regular displacements of a few hundred microns ranging from 0.5 to 1.5Hz. With digital signal processing of 2/3Hz high pass filtering, precise evaluation FBM was possible while rejecting gross movements due to maternal breathing, etc. Although further studies are required for clinical application, it appears that quantitative assessments of FBM should be possible with this technique.

[A case of rheumatoid arthritis with immunotactoid glomerulopathy].

The patient was a 64-year-old female who had been treated by a local doctor for rheumatoid arthritis and hypertension for 10 years. Malaise and edema developed since July, 1990, and as proteinuria and renal dysfunction were noted, the patient was admitted to our hospital on November 2. On admission, BUN was 33mg/dl, creatinine was 2.5mg/dl, and proteinuria was about 3g/day. Renal biopsy was performed after admission. Light microscopy revealed nodular lobulation of glomeruli and occlusion of loops. Dylon staining was negative. Immunofluorescent study showed granular deposition of IgG, IgM, C3, C4, Clq in the glomerular basement membrane and mesangial area. Electron microscopy showed a large amount of electron dense deposits in the subendothelium and mesangial area and dense aggregation of tubular structure in the deposit, part of which exhibited a profile of fingerprint deposit. The tubular structures were classified into three major types, which were 120, 100, and 50nm in diameter. From these findings, a diagnosis of immunotactoid glomerulopathy was made. After renal biopsy, plasmapheresis and prednisolone were administered, and the patient has been managed conservatively to date.

[Clinical study of post-term pregnancy].

One hundred fourteen cases of post term pregnancy were clinically analyzed and classified. 1. In case of primipara, the duration of labor was longer and the frequency of prolonged labor (greater than 24 hours) was higher than that of the same number of cases of term delivery at 39 weeks. 2. The main cause of prolonged labor was weak pains. 3. The mean newborn umbilical artery pH in prolonged labor cases was not lower than that in other cases. 4. The cases of post-term pregnancy were classified into 4 groups and compared each other in several respects. Group I: prolonged labor(-), fetal distress(-), vaginal delivery. Group II: prolonged labor(-), fetal distress(+), vaginal delivery. Group III: prolonged labor(-), fetal distress(+), cesarean section. Group IV: prolonged labor(+). 1) The length of the fundus uteri: Group III: minimum after 31 weeks: Group IV: maximum after 35 weeks. 2) The body weight of the newborn and the weight of the placenta: Group III: minimum: Group IV: maximum. 3) The titer of E3 in urine: Group III: obviously low. 4) The frequency of Clifford's sign: Group III: high frequency of meconium staining. Cases of post-term pregnancy were composed of several groups with different clinical features.

Synchronous cancer of squamous cell carcinoma of the lung and lymphocytic leukemia (B-cell type): a case report.

The patient was an 82-year-old male who consulted us over a lymph node enlargement in the right side of his neck. The WBC was 23,700/microliters (lymphocytes 80%), and a diagnosis of chronic lymphocytic leukemia (CLL) was given based on the findings of a bone marrow puncture and lymph node biopsy. During the observation period, abnormal shadows accompanied by cavitation appeared in the right lower lung field, and CT-guided percutaneous biopsy findings indicated a squamous cell carcinoma of the lung. No metastatic lesions were noted. No aggressive surgical or internal treatments were performed in consideration of the patient's age. The patient was admitted again and died of respiratory failure due to lung carcinoma. Reports of CLL and other malignant tumors arising in one patient are rare in Japan, and those of their occurring concurrently are even rarer. In the present report, a case of synchronous cancer of squamous cell carcinoma of the lung and CLL is described, together with a review of the literature.

Progression of avascular necrosis of femoral head and choice of treatment.

Observations of the disease course mainly by X-ray were made in 52 patients (85 joints) with avascular necrosis of the femoral head to determine the prognosis and to decide on treatment. The progression of the necrotic area is related to the activity of the original disease, to the size of the necrotic area in the early stages of the disease, and to whether or not the patient received steroid treatment. On the basis of the size and location of the necrotic area, the disease process is divided into four stages, I to IV. The affected heads are also classified into six types according to their site and extent, the degree of flatness of their weightbearing surface, and the presence of cystic lesion. Preventive treatment and conservative observation or transtrochanteric anterior rotational osteotomy and vascularized pedicle bone graft are applicable to cases in Stages I and II. Total hip joint arthroplasties and salvage procedures are performed in Stages III and IV. The usefulness of 99mTc bone scintigraphy was unexpectedly disappointing for diagnosis of the stages of the disease. However, MRI was sensitive for the diagnosis in the early stages of the disease.

[Clinical experience with recently produced devices for use in spinal surgery].

The authors reported their clinical experience with devices, which have been recently introduced for spinal surgery. The main roles of such devices are to correct malalignment of the spine and stabilize the spinal instability. Although Harrington's device has been widely used since 1962, its use is gradually decreasing due to its having several weak points, such as its low resistance to distorting forces and its imperfect ability to correct malalignment of the spine. In the present manuscript, the authors describe clinical experience with three kinds of spinal devices. They are Steffee's transpedicular screw for 5 cases of lumbar spondylolisthesis, Roosen-Trauschel compression clamp for 2 cases of atlanto-axial dislocation and Kaneda's device for one case of metastatic T-11 tumor. Good fixation was obtained in 4 cases using Steffee's devices. The injury of the nerve root is one of the most serious disadvantages in this device. Although Roosen-Trauschel compression clamp needs improvement in some aspect, it was able to bring about good stabilization without using sublaminar wiring, which may be a dangerous procedure in the cervical region. Kaneda's device seemed to be useful for the thoracolumbar lesion. Although it needs a relatively radical approach, such as thoracotomy, Kaneda's device brought about good stabilization and corrected the kyphosis. The devices, which are described in the present manuscript, will probably play important roles in spinal surgery in the future.

[Chondroblastoma of the temporal bone--report of a case and a review of the literature of 54 cases].

Chondroblastoma of the skull is a rare tumor and only 54 cases have been reported to date. A case of chondroblastoma arising from the squamous part of the left temporal bone is reported. A 34-year-old woman had 6-month history of left conductive hearing disturbance and tenderness in the left temporal region. Plain skull X-ray showed a well demarcated osteolytic lesion in the temporal bone. CT demonstrated a heterogeneously high density mass, with enhancement. T1-weighted MRI showed a low intensity mass while T2-weighted images showed no signal area. The left external carotid angiograms showed a marked staining supplied by the left middle meningeal artery. This tumor grew destroying the left temporal squama and pyramidal bone, and extended to the external auditory canal and the middle ear cavity. The tumor was subtotally resected. Histologically, this tumor consisted of clusters of round or polygonal chondroblasts with oval or grooved nuclei and well-defined cell border. Multinucleated giant cells were also observed. Chondroid matrix was found in some areas. Immunohistochemically, the tumor cells were positive for S-100 protein. These findings lead us to the diagnosis of chondroblastoma. The diagnosis, histology, therapy, and prognosis of chondroblastoma are discussed including the review of 54 cases in the literature.

[Ventricular puncture; is it a safe procedure?].

Four hundred and fifty patients underwent 755 ventricular punctures as a procedure for continuous ventricular drainage (CVD), or ventriculoperitoneal/ventriculoatrial shunt (shunt) between April, 1983 and March, 1990. In all patients, postoperative findings on CT scan and related clinical manifestations were studied. Of the total of 755 punctures in 100 cases (13.2%) some high density areas (HDA) (i.e. intracerebral hemorrhage) appeared on CT scans, and, of these, HDA larger than 3 cm in diameter (Type C) was seen in 12 cases (1.6%). On the other hand, some low density areas (LDA) appeared in 448 cases (59.3%), and, of these, an extensive LDA (Type C) was seen in 65 cases (8.6%). LDA on CT scan tended to improve gradually with time, and in one third of the cases it was undetectable one year after puncture. Nine percent of the LDA decreased in density, and they were regarded as areas of infarction or porencephalus. The factors considered to affect postoperative HDA were 1) old age, 2) puncture in the occipital region, 3) puncture in a previously damaged brain area, 4) repuncture using the same tract within a short interval from the previous puncture, 5) CVD. Conversely, factors that affected the LDA were 1) low age, 2) puncture in the occipital region, 3) puncture to normal, non-damaged brain, 4) puncture using a new tract. Ten out of 12 patients with large HDA (Type C) showed deterioration of consciousness level or epilepsy as an acute manifestation, and a prolonged disturbance of consciousness. Mental change and/or homonymous hemianopia were seen in all surviving patients in the chronic phase.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case report of cervical disc hernia presenting fecal incontinence evoked by walking; spinal intermittent rectal dysfunction].

Intermittent clinical manifestations, the representative one of which is claudication, can be classified into two types; neurogenic and vasculogenic. Although cauda equina lesions are well known as a neurogenic cause, spinal disorders, especially cervical or thoracic cord lesions, have been paid more attention to by several authors recently. We encountered a 42-year-old man with cervical soft disc hernia and ossification of longitudinal ligament, who showed intermittent rectal dysfunction evoked by walking. This peculiar clinical manifestation successfully disappeared after surgical decompression of the spinal cord. Such an interesting case has not been reported in the world literature. The patient was admitted because of numbness in both hands. Neurological examinations on admission showed neither motor weakness nor abnormally increased tendon reflex. Hypesthesia and hypalgesia were noticed below the Th4 dermatome on both sides. Adding to those symptoms, he complained of fecal incontinence evoked by walking 100 meters. This rectal dysfunction became gradually worse. At last he showed fecal incontinence after walking only 10 meters. This was ten days after his admission. Myelogram and computed tomographic scan revealed a cervical soft disc hernia at the C5/6 level and findings of OPLL at the C5 and C6 level. Anterior cervical approach for OPLL and soft disc was used for bone graft insertion from the C4 to the C7 vertebral body. The rectal dysfunction completely disappeared after the operation. The possible mechanisms of intermittent rectal dysfunction evoked by walking were discussed.

[Renal cell carcinoma metastasizing to choroid plexus of lateral ventricle; a case report].

A rare case of renal cell carcinoma metastasizing to the choroid plexus of the lateral ventricle is reported. A 59-year-old woman was admitted to our institution on November 26, 1987 complaining of left-half headache for one month. She had a past history of right nephrectomy due to renal cell carcinoma 4 years before admission, and of right radical mastectomy due to breast cancer 10 years before admission. She had no abnormal neurological findings and laboratory results were normal. CT scan revealed a well-circumscribed, apparently homogeneously enhancing mass in the left lateral ventricle with mild ventricular dilatation. Left vertebral angiogram showed a tumor stain fed by the left posterior choroidal artery. The most likely diagnosis was thought to be metastasis of renal cell carcinoma. The patient underwent the operation for tumor removal via the posterior interhemispheric transcallosal approach 14 days after admission. A histological examination of the tumor determined the diagnosis of clear-cell type renal cell carcinoma. The operation was uneventful and the patient was discharged 20 days after operation without neurological deficit. But she had recent-memory disturbance, low activity, and gait disturbance in May 1989. CT scan revealed ventricular dilatation and tumor recurrence at the same site. She also suffered from diabetes due to the regrowth and invasion of primary tumor to the pancreas. She was discharged free of neurological defects after ventriculo-peritoneal shunt. In December 1989, she gradually deteriorated due to the regrowth of the intraventricular metastatic lesion, and now she is bedridden. Choroid plexus metastasis is quite rare, and, to our knowledge, only three cases have been described.

[Assessment of ovarian tumors by transvaginal scanning and clinical significance of ultrasonic examination in postoperative follow up].

A hundred and sixty cases of ovarian tumor were preoperatively examined by transvaginal (TV) and Transabdominal (TA) scanning and accuracy of the assessment for histologic type was compared. We also examined 56 cases of postoperative ovarian cancer by ultrasonography to detect recurrent lesions. Positive predictive values for serous cysts and endometrial cysts were higher with TV than TA. Sensitivity for cancer was also high, but false negative rate was lower with TV than TA. The results showed that scatter from small particles and small mural echo in the cystic mass are visualized more clearly with TV, so that TV is more useful for assessing histological type of ovarian tumors. Sensitivity and specificity of ultrasonographic examination for detecting intraperitoneal recurrent lesions were 88.2% and 97.4%, respectively. Those for extraperitoneal lesions were 86.7% and 97.6%. Ultrasonography is suggested to be useful for detecting recurrence ovarian cancer especially for experitoneal lesions, as laparoscopy is not effective for such lesions.

[Basic research for interferon gene therapy against malignant glioma].

In order to establish new interferon therapy against malignant glioma by selective transfection of its gene, we developed a novel transfection system using liposomes bearing positive charges on their surface and entrapping plasmids containing the HuIFN- beta gene (pSV2IFN-beta). The liposomes were composed of N-(alpha -trimethylammonioacetyl) -didodecyl-D-glutamate chloride (TMAG), dilauroylphosphatidylcholine (DLPC), and dioleoylphosphatidylethanolamine (DOPE) as a molar ratio of 1:2:3. The liposomes were not observed to have cytotoxicity for human glioma cells, when applied at a rate less than 15nmol/ml. This liposome-mediated transfection of the gene into the cultured glioma cells (U-251-MG) resulted in the secretion of HuIFN- beta into the medium. The HuIFN- beta level in the culture media of glioma cells reached 23 IU/ml after 96h of incubation. When the pSV2IFN-beta containing liposomes were coupled with a monoclonal antibody (G-22 MCA) against glioma-associated antigen (G-22), the level of HuIFN-beta in the medium was 181 IU/ml, resulting in a 7-fold increase. It was indicated that this transfection system was a safe and selective method in the case of TMAG/DLPC/DOPE liposomes.
