[Enzymeimmunoassay of human interferon-alpha using monoclonal antibody].

Five monoclonal antibodies to human rIFN-alpha A have been obtained and used them in a sandwich enzymeimmunoassay for IFN-alpha. In the assay, rIFN-alpha A was measured from approximately 100 U/ml to 1,000 U/ml, but the sensitivity and the extent to measurement were different among using monoclonal antibodies. Protease or heat denatured IFN-alpha could not be detected in the enzymeimmunoassay and thus only a biologically active form of IFN-alpha was measurable. The reproducibility of the enzymeimmunoassay was considerably high (variation coefficient was lower than 10%) and the correlation of IFN-alpha titers with CPE method was observed slightly. Although the same CPE titers, natural IFN-alpha was detected only 20% compared with rIFN-alpha A in the enzymeimmunoassay, so the assay system seemed to be very specific for rIFN-alpha A. From this observation, it was suggested that the monoclonal antibodies used in the enzymeimmunoassay should recognize a epitope specifically existed on the rIFN-alpha A molecule.

[Treatment of organic impotence. Self-administered intracavernous injection of papaverin HCI (a preliminary report)].

Intracavernous injection of 40 mg papaverin HCI caused erection sufficient for intromission in all six cases of organic impotence. The patients were taught to self inject one corpus cavernosum just prior to coitus and reported that they were able to have coitus at home. No complications were encountered. This method should be tried first as an alternative to penile implant for patients with organic erectile failure.

[Clinicopathologic study of thyroid carcinoma infiltrating the trachea].

A clinicopathologic study of 14 cases of thyroid carcinoma with tracheal infiltration and 70 control cases of thyroid carcinoma without tracheal infiltration was performed. The cases with tracheal infiltration were classified into well differentiated carcinoma in 5 cases, poorly differentiated carcinoma in 6, undifferentiated in 2, and squamous cell carcinoma in one, thus showing a higher frequency (54.5%) of poorly differentiated carcinoma than that (11.4%) in the control cases. A comparative study of biological behavior between well differentiated and poorly differentiated carcinomas revealed that the latter was more aggressive, and the lymphnode metastatic rate of which was higher. The prognosis of poorly differentiated carcinoma was poorer than that of well differentiated carcinoma. Selective en bloc resection of the tumor including the larynx and trachea was followed by the improved prognosis of advanced thyroid carcinoma with tracheal infiltration.

[Clinico pathological study on prognostic factors of sarcoma].

A clinicopathologic study of 48 patients of sarcoma was performed. This malignant neoplasm principally of middle and late adults occurred most often in the body surface (58%). Forty-six patients were treated by surgery with wide or marginal or palliative resection. In two only biopsy was done. Adjuvant chemotherapy was added in 27 patients. Histological grading of surgical specimens was Stage I in 9, II in 16, III in 18 and IV in 5. The evaluation of prognosis showed that wide resection with lymph node dissection was most excellent procedure as the operative methods. Regarding histological grading, significant difference between Stage I + II and III + IV could be detected in the survival rates (p less than 0.05). The adjuvant chemotherapy used did not affect the prognosis.

Horseradish peroxidase studies on thalamic and striatal connections of the mesial part of area 6 in the monkey.

In monkeys (Macaca fuscata), horseradish peroxidase (HRP) was injected in the mesial part of area 6a alpha and 6a beta of the Vogts (presumed supplementary motor area). Groups of retrogradely labelled neurons were found in the dorsolateral half of VA, VLc, X and VLm. Other groups of retrogradely labelled neurons extended through Pcn, Csl, the dorsal edge of Cn.Md, Pf, Cl, MDmf, MDdc and MDpc in the thalamus and the claustrum. Anterogradely labelled terminals were found in turn in these thalamic portions. The neostriatum, the claustrum and the medial part of pars parvocellularis of the red nucleus were noted to receive dense anterogradely labelled terminals. These connections are similar to those of the dorsolateral part of area 6 (premotor area) with some topographical differences.

Electrophysiological and morphological studies on thalamic neurons receiving entopedunculo- and cerebello-thalamic projections in the cat.

Electrophysiological studies on the entopedunculo- and cerebello-thalamic projections were performed by intracellular recordings in the thalamic VA, VL and VM nuclei of cats under sodium pentobarbital anesthesia. Identification of the thalamic neurons were performed electrophysiologically by antidromic activation on stimulation of the precruciate cortex (areas 4 and 6) and the caudate nucleus, and morphologically by intracellular staining with HRP through recording microelectrodes. One hundred and sixty-three neurons were collected in the VA, VL and VM nuclei. In 79 neurons penetrated in the medial and ventral parts of the VA and VL nuclei, stimulation of the entopeduncular nucleus induced monosynaptic IPSPs (latency of 1.1-3.5 ms, mean 2.07 ms). Sixteen neurons were identified as thalamo-cortical relay neurons and 3 were activated only orthodromically by precruciate stimulation. Seventy-eight neurons located dorsolaterally to the entopeduncular-influenced neurons received only cerebellar EPSPs. Only 6 neurons showed convergence of entopeduncular and cerebellar inputs. They were scattered around the border between the entopeduncular and cerebellar projection areas. Sixteen neurons could be stained intracellularly by HRP injection. From the pattern of dendritic arborization, two types of neurons can be distinguished: neurons whose dendrites spread radially in all directions and neurons whose dendrites extend mainly along the long axis of the soma for a long distance in the frontal plane, respectively. The former are relay cells to the cerebral cortex or the caudate nucleus (i.e. projection neurons) and the latter appear to be interneurons in the thalamus.

Radical operation for thyroid carcinoma invading the trachea.

We report 12 cases of well-differentiated thyroid carcinoma that invaded the trachea. In all of these cases, we performed a hemithyroidectomy, including the isthmus, with an accompanying neck dissection and resection of the trachea. Six of 12 patients experienced hemoptysis, and a diagnosis of tracheal invasion was made preoperatively in nine patients by tracheal endoscopy and computed tomography. Histologic diagnosis was confirmed by a preoperative biopsy in one case only. An end-to-end anastomosis of the trachea was performed in five patients, an anastomosis between the cricoid cartilage and the trachea was performed in five patients, and an anastomosis between the thyroid cartilage and the trachea was performed in two patients. One patient with a recurrence of tumor died of tracheal bleeding 11/2 years later. One patient died of massive gastrointestinal bleeding postoperatively. The remaining ten patients have been doing well from three months to five years two months postoperatively.

Anaesthetic management with morphine in phaeochromocytoma.

Morphine was used as the principal anaesthetic agent for five patients undergoing resection of phaeochromocytoma, and changes in plasma catecholamines were monitored. Phenoxygenzamine (0.5 mg X kg-1 X day-1) was orally given one week before operation. After premedication with hyoscine (0.4 mg) and diazepam (20 mg) of meperidine (50 mg), anaesthesia was induced with an infusion of morphine (1 mg X kg-1) over 30 min and maintained with diazepam, nitrous oxide and oxygen. Pancuronium was used as the muscle relaxant. In three of the five patients, blood pressure transiently rose (ranging from 15 to 60 mg) following orotracheal intubation, but vasodilators were not required. In two of the three, plasma norepinephrine increases were 1.1 and 1.3 ng X ml-1. In the other two patients, whose blood pressure was stable, the change in plasma norepinephrine was 0.1 and 0.7 ng X ml-1. After reaching a peak at the time of ligation of drainage vein from the tumour, plasma catecholamines rapidly decreased to the preoperative levels, but the blood pressure was well maintained in all cases. Ventricular arrhythmias did not occur despite the increased levels of endogenous catecholamines. These findings suggest that morphine can be used as an alternative anaesthetic agent during operation for phaeochromocytoma and that exaggerated pressor responses to morphine (when given slowly) seem not to occur in phaeochromocytoma.

Gastric inhibitory polypeptide secretion after radical pancreatoduodenectomy.

To elucidate the role of gastric inhibitory polypeptide (GIP) in the alteration of insulin secretion following pancreatoduodenal resection, in which the main sources of GIP are removed, plasma levels of GIP were measured for 180 minutes after oral glucose administration, both before and after radical pancreatoduodenectomy in nine patients with periampullary cancer. Fasting plasma levels of GIP remained much the same before and after surgery, and were not different from those in normal controls. The levels of GIP after glucose ingestion were significantly greater in the preoperative patients than in normal controls throughout 180 minutes. After pancreatoduodenectomy, the postglucose levels significantly diminished but remained within normal limits. Changes in plasma levels of insulin early after glucose ingestion in these patients, however, were significantly less both before and after surgery than in normal controls, and were not concomitant with the initial increase in plasma GIP. On the other hand, plasma levels of insulin greatly increased immediately after glucose ingestion in accordance with a rapid elevation of plasma GIP in 11 gastrectomized patients in whom the duodenum and the pancreas were preserved intact and who served as the control group. Thus, the diminution in GIP secretion following pancreatoduodenectomy may relate to the lack of main sources of this gut hormone and not to factors involved in the reconstruction of the alimentary tract. We conclude that the impaired insulin secretion following oral glucose ingestion in patients before and after pancreatoduodenectomy does not relate to the secretion of GIP.

Protective mechanisms against infection by Listeria monocytogenes: accumulation and activation of macrophages.

The protective mechanisms against Listeria monocytogenes were analysed in mice. Listeria immune mice were rechallenged with viable listeria, and the degree of activation of macrophages, the degree of accumulation of macrophages to the infected sites, and these mechanisms were studied. Enhanced acquired resistance to the reinfection became detectable from the early stage of immunization. It was observed that activation of macrophages was mediated by pretreatment with antigen from the early stage of immunization. Enhanced accumulation of macrophages at the infected sites was observed only in the early stage. Enhanced accumulation and activation of macrophages in the early stage were generated by mediators from listeria antigen sensitized T lymphocytes which were different from classical tuberculin type. Enhanced activation of macrophages in the later stage was generated by MAF and/or MIF from sensitized T lymphocytes of classical tuberculin type. It was suggested that acquired cellular resistance to listeria in the early stage depends on both activation and accumulation of macrophages, and that in the later stage depends on activation of macrophages.

[Treatment of maxillary cancer].

In order to avoid post-operative cosmetic and functional disabilities, simultaneous combined surgery, radiotherapy and regional chemotherapy was introduced in treating carcinomas of the maxillary sinus. The combined therapy consists of a reduction operation succeeded by daily cleaning of the residual tumor, external irradiation and intra-arterial infusion of 5-fluorouracil and broxuridine. Thirty-six cases of squamous cell carcinoma of the maxillary sinus were treated between 1974 and 1981, and followed up for more than 2 years. Some modifications were made in order to decrease the incidences of local recurrences as well as distant metastasis. The result of these modifications in the combined treatment is too short of time and cases to estimate. However, 5 year survival rate of 10 cases analysed by life table method showed 88%.
