Histologic and immunologic study of uterine biopsy tissue of women with incipient abortion.

Biopsy specimens taken from the region of the placental bed were examined for the presence of phloxinophilic granulated mononuclear cells in women with a history of recurrent miscarriage and who would eventually miscarry a current pregnancy. They were compared with biopsy specimens from women with intact pregnancies presenting for elective termination of pregnancy and those with "missed abortion." Cells with large cytoplasmic granules (greater than or equal to 1 micron) were abundant in the group of ongoing pregnancies whereas cells with smaller granules (less than 1 micron) that were similar to large granular lymphocytes were more abundant relative to cells with large granules in the biopsy specimens from failing pregnancies. Immunosuppressive activity was tested in the supernatants of cultured biopsy samples of each group and found to be significantly lower in the incipient miscarriage group. These findings could represent alterations associated with the process of miscarriage, such as inflammation, or there may be deficient suppressor cell activity at the fetomaternal interface as the reason for "rejection" of the early embryo.

T lymphocyte subsets in leprosy. A study of 24 Egyptian patients.

Phenotypic analysis was done on 24 Egyptian leprosy patients and 11 healthy controls. The type of leprosy, duration of disease at the time of testing, and age were found to affect T cell subset distribution. As compared with controls, neural leprosy tended to have a decreased total T cell percentage, borderline leprosy an increased T suppressor cell percentage, and reactional borderline leprosy an increased T helper/suppressor ratio. Patients with the disease for less than 1 year had a higher mean percentage of T suppressor cells and a lower mean T helper/suppressor ratio than patients with leprosy for more than 1 year. The same was true in older (50-70 years old) versus younger (12-41 years old) patients.

High pressure freezing comes of age.

High pressure freezing permits the successful cryoimmobilization of thick biological specimens (up to approx. 500 microns). A very high yield of adequately frozen specimens, in which no segregation patterns due to ice crystal formation is apparent after freeze-substitution or freeze-fracturing, is obtained with suspensions of microorganisms as well as plant and animal tissue. This very high yield is attributed to an optimized transfer of pressure and cold to the biological specimen. This is achieved by replacement of extraspecimen water or buffer by 1-hexadecene, a chemically inert, hydrophobic paraffin oil of low viscosity and low surface tension.

[Septic melioidosis following a visit to India].

A case is reported of lethal septicemic melioidosis due to Pseudomonas pseudomallei in a 40-year-old woman who had been in India. The epidemiology, clinical findings and management of this unusual disease are discussed. The diagnostic value of serological tests and in vitro sensitivity of Pseudomonas pseudomallei to various antibiotics are outlined. Melioidosis should be considered in the differential diagnosis of any febrile condition in a person returning from a tropical country.

Idiotypic relationships between allotypically different rabbit antibodies.

Homozygous rabbits genotypically different for the allotypy of the VH region and the C kappa region of their immunoglobulins (al/al, b4/b4 and a3/a3, b5/b5, respectively) were transferred together at the blastocyst stage to be born from the same foster mother. At the time of transfer the foster mothers were active anti-anti-idiotypic (Ab3) antibody producers and, according to their own genotype, the idiotypes (Ab1) which initiated the immunization cascade ending with their Ab3 were allotypically either a1,b4 or a3,b5. This experimental scheme was carried out, at the level of the Ab1 and of the Ab'1 (antibodies produced by the transferred rabbits subjected to the Ab3 action), with three different exogenous antigens, namely a protein, a polysaccharide and a hapten which elicited in normal rabbits the production of antibodies with private idiotypic specificities. Two of them, the protein and the polysaccharide enabled us to find idiotypic relationships between antibodies totally different for their allotypy and particularly for the allotypy of the variable region of their heavy chains. In these examples, the structural coercions implied by the different allotypic markers of the VH did not seem to prevent the association of at least related genetic elements responsible for the observed idiotypic community. In these idiotypic relationships, only subpopulations of the Ab1 and of the Ab'1 were implicated and they could be different for the diverse littermates.

Bone marrow transplantation from an unrelated donor for Fanconi's anaemia: two unusual complications.

An eight-year-old boy with Fanconi's anaemia received a bone marrow transplantation from an unrelated donor. Sustained engraftment was achieved, but he developed major complications including acute grade III graft-versus-host disease, autoimmune haemolytic anaemia with positive Coombs' test and free antibodies against Rhesus group, and a virus-associated haemophagocytic syndrome; this latter complication was successfully treated with acyclovir. Sixteen months after transplantation he has full haematopoietic reconstitution, but with persisting mild autoimmune haemolysis.

[Rehabilitation of young craniopharyngioma patients].

The course of rehabilitation in 11 young patients with craniopharyngeoma was investigated. With intelligence well-preserved, neuropsychological diagnosis revealed impairments in rate and persistence in virtually all patients; behaviour disorders were present in 8 of the 11 patients, with efforts to distinguish psychoorganic from psychoreactive causality however not always successful. The neuropsychological deficits and behavioural deviations mentioned, rather than complications of a medical nature (1 of 11), resulted in only 2 patients having been able to maintain their premorbid educational/vocational status. The rehabilitative outcomes obtained in patients with clear-cut indications of diencephalic lesion (among them variations in vigilance, polyphagia, adipsia) had been unsatisfactory, whereas, in the other behaviourally disordered patients, successful outcomes were achieved by a therapeutic approach tailored to the specific needs of the individual condition.

A new 23 valent pneumococcal vaccine: immunogenicity and reactogenicity in adults.

A 23 valent pneumococcal vaccine containing 25 micrograms of each polysaccharide was administered to 30 healthy adults. Adverse reactions were always mild, consisting only of local pain and erythema in most subjects. Pre- and four weeks post-vaccination sera were analysed by radioimmunoassay. Geometric mean titre (GMT) ratios were between 2.3 and 14.3 (overall mean X 5.7) according to type. A level of 300 ng of antibody N/ml, the supposedly protective level, was achieved in at least 86.6% of the subjects for all the serotypes.

[Monilial granuloma treated successfully with ketoconazole over a period of 30 months].

A case of monilial granuloma of 12 years duration is being successfully treated with ketoconazole administered orally in doses of 200 mg per day. Trush, onyxis and keratotic granulomatous lesions of the face and hands have dramatically regressed. In view of persistent anomalies of cellular immunity and complete deficiency in serum and salivary IgA, treatment with ketoconazole at the same dosage level has now been continuously pursued for 30 months without any evidence of toxicity.

Preferential uptake of norepinephrine into dopaminergic terminals of a synaptosomal preparation from rat cerebral cortex.

In a synaptosomal preparation from male rat cerebral cortex only 34% of total norepinephrine (NE) uptake could be inhibited by nanomolar concentrations of desmethylimipramine (DMI) with an apparent IC50 value of 0.37 nM. The residual uptake was efficiently inhibited by micromolar concentrations of DMI (IC50 = 4.0 microM). In synaptosomes from the hypothalamus, 74% of total NE uptake could be blocked by DMI with an IC50 of 0.1 nM whereas in synaptosomes from the striatum the IC50 for DMI inhibition was 3.8 microM. It is concluded that in synaptosomes from rat cerebral cortex only 34%, and in synaptosomes from the hypothalamus 74% of total NE are taken up by noradrenergic nerve terminals whereas the residual NE uptake occurs in dopaminergic nerve endings.

Competition by monophenolic estrogens and catecholestrogens for high-affinity uptake of [3H](-)-norepinephrine into synaptosomes from rat cerebral cortex and hypothalamus.

High affinity uptake of [3H](-)-norepinephrine (NE) was investigated in synaptosomes from rat cerebral cortex (Km = 360 +/- 30 nM) and hypothalamus (Km = 307 +/- 90 nM). Estrogens but not androgens, glucocorticoids or progestin interfered competitively with NE uptake. Ethinylestradiol was the most effective competitor tested, its Ki value being 200 nM in the cortex and 144 nM in the hypothalamus. Stereospecificity of the inhibitory effect of estradiol-17 beta with a preference for the 17 beta-hydroxy group was indicated by the ineffectiveness of estradiol-17 alpha and estrone as competitors. A-ring substitution of estradiol-17 beta or ethinylestradiol by hydroxyl groups in positions 2 and 4 (yielding catecholestrogens) or methyl substitution in positions 2 and 4 (yielding methylestrogens) significantly reduced the inhibitory potency of the estrogen. Methoxylation in positions 2, 4 or 11 beta completely abolished the competitive action of estradiol-17 beta or ethinylestradiol on NE uptake.
