Computation of ventilation-perfusion ratio with Kr-81m in pulmonary embolism.

Diagnostic difficulties occur in pulmonary embolism (PE) during visual analysis of ventilation-perfusion images in matched defects or in chronic obstructive lung disease (COPD). In 44 patients with angiographically confirmed PE and in 40 patients with COPD, the regional ventilation-perfusion ratios (V/Q) were therefore computed using krypton-81m for each perfusion defect, and were displayed in a functional image. In patients with PE and mismatched defects, a high V/Q (1.96) was observed. A V/Q greater than 1.25 was also found in nine of 11 patients having PE and indeterminate studies (studies with perfusion abnormalities matched by radiographic abnormalities). COPD was characterized by matched defects and low V/Q. The percentage of patients correctly classified as having PE or COPD increased from 56% when considering the match or mismatched character to 88% when based on a V/Q of 1.25 in the region of the perfusion defect. This quantitative analysis, therefore, seems useful in classifying patients with scintigraphic suspicion of PE.

Myocardial macroaggregate uptake on successive lung perfusion scans.

In a patient with an Eisenmenger syndrome and multiple intraventricular communications, myocardial uptake of macroaggregated albumin appeared during a perfusion lung scan. This pattern can be explained by an increase of arterial pulmonary hypertension heightening the proportion of the right-to-left shunt.

[Noninvasive evaluation of aortocoronary bypass graft patency with tomodensitometry and exercise myocardial scintigraphy].

The control of the patency of aortocoronary bypass grafts necessitates further coronary angiography, an invasive investigation which is difficult to perform routinely because over 60 p. cent of operated patients are asymptomatic. Non-invasive methods have been proposed for this task, including computerised axial tomography (CAT) and exercise Thallium 201 myocardial scintigraphy (EMS). The aim of this study was to assess the relative value of CAT and EMS, alone and in association, in comparison with coronary angiography. Thirty six patients (35 men, 1 woman) with a mean age of 54 years were studied. These patients had a total of 59 bypass grafts inserted an average of 23 months before investigation (20 single, 10 double, 5 triple and 1 quadruple bypass grafts). CAT scanning was performed the day before coronary angiography. Sections of the thorax 7 mm thick were recorded after intravenous injection of contrast medium. A patent graft was identified as an opacity increasing after the injection of contrast on one of the aortic walls. During coronary angiography a graft was declared to be patent when it was opacified selectively or during aortography, and when the grafted coronary artery was seen to be revascularised. The quality of the distal coronary bed was also evaluated (implantation of the graft, distal and collateral vessels). Twenty three patients (with a total of 36 grafts) also underwent EMS on the same day as CAT scanning. Normal fixation in the revascularised territory was taken as evidence of a patent graft. --Coronary angiography showed that 44/59 grafts were patent at 24 months. --CAT scanning was unable to evaluate 12/59 grafts. Thoracic metallic clips created stratified artifacts and analysis of the section was impossible (20 p. cent of CAT investigations were non-contributive); of the interpretable investigations, 40/47 grafts were correctly assessed (85 p. cent): 30/32 patent grafts and 10/15 occluded grafts. --The results of EMS were less reliable; 23/36 grafts correctly assessed (64 p. cent), 18/27 patent grafts and 5/9 occluded grafts. However, EMS provides complementary information to that provided by CAT scanning, especially with respect to the distal coronary bed. When the two methods were used together, 15/15 good surgical results (patent grafts with good distal vascularisation) and 6/8 poor results (patent grafts but poor distal vascularisation), were identified. We conclude that these two atraumatic methods, CAT scanning and EMS, which may be performed on out-patients, are valuable for the routine assessment of the patency of coronary bypass grafts. Coronary angiography could therefore be reserved for those patients in whom further surgery is being considered.

Biological effects and irradiation dose induced in human lymphocytes in vitro by an intracellular radionuclide:99mTc.

Since lymphocyte radiolabeling is widely used for in vitro and in vivo studies and may interfere with the properties of the cells, the purpose of this work was to investigate the effects of 99mTc labeling on lymphocyte morphology and functions. Two ranges of 99mTc activity were used, 37 and 370 MBq, and their effects were compared to those produced by a graduated external irradiation. Electron microscopy was performed and T cells were studied using biological markers (E-Rosette formation, cell proliferation under lectin reactivity, and specific cell-mediated cytolysis). After 370-MBq labeling, an intense cytotoxic effect was observed on cell morphology, drastically impeding T-lymphocyte functions. Thirty-seven-megabecquerel labeling and sham procedure did not significantly affect cell functions. The external irradiation dose-response curve indicated a clear radiation-induced inhibitory effect, especially over 10 Gy. This effect was less intense than that obtained with 370-MBq 99mTc labeling. The labeling cytotoxic effect can then be attributed to a radiolesion due to a radiation dose which is probably higher than 10 Gy. In fact, the dose received by lymphocytes during the 370-MBq labeling procedure was estimated as 70 Gy by calculations, taking into account all emissions of 99mTc.

Diagnostic value of image processing in myocardial scintigraphy.

The diagnostic value of stress myocardial analog scintigrams, and of five image-processing methods, was assessed by a decisional analysis in 96 patients undergoing coronary arteriography. The methods involved digitalization, nine-point binomial smoothing, background subtraction by linear interpolation, stationary filtering, and a combination of them. The difference between after-test probabilities of having the disease with a positive or a negative examination provided a discriminant index for different prevalences of the disease. Though the processing methods failed to improve the detection of a circumflex stenosis, the stationary filter significantly increased the diagnostic value for the detection of stenosis in a left anterior descending artery for a large range of prevalence, and in a right coronary artery at high prevalence. Thus, the filter seemed to provide a useful tool for enhancing the diagnostic value of myocardial scintigraphy.

Automatic construction of pulmonary functional images.

Dynamic study of regional ventilation of Xenon-133 with a gamma camera requires the medical interpretation of many analog scintigraphic images and time-consuming curve analysis. We have developed a program for the immediate creation of two functionally digitalized images by minicomputer which collects the data from the whole study. These images are constructed by assigning a value to each pont which depends upon the dynamic phenomenon. The program automatically computes the working files of the dynamic acquisition, performs the background subtraction, and delineates the boundaries of the lungs using isoactivity lines. Then the functional images of transit time and clearance of the radioactive gas in the alveoli are reconstructed. This processing takes less than 1 min and allows both time sharing and enhancement of interpretation.

Renin uptake by the right kidney in the rat following partial clamping of the left renal artery.

Renin secretion from both kidneys was studied in eleven Wistar rats for 40 min after partial clamping of the left renal artery. 14C Inulin and 3H para aminohippuric acid clearance and the arterial and venous plasma renin concentrations were measured in each kidney during control and experimental procedures. After clamping, the arterial plasma renin concentration increased and correlated positively with the plasma renin concentration in both renal veins. However, the plasma renin concentration of the right renal vein was lower than the arterial concentration (P < 0.01). The slope correlating the two variables is significantly lower than unity (P < 0.05, n = 19). As the left kidney secreted a large amount of renin, calculation of secretion by the right kidney decreased to negative values, implying renin uptake by this kidney. These data suggest the existence of a balance between the two kidneys. Renin secretion appears to maintain an equilibrium between release by the juxtaglomerular apparatus and removal from the blood into interstitial space. Release seems to predominate in the hypoperfused kidney, and removal in the contralateral kidney. This may result from the increase of the circulating angiotensin II, since we observed a direct correlation between renin uptake and the arterial plasma renin concentration.

99mTc labelled human leukocytes. An in vitro functional study.

A new method of human leukocyte labelling is described. 20 ml of human blood are allowed to sediment in a dextran solution and the remaining red blood cells are removed using NH4C1. The leukocyte-rich suspension is incubated with a Sn-pyrophosphate solution, and then labelled, using 20 mCi 99mTc pertechnetate. Labelling efficiency is about 30%. Functional activity was shown to be satisfactory: (1) appearance under electron microscopy was quite normal; (2) Klebsiella ingestion, zymosan-stimulated oxygen consumption and iodination were similar in labelled and unlabelled cells, and (3) chemotaxis revealed no alteration in cell migration.

"In vivo" kinetics of 99m-technetium labelled leucocytes in dogs and the effects of an abscess.

A new labelling method of blood white cells using "cold" pyrophosphate and 99m-technetium pertechnetate was assessed in 6 normal dogs and 6 dogs with turpentine induced abscesses (8). In normal dogs, blood kinetic studies showed a two component exponential disappearance pattern of the radioactivity, the late half-life being from 4 to 20 hours. Direct organ counting and external computer assisted gamma camera counting showed an immediate accumulation in the lungs followed by a progressive decrease. There is a marked uptake into the liver and spleen. In bearing abscesses dogs, there is an obvious increase in local radioactivity, in spite of a decreased local blood supply, allowing scintigraphic demonstration of such foci in a limb. However, the abscess model is far from perfect and further studies are required before there is a routine use in man for the detection of abscesses.

[Thyroid nodule secondary to subacute thyroiditis. 8 cases (author's transl)].

This description of 8 cases of thyroid nodules secondary to subacute thyroiditis serves to emphasise the relative prevalence of the circumscribed forms. These nodules, variable in size, firm and painful, developed after a period of cervical discomfort and pain. A contrast between a raised or normal blood hormone level and a very low uptake of I131 was found in five cases. It was all the more valuable when there was no iodine overload. Scintigraphy showed the nodule to be cold in 6 cases, isofixing in one and hot in one case. An essential diagnostic criterion is the rapidly favourable course: 4 nodules disappeared completely, 3 regressed in less than 6 months and one persisted, justifying excision with histopathological examination since localised thyroiditis of this type may be the presenting feature of a carcinoma.

Renal consequences of irradiation of the spleen in lymphoma patients.

The authors have studied the consequences of irradiation of the upper part of the left kidney during treatment of the spleen. Seventy-four patients with lymphoma were entered in the study. They have all been followed up for between three and five years. Renal function was assessed by physical examination, biological tests, intravenous pyelograms and nephrotomographies, isotope scans with 197Hg neohydrin, dynamic studies with 131I hippuran and 99Tcm DTPA. No significant changes have been noted for blood pressure, biological tests or IVP. Eighteen months after irradiation renal tomograms showed cortical atrophy of the superior pole of the left kidney. 197Hg neohydrin started to show reduced uptake of the isotope in the irradiated region of the left kidney by the eighth month. 131I hippuran and 99Tcm DTPA detected abnormalities respectively five and three months after irradiation. No compensatory hypertrophy nor change in the overall kidney function have been noted. In Hodgkin's disease, radiotherapy has proved its efficiency in splenic involvement and the lack of secondary renal side effects. On the other hand, morbidity was shown to occur in 5% of cases after splenectomy. From these data, it is proposed to prefer radiotherapy rather than surgery for splenic treatment.

[Fluorescent thyroid scanning and intrathyroidal iodine quantification: study in 140 patients (author's transl)].

The system consists of twenty individual sorces of one Ci of 241Am and a high resolution lithium drifted silicon detector. A dual channel analyser permits to measure intrathyroidal stable iodine. The scan requires only 13 minutes and can be made with a very low radiation dose (less than 10 mrads). It provides good quality images and remains available in cases of flooded iodine pools or iodine induced thyrotoxicosis. It is of great interest to follow up subacute thyroiditis and to diagnose autonomous nodules without TSH stimulation. Thyroid stable iodine was measured in vivo in 104 patients. Normal female thyroid glands contained 5,5 +/- 1 mg stable iodine. In hyperthyroidism (10 patients), iodine values are low or elevated. An inverse relationship exists between thyroidal stable iodine and values of T4 or T3.

Complications of total abdominal and spleen irradiation in patients with lymphomas.

Fifty patients with non-Hodgkin's lymphoma were treated with total abdominal irradiation to a dose of 3000 rad by anterior and posterior fields treated the same day. Fourteen patients developed complications, four with intestinal obstruction due to stenosis and three with total or partial intestinal irradiation damage. These bowel complications were only seen in patients who had previously had a laparotomy. The ways of preventing these complications are discussed. We have studied renal function by means of clinical examination, biologic and radiologic investigations, and scanning in 75 patients who have received splenic irradiation to a dose of 4000 rad. With a mean follow-up time of 36 months, this study has shown that there was no significant change in blood pressure or in biochemical measurements. However, at the 17th month, nephrotomograms demonstrated cortical atrophy of the upper role of the kidney; at the 8th month, low uptake in the irradiated area was seen on 197Hg neohydrine scanning and a decrease in renal plasma flow in the left kidney during dynamic studies with Hippuran.
